In this study, we investigated the effects of neuromuscular electrical stimulation (NMES) on the treatment of 20 acute stroke patients with dysphagia. For both the treated and control groups, the basic facial stimulation training was conducted for 30 minutes, five times a week, for four weeks. NMES was performed on the treated group only, for 30 minutes each time. Both groups were evaluated according to the functional dysphagia scale (FDS) using a videofluoroscopic swallowing study (VFSS). After the treatment was performed for four weeks, the FDS results of the treated group showed a significance difference in oral transit time in the oral phase and in the triggering of pharyngeal swallow fluid, laryngeal elevation and epiglottic closure, nasal penetration, residue in valleculae, coating of pharyngeal wall after swallow fluid, and pharyngeal transit time in the pharyngeal phase. In addition, the treated group showed a significant difference in laryngeal elevation and epiglottic closure, nasal penetration, and pharyngeal transit time in the pharyngeal phase after the treatment compared to the control group. The results of this study showed that neuromuscular electrical stimulation may be an effective method of treating dysphagia in acute phase stroke patients.
본 논문은 근육의 기능을 조절하는 신경말단에 전기적인 자극을 가하여 신경의 반응 정도를 측정하는 플랫폼 구현에 관한 연구로써, 전기 자극에 대한 신경반응이 가해지는 전류량, 가해지는 전류지속시간, 전극위치에 따른 반응을 측정하였다. 신격자극의 전극 위치는 표면말달 운동신경이면 어느 신경이든지 가능하고, 신격자극 양식에는 단순연축자극(Single Twitch Stimulation), 사연속자극(Train-of-four, TOF), 두 집단 발사자극(Double Burst Stimulation, DBS)이 있다. 임베디드 시스템기반으로 가기위한 저전력 MCU를 선정하고, 기본적인 신경자극반응 측정 센서의 민감도를 알아보기 위해 센서 인터페이스를 구성하여 반응정도를 측정해야 한다. 그리고 측정된 Data의 정확도를 높이기 위해 고성능의 AD Convertor 선정하여 플랫폼을 구현하였다.
Purpose : The purpose of this study was to compare the effectiveness of both neuromuscular electrical stimulation(NMES) and isometrical exercise(IE) to strengthen the quadriceps femoris muscle. The relationships between the strength changes and the relative force and duration of training contractions were also studied. Methods : The subjects were divided into three group. The control group(n=6) received no exercise and/or stimulation. The isometric exercise (IE) group(n=6) performed maximum isometric contractions, and the neuromuscular electrical stimulation(NMES)(n=6) engaged electrically stimulated isometric contractions, three days a week for four weeks. Results : Results showed that both IE group and NMES group were found to have an increase in strength significantly greater(p<0.05) than the control group at 4 week. But between IE group and NMES group were not found to have an difference in strength significantly. Conclusion : The relative increase in isometric strength, using IE and NMES, may be determined by the ability of the subjects to tolerate longer and more forceful contractions. Suggestions for further research and implications for the clinical of IE and NMES for strength-training are discussed.
Objectives : The purpose of this study was conducted to find change and development of the proprioceptive neuromuscular facilitation(PNF). Methods: This is a literature study with books, seminar and book for the international courses. Results: Combination of isotonic has been started by Mr and Mrs Johnson after Miss Knott passed away. It is that the terms of maximum resistance changed into optimal or appropriate resistance. It is focus on activities. Maximum resistance focus not only activities but also irradiation have to fit with normal functions. PNF has enough for the motor control concepts and international classification of functioning(ICF)of WHO. Conclusions: PNF has changed and developed with giving resistance method. It is that changed from maximal resistance to optimal resistance. But it is smaller concept than maximum resistance. Even though PNF founded 60 years ago, it is enough for new concepts that is motor control and learning.
