  • The oriental medical treatment has many strong points when compared to the western medical treatment, but there are still problems when there is no scientific proof given concerning the effectiveness of a certain treatment. Moreover, experiments are being conducted where new medical treatments applied to patients suing both oriental and western techniques. Especially, in oriental acupuncture, there is no objective data on which an oriental medicine doctor can decide the injection time for acupuncture needle by considering a patients body condition, age and the condition of the diseases. From computer simulation results, it has been that the electronic needle is more efficient than the conventional method of acupuncture needle.

Evaluation of an Experimentally Designed Stereotactic Guidance System for Determining Needle Entry Point during Uniplanar Fluoroscopy-guided Intervention

  • Background: In discography performed during percutaneous endoscopic lumbar discectomy (PELD) via the posterolateral approach, it is difficult to create a fluoroscopic tunnel view because a long needle is required for discography and the guide-wire used for consecutive PELD interrupts rotation of fluoroscope. A stereotactic system was designed to facilitate the determination of the needle entry point, and the feasibility of this system was evaluated during interventional spine procedures. Methods: A newly designed stereotactic guidance system underwent a field test application for PELD. Sixty patients who underwent single-level PELD at L4-L5 were randomly divided into conventional or stereotactic groups. PELD was performed via the posterolateral approach using the entry point on the skin determined by premeasured distance from the midline and angles according to preoperative magnetic resonance imaging (MRI) findings. Needle entry accuracy provided by the two groups was determined by comparing the distance and angle measured by postoperative computed tomography with those measured by preoperative MRI. The duration and radiation exposure for determining the entry point were measured in the groups. Results: The new stereotactic guidance system and the conventional method provided similarly accurate entry points for discography and consecutive PELD. However, the new stereotactic guidance system lowered the duration and radiation exposure for determining the entry point. Conclusions: The new stereotactic guidance system under fluoroscopy provided a reliable needle entry point for discography and consecutive PELD. Furthermore, it reduced the duration and radiation exposure associated with determining needle entry.

  • Objectives This paper focused on developing the acupuncture-moxibustion device for head. Disadvantages of the existing devices for head moxibustion are its hume and inconvenience in operating because of the hair. Methods The proposed acupuncture-moxibustion device for GV20(Baihui) is 10mm long as needle body and 6mm long as needle root whose feature are 1~2 turns spiral coil. The spiral coil contains the moxa in order not to drop the moxa while operating Results The direct temperature on the burning moxa were measured by TK-305 infrared thermometer. But the direct temperature on the needle of acupuncture-moxibustion device were not able to measure by TK-305 infrared thermometer. By definition equation, the indirect temperatures on the needle of acupuncture-moxibustion device were able to be calculated. Conclusion A very safe, small and cheap acupuncture-moxibustion device for GV20(Baihui) of head is proposed. It will be useful to generate the heat shock protein and to operate in preventing and curing the Alzheimer's disease.

  • Objectives : Chim-Gu-Kyung-Heom-Bang(CGKHB; 鍼灸經驗方) is the needle and moxa specialized document written by the doctor specialized in needle and moxa treatment of Joseon(朝鮮), Heo Im(1570-1647). The document was published in April 1644(22nd year of King Injo). CGKHB contains the needle and moxa treatment techniques accumulated by the Joseon Dynasty as well as the personal experience of Heo Im. The aim of this study is to restore the past treatment method as a method of Needle Insertion Method at CV12(NIM-CV12, needle to penetrate blood vessel technique) in CGKHB.. Methods : Through Dong-Yi-Bao-Gam(DYBG; 東醫寶鑑), the implication of Korean medicine study of the Jungwan(CV12) has been studied. Next is the contemplation of the NIM-CV12 of CGKHB with the Acupuncture Treatment Using Jungwan(中脘鍼法) of Park Tae-won and Acupuncture Treatment for Byeokjeok(癖積鍼法) of "Geup Yubang". Results and Conclusions : 1. CV12 is one of the acupuncture points representing stomach(脾胃), middle energizer(中焦), phlegm-fluid retention(痰飮) and greater yin(太陰). 2. NIM-CV12 of CGKHB is the technique to penetrate the needle into the CV12 strictly relying on tactile sense of a person who give the penetration. This CV12 administration was carried out at intervals of every 7 or 8 day. During the administration period, the patient was not allowed to intake excessive amount of food. 3. The Acupuncture Treatment Using Jungwan(中脘鍼法) of Park Tae-won and Acupuncture Treatment for Byeokjeok(癖積鍼法) of "GeupYubang", existed in the same era of the NIM-CV12 of CGKHB, have similar linkage to the NIM-CV12 of Heo Im.

