A Study on the Importance, Performance, and Educational Needs of the Community Pharmacy Practice Education (지역약국 필수 실무실습 교육의 중요도, 수행도 및 교육요구도 분석 연구)

  • Objective: The purpose of the study was to analyze the difference in the importance and performance of the educational content of the community pharmacy practice and find the factors that influence it. Methods: A 5-point Likert scale questionnaire asked about the importance and performance of the pharmacy practice education items to community pharmacy preceptors, current students, and professors. Data were analyzed using Microsoft office 365 Excel and IBM SPSS 21.0. Results: A total of 117 people participated. The averages of importance and performance were 4.34±0.34 and 3.71±0.45, respectively. Except for the computational entry of prescriptions and medication history, the average score for the importance of all educational items was statistically significantly higher than the one for performance. Importance Performance Analysis (IPA) identified over-the-counter medication counseling, over-the-counter drug selection consultation, pharmacy management, adverse drug reaction monitoring, and understanding and implementation of drug use evaluation as the items that needed focused effort. Borich's needs were highest in the adverse drug reaction monitoring. Conclusions: Overall, high importance and low performance indicate a need for improvement in community pharmacy practice education. Since the demand for education was high in over-the-counter medication counseling, adverse drug reaction monitoring, and OTC drug selection consultation, various supports are urgently needed to strengthen pharmacy practice education in this area in the future.

School Dietitians Need Useful Nutrition Counseling Materials (학교영양사가 요구하는 영양교육 교재의 특성)

  • The purpose of this study was to investigate dietitians' needs of teaching materials about nutrition education for school children in Kyonggi and Chungnam(including Daejeon) areas. This study was carried out using questionnaires via e-mail or in class to the subjects of 166 elementary school dietitians as 68 in Kyonggi and 98 in Chungnam. The number of small(less than 700 meals), medium(700-1400 meals) and large(over 1400 meals) schools based on served meals daily were 48, 62 and 56, respectively. The results are as follows: Frequency of nutrition education for students as well as for teachers was samely very low as 'once per two months' and the education used to perform during lunch time mostly. For preparing nutrition education they obtained informations from the internet(64.0%). The main topics of nutrition education they used to covered were 'et's eat breakfast', 'table manners', 'the relations of body and nutrients', 'food waste', 'foods and calorie', etc.. The largest limit of nutrition education practices faced by school dietitians were pointed out as 'lacks of teaching materials' and 'counseling techniques'. Most dietitians wanted CD or substantial teaching materials for the practices, and required their contents covering 'cooking', 'food and calories', 'let’s eat various foods', 'relations of body and nutrients', 'regular meal is important’ and etc.. Since we expect nutrition teachers to be realized soon, nutrition education/counseling is emphasized more as their duty. Even though school dietitians revealed poor self-confidence, they were positive to get improved by reeducation of nutrition counseling methods and skills. If easy and suitable education materials(CD type) are developed as good as the school dietitians desired, the materials could contribute much to better nutrition education/counseling practices at schools.

An Analysis of Health Counseling in One Rheumatic Center Homepage by the Internet (일 병원 류마티스센터 홈페이지의 인터넷 상담내용 분석)

  • Due to the development in computer mediated communication, there are many homepages on the web, which provide medical on health counseling and education. The internet is becoming increasingly important and essential in medicine. But little was known about what problems can be counseled and what difficulties are met during the computer mediated counseling. This study was performed to investigate the content and purpose of health counseling on the web. The data were collected from March 1st, 2002 to February 28th, 2003, from 199 questioners visited one rheumatic center homepage questionary/answering(Q/A) site. Most of the questioners consulted health problems of their own or their families. The 51.3% of them were diagnosed medically Rheumatoid arthritis was the most frequent disease and followed systemic lupus erythematosus, gout and ankylosing spondylitis. Most of the questioners needed to know treatment principles, diagnosing process, and information regarding the clinic. During the one-year period, the average number of users questioning was 29.4 cases and answering was 44.8 cases. Above findings suggest that computer mediated health counseling of rheumatic patients may be useful supplement of health education for them. Communication using internet has become popular, but this kind of approach needs a proper guideline. It is highly recommended that nurses be familiar with the e-health and e-medicine.

