Effects of TENS & Auricular of Somatic on Experimental Pain Threshold (외이경혈자극과 경피신경전기자극이 체성감각에 미치는 영향)

  • The purpose of this study were to examine the effect of high intensity, low frequency transcutaneous electrical nerve stimulation at auricular acupuncture points on experimental pain threshold measured at the wrist and to determine the changes in effect over time. Forty-two healthy adult men and women were assigned randomly to one of three treatment groups. Group 1(n=15) received TENS to appropriate auricular points for wrist, Group 2(n=12) received TENS to wrist, and Group 3(n=15) received no TENS. We measured experimental pain threshold at the wrist after an electrical stimulus during one pre-treatment and three post-treatment time periods. Group 1 and 2 showed stastically significant increase(p<0.05) in pain threshold after treatment whereas the Group 3 did not. Group 1 showed a significant increase in pain threshold than Group 2. These results suggest that high intensity, low frequency TENS applied to appropriate auricular acupuncture points can increase pain threshold.

Differential gene expression pattern in brains of acrylamide-administered mice

  • The present study was performed to evaluate the relationship between the neurotoxicity of acrylamide and the differential gene expression pattern in mice. Both locomotor test and rota-rod test showed that the group treated with higher than 30 mg/kg/day of acrylamide caused impaired motor activity in mice. Based on cDNA microarray analysis of mouse brain, myelin basic protein gene, kinesin family member 5B gene, and fibroblast growth factor (FGF) 1 and its receptor genes were down-regulated by acrylamide. The genes are known to be essential for neurofilament synthesis, axonal transport, and neuroprotection, respectively. Interestingly, both FGF 1 and its receptor genes were down-regulated. Genes involved in nucleic acid binding such as AU RNA binding protein/enoyl-coA hydratase, translation initiation factor (TIF) 2 alpha kinase 4, activating transcription factor 2, and U2AF 1 related sequence 1 genes were down-regulated. More interesting finding was that genes of both catalytic and regulatory subunit of protein phosphatases which are important for signal transduction pathways were down-regulated. Here, we propose that acrylamide induces neurotoxicity by regulation of genes associated with neurofilament synthesis, axonal transport, neuro-protection, and signal transduction pathways.

Anxiety and fatigue related to dental practices of dental hygiene students (치위생과 학생들의 임상실습관련 불안과 피로 조사)

  • Objectives : The aim of the study is to investigate anxiety, fatigue and stress of dental hygiene students in clinical practice and to improve the psychological support program. Methods : A self-reported questionnaire was filled out by 428 dental hygiene students in 4 universities in Gwagju and Jeonnam. Data were analyzed for anxiety, stress and fatigue, general characteristics using SPSS 18.0 program for descriptive statistics, ANOVA, and correlation. Results : Saliva suction showed the most high score in treatment($mean{\pm}SD=2.92{\pm}1.13$) and was followed by children dental care($2.79{\pm}1.21$), preventive dental care($2.38{\pm}1.04$), maxillofacial radiology($2.35{\pm}1.15$). During the clinical practice, students felt uneasy when they stayed in relative home(p<0.05). Poor interpersonal relation(p<0.001), poor health condition(p<0.01), low satisfaction with major(p<0.01), the other cause of major choice(p<0.01) influenced on their stress. Satisfaction with major was closely related to clinical practice performance, anxiety, physical fatigue and mental fatigue. Conclusions : In order to reduce physical, mental, neuro sensory fatigue of students in clinical practice, it is recommended to reinforce the patient care etiquettes in curricula.

A Case of Cecal Volvulus Presenting with Chronic Constipation in Lissencephaly

  • Cecal volvulus is uncommon in pediatric patients and there are few reports of cecal volvulus with cerebral palsy. Here, we report the case of a 19-year-old male patient who presented with abdominal distension, a history of cerebral palsy, refractory epilepsy due to lissencephaly, and chronic constipation. An abdominal x-ray and computed tomography without contrast enhancement showed fixed dilated bowel intensity in the right lower abdomen. Despite decompression with gastric and rectal tube insertion, symptoms did not improve. The patient underwent an exploratory laparotomy that revealed cecal volvulus. Cecal volvulus usually occurs following intestinal malrotation or previous surgery. In this patient, however, intestinal distension accompanying mental disability and chronic constipation resulted in the development of cecal volvulus. We suggest that cecal and proximal large bowel volvulus should be considered in patients presenting with progressive abdominal distension combined with a history of neuro-developmental delay and constipation.

