Objectives: Previous investigations have presented some evidence of late cognitive effects in dental personnel exposed to metallic mercury. We wanted to examine if Norwegian dentists have an increased prevalence of symptoms consistent with neurological and/or cognitive malfunction. Methods: The study group consisted of 406 dentists from central Norway and 217 controls from the general population, all under the age of 70. They had responded to a standardised postal questionnaire (Euroquest) inquiring about seven symptoms in regard to neurology, psychosomatics, memory, concentration, mood, sleep disturbances, and fatigue. A score was calculated for each symptom based on 4 to 15 single questions scored on a scale from 1 (seldom or never) to 4 (very often). Results: The dentists and controls had a participation rate of 57.2 % and 42.9 % respectively. The dentists reported no more cognitive symptoms than the controls, with low average symptom scores from 1.16 for neurological symptoms in males to 1.73 for fatigue in females. Corresponding figures for the controls were 1.22 and 1.77. There were a total of 1.2 % of the dentists and 1.8 % of the controls who reported having three or more of the seven symptoms "often" or more frequently. Conclusion: Norwegian dentists do not report more cognitive and neurological symptoms than controls from the general population.
Mitochondrial dysfunction contributes to the pathogenesis and prognosis of many common disorders, including neurodegeneration, stroke, myocardial infarction, tumor, and metabolic diseases. Ginsenosides, the major bioactive constituents of Panax ginseng (P. ginseng), have been reported to play beneficial roles in the molecular pathophysiology of these diseases by targeting mitochondrial dysfunction. In this review, we first introduce the types of ginsenosides and basic mitochondrial functions. Then, recent findings are summarized on different ginsenosides targeting mitochondria and their key signaling pathways for the treatment of multiple diseases, including neurological disorders, cancer, heart disease, hyperglycemia, and inflammation are summarized. This review may explain the common targets of ginsenosides against multiple diseases and provide new insights into the underlying mechanisms, facilitating research on the clinical application of P. ginseng.
작업치료사의 환자중심훈련(patient-centered practice)은 치료사에게 환자와 그 가족의 관점에서 능동적으로 환자의 기능부전을 찾아야 가능하다. 본 연구는 신경계 작업치료사가 검사 및 평가를 실시하기 위하여 현재 임상에서 사용하는 평가도구의 사용빈도 및 평가도구 선택시 고려사항을 알아보고, 평가에 관한 향후 방향을 제시하기 위함이었다. 연구대상자는 신경계 질환자를 대상으로 치료하는 서울 경기지역에 근무하는 작업치료사 66명이었다. 설문지는 연구대상자의 일반적인 특성, 평가관련 정보, 신경계 작업치료사가 사용하는 평가도구의 내용, 성인 평가도구 및 아동평가도구에 관한 내용으로 구성되었다. 결과분석은 설문에 응답한 66명의 자료를 대상으로 기술적 통계량을 사용하였다. 연구결과, 1일 환자 수는 10인 이상 15인 미만이 가장 많았으며, 초기평가는 20분 이상 40분 미만이 가장 많았고, 환자 재평가 주기는 1개월과 기능적 차이가 보일 때가 가장 많았다. 평가도구는 신경계 관련 도구에만 국한되지 않았으며, 평가도구 선택시 고려사항은 평가도구의 신뢰도와 타당도가 가장 높았다. 성인을 대상으로 사용한 평가도구는 '상지운동기능'은 JHFT, '인지 및 지각'은 MMSE-K, '일상생활동작'은 MBI, '작업수행'은 COPM이 가장 많았다. 또한 아동을 대상으로 한 평가도구는 '인지-지각 평가도구'는 MVPT, '일상생활동작 평가도구'는 Wee-FIM이 가장 많았다. 마지막으로 영역에 따른 평가도구 선택시 고려사항도 평가도구의 신뢰도와 타당도가 가장 높았다. 본 연구결과를 통하여 서울.경기지역에 근무하는 신경계 작업치료사의 영역별 평가도구의 사용빈도와 그 평가도구를 선택할 때 고려하는 사항을 알아볼 수 있었고, 이 결과를 바탕으로 향후 신경계 작업치료사가 평가를 수행할 때 환자수행중심 평가뿐만 아니라 환자와 보호자의 관점을 통합하여 치료계획을 수립할 필요성이 있다고 제언하는 바이다.
