Fine Needle Aspiration Cytology of Biphasic Synovial Sarcoma - A Case Report - (이중상 활막육종의 세침흡인 세포학적 소견 -1예 보고-)

  • Synovial sarcoma is a rare soft tissue tumor accounting for 5-10% of soft tissue sarcomas. Most arise in the extremities and trunk, and a small proportion occur in the abdominal wall, head and neck legion, and mediastinum. It manifests different phenotypic subtypes that render their cytologic evaluation challenging. Moreover, cytomorphologic description of the epithelial component of synovial sarcoma is rare in Korea. We report a case of biphasic synovial sarcoma on the right lower extremity in a 49-year-old woman diagnosed by fine needle aspiration cytology. The aspirate was moderately cellular and composed of a mixture of tissue fragments and dissociated cells with bland chromatin, inconspicuous nucleoli, and oval to spindle-shaped cytoplasm. Mitosis was rare. A monolayer sheet of epithelial component was seen. The cells in this monolayer sheet had more abundant distinct cytoplasm, round nuclei, and prominent micronucleoli. Histologic examination showed a biphasic pattern consisting of mostly sarcomatous stroma and a few small glandular areas at the periphery of the tumor.

Analysis of Underwater Discharge Characteristics Caused by Impulse Voltages (임펄스전압에 의한 수증방전특성의 분석)

  • This paper describes underwater discharge phenomena and breakdown characteristics in case that the standard lightning impulse voltage is injected to the needle and spherical electrodes installed in the hemisphere water tank. The objective of this work is to understand the basic features related to transient ground impedance against lightning surges. The discharge luminous images were observed and the dependence of breakdown voltage on the polarity of applied voltage and water resistivity were investigated. As a consequence, streamer corona is initiated at the tip of needle and spherical electrodes and is propagated toward grounded tank with stepwise extension. The breakdown voltage characteristics measured as a function of water resistivity showed V-shaped curves. Breakdown voltage and time curve of needle electrode is higher than that of spherical electrode.

Fine Needle Aspiration Cytology of Kimura's Lymphadenitis with Characteristic Warthin-Finkeldey Type Polykaryocytes - A Case Report - (특징적인 Warthin-Finkeldey형의 다유핵 거대세포 소견을 보인 기무라 림프절염의 세침흡인 생검소견 - 1예 보고 -)

  • Kimura's disease is a chronic Inflammatory disorder of unknown etiology, presenting usually as a painless subcutaneous swelling in the head and neck region or in the lymph nodes. We experienced a case of Kimura's lymphadenitis with characteristic Warthin-Finkeldey type polykaryocytes by fine needle aspiration cytology. The patient was a 10-year old male with two enlarged lymph nodes in the postauricular area. Fine needle aspiration cytology from the lymph nodes disclosed hypercellular smears with some scattered eosinophils and polykaryocytes in a polymorphous lymphoid background. There were also fragmented vessel wails and activated endothelial cell clusters in the slightly necrotic background. The Warthin-Finkeldey type polykaryocytes had three to thirty nuclei and prominent nucleoli with cytoplasmic borders. Their nuclei were arranged in grapevine or ring shaped clusters. As these polykaryocytes could also be found in lymph nodes and extranodal tissues of both reactive and neoplastic lymphoid disorders, polykaryocytes themselves are clinically nonspecific. However, the morphologic features of the Warthin-Finkeldey type giant cells are quite different from the foreign body type or Langhans' type giant cells. When the characteristic cytologic features of Kimura's disease such as significant number of eosinophils in a background of lymphoid cells asd proliferation of vessels and endothelial cells are also observed in the smear, it is possible to suggest this diagnosis in the appropriate clinical setting.

