장기요양보험 등급판정 대상자의 지역별 방문간호 서비스 이용 만족도 및 요구도: 대도시, 중소도시, 농촌 지역을 중심으로 (Regional Comparison of Home Visiting Care Service Satisfaction among Elder Beneficiaries of the Long Term Care Insurance)

  • Purpose: This study aims at comparing levels of service satisfaction with and needs for visiting nurse(VN) services among their beneficiaries in order to make a plan for extending VN services. Methods: The number of the subjects of this study is 270 in total. They were selected in random sampling in which 30 VN service recipients were randomly selected from each of 7 regions based on their grades and region types. Results: The highest total score for satisfaction with the VN services was 46.30, which the rural residents gave to their received VN services, then the metropolitan residents gave 44.57, and the small and middle size cities' residents 43.64. On the other hand, the highest total score for needs for VN services was 33.03, which the small and middle size cities' residents gave to their received VN services, then the rural residents gave 31.68, and the metropolitan residents 30.88. Conclusion: As the small and middle size cities' residents showed low satisfaction with and high needs for local VN services, policies need to be made to promote service providers' expertise and accessibility.

노인장기요양보험 인정자의 미이용 관련요인 분석: 전남지역을 대상으로 (Factors Associated with the Non-Use of Beneficiaries of Long-Term Care Insurance Service: The Case of Jeollanam-do Province)

  • Background: This study aimed to explore factors associated with the non-use of beneficiaries of long-term care insurance services for the elderly in Jeollanam-do Province by analyzing a dataset obtained from National Health Insurance Service. Methods: The study sample consists of 1,663 individuals who were evaluated as eligible for long-term care insurance services in Jeollanam-do Province during the period of July 1, 2008 through June 30, 2009. As a dependent variable, the non-use of the service was defined as one when a beneficiary had used it once or more times during one year after he or she was evaluated as eligible and as zero otherwise. A proportion analysis was conducted to describe characteristics of study sample. Chi-square tests were used to compare general characteristics between beneficiaries who had used the services and those who had not used them. Multiple logistic regressions were performed by three models including additional sets of explanatory variables such as socio-demographic characteristics, health conditions, and economic status. Results: Main results are summarized as follows. The proportion of beneficiaries who had not used the service was 14.5% of all beneficiaries. According to the results from the model using all explanatory variables, the factors associated with the non-use of the services were residence location, dwelling place, type of desired service, level of care needs, and instrumental activities of daily life limitations. Conclusion: In particular, regarding the type of desired service, the cash benefit showed a high likelihood of the non-use of the service; it had an odds ratio (OR) of 50.212 (95% confidence interval [CI], 24.00-105.04) compared with home service. In case of dwelling place, a hospital showed also a high likelihood of the non-use with an OR of 20.71 (95% CI, 10.12-42.44) compared with home.

대구광역시 노인복지시설 유형별 수요추정 (A Study on the Forecast of Bed Demand ofr Institutional Long-term Care in Taegu, Korea)

  • The purpose of this study was to estimate the forecast of bed demand for institutional long-term care for the elderly persons in Taegu Metropolitan City. The study subject was the total 1,877 elderly persons over age 65 living in Taegu. Among them 1,441 elderly persons were sampled from community and 436 were from the elderly admitted 5 general hospitals. Data collection was carried out by interview from 25 August to 25 December 1997. The measuring instrument of this study was the modified tool of CARE, MAI, PCTC, and ADL which were examined for validity and reliability. In order to forecast bed demand of Nursing Home, this study revised prediction techniques suggested by Robin. The results were as follows : 1. OLDi of Taegu City were 122,202 by the year 1998 and number of Low-Income Elderly Persons were 3,210. 2. The Level I : Senior Citizen Home $ADEMi=\frac{AQi * ASTAYi}{365 * AOCUi}$. AQi = OLDi * LADLi * NASi * ALONi * LIADLi * AUTILi. Predicted number of bed demand for Home Based. Elderly Persons were 4,210 and Low-Income Elderly Persons were 1,081 and Total Elderly Persons were 5,291 by the year 1998, 6,343 by the year 2000 and 8,351 by the 2005. 3. The Level II : Nursing Home $BDEMi=\frac{(BQ1i+BQ2i) * BSTAYi}{365 * BOCUi}$. BQ1i = OLDi * HADLi * ALONi * HIADLi BQ2i = OLDi * HADLi * FAMi * OBEDi Predicted number of demand for Total Elderly Persons were 668 by the year 1998, 802 by the year 2000 and 1,055 by the 2005. 4. The Level III : Nursing Home $CDEMi=\frac{COLDi * HDISi * CUTILi * CSTAYi}{365 * COCUi}+OQi/10$ Predicted number of demand for Total Elderly Persons were 1,899 by the year 1998, 2,311 by the year 2000 and 3,003 by the 2005. 5. Predicted number of bed demand of long-term care facilities in the year 1998 according to Levels were 4.3% among elderly persons in Taegu by Level I, 0.5% by Level II and 1.5% by Level III. Number of elderly persons in current long-term care facilities were 458 in LevelI I,284 in Level II. 6. Deficit number of bed demand of long-term care facilities were 4,833 in Level I, 384 in Level II, 1,899 in Level III for the elderly persons in Taegu Metropolitan City.

