말초성 안면마비에서 중두개와 접근법에 의한 안면신경 감압술 : 3례 보고 (Facial Nerve Decompression via Middle Fossa Approach : Report of Three Cases)

  • Objective : Several conservative treatments have been tried in peripheral facial nerve paralysis, because 80% of patients recover spontaneously. Surgical decompression may be helpful to the residual, medically intractable patients. We present here our experiences of facial nerve decompression via middle fossa approach, which seems to be one of good surgical therapeutic options for medically refractory peripheral facial nerve paralysis. Method : Three cases of medically intractable peripheral type facial paralysis were microscopically operated via middle cranial fossa approach to decompress the labyrinthine segment of the facial nerve and geniculate ganglion by searching landmarks of middle meningeal artery, greater superficial petrosal nerve and facial hiatus. Results : After operation, two cases of Bell's palsy improved substantially and one case of post-traumatic facial paralysis improved partially. Conclusion : This report is presented to describe the surgical facial nerve decompression via middle fossa for early control of peripheral type facial paralysis. Surgical decompression of edematous peripherally paralysed facial nerve could be preferred to conservative treatment in some patients although more surgical experience should be required.

The Effects of Aqueous Extracts of Aconiti ciliare tuber on Functional Recovery after Sciatic Crushed Nerve Injury in Rats

  • Objective : The aim of this study was to evaluate the effects of Aconiti ciliare tuber on the descending pain and the recovery of locomotor function that results from sciatic crushed nerve injury in rats. Method : In order to assess the effects of the aqueous extract of Aconiti ciliare tuber on the recovery rate of locomotor function, we investigated the walking track analysis, and for the effects on the pain control we investigated brain-derived neurotrophic factor (BDNF) and inducible nitric oxide synthase (iNOS) expression in the sciatic nerve and on the expressions of c-Fos in the ventrolateral periaqueductal gray (vlPAG) region resulting from the sciatic crushed nerve injury in rats. Result : Treatment with Aconiti ciliare tuber significantly enhanced the SFIvalue, enhanced BDNF expression, decreased iNOS expression, and suppressed c-Fos expression. The present results showed that Aconiti ciliare tuber facilitated functional recovery following sciatic crushed nerve injury in rats. The recovery mechanisms of SFI by Aconiti ciliare tuber might be ascribed to the increase of BDNF expression for nerve regeneration and reinnervation and to the suppression of iNOS expression for inhibiting nerve inflammation. Conclusion : In this process it has been shown that Aconiti ciliare tuber can be used for pain control and functional recovery from peripheral nerve injury.

  • Biplane fluoroscopy is usually used in angiography. Biplane fluoroscopy gives a biplane image with high resolution during the performance of operations. Trigeminal nerve blocks are effective treatment modalities for trigeminal neuralgia, and maxillary nerve block is the most dangerous procedure among them. The anatomic structures can change after head and neck surgery, so the trigeminal nerve block procedures cannot be done so easily. We used biplane fluoroscopy in these difficult cases. Our first case was a 60-year-old man who had undergone maxillary nerve block. The second case was of a 64-year-old man who had pulsed radiofrequency lesioning of mandibular nerve performed after head and neck surgery. With biplane fluoroscopy, we got good results without any complications.

당귀가 rat의 손상된 좌골신경 재생에 미치는 영향 (Improved Axonal Regeneration Responses in the Injured Sciatic Nerve of Rats by Danggui Treatment)

  • Objective: This study was performed to examine Danggui (DG, Angelica gigas Nakai)'s potential activity for promoting axonal regeneration in the injured peripheral nerve. Methods: Using the sciatic nerve in the rats, DG extract 5 ${\mu}l$(10 mg/ml in 0.5% saline) was dripped into the injury site of the nerve. Results: DG treatment facilitated axonal elongation responses in the distal portion to the injury site. GAP-43 protein levels were upregulated by DG treatment in the injured nerve and also in the DRG, suggesting the induction of GAP-43 expression at gene expression level after nerve injury. Phospho-Erk1/2 protein levels were upregulated in the injured nerve area and also in the DRG, suggesting retrograde transport of phospho-Erk1/2 protein from the injury area to the cell body. Cdc2 protein levels were slightly upregulated by DG treatment. DG treatment increased the number of non-neuronal cells in the distal portion to the injury site. Conclusions: The present data suggest that DG is effective for enhanced axonal regrowth after sciatic nerve injury.

랫드의 대퇴 신경중 ATPase 효소활성에 미치는 납의 영향 (Effects of lead on ATPase activity in the sciatic nerve of Sprague-Dawley rat)

  • Nerve conduction impairment in lead neuropathy has been empirically linked to altered nerve myo-inositol metabolism. In most cases of neuropathy, abnormal myo-inositol metabolism is associated with abnormal $Na^+/K^+$ATPase provides a potential mechanism to relate defects of the myo-inositol metabolism in the peripheral nerve treated with lead. Therefore, the effect of lead on the rat sciatic nerve $Na^+/K^+$ATPase and other ATPase of sciatic nerve was studied. ATPase activity was measured enzymatically in sciatic nerve homogenates from 2-wk lead treated neuropathy rats and age-mached controls administered myo-inositol. $Na^+/K^+$ATPase components were assessed by ouabain inhibition or the omission of sodium and potassium ions. Lead reduced 50% reduction in the $Na^+/K^+$ATPase activity in homogenates of sciatic nerve. The 50% reduction in the $Na^+/K^+$ ATPase activity was selectively prevented by myo-inositol treatment. This study suggests that the toxic mechanism of the lead on peripheral nerve may be through reduction in $Na^+/K^+$ATPase activity which has been linked to axonal transport slowing in the rat model of lead neuropathy, via direct changes by the perturbation of the intracelluar sodium or potasium level.