Percutaneous Fine Needle Aspiration Cytology of Thymoma (흉선종의 경피 세침흡인 세포학적 검색)

  • The fine needle aspiration (FNA) cytologic findings in 16 cases of histologically confirmed thymoma are reported. The aspirates were obtained under fluoroscopic guidance. The cytologic diagnoses were inadequate sample in one case, thymoma in 12(75%), small cell carcinoma or thymoma in 1, benign mesenchymal tumor in 1, and germ cell tumor in one. The cytologic features were detailed according to the constituent epithelial cell type, and into 4 small of epithelial cells and lymphocytes. Fifteen cases were classified into 4 small epithelial cell type, 6 intermediate epithelial cell type, 1 large epithelial cell type, 1 large pleomorphic epithelial cell type, and 3 spindle-shaped epithelial ceil type. Cytologic differential diagnosis was discussed, and the important criteria for the cytologic diagnosis of thymoma were reviewed. This review leads us to think that nonoperative cytologic approaches in the diagnosis of the thymoma are possible, and that correct cytologic diagnosis of thymoma with FNAs can easily be made, if adequate samples are obtained However the invasiveness and histologic type could not be predicted by cytological features only. Knowing various cytologic and histologic features of thymoma will be helpful for the diagnosis of thymoma and the differential diagnosis of modiastinal tumors.

  • Angiomyolipoma(AML) of the liver is a rare benign tumor; about 50 cases, almost solitary, have been reported. We present here a extremely rare case of multiple AML in the liver diagnosed by fine needle aspiration cytology(FNAC). Two large masses were found in a 51 year-old woman complaining of abdominal discomfort by computed tomography and several smaller masses are scattered in the liver. FNAC was performed, showing bundles of spindle shaped smooth muscle cells intermingled with mature fat cells. FNAC may be a valuable method for definitive diagnosis of hepatic AML. The diagnosis was further confirmed by histologic examination with immunohistochemical studies.

  • This paper describes the dielectric characteristics of $SF_{6}$ gas in a non-uniform electric field under overvoltages. The breakdown voltage-time characteristics and the breakdown voltage-gas pressure characteristics are measured within a gas pressure range extending from 0.1 to 0.5 [Mpa] for the plane electrode system with a needle-shaped protrusion. The curvature radius of the needle protrusion is 0.5[mm]. Also, the growth process of the predischarge are simultaneously observed. As a result, it is found that the breakdown mechanism and predischarge phenomena are closely associated with the polarity and waveforms of the testing voltage.

  • This paper describes dielectric characteristics of SF$_{6}$ gas stressed by the non-oscillating and oscillating impulse voltages in inhomogeneous fields disturbed by metallic protrusion. The breakdown voltage-time (V-t) characteristics and breakdown voltage-gas pressure (V-p) characteristics and their characteristics are statistically investigated with positive and negative very fast transient overvoltages. The experiments were carried out using a needle-to-plane gap geometry in the gas pressure ranges from 0.1 to 0.5 MPa. The gap separation gas 22 mm, and the needle-shaped protrusions were made of stainless steel 10 mm in length and 1.0 mm in diameter.r.

  • Ganglioneuromas are a fully differentiated tumor that contains no immature elements. The majority of sanglioneuromas are diagnosed in patients older than 10 years and are most often located in the posterior mediastinum, followed by the retroperitoneum. The location of these tumors in the parapharyngeal region is extremely uncommon and there are only a few reports on the cytologic appearance of the tumor. We report a case of ganglioneuroma presenting in a parapharyngeal location in a 4 year-old boy, diagnosed by fine needle aspiration cytology. The smears revealed scattered large oval to polygonal cells with voluminous, granular cytoplasms. The nuclei were one to two in number and had a prominent nucleolus. Clusters of benign spindle-shaped cells were also present.

  • This paper presents dielectric characteristics of $SF_6$ gas stressed by the non-oscillating and oscillating impulse voltages in inhomogeneous fields disturbed by metallic protrusion. The propose of this investigation is to present the predischarge development and dielectric characteristics of $SF_6$ gas under VFTOs. The experiments were carried out using a needle-to-plane and two needles-to-plane gap geometries in the gas pressure ranges from 0.1 to 0.5 MPa. The gap separation was 22 mm and the needle-shaped protrusions were made of stainless steel 10 mm in length and 1.0 mm in diameter. As a result, it was found that the breakdown mechanism and predischarge current are principally dependent on applied voltage waveform and polarity.

  • A 33-year-old woman who was diagnosed as medullary carcinoma by fine needle aspiration of thyroid mass is presented. The smear revealed dispersed pattern and small clusters of cells without follicular or papillary structures. The nuclei were round, oval or spindle shaped and eccentrically located. The cytoplasm was abundant and showed red-stained cytoplasmic granulation and tail-like projection with indistinct border. Clumps of amorphous, light-green material were intermingled with tumor cells.