장기요양시설 노인의 기능상태에 따른 간호서비스 필요도 예측 (Predictors of Nursing Service Need for Nursing Homes Residents)

  • Purpose: The purposes of this study were to explore the functional status of elderly residents and to analyze time use, and finally identify factors to predict nursing care needs in relation to functional status and health related variables. Methods: In this study a descriptive-correlational design was used. Functional status of participants was obtained through interviews, and nursing care time was examined using a 1 min time-motion study with a standardized instrument developed by Korea Long-Term Care Planning Committee (2005). Results: The mean total functional score was 65 (range 28-125) and mean total nursing care time was 144.15 min per day. There were significant positive relationships between total nursing care time, marital status, back pain, dementia, and vision impairment. Multiple regression analyses showed that a liner combination of number of illnesses, types of primary disease, ADL, IADL, cognitive function, nursing demand, and rehabilitation demand explained 42.8% of variance of total nursing time. ADL (${\beta}$=-.533) was the most significant predictor of nursing service need. Conclusion: Identifying factors that result in variations of service need has implications for adequate nursing service, estimation of optimum nurse to patient ratio, quality of care and patient safety.

A Study on TQM of Health Care Sector

  • The study attempts to review TQM models used in the health care sector and the obstacles to the application of TQM in the sector. Even though the TQM models in the manufacturing and service sectors were successfully applied, the applicability of TQM in the health care sector is still in question. The reason is the unique characteristics of the medical sector such as medical and management practices. The most of the TQM models in the health care sector come from manufacturing industries. The importance of the professional groups is, however, more emphasized in the sector than in manufacturing sector. The role of the groups are idiosyncratic to the sector. They generate some obstacles to the application of TQM in the sector. The barriers include cultural obstacles of health care organization. It naturally follows that the TQM in the health care sector requires the change of the organizational culture of the sector. The culture embraces the norms, rules, regulations, compensation system, morale, practices, and common experiences. To change the culture needs long term effort and modification of the rules, regulations, compensation system, and practices. It also requires staffs' training in the problem solving methods. The TQM in the health care sector needs that the interested parties should change. Since doctors group and nurses group are controlled in the bureaucratic and authoritative manner, they should learn the problem solving techniques which require the interaction with other groups. The management also needs to learn management skills and get thorough training on them.

간호·간병통합서비스 병동 간호사의 우울 예측 요인 (Factors Influencing Depression of Nurses among Comprehensive Nursing Care Service Ward)

  • Purpose: The purpose of this study was to identify depression and its influencing factor of nurses among comprehensive nursing care service ward. Methods: The research was cross-sectional descriptive study. The subjects of the study were 173 nurses of comprehensive nursing care service ward in Gyeonggi-do. Data collection was done using self-reported structured questionnaires asking about job stress, emotional labor, job burnout, somatic symptoms, turnover intention, depression and general characteristics from May 16 to June 3, 2016. Data were analyzed with SPSS/WIN 19.0 program. Results: Mean scores of job stress were 159.15(range 43~215), emotional labor 30.90(range: 9~45), job burnout 59.45 (range: 0~132), somatic symptoms 24.03 (range: 12~60), turnover intention 5.61 (range 0~18) and depression 19.25 (range: 0~60) and reported as depression in 63.6%. Somatic symptoms (${\beta}=.26$, p<.001), job burnout (${\beta}=.37$, p<.001) had significant associations with depression and the most important variable was job burnout. Conclusion: It is necessary to apply the developed program to reduce job burnout in hospitals and to prevent and/or control depression. And also, it needs to improve working conditions and increase the nursing staff to reduce somatic symptoms for nurses among comprehensive nursing care service ward.

가족수발자가 인지하는 표준장기요양이용계획서의 질과 만족도, 활용도 간의 관계분석 (An Analysis of Relationships among Quality, Satisfaction and Utilization Perceived by Family Caregivers in Standard LTC Utilization Plan)

