Does total regression of primary rectal cancer after preoperative chemoradiotherapy represent "no tumor" status?

  • Purpose: Insistence that total regression of primary tumor would not represent long-term oncologic outcomes has been raised. Therefore, this study aimed to evaluate the outcomes of these patients after preoperative chemoradiotherapy (PCRT) and radical surgery and to evaluate the associated risk factors. Methods: We included 189 patients with rectal cancer who showed total regression of the primary tumor after PCRT, followed by radical resection, between 2001 and 2012. Recurrence-free survival (RFS) was calculated using the Kaplan-Meier method, and the results were compared with 77 patients with Tis rectal cancer who received only radical resection. Factors associated with RFS were evaluated using Cox regression analysis. Results: Sphincter-saving resection was performed for 146 patients (77.2%). Adjuvant chemotherapy was administered to 168 patients (88.9%). During the follow-up period, recurrence occurred in 17 patients (9%). The 5-year RFS was 91.3%, which was significantly lower than that of patients with Tis rectal cancer without PCRT (P = 0.005). In univariate analysis, preoperative CEA and histologic differentiation were associated with RFS. However, no factors were found to be associated with RFS. Conclusion: RFS was lower in patients with total regression of primary rectal cancer after PCRT than in those with Tis rectal cancer without PCRT, and it would not be considered as the same entity with early rectal cancer or "disappeared tumor" status.

Spontaneous Regression of Non-Small Cell Lung Cancer in a Patient with Idiopathic Pulmonary Fibrosis: A Case Report

  • Treatment of lung cancer in patients with idiopathic pulmonary fibrosis (IPF) is difficult because the mortality rate after surgery or chemotherapy is high for these patients. Spontaneous regression of cancer is rare, especially in lung cancer. A 62-year-old man, previously diagnosed with IPF, presented with stage IIIC (T2N3M0) non-small cell lung cancer. About 4 months later, spontaneous regression of the primary tumor was observed without treatment. To the best of our knowledge, this is the first report of spontaneous regression of lung cancer in a patient with IPF.

항암화학요법을 받는 폐암환자의 불확실성 영향요인 (Influencing Factors on Uncertainty of Patients Undergoing Chemotherapy for Lung Neoplasms)

  • 본 연구는 항암화학요법을 받는 폐암환자의 불확실성과 불확실성 영향요인을 파악하기 위한 서술적 상관관계 연구이다. 연구 대상자는 일 대학병원에서 항암화학요법을 받고 있는 폐암환자 111명을 대상으로 하였다. 자료는 2014년 7월 25일부터 12월 31일까지 수집하였다. 수집된 자료는 SPSS Window 18.0 Program을 이용하였고, 빈도와 백분율, 평균과 표준편차, t-test, ANOVA, Scheffe test, median, standard deviations, Pearson's product moment correlation coefficient와 multiple regression analysis로 분석하였다. 연구 결과 항암화학요법을 받는 폐암환자의 불확실성은 평균 평점 2.61(${\pm}0.46$)점으로 다른 질병을 가진 환자의 불확실성 정도보다 높았다. 항암화학요법을 받는 폐암환자의 불확실성과 질병의 심각성, 증상의 일관성, 사건의 친숙성, 의료인에 대한 신뢰, 일상 활동 능력, 자기 효능감, 사회적 지지와의 관계를 분석한 결과 불확실성과 질병의 심각성 간의 상관계수는 .74(<.01), 불확실성과 증상의 일관성 간의 상관관계는 .27(<.01)이었다. 질병에 대한 심각성이 불확실성에 가장 영향을 미치는 변수로 나타났으며, 전체 설명력은 54%이었다. 이상에서 항암화학요법을 받는 폐암환자의 불확실성을 감소시키기 위한 간호중재가 필요할 뿐만 아니라 폐암 환자의 간호중재에는 질병의 심각성을 고려한 간호중재가 이루어져야 할 것이다.