  • Optic nerve injury serious enough to result in blindness had been reported to occur in 3% of facial fractures. When blindness is immediate and complete, the prognosis for even partial recovery is poor. Progressive or incomplete visual loss may be ameliorated either by large dosage of steroid or by emergency optic nerve decompression, depending on the mechanism of injury, the degree of trauma to the optic canal, and the period of time that elapses between injury and medical intervention. We often miss initial assessment of visual function in management of facial fracture patients due to loss of consciousness, periorbital swelling and emergency situations. Delayed treatment of injuried optic nerve cause permanent blindness due to irreversible change of optic nerve. But by treating posttraumatic optic nerve injuries aggressively, usable vision can preserved in a number of patients. The following report concerns three who suffered visual loss due to optic nerve injury with no improvement after steroid therapy and/or optic nerve decompression surgery.

좌골신경 손상에 대한 한약물 치료효과에 대한 실험논문 분석 - 국내외 한의계 연구를 중심으로 - (Effects of the Herbal Medicine in Animal Models for Treatment of Sciatic Nerve Impairment: A Review of Animal Study Reports)

  • Objectives This study is to review the effect of herbal medicine on treatment of sciatic nerve injury induced animal models reported in domestic & foreign journals. Methods 5 electronic databases (Pubmed, CAJ, RISS, Oasis, Koreantk) were searched with term as sciatic nerve injury, and animal study reports on sciatic nerve impairment with herbal medicine treatment were extracted. Results 31 articles were reviewd. All studies used SD rat. 26 studies used crush injury at the sciatic nerve using haemostatic forcep and 5 studies used sciatic nerve transection. 15 studies used single herb extract and 16 studies used complex herb medicine. Angelica gigantis radix was the most frequently used herb. Each study showed significant changes of improvement indicators from sciatic nerve impairment. Conclusions Various herb medicine are expected to have positive effects on sciatic nerve impairment.

열상에 의하지 않은 표재 비골 신경과 비복 신경 병변의 치료 (Treatment of Superficial Peroneal and Sural Nerve Lesions Unrelated to Laceration)

  • Purpose: To review the results of surgical treatment for superficial peroneal and sural nerve lesion unrelated to laceration. Materials and Methods: Eleven superficial peroneal and sural nerve lesions in eleven patients were surgically treated at our hospital with follow up of average 20.7 months (range, 9-64 months). The anatomical locations of the lesion were on superficial peroneal nerve in seven patients including two patients having ganglion and sural nerve in four patients. Two patients were male and the average age at surgery was 41.5 years (range, 23-57 years). Six cases developed after repetitive sprain and five cases had no trauma history. Clinical results were assessed according to the criteria of Pfeiffer and Cracchiolo. Results: The methods of operation were proximal resection of the nerve lesion in nine cases and removal of ganglion only in two cases. The results were excellent in four cases, good in five cases, fair in one case and poor in one case. Ten cases (10 patients) were satisfied with the result of treatment. Conclusion: We can expect satisfactory results of surgical treatment for superficial peroneal and sural nerve lesion unrelated to laceration.

침 치료가 신경 재생 및 회복에 미치는 영향에 대한 연구 동향: PubMed를 중심으로 (Current Research Trend on Acupuncture Treatment for Nerve Regeneration and Recovery: Based on the Data of PubMed)

  • Objectives : The purpose of this study is to explore the current research trend on acupuncture treatment for nerve regeneration and recovery effect. Methods : We investigated the researches so far, on acupuncture treatment for the nerve regeneration and recovery via searching Pubmed from 2005 up to October 2014. Data were extracted from the included studies regarding the authors, countries, type of nerve injury, type of acupuncture, treatment period, acupuncture points, assessment tool and results. Results and Conclusions : Twenty-four research papers were included in the review. Outcomes were measured by immunohistochemical results, motor behavior scores, and electrocphysiological results. All but one study favored acupuncture and electroacupuncture treatment for nerve regeneration and recovery regardless of type of nerve injury and acupuncture modality. Acupuncture treatment may have a potential for nerve regeneration and recovery and further research is required.

안면경련 환자에서의 안면 신경차단 -27예의 환자분석- (Facial Nerve Block for the Treatment of Facial Spasm -A retrospective analysis of 27 patients-)

  • Twenty seven patients with hemifacial spasm were treated by facial nerve block at the foramen stylomastoideum and O'Brien block from January 1992 to March 1994. There were 16 female and ll male patients, a 1.5:1 ratio respectively. Most patients were 40~60 years old. Among the 27 patients treated by nerve blocks, 26 responded well to the facial nerve block, including two cases of O'Brien block. One patient failed to respond to the facial nerve block. Induced facial palsy disappeared within one or two months within the majority of patients. Among the 26 patients who received nerve block, 6 patients required a second block within 3 to 9 months. After successful nerve block, all patients were free of spasm for 1 to 16 months. Although the follow-up period was short in duration, these results suggest that facial nerve block is a satisfactory and reliable method in the treatment of facial spasm.