Clinical neuropsychological tests were developed originally for the diagnosis of neurological and neuro-surgical diseases. Recently, these tests are being introduced to psychiatric patients. Authors had the experience to use these tests in pychiatric outpatient clinic. Results were as follows. There was a significantly increase in language and attentional function in residual schizophrenia compared to normal control. In chronic neurosis, as visuospatial function was reduced, language and attentional functions were enhanced. With these results, authors suggest that application of neuropsychological tests in psychiatric patients may be very helpful in classifying the subgroups of disease, in selecting the modality of treatment, and in expecting prognosis.
Purpose: The purpose of this study was to examine the effects of the one-time aerobic exercise on pulmonary function, oxygen saturation, and smoking in male college students in their 20's. Methods: The experiment was performed on 11 healthy men who had no musculoskeletal or neurological diseases and who smoked. The subjects performed an aerobic exercise for 30 minutes, using a step box. Before and after the exercise, forced vital capacity (FVC), forced expiratory volume for one second (FEV1), and oxygen saturation were measured. Results: Between the two measurements, there were no significant differences in oxygen saturation (p>.05). However, the differences in FVC and FEV1 were statistically significant (p<.05). Conclusions: The results of this study demonstrate that a one-time aerobic exercise improves pulmonary function.
PURPOSE: To prevent secondary complications from decreased pulmonary functions and promote neurological recovery, identification of respiratory capacity change patterns depending on different postures of stroke patients and investigation of their properties are needed for active rehabilitation. Therefore, this study was conducted to investigate the changes in vital capacity in response to different positions and to implement the results as clinical data. METHODS: A respiratory function test was administered to 52 patients with stroke in the sitting, supine, paretic side lying, and non-paretic side lying positions. Pulmonary function indexes used for comparison were forced vital capacity (FVC), forced expiratory volume at 1 second (FEV1), forced expiratory flow 25-75% (FEF 25-75%), and maximum voluntary ventilation (MVV). One-way repeated ANOVA was used for analysis, and post hoc analysis was conducted using least significant difference (LSD). RESULTS: All pulmonary function indexes were measured in the order of sitting, paretic side lying, supine, and non-paretic side lying positions. Excluding the FEF25-75% and MVV of the supine compared with the paretic side lying position, all other pulmonary function indexes differed significantly (p<.05). CONCLUSION: There are differences in pulmonary function indexes depending on different postures of stroke patients, and the study showed that the non-paretic side lying position yielded the greatest effect on lung ventilation mechanisms. Based on these results, appropriate postures need to be considered during physical therapy interventions for stroke patients.
뇌졸중은 전 세계적으로 신경계 장애를 일으키는 주요 원인 중 하나이며, 뇌졸중 환자는 다양한 운동, 인지 및 정신 장애를 나타낸다. GPR88은 orphan G protein coupled receptor이며 striatal medium spiny neurons에서 높게 발현이 되며, GPR88이 결손이 된 경우 motor coordination과 motor learning에 문제가 발생하게 된다. 본 연구에서는 Western blot 및 real-time PCR을 사용하여 허혈성 마우스 모델에서 GPR88 발현이 감소함을 발견 하였다. 또한, 뇌에서 유래한 세 가지 유형의 세포들, 뇌혈관내피세포(brain microvascular endothelial cells), 미세 아교세포(microglial cells) 및 신경 세포들에서 GPR88의 발현정도를 확인한 결과, HT22 신경 세포에서 GPR88의 발현이 가장 높음을 관찰하였고, 뇌졸중과 유사한 실험조건인 oxygen glucose deprivation (OGD) 조건에 배양한 HT22 신경세포에서 GPR88의 발현이 감소하였다. 또한 GPR88 효현제인 RTI-13951-33 (10 mg/kg)을 전처리후에 뇌허혈을 유발하였을 때, infarct volume의 감소, vestibular-motor function 및 neurological score의 개선효과를 관찰할 수 있었다. 이러한 결과는 GPR88이 허혈성 뇌졸중을 포함한 CNS 질환의 치료를 위한 잠재적인 약물표적이 될 수 있음을 제시한다.