  • Burning acupuncture is one of the methods of curing and preventing many kinds of diseases by inserting and taking out of the needle into particular points of the body so called acupuncture points, using a heated Dae-Chim which is one of the nine kinds of needles used in ancient times or by inserting a thick needle after its been heated to certain temperature. Nowadays in China, Burning Acupuncture is used thoroughly from musculoskeletal diseases to, internal medicine, gynecology and many other clinical departments. But in Korea, this method of cure is practiced by only a few clinicians, therefore by studying historical documents, our object is to recognize the importance of the Burning acupuncture and to offer the basis for the scientific researches and clinical application.

Effect of Microcurrent Stimulation at Hand Somatic Acupuncture Points on Experimental Pain Threshold (미세전류가 수부체성경혈점의 실험적 통증 역치에 미치는 영향)

  • The purposes of this study were 1) to examine the effects of microcurrent electrical neuromuscular stimulation 2) to compare surface electrode with needle electrode at somatic acupuncture points on experimental pain threshold measured at the distal end of the radius 3) to determine the changes in effect over time. A total of sixty healthy adult male and female subjects were assigned randomly to one of two experimental group or to a control group. Group 1(n=20) received MENS(Microcurrent Electrical Neuromuscular Stimulation) with surface electrode. Group 2(n=20) received HENS with needle electrode. Group 3(n=20) received no MENS. It measured experimental pain threshold at the wrist on pretreatment, 0 min after treatment. 15 min after treatment, 30 min after treatment in two experimental group. The results were as follows: 1. Experimental pain thresholds were higher in males than females(P<0.01). 2. Only the experimental group exhibited a significant increased in pain threshold after MENS treatment(P<0.05). 3. Surface electrode group increased significant pain threshold 0 min, 15 min after treatment, but greatly decrease 30 min after treatment. 4. Needle electrode group increased significant pain threshold 0 min, 15 min after treatment, but decrease 30 min after treatment. The results suggest that MENS applied to appropriate somatic acupuncture point can increase pain threshold. Further research is needed to assess the effects of greatly variety intensity of MENS of pain sufferes.

Analysis of the Physiological Signal Measured by Oriental Medicine Instruments in the View of the Rule of Promoting and Counteracting Relation of the Five Evolutive Phases (한방기기(韓方機器)로 측정(測定)한 생리신호(生理信號)의 오행(五行) 생극제화규율적(生剋制化規律的) 해석(解析))

  • The physiological signals measured by Oriental Medicine instruments have been analyzed quantitatively in the view of the rule of promoting and counteracting relation of five evolutive phases theory. We tried to reduce the physiological signals measured by EAV(Elec-tro-Acupuncture according to Voll) and IR thermography to the representation of five evelutive phases. The EAV index and local skin temperature on acupuncture points of each phases measured and normalized so that the total value of five phases became unity. We assumed that the normalized EAV index and local skin temperature mean the deficiency or excess of Qi for each phases. The state of Qi distribution for each phases were approximately agree with the diagnostic pattern of O. M. doctor. Taking account of the Qi distribution state of·or the five evolutive phases, we performed a proper needle insertion on acupuncture points to induce the distinct change of Qi for each phases. We compared the measured results with the predictions of Qi variation by the rule of pro- moting and counteracting relation over the five evolutive phases. For all cases, the variation of Qi in the own phase on which a needle insertion was performed were exactly same to the theoretical prediction and partial agreement was shown for the other four phases. The same analysis was carried to the results of skin temperature measurements at accupoints. We found that the local skin temperature at accupoints of each phases shelved a finite change by the needle insertion and the behavior- of its change were strongly correlated to the rule of promoting and counteracting relation of five evolutive phases.

Documentary comparative study on the chief virtues of the Back-Su(兪) points and the Front-Mo(募) points (배유혈(背兪穴)과 복유혈(腹募穴)의 주치증(主治症)에 관한 문헌적(文獻的) 비교(比較) 연구(硏究))

  • From documentary comparative study on the chief virtues of the Back-Su(兪) points and the Front-Mo(募) points, the following results are obtained : 1. Contrast the Back-Su(兪) points with the Front-Mo(募) points, the needle-steadying depth of the Front-Mo(募) points is deeper than that of the Back-Su(兪) points. and Moxibustion dosage of the Front-Mo(募) points is more than that of the Back-Su(兪) points. 2. In the case of Kimun(LR14) and Kansu(BL18), Kergwol(CV12) and Shimsu(BL15), Chungwan(CV12) and Wisu(BL21), Kyungmun(GB25) and Shinsu(BL23), the Back-Su(兪) points can be chiefly used for the treatment of acute disease, external disease, exess disease and Jang(臟)-disease. contrary the Front-Mo(募) points can be chiefly used for the treatment of chronic disease, internal disease, deficiency disease and Bu(腑)-disease. 3. In the case of Kwangwon(CV4) and Sojangsu(BL27), Serkmoon(CV5) and Samchosu(BL22), Changmun(LR13) and Bisu(BL20), Chungbu(LV1) and Pesu(BL20), Chungguk(CV3) and Banggwangsu(BL28), the Front-Mo(募) points can be used for the treatment of unbalanced visceral funtion. contrary the Back-Su(兪) points can be used for not only the treatment of unbalanced visceral funtion but also the treatment of attached organs. 4. The Back-Su(兪) points can be used for the treatment of acute disease, external disease, exess disease, Jang(臟)-disease and attached organs-disease. contrary the Front-Mo(募) points can be chiefly used for the treatment of chronic disease, internal disease, deficiency disease, and Bu(腑)-disease.