Practitioners' Needs Analysis to Improve the Youth Theft Prevention Program (청소년 절도비행 재범방지 프로그램 개선을 위한 실무자 요구조사 연구)

  • This study is aimed to improve the youth theft prevention program with rationality and practicality, based on the needs of practitioners who conduct these programs in fields. The survey questionnaire was prepared based on previous researches, and the survey was conducted on 72 probation officers and education instructors nationwide running a youth theft prevention program. As a result, practitioners reported the lack of self-control and high impulsivity as the main characteristics of the theft delinquent adolescents. And they recognized peer alignment and family problems as the causes of theft. As for the contents of the program, they reported that recidivism prevention skills such as understanding psychological problems, peer relationships, raising a sense of law compliance, anger, impulse control, dealing with negative emotions, and finding alternative behaviors, as well as moral development, are necessary. These results indicate that multi-dimensional interventions such as individuals, families, and peers are needed to prevent recidivism of juvenile theft. Implications of these results, future research directions, and limitations and significance of the study were discussed.

A Target Selection Model for the Counseling Services in Long-Term Care Insurance (노인장기요양보험 이용지원 상담 대상자 선정모형 개발)

  • In the long-term care insurance (LTCI) system, National Health Insurance Service (NHIS) provide counseling services for beneficiaries and their family caregivers, which help them use LTC services appropriately. The purpose of this study was to develop a Target Selection Model for the Counseling Services based on needs of beneficiaries and their family caregivers. To develope models, we used data set of total 2,000 beneficiaries and family caregivers who have used the long-term care services in their home in March 2013 and completed questionnaires. The Target Selection Model was established through various data-mining models such as logistic regression, gradient boosting, Lasso, decision-tree model, Ensemble, and Neural network. Lasso model was selected as the final model because of the stability, high performance and availability. Our results might improve the satisfaction and the efficiency for the NHIS counseling services.

Clinical Nurses' Attitudes towards Termination of Pregnancy, Knowledge of, and Information Needs for, Prenatal Genetic Screening and Diagnosis (임상간호사의 낙태 태도, 산전기형아 검사 관련 지식도 및 정보요구도)

  • Purpose: There has been a considerable increase in the number of women giving birth at advanced age. The genetic screening of such women is highly desirable. Clinical nurses, however, are not adequately trained to assist such clients. This study aims at identifying the educational needs of nurses in order for them to provide better care and treatment for such women. Methods: 206 South Korean clinical nurses participated in this study. Study variables were measured by nurses' attitudes toward terminating pregnancy (ATP), knowledge of prenatal genetic screening and diagnosis (K-PGSD), and information needs for prenatal genetic screening and diagnosis (I-PGSD). The statistical analysis included T-test, analysis of variance and Pearson's Correlation Coefficient. Results: Mean scores were 34.57±5.73 for ATP, 16.44±3.04 for K-PGSD, and 78.81±10.95 for I-PGSD. The findings demonstrate that nurses have high information needs (I-PGSD) to take better care of women who have positive results from their amniocentesis tests. Conclusion: Information needs among clinical nurses are not currently being met. Education for nurses must include training in counseling to encourage patients' autonomous decision-making regarding their pregnancies.

Alcohol Problems and Related Service Needs in Urban Korean Community (경기도 일지역의 문제음주실태와 알코올 미충족욕구 조사연구)

  • Objectives : The purpose of this study is to investigate the characteristics of drinking patterns and the prevalence of hazardous drinking and alcohol use disorder with the assessment of alcohol policy needs in community population of urban city in Korea. Methods : Data was collected from 1,000 residents in a single urban city in Korea, from June 1 to July 31, 2011 by a questionnaire. As research tools, socio-demographic characteristics composed of variables based on study purpose, alcohol problems and alcohol use disorder state scales composed with Alcohol Use Disorder Identification Test(AUDIT) and Cut down, Annoyed, Guilty, Eye-opener(CAGE), and the assessment of alcohol policy needs in community population questions were used. Results : The lifetime experience of alcohol drinking was 97.4%. For the frequency of drinking, 21.9% of population responded 2 to 4 times per weekly. 24.4% of male had one time or more high risk drinking, defined as having four or more drinks in a sitting, per week. 51.6% experienced the first time drinking of alcohol in adolescence. The lifetime prevalence of hazardous drinking and alcohol use disorder were 14.9% and 3.3%, respectively using AUDIT. Alcohol use disorder were 9.6% with the CAGE scale. Problem drinking were 8.4% in this study. They requested the program such as prevention education(57.2%), construction of liaison system to treatment facilities(50.1%), alcohol counseling and intervention(41.7%), and rehabilitation program(39.8%). Conclusions : These results demonstrate that alcohol problem and alcohol use disorder were much prevalent, and these problem started in the earlier age. It is necessary to develop an effective alcohol prevention program or policy with considering the needs in community population.