최근의 연구결과를 통해서 중추 신경계와 위장관은 장-뇌 축을 따라서 양방향의 상호작용이 일어나고 있다는 것이 분명해지고 있다. 전임상 연구로부터 장내 마이크로비오타가 다양한 생리적 기능을 통해서 중추 신경계의 기능을 조절할 수 있음이 밝혀지고 있다. 폴리페놀 화합물은 과일, 채소, 차, 커피, 와인과 같은 식품에 존재하는 식물 유래의 물질로, 항산화, 항염증, 항균, 면역 조절, 항암, 혈관 확장 및 프리바이오틱스와 유사한 효과를 보유하고 있어 식이를 통해 섭취할 경우 건강에 직접적인 효과를 나타낸다. 최근 들어 폴리페놀 화합물이 인지 기능뿐만 아니라 산화적 스트레스 및 염증성 손상에 대해 작용하는 신경 보호에 유익한 효과를 줄 수 있다는 증거가 보고되고 있다. 본 총설에서는 신경 세포 신호 전달 경로의 자극, 신경 염증, 혈관 기능 및 장내 마이크로비옴과의 상호작용에 따른 폴리페놀 화합물의 신경 보호 효과와 관련된 작용 메커니즘에 대한 일반적인 개요를 제시한다. 폴리페놀 화합물의 대사 산물은 혈액-뇌 장벽을 가로 지르는 신경 전달 물질을 이용하고 뇌 혈관 시스템을 조절하여 작용하거나, 간접적으로 장내 마이크로비오타에 작용한다. 또한, 폴리페놀 화합물은 노화 관련 인지 기능 저하 및 신경 퇴행과 같은 신경계 질환을 다양한 생리 기능을 통해 효과적으로 관리할수 있다는 사실이 제시되고 있다. 폴리페놀 화합물은 신경 염증을 감소시키고 기억과 인지 기능을 향상 시키며 장내 마이크로비오타를 조절하는 능력을 지니고 있기 때문에 신경계 질환의 예방 및 치료에 있어 잠재적인 기능성 식품으로 주목 받을 것으로 기대된다.

Objective : Through our previous clinical trials, the demonstrated therapeutic effects of MSC in chronic spinal cord injury (SCI) were found to be not sufficient. Therefore, the need to develop stem cell agent with enhanced efficacy is increased. We transplanted enhanced Wnt3-asecreting human mesenchymal stem cells (hMSC) into injured spines at 6 weeks after SCI to improve axonal regeneration in a rat model of chronic SCI. We hypothesized that enhanced Wnt3a protein expression could augment neuro-regeneration after SCI. Methods : Thirty-six Sprague-Dawley rats were injured using an Infinite Horizon (IH) impactor at the T9-10 vertebrae and separated into five groups : 1) phosphate-buffered saline injection (injury only group, n=7); 2) hMSC transplantation (MSC, n=7); 3) hMSC transfected with pLenti vector (without Wnt3a gene) transplantation (pLenti-MSC, n=7); 4) hMSC transfected with Wnt3a gene transplantation (Wnt3a-MSC, n=7); and 5) hMSC transfected with enhanced Wnt3a gene (1.7 fold Wnt3a mRNA expression) transplantation (1.7 Wnt3a-MSC, n=8). Six weeks after SCI, each 5×105 cells/15 µL at 2 points were injected using stereotactic and microsyringe pump. To evaluate functional recovery from SCI, rats underwent Basso-Beattie-Bresnahan (BBB) locomotor test on the first, second, and third days post-injury and then weekly for 14 weeks. Axonal regeneration was assessed using growth-associated protein 43 (GAP43), microtubule-associated protein 2 (MAP2), and neurofilament (NF) immunostaining. Results : Fourteen weeks after injury (8 weeks after transplantation), BBB score of the 1.7 Wnt3a-MSC group (15.0±0.28) was significantly higher than that of the injury only (10.0±0.48), MSC (12.57±0.48), pLenti-MSC (12.42±0.48), and Wnt3a-MSC (13.71±0.61) groups (p<0.05). Immunostaining revealed increased expression of axonal regeneration markers GAP43, MAP2, and NF in the Wnt3a-MSC and 1.7 Wnt3a-MSC groups. Conclusion : Our results showed that enhanced gene expression of Wnt3a in hMSC can potentiate axonal regeneration and improve functional recovery in a rat model of chronic SCI.