Purpose : This study aimed to investigate how neuromuscular electrical stimulation (NMES) affects the balance ability of patients who have undergone total knee arthroplasty owing to osteoarthritis. Methods : Thirty patients who had undergone total knee arthroplasty were randomized to an experimental group (n=15) and a control group (n=15). The experimental group received conventional physical therapy for 50 minutes and NMES treatment for 30 minutes, whereas the control group received conventional physical therapy for 50 minutes and active range of motion (AROM) exercises for 30 minutes. Within-group and between-group changes in static and dynamic balance ability before and after the 4-week intervention were analyzed. Results : In the within-group comparison, sway velocity in the center of gravity and total distance were significantly improved in both the experimental and control groups (p<.05), with no significant differences between the groups (p>.05). In the within-group comparison, both the experimental and control groups showed significant improvement in the functional reach test and movement velosity (p<.05). In the between-group comparison, the experimental group showed a significantly better improvement than the control group in the functional reach test (p<.05), but there was no significant difference in the movement velosity test (p>.05). Conclusion : In this study, NMES improved the static and dynamic balance in patients who had undergone total knee arthroplasty. Compared with AROM exercises, there was a greater effect on dynamic balance partially; however, the overall effect was similar. Therefore, NMES may be one option among various interventions to improve the balance ability in patients who have undergone total knee arthroplasty. In particular, this method may be effective when it is difficult to apply balance training for patients with total knee arthroplasty in a clinical setting.
The purpose of this study was to compare the effects of one proprioceptive neuromuscular facilitation technique and static stretching on calf muscle tightness. The subjects consist of 9 hemiplegics, and 9 quadriplegics. The eighteen subjects were randomly divided into 3 groups: prorioceptive neuromuscular facilitation(6 persons), static stretching(6 persons) and control(6 persons). Contract relax antagonist contract and static stretching techniques were applied continuously for twenty minutes each. Of the many proprioceptive neuromuscular facilitation techniques, only the contract relax antagonist contract technique was applied. The static stretching technique was applied with the subject placed in standing on a seventy degree inclined tilt table for twenty minutes. A wedge was placed under the feet to obtain maximum dorsiflexion. Wedge thickness varied with each subject. Results revealed: (1) a significant difference between the experimental and the control groups(p<0.05). (2) a significant difference between contract relax antagonist contract and static stretching groups(p<0.05). (3) At day five, the final increments were: contract relax antagonist contract $11.9{\pm}1.90^{\circ}$, static stretching $7.7{\pm}2.3^{\circ}$(mean${\pm}$standard deviation).
Purpose: This study investigated the effects of proprioceptive neuromuscular facilitation (PNF) respiratory muscle strengthening exercise on the pulmonary function, back pain, and disability level of patients with chronic low back pain (CLBP). Methods: There were 24 CLBP patients randomly divided into the experimental group (n = 12) who took part in PNF respiratory muscle strengthening exercise and the control group (n = 12) who performed a mock treatment. Both groups performed their respective interventions for 15 min five times per week for four weeks. The pulmonary function was measured using a portable spirometer. The back pain of the participants was assessed using a visual analogue scale. The Oswestry disability index was conducted to measure disability level. A paired t-test was performed to compare within-group changes before and after the PNF respiratory muscle strengthening exercise. Differences between the experimental and the control groups were analyzed using an independent t-test. For all tests, the level of statistical significance was α = 0.05. Results: The within-group change in pulmonary function was significantly different in the experimental group (p < 0.05), and the levels of back pain and disability were significantly different between the groups (p < 0.05). There was also a significant between-group difference in pulmonary function, back pain, and disability level after intervention (p < 0.05). Conclusion: These results suggest that PNF respiratory muscle strengthening exercise enhances pulmonary function and reduces levels of back pain and disability in patients with CLBP.