Smoking Cessation Strategies Targeting Specific Populations

  • Quitting smoking helps smokers maintain their health and extend their lifespan by 10 or more years. Treatment strategies for smoking cessation should be tailored to individual smokers with special needs based on their specific circumstances. It is recommended that pregnant women adopt smoking cessation through counseling and behavioral interventions because the safety of medications has yet to be established. Counseling is the main strategy for smoking cessation in adolescents and nicotine replacement therapy can be used with caution in individuals with serious nicotine dependence. It is important for smokers with psychiatric diseases to quit smoking following accurate assessment of their depression status. Nicotine replacement therapy, varenicline, and bupropion can be used for smoking cessation in smokers with psychiatric disorders. The incidence of cardiovascular disease decreased according to the smoking status and the duration of smoking cessation. In smokers with chronic obstructive pulmonary disease (COPD) who used a combination of counseling and pharmacotherapy the quitting rate was more than twice as high as subjects who used behavioral interventions alone. Varenicline can be used as the most effective anti-smoking drug by most smokers including those with psychiatric disorders, cardiovascular disease, and COPD.

Analysis of Unmet Medical Needs according to Mental Health (정신건강유형에 따른 미충족 의료 현황 분석)

  • Objectives : This study investigated the prevalence of unmet health care needs among Korean adults and related factors. Methods : The study participants were adults over the age of 20 mental health experience from the Korea Health Panel in 2012(n=4,730). Statistical analysis methods used in this study were the ${\chi}^2$-test, Logistic Regression Analysis and other basic statistics such frequency-and percentage using SPSS version 22.0. Results : (1)Significant variables of stress: Factors were age, economic activity, subjective health status, and activity limitation. (2)Significant variables of depression: Factors were age, income class(low) and activity limitation. (3)Significant variables of suicidal impulse: Factors were age, chronic diseases, income class, and activity limitation. Conclusions : Stress, depression, and suicidal impulse can be unmet medical factors; therefore improvement measures and mental health counseling programs in response to suicide impulses, should be developed. Thus there is a need for a health sciences approach.

Analysis of Perceived Job Training and Educational Needs for the Public Health Promotion Program Personnel (보건소 건강증진사업 담당자의 직무교육에 대한 인식 및 교육요구도)

  • Objectives: This study is for understanding the perceived job training of the public health promotion program officials and analysis the educational needs in order to serve as the basis for the development of the future educational programs. Methods: To this end, we have developed a survey based on references and consultations with experts. We had explained the purpose and the intent of the survey to nationwide public health directors in advance and an online questionnaire was conducted for health promotion program and service providing personnel; the results from a total of 763 survey respondents were used in the final analysis. Collected data were analyzed through SPSS Win 21.0. Results: The results shows that the tasks of public health promotion personnel are 'business practices,' 'planning and evaluation related work' and so on in order of importance while the duties of service providing personnel are 'counseling,' 'education' and so on. One of the factors affecting field applications of education is 'awareness of the connection between education and career development' which accounts for 33% of explanatory power; the selected six other factors constitute 41.7%. Necessary educational contents for public officials are 'public health service planning,' 'report writing skills' and 'project evaluation methods.' On the other hand, for service providers, the contents are 'counseling methods,' 'development of educational materials,' 'monitoring health status' and so on. Conclusions: In order for trained service personnel to accord with the changed health promotion environment and the demand of local residents, Public Health Promotion officials should increase educational opportunities based on the competencies for each job, provide continuous learning opportunities and information even after the training, and finally, create a system that can link to career development.