  • Background : Eye diseases refer to a wide range of disconveniences from conjunctivitis, pterygium, glaucoma to even blindness. Acupuncture has been widely used in Korea throughout the history and provides an efficient method in the treatment of them. Objectives : Establish a distinctive and efficient acupuncture method for the treatment of eye diseases based in literature research. Method : We reviewed four Korean medical literature, "Guide to Swollen Sore Treatment[治腫指南]", "Treasured Mirror of Eastern Medicine[東醫寶鑑]", "Experiential Prescriptions of Acupuncture and Moxibustion[鍼灸經驗方]", and "Essential Rhymes on Acupuncture and Moxibustion by Master Saam[舍岩鍼法]", and analyzed the therapeutic characteristics in the treatment of eye diseases. Result : 1. According to "Guide to Swollen Sore Treatment[治腫指南]", various methods were applied in the treatment of eye diseases. We can cite salt water washing method after needling, pricking bloodletting method using three-edged needle, surgery method using bent needle and lance needle, or sore treatment using sliced bean-curd and ground Aristolochiae Fructus among others. Acupuncture points like GV20[百會], BL1[睛明], EX-HN5[太陽], GB20[風池], GV24[神庭], GB1[瞳子髎], and GB15[臨泣] were mostly needled. 2. In "Treasured Mirror of Eastern Medicine[東醫寶鑑]", pricking bloodletting method were most frequently used in comparison to single acupuncture or moxibustion methods. Applied points were GV20[百會], BL1[睛明], LI4[合谷], EX-HN5[太陽], GB37[光明], BL18[肝兪], GB20[風池], BL2[攢竹], GB1[瞳子髎], and ST36[三里]. Also selections of adjacent points were considered important. 3. In respect to treatment methods "Experiential Prescriptions of Acupuncture and Moxibustion [鍼灸經驗方]" has some similarity to "Treasured Mirror of Eastern Medicine[東醫寶鑑]" as pricking bloodletting method were mostly used. Also focused on normal Qi flow through meridian. Points like BL18[肝兪], BL1[睛明], LU5[尺澤], EX-HN5[太陽], LI4[合谷] were used. 4. "Essential Rhymes on Acupuncture and Moxibustion by Master Saam[校勘舍岩道人鍼法]" considered visceral pattern identification method fundamental in the diagnosis and treatment of eye diseases. Specifically, Liver, Heart, Stomach, Lung, Kidney identification methods are presented. Combined both corresponding and connecting meridians supplementation and draining methods according to mother-child relation. Also Saam master's own experiential prescriptions are noted. Conclusions : After previous study on stroke, we could also find various efficient methods according to eye diseases, through literature research of korean medical classics. This study will concurrently result in establishing distinctive therapeutic method characteristic of Korea.

Effects of Acupuncture at the Sea Point on the Changes of Plasma and Tissue Levels of NO, nNOS, Norepinephrine in Rats (12경맥(經脈)의 오수혈중(五輸穴中) 합혈(合穴) 침자(鍼刺)가 백서(白鼠)의 혈위(穴位) 조직내 NO, nNOS와 조직 및 혈장 Norepinephrine의 변화에 미치는 영향(影響))

  • Objectives : This study is to observe the changes in the expression of neurotransmitters, such as NO, nNOS, and NE, upon the needle insertion to the sea points, which is one of the five transport points. Methods : Needles were inserted into rats, on both left and right sides of all sea points, including the LU5, PC3, HT3, LI11, TE10, SI8, SP9, LR8, KI10, ST36, GB34, and BL40, which are the sea points of five transport points for 12 meridian vessels. After insertion, needles were retained for five minutes. After the retention, blood was drawn via cardiac puncture, and tissues of each point near meridian vessels were extracted to examine the changes in the expression of NO, nNOS and NE. Results : In terms of the effect in NO production, there was a significant decrease only in the LU5 point, whereas there was a significant increase in the TE10 point alone. In terms of the expression of nNOS within tissues, none of the experimental groups showed significant changes based on the results of immunohistochemistry and western blotting. Regarding the formation of norepinephrine within tissues, the HT3, SP9, and KI10 point showed a significant decrease, while the PC3 and LR8 point showed a significant increase. Production of plasma norepinephrine was significantly increased at the TE10, SP9, LR8, GB34, and BL40 point. Conclusions : The effect of needles applied at the sea points of five transport points of 12 meridian vessels on the functions of NO, nNOS, and NE could be observed, and it is considered that the effect of needle stimulation on nervous system disorders could be studied through additional researches based on this one.