Purpose: The purpose of this study was to investigate the effects of neck patterns in proprioceptive neuromuscular facilitation (PNF) for neck movement and the neck disability index (NDI) among adults with forward head posture. Methods: Thirty-nine subjects were randomly assigned into two groups. Subjects in the proprioceptive neuromuscular facilitation exercise group (PNFG, n = 20) received 20 minutes of PNF neck pattern (flexion-Rt. lateral flexion-Rt. rotation followed by extension-Lt. lateral flexion-Lt. rotation) 3 times weekly for 4 weeks. Outcomes were measured using absolute rotation angle (ARA), anterior weight bearing (AWB), range of flexion and extension motions (RFEM), and neck disability index (NDI) methods before and after the 4-week intervention period. Results: There were significant effects for the PNFG, pre- and post-intervention, in ARA, AWB, RFEM, and NDI. There were significant differences in ARA, AWB, RFEM, and NDI compared with CG. Conclusion: The results of this study suggest the PNF neck pattern could be beneficial for adults with forward head posture.Purpose: This study investigates how abdominal muscular exercise based on proprioceptive neuromuscular facilitation (PNF) can affect chronic low back pain patients in terms of their pulmonary function, pain, and functional disability indexes. Methods: Fourteen target subjects with chronic low back were randomly assigned to the control group (n = 7) that performed abdominal muscle exercises and the experimental group (n = 7) that performed PNF abdominal muscular exercises. The exercises were performed five times a week for six weeks. To check the change in pulmonary function, the forced expiratory volume at one second (FEV1) and visible analogue scale (VAS) were measured to check the pain level. The disability level caused by back pain was measured by the Oswestry Disability Index (ODI). A paired t-test was applied to compare the differences between the groups before and after the intervention, and an independent t-test was used to compare the differences between the groups. The level of statistical significance was set as ${\alpha}=0.05$. Results: Before and after the intervention, the experimental group showed a significant change in FEV1 (p < 0.01), and both the experimental and the control groups showed significant changes in VAS and ODI (p < 0.01). A comparison of the differences between the groups indicated that the experimental group showed more significant changes in FEV1 (p < 0.05). Conclusion: According to the study results, PNF abdominal muscular exercise effectively improved pulmonary function, pain, and functional disability indexes in subjects with chronic back pain. The proposed program can be applied to chronic back pain patients as a useful therapy.
Purpose: This article was conducted to determine the immediate effects of unilateral contract-relax (CR) stretching on contralateral knee extension range and to compare both sides of the knee extension range between experimental and control groups. Methods: This study recruited 16 adult males and females with straight leg raising abilities below $90^{\circ}$. The subjects were randomly divided into an experimental group and a control group comprising 8 subjects each. The experimental group performed direct CR stretching on the right hamstring muscles with straight hip extension adduction, and the control group performed indirect CR stretching on the right hamstring muscles with straight hip flexion abduction. Each group performed CR stretching 4 times with 4 repetitions comprising 10 sec of contraction and a 10 sec break between repetitions. Before and after the CR stretching exercises, the subjects' passive knee extensions were measured at the hip in a $90^{\circ}$ flexed position. The subjects' peak force on the right leg and peak pressure on the left leg during each CR stretching exercise were also measured. Results: After doing CR stretching 4 times, each group showed a significantly increased passive knee extension range on both sides, and there was no difference in the passive knee extension ranges between the groups. The peak force on the right leg was significantly higher in the experimental group than the control group. There was no difference in peak pressure between the groups. Conclusion: After applying unilateral CR stretching, the study subjects experienced a significantly increased passive knee extension range on the contralateral side. For patients who find it difficult to apply stretching techniques to knee joints directly, the use of the proprioceptive neuromuscular facilitation technique of CR stretching may be useful in improving the range of the knee joint on the contralateral side without direct treatment.
This study assigns each 8 of 24 normal persons to control group(Group I), strength increase group(Group II) and endurance increase group(Group III) to analyze differences in changes of strength and endurance with surface electromyography and kinetics according to application modes of neuromuscular electrical stimulation(NMES). Group I had not any treatment, group II performed 15 repeated contraction with 60% intensity of maximal voluntary isometric contraction(MVIC) by setting 10-sec on time and 50-sec off time and group III conducted 30 repeated contraction with 30% intensity of MVIC by setting 10-sec on time and 20-sec off time. For neuromuscular electrical stimulation, 2,500 Hz of Russian current, 35 pps of pulse rate and 200 of pulse width. Neuromuscular electrical stimulation was conducted by five times for total 4 weeks. Before and after experimentmotor unit action potential of vastus medialis, rectus femoris and vastus lateralis were measured with sEMG, median frequency(MDF) was analyzed, and thus the following results were obtained. There was significant difference in the period of measuring vastus medialis and rectus femoris in change of MDF and interaction among groups with analysis of surface electromyography before and after neuromuscular electrical stimulation(p<.001) and in particular, there was a remarkable change among groups according to the period of measurement. In conclusion, NMES influenced changes of strength and endurance according to its application modes and in particular, it was found that strength increment application had a significant influence on strength increment in applying short-time NMES.