A Review of Postpartum Depression: Focused on Psychoneuroimmunological Interaction (산후 우울의 고찰: 정신신경면역계 상호작용을 중심으로)

  • Purpose: The purpose of this review was to describe a psychoneuroimmunology (PNI) framework for postpartum depression (PPD) and discuss its implications for nursing research and practice for postpartum women. Methods: This study explored the role of hypothalamic-pituitary-adrenal (HPA) axis and inflammation as possible mediators of risk factors for PPD through literature review. Results: From this PNI view, human bodies are designed to respond with the reciprocal interactions among the neuro-endocrine and immune system when they are faced with physical or psychological stressors. Chronic stress induces alterations in the function of HPA axis, and a chronic low-grade inflammatory response is associated with depression. The dysfunctions of cytokines and HPA axis have been observed during the postpartum period. Stress promotes glucocorticoid receptor resistance, which can promote inflammatory responses. This, in turn, can contribute to the pathophysiology of depression. This can especially affect populations at vulnerable time-points, such as women in the postpartum. Conclusion: From a PNI perspective, well-designed prospective research evaluating the role of stress and inflammation as an etiology of PPD and the effect of stress reduction is warranted to prevent PPD.

Relationship of Level of Stress, Life Style, Subjective Symptoms and Clinical Diagnosis in Clients taken Multiphasic Screening Program (종합검진 수검자의 스트레스 수준과 생활습관, 자각증상 및 임상적 진단의 관련성)

  • To improve wellness and quality of life by recognizing the health efforts of stress, the author estimated the relationships between stress, subjective symptoms and clinical diagnosis through a questionnaire and a battery of specified laboratory tests; electrocardiography, blood pressure, cholesterol, aspartate aminotransferase(AST), alanine aminotransferase(ALT), gamma glutamyl transferase$(\gamma-GTP)$, fasting blood sugar, gastro-endoscopy or UGI, abdominal sonography, etc. The data was gathered from 337 clients who were undergoing multiphasic screening program at a University Hospital from January to March 1998. The mean age of subjects was $46.5{\pm}11.2$ years and the mean of body mass index was $24.0{\pm}3.7kg/m^2$. The mean vol of stress was $18.5{\pm}6.0$ expressed as the score out of 40. By general characteristics and lift style among male, mean level of stress was significantly higher in case of lower socioeconomic status, habitual drug use, longer daily working time(>10 hours), no regular exercise, drinkers, irregular meal, skip-ping breakfast(p<0.05). In case of female, that was significantly higher in case of lower education, lowe. socioeconomic status, longer daily working time(>10 hours), no regular exercise, drinkers, smokers, irregular meal, skipping breakfast(p<0.05). Significant correlations were observed between stress and subjective symptoms in all kinds of organ system (p<0.01). Correlation coefficients of stress among male were relatively high with neuro-psychiatric symptom$(\gamma=0.476)$ and cardio-vascular symptom$(\gamma=0.361)$ in order, and correlation coefficients of stress among female was highest with neuro-psychiatric symptom$(\gamma=0.371)$. The prevalence of the diagnosis through the battery of laboratory tests was high in upper gastrointestinal disorders and hypercholesterolemia in order in both sex group. Among male the mean score of stress was significantly high in ulcerative peptic disorder of upper gastrointestine and hepatopathy in order (p<0.05) . Among female that was significantly high in diabetes mellitus. In summary, it is likely that there are associations between stress, subjective symptoms and clinical diagnosis. To promote wellness and quality of life it would be of value that periodic stress evaluation program and stress management including apropriate control of smoking and drinking, regular exercise and meal.