  • 표준장기요양이용계획서는 노인장기요양보험 이용자와 가족의 급여이용을 지원하기 위한 목적으로 도입된 일종의 케어플랜이라고 할 수 있다. 본 연구는 가족수발자가 인지하는 표준장기요양이용계획서의 질과 만족도, 활용도를 파악하고 이들 간의 관계를 파악하는 것을 목적으로 한다. 연구자료는 2008년 11월~12월 현재 노인장기요양보험 1~3등급 판정을 받고 재가장기요양서비스를 이용하고 있는 수급자의 가족수발자를 대상으로 가정방문면접조사를 통해 수집하였고, 총 944명의 가족수발자 가운데 표준장기요양이용계획서를 읽어본 경험이 있으며 설문을 완성한 351명이 최종분석대상이 되었다. 조사도구는 가족수발자와 수급자의 인구사회학적 특성 및 장기요양특성, 질 측정항목(9개), 전반적 만족도 측정항목(1개), 활용도 측정항목(2개) 등을 포함하였고, 질, 만족도 및 활용도 간의 관계를 파악하기 위해 경로분석을 실시하였다. 연구결과, 표준장기요양이용계획서의 질은 장기요양 욕구평가 영역, 장기요양 이용계획 영역, 월 한도액 관리지원 영역 등 세 개 영역으로 구분되었고, 모든 영역이 만족도 및 활용도와 유의한 상관관계를 나타냈다. 다음으로 경로분석 결과, 표준장기요양이용계획서의 장기요양 욕구평가 영역의 질(간접효과, 경로계수=0.077)과 전반적 만족도(전체효과, 경로계수=0.324)가 활용도에 통계적으로 유의한 영향을 미쳤다. 본 연구결과는 표준장기요양이용계획서의 역할을 향상시키기 위한 전략을 개발하는데 도움이 될 것으로 기대된다.

국내 요양병원 종사자의 감염관리 수행도 관련요인 연구: 체계적 고찰 및 메타분석 (Factors related to infection management performance of health workers at Long-Term Care Hospitals in Korea: systemic review and meta analysis)

  • 본 연구는 국내 요양병원 종사자를 대상으로 한 감염관리 수행도 관련요인을 파악하고, 각 요인별 효과크기를 산출하기 위해 실시한 메타분석 연구이다. 대상 문헌은 KMBASE, Research Information Sharing Service (RISS), Korean studies Information Service System(KISS), DataBase Periodical Information Academic(DBpia), National Library of Korea, Pubmed, EMBASE가 이용되었고, R3.5.1을 이용하여 분석하였다. 총 22개의 관련요인 중 사례수가 3개 이상 관련요인을 메타분석 한 결과, 그 효과크기가 성별(.16), 연령(.30), 학력(.32), 병상수(.28), 감염관리 교육필요성(.44), 감염관리 교육경험(.25), 감염관리 인지(지식)(.70)으로 나타났다. 이러한 결과를 바탕으로 요양병원 종사자의 감염관리 수행을 향상시키기 위한 프로그램 개발 및 적용에 근거자료로 활용될 것이다.

델파이 기법을 이용한 우리나라 재가간호서비스 연계방안 (Strategies of Home Health Care Services Linkages in Korea Based on Delphi Technique)

  • 현재 우리나라의 재가간호서비스는 보건소의 맞춤형 방문건강관리사업과 의료기관 가정간호사업, 그리고 노인장기요양보험의 방문간호서비스의 독립성과 전문성을 인정하면서, 각 영역간의 연계를 통해 사업의 중복성을 최소화하는 연계방안이 필요한 상황이다. 이에 본 연구는 재가간호서비스의 연계방안을 도출하기 위해 전문가들의 의견을 바탕으로 합의를 이끌어내는 델파이 기법을 활용하였다. 그 결과 제도적 차원의 연계와 의료적 차원의 연계라는 두 영역 안의 총 24개 항목이 연계방안으로 도출되었다. 재가간호서비스의 연계방안으로 도출된 24개의 항목은 재가간호사업 체계의 개선과 제공되는 서비스 질의 향상, 대상자의 만족도 증대 효과를 가져 올 수 있을 것으로 기대되며, 서비스의 중복을 최소화할 수 있는 방안을 모색함으로서 국가적 차원에서 복지 및 국민 건강관리의 효율성을 증대할 것으로 보인다. 이를 통해 궁극적으로 국민의료비의 절감과 더불어 재가간호서비스 증진에 기여할 것으로 여겨진다.

고령자 대상 의사-의료인간 ICT활용 협진 모델개발 및 실증에 관한 연구: 장기요양서비스 중 재가노인대상 방문간호서비스를 중심으로 (The Development and Effect of Doctor-Visiting Nurses ICT Communication System for the Elderly: Focusing on Visiting Nursing Services for Long-term Care Patients)

  • Purpose: The purpose of this study was to examine effectiveness and usefulness of Information Communication Technology (ICT) in communication between physician and visiting nurses who provide visiting nursing services under long-term care insurance. Methods: Structured questionnaires were used to measure usefulness and satisfaction of the system, both accessibility and convenience to visiting nurses (31 people) and users (182 people). Results: From the user perspective, accessibility and convenience in terms of service users were both satisfactory as shown by users' percentage. No statistically significant difference was found for satisfaction between before and after using the system. The usefulness of the system for visiting nurses was satisfactory for most the nurses. Also most nurses answered that the system is needed and is very useful. Most of the participants (both visiting nurse and service users) were satisfied with use of the ICT system. However, there was no statistically significant difference in satisfaction between the pre and post service because the service provision period was too short (three months). Conclusion: The consensus from both users and service providers is that an ICT based visiting nursing system needs to be introduced but a more user - friendly environment for system development will be needed.