방사선치료를 받는 유방암 환자의 피로, 수면장애, 삶의 질에 대한 연구 (Fatigue, Sleep Disturbance, and Quality of Life among Breast Cancer Patients Receiving Radiotherapy)

  • Purpose: The purpose of this study was to examine fatigue, sleep disturbances, and quality of life (QOL) among patients with breast cancer receiving radiotherapy. Methods: A cross-sectional, descriptive design was used. Data were collected through questionnaires distributed to 201 breast cancer patients in a hospital. The data were analyzed using SPSS 21.0. Results: The fatigue scores showed significant differences depending on exercise and duration since diagnosis. The sleep disturbance scores showed significant differences depending on duration since diagnosis. QOL scores showed significant differences depending on exercise, duration since diagnosis, and treatment site. Fatigue and sleep disturbances (r=.40, p<.001) showed statistically significant positive correlations, while fatigue and QOL (r=-.55, p<.001), and sleep disturbances and QOL (r=-.45, p<.001) showed statistically significant negative correlations. The multiple regression analysis, which was used to determine the variables influencing on QOL after radiotherapy, resulted in a significant regression model (F=23.88, p<.001), which accounted for approximately 45% of the explanatory power. Fatigue (${\beta}=-.39$, p<.001) and sleep disturbances (${\beta}=-.27$, p<.001) were revealed to adversely affect quality of life. Conclusion: The nursing intervention is necessary to reduce fatigue and sleep disturbance and to promote exercise in order to enhance QOL of patients with breast neoplasm while receiving radiotherapy.

건강보험환자와 의료급여환자의 상급종합병원 입원이용 비교 (Utilization Patterns of National Health Insurance and Medical Aid Inpatients in Tertiary Hospitals)

  • The objective of this study is to analyze the utilization patterns of national health insurance and medical aid inpatients in tertiary hospitals. For the analysis, the study utilize the nationwide data on '2010 Survey of Patients' of Ministry of Health & Welfare. The statistical methodology used in the study is the logistic regression model. This study has three major findings. First, utilization rate of national health insurance inpatients was higher than medical aid inpatients in tertiary hospitals. Second, the significant affecting demographic factors in utilizing tertiary hospitals were sex, age, surgery case, treatment result, inpatients residence region and short length of stay. Third, compared to other disease groups, the inpatients on 'congenital malformation, deformity and chromosomal abnormalities', 'factors influencing health status and contact with health services' and 'neoplasm' groups are more likely to utilize tertiary hospitals. Finally, according to residence region, Seoul residence inpatients in both of national health insurance and medical aid more likely to utilize tertiary hospitals than other region inpatients.

Clinicopathological Features and Prognosis of Gastroenteropancreatic Neuroendocrine Tumors: Analysis from a Single-institution

  • Background: The gastroenteropancreatic neuroendocrine neoplasm (GEP-NEN) is the most common type of neuroendocrine neoplasm. We summarized data in our centre to investigate the clinicopathological features, diagnostic methods, therapeutic approaches and prognosis for this neoplasm to increase knowledge of this disease in Asian populations. Method: A total of 122 patients treated at Sun Yet-san Memorial Hospital of Sun Yat-sen University between January 2000 and December 2011 were analyzed retrospectively. Results: Pancreas was the most common site of involvement (65/122, 53.3%); this disease has no special symptoms; positive rates of chromogranin A (CgA) and synaptophysin (Syn) were 81.1% and 87.7%, respectively. The positive rate of Syn had statistical difference among the three grades, but not CgA. Some 68 patients had G1 tumors, 32 G2 tumors and 22 G3 tumors, and Chi-square test showed that higher grading was correlated with worse prognosis (${\chi}^2=32.825$, P=0.0001). A total of 32 patients presented with distant metastasis, and 8 cases emerged during following up. Cox proportional hazards regression modeling showed that the tumor grade (P=0.01), lymphatic metastasis (P=0.025) and distant metastasis (P=0.031) were predictors of unfavorable prognosis. The overall 5-year survival rate was 39.6%, the 5-year survival rate of G1 was 55.7%, and the G2 and G3 were 34.2% and 0%, respectively. Conclusions: The incidence of gastroenteropancreatic neuroendocrine tumors has risen over the last 12 years. All grades of these diseases metastasize readily, and further research regarding the treatment of patients after radical surgery is needed to prolong disease-free survival.