  • This paper describes a phase-shift pulse-width modulation and pulse-frequency modulation series resonant high-frequency inverter using IGBT(Insulated-Gated Bipolar Transistor) for the power control of high-frequency induction heating using neuro-fuzzy, which is practically applied for 20KHz~500KHz induction-heating and melting power supply in industrial fields. The adaptive frequency tracking based phase-shifting PWM(Pulse-Width Modulation) regulation scheme is presented in order to minimize switching losses. The trially-produced breadboards using IGBT are successfully demonstrated and discussed.

  • Arare case of bilateral abducens nerve paralyses after rupture of a left posterior communicating artery(PcomA) aneurysm with multiple unruptured aneurysms in a 46-year-old female is presented. Sudden left abducens nerve paralysis followed by progressive right abducens nerve paralysis were present without additional neuroophthalmological signs. Postoperatively, bilateral abducens nerve paralyses gradually recovered and disappeared in 2 weeks. The authors reviewed and discussed the possible mechanisms involved in this uncommon neuro-ophthalmological manifestation.

Changes in EEG According to Attention and Concentration Training Programs with Performed Difference Tasks (주의·집중훈련 프로그램의 두 가지 과제수행에 따른 뇌파 변화)

  • Purpose: The purpose of this study was to investigate changes in EEG through attention. Concentration training and performing tasks are important factors in the improvement of motor learning ability. Methods: In the experiment, 22 healthy people were divided into two groups: the trail making test (TMT) group and the computerized neurocognitive function test (CNT) group. A one-way Neuro Harmony M test to see whether there was a significant difference among the groups. Results: The TMT group showed a significant increase in ${\alpha}$ wave, ${\alpha}$ wave sequence, and ${\beta}$ wave sequence; however, there were no significant differences in SMR wave, SMR wave sequence, and ${\beta}$ wave. The CNT group showed increases in ${\alpha}$ wave, ${\alpha}$ wave sequence, SMR wave, SMR wave sequence, and ${\beta}$ wave sequence; however, there was no significant difference in ${\beta}$ wave. In EEGs before and after two performance tasks were changed, there were significant differences in ${\beta}$ wave, SMR wave, SMR wave sequence; however, there were no significant differences in ${\alpha}$ wave sequence, ${\beta}$ wave, and ${\beta}$ wave sequence. Conclusion: Attention training and concentration training offer feedback and repetition for constant stimulus and response. Moreover, attention training and concentration training can contribute to new studies and motivation by developing fast sensory and motor skills through acceptable visual and auditory stimulation.

Hybrid adaptive neuro-fuzzy inference system method for energy absorption of nano-composite reinforced beam with piezoelectric face-sheets

  • Effects of viscoelastic foundation on vibration of curved-beam structure with clamped and simply-supported boundary conditions is investigated in this study. In doing so, a micro-scale laminate composite beam with two piezoelectric face layer with a carbon nanotube reinforces composite core is considered. The whole beam structure is laid on a viscoelastic substrate which normally occurred in actual conditions. Due to small scale of the structure non-classical elasticity theory provided more accurate results. Therefore, nonlocal strain gradient theory is employed here to capture both nano-scale effects on carbon nanotubes and microscale effects because of overall scale of the structure. Equivalent homogenous properties of the composite core is obtained using Halpin-Tsai equation. The equations of motion is derived considering energy terms of the beam and variational principle in minimizing total energy. The boundary condition is assumed to be clamped at one end and simply supported at the other end. Due to nonlinear terms in the equations of motion, semi-analytical method of general differential quadrature method is engaged to solve the equations. In addition, due to complexity in developing and solving equations of motion of arches, an artificial neural network is design and implemented to capture effects of different parameters on the inplane vibration of sandwich arches. At the end, effects of several parameters including nonlocal and gradient parameters, geometrical aspect ratios and substrate constants of the structure on the natural frequency and amplitude is derived. It is observed that increasing nonlocal and gradient parameters have contradictory effects of the amplitude and frequency of vibration of the laminate beam.