Association Assessment among Risk Factors and Breast Cancer in a Low Income Country: Bangladesh

  • Background: In the low incoming country Bangladesh, breast cancer is second most common neoplasm and is increasing at an alarming rate among females. Lack of awareness and illiteracy are contributory factors for late presentation and therefore mortality. Purpose: To examine associations of different factors with breast cancer mortality and to raise awareness among the women of society in Bangladesh. Materials and Methods: This descriptive case-control study was conducted on 160 participants from April 2011 till July 2014. Through a valid questionnaire covering personal and family history, data were collected by face to face interview. For analyzing correlations among factors with breast cancer data, binary logistic regression, Pearson's ${\chi}^2$-value, odd ratios and p-value tests were conducted with SPSS version 20. Results: The mean age of the patients was 43.0 ($SD={\pm}11.12$). In ascending order the leading significant factors were hormone therapy (p<0.0000, OR=4.897), abortion (p<0.0001, OR=3.452), early start menarche (p<0.0002, OR=3.500), family history (p<0.0022, OR=3.235), and late menopause (p<0.0093, OR=3.674) with both ${\chi}^2$ test and logistic regression analyses. Non-significant factors were cancer experience, fatty food habits, marital status and taking alcohol. Conclusions: Regarding the investigation of this study, significant and insignificant factor's correlation visualization with breast cancer will be helpful to increase awareness among Bangladeshi women as well as all over the world.

Parental Age-Related Risk of Retinoblastoma in Iranian Children

  • Background: Retinoblastoma is a rare malignant intraocular neoplasm. About 90% of cases feature a germline mutation in the RB1 gene and these will develop retinoblastoma during their early childhood. An association between mutations in germline cells and aging has been demonstrated. This suggests a higher incidence of childhood cancer including retinoblastoma among children of older parents. Materials and Methods: In the present study we aimed to determine the association of paternal and maternal age with an increased risk of retinoblastoma in a case-control study in Iranian population. The study was carried out on 240 persons who were born during 1984-2012 in Mahak and Mofid hospitals in Tehran, Iran. The statistical analysis included studying the mean age of parents and in order to know whether parental age of patients is different from parental age of control group, (t-test) compare averages test is used perfectly. By binary logistic regression, odds ratios (ORs) and 95% confidence intervals (CIs) were calculated. Results: The results of statistical analysis including the study of mean parental age by the use of (t-test) compare averages test showed a significant difference between parental ages of patients and controls. Logistic regression showed that coefficients were significant for maternal but not paternal age. Conclusions: Our findings indicate that advanced maternal age can increase the risk of retinoblastoma in offspring, but the paternal age has no significant effect.

Iranian Cancer Patient Perceptions of Prognosis and the Relationship to Hope

  • Background: The aim of this study was to investigate Iranian cancer patient perceptions of their prognosis, factors that influence perceptions of prognosis and the effect this has on patient level of hope. Materials and Methods: Iranian cancer patients (n=200) completed self-report measures of their perceptions of their prognosis and level of hope, in order to assess the relationship between the two and identify factors predictive of perceptions by multiple linear regression analysis. Results: Cancer patients perceived of their prognosis positively (mean 11.4 out of 15), believed their disease to be curable, and reported high levels of hope (mean 40.4 out of 48.0). Multiple linear regression analyses demonstrated that participants who were younger, perceived they had greater family support, and had higher levels of hope reported more positive perceptions of their cancer prognosis. Conclusions: Positive perceptions of prognosis and its positive correlation with hope in Iranian cancer patients highlights the importance of cultural issues in the disclosure of cancer related information.

신경계 증상을 동반한 부분적으로 자연완화된 소세포폐암 (Partial spontaneous remission of small cell lung carcinoma with neurologic symptom)

  • Small cell lung carcinoma (SCLC) is a cancer that shows aggressive behavior, early spread to distant sites, and frequent association with distinct paraneoplastic syndromes. Spontaneous remission of cancer, particularly of SCLC, is a rare biological event. Cases involving spontaneous regression of SCLC were reported, and were associated with paraneoplastic syndromes of the nervous system. This article reports on a 78-year-old man with SCLC in remission, with neurological symptoms. The patient visited the hospital because of generalized weakness, and imaging studies revealed a mass in the lower lobe of the left lung, pathological evaluation showed SCLC. The patient refused oncologic treatment and was treated only with conservative care. In follow-up study the diameter of the mass had decreased from initial 32 mm, 9 months after admission to 20 mm, 17 months after admission to 13 mm. The patient kept complaining of generalized weakness, dizziness, and paresthesia of limbs. We assumed that, in this case, the spontaneous remission of lung cancer was related to the immunologic response directed against the tumor, which is believed to be an important factor in the pathogenesis of paraneoplastic neurologic syndromes.
