수삼양경 락혈의 깊이별 침자가 백서의 nNOS, NO 및 Norepinephrine의 변화에 미치는 영향 (Effects of Acupuncture at Varying Depths at the Connecting Point on the Changes of Levels of nNOS, No and Norepinephrine in Rats)

  • Objectives : This study was intended to observe the changes in the expression of neurotransmitters, such as nNOS, NO and NE upon the needle insertion at varying depths at the connecting point. Methods : Needles were inserted into rats, on both left and right sides of the connecting point, including the LI6, SI7 and TE5 acupoints which are three yang meridians of the hand. After insertion, needles were retained for five minutes. Each acupuncture groups were treated acupuncture at each acupoint and at the depths of superficial, middle and deep layer. After the retention, blood was drawn via cardiac puncture, and tissues of each point near meridian vessels were extracted to examine the changes in the expression of nNOS, NO and NE. Results : In terms of the effect in nNO production, there was a significant increase only in the middle and deep layer at SI7 acupoint, but there was no significant change in the expression of NO. Regarding the formation of norepinephrine within tissues, the middle layer on LI6 acupoint, the middle layer and the deep layer on TE5 acupoint showed a significant increase, while production of plasma norepinephrine was significantly decreased at the middle layer and the deep layer on LI6 acupoint and the deep layer on SI7 acupoint. Conclusions : The effect of needles applied at the connecting point of three yang meridians on the activities of nNOS, and NE could be observed, and it can be induced that the effect of needle stimulation on disrupted nervous system can be examined through additional researches based on this one.

-침술효과 객관화를 위한 연구- 접지와 절연조건이 경락전위에 미치는 영향 (Influence on Meridians Electric Potential for Electrical Ground and Insulation)

  • When a patient is positioned at insulated bed and practitioner is positioned at insulated floor, that condition could be a cause of lessening effect in acupuncture practice. We investigated how Ground connection and Insulation could influence on the electrical meridian potential between practitioner and patient during acupuncture practice. We treated 30 normal healthy subjects with acupuncture and measured changes in the electrical potential between the stomach meridian points ST-39 and ST-37 in response to light touch after insertion of a needle at ST-36. At first, we stimulated non-insulated needle and measured electrical potentials for non ground, patient ground only, practitioner ground only, all ground respectively. Then we used insulated needles and measured same as above. Non-insulated all ground subject elicited positive mean potential $44.6{\pm}19.2{\mu}V$ and showed $181.4{\pm}59.7{\mu}V$ peak to peak potential. Practitioner ground only showed negative mean potential of $51.5{\pm}9.3{\mu}V$ and $367.4{\pm}27.8{\mu}V$ of peak to peak potential. Patient ground only revealed no mean potential of $2.9{\pm}1.3{\mu}V$, $16.4{\pm}11.9{\mu}V$ of peak to peak potential. All ground showed no mean potential of $1.6{\pm}0.7{\mu}V$, $3.3{\pm}1.9{\mu}V$ of peak to peak potential. In case of Insulated condition, there elicited no mean potential. Non ground and Practitioner only showed weak value of peak to peak potential as $7.8{\pm}2.6{\mu}V$ and $8.4{\pm}2.5{\mu}V$ each. But patient only and all ground showed no potential as $4.7{\pm}2.2{\mu}V$, $3.4{\pm}2.2{\mu}V$, respectively. Therefore, Ground connection modulate the energy transfer course between practitioner and patient. Insulation block energy transfer but static electrical impulse between practitioner and patient.

미녹시딜을 내포한 용융형 고분자 마이크로 니들의 길이에 따른 특성 및 발모 효능 관찰 (Characteristics and Hair Growth Efficacy of Water Dissolving Micro-needles Containing Minoxidil Regarding Length of Micro-needles)

  • 본 실험에선 미녹시딜을 함유한 수용성 고분자 젤로부터 solvent casting 방법으로 250과 $750{\mu}m$ 두 가지 길이의 미녹시딜이 함유된 용융형 고분자 마이크로 니들(micro-needle)을 제작하였다. 제작된 미녹시딜을 함유한 용융형 고분자 마이크로 니들은 기계적 강도가 입증된 고분자 니들과 동일한 형태 및 기계적 강도를 나타냈으며, 성공적으로 피부를 투과함을 확인하였다. 제조된 고분자 마이크로 니들의 경우 피부 적용 후 최소 30분 이상은 부착되어야 전체 니들의 50% (v/v) 이상이 피부 내로 용융되었으며, 니들 적용부위에서 서서히 약물의 방출 및 확산을 확인할 수 있었다. 또한 C57BL/6 동물 모델을 통해 미녹시딜이 함유된 250, $750{\mu}m$ 길이의 고분자 마이크로 니들이 무처리군 혹은 미녹시딜 처리군에 비해 발모에 긍정적인 영향을 나타냄을 확인할 수 있었다.

무작위 대조 시험을 통한 족관절 염좌의 자침 중 심자와 천자의 비교 연구-준비 조사 (Comparison of Superficial and Deep Acupuncture in the Treatment of Ankle sprain : A Randomized Controlled Trial-Pilot study)

  • Objective : The aim of the study was to compare the therapeutic effect of the superficial and in-depth insertion of acupuncture needles in the treatment of patients with ankle sprain. Design : A prospective randomized single-blind study of superficial and deep acupuncture was conducted. Setting : The study was conducted in the Kang-nam Oriental Hosp. of Dong-Guk University. Patients: The study comprised 14 patients with ankle sprain who were divided into two groups (A and B). Intervention : In group A, the needle was introduced in the skin at a depth of 2 mm, whereas in group B the needle was placed deeply into muscular tissue or the articular capsule at a depth more than 1.5 Cm The treatment was planned for a duration of 1 week, 3 times. Outcome Measures : The intensity of pain was evaluated with the Ankle grade pain chart(AGPC) before and after treatment and at the 1-week follow-up examination. The AGPC includes Swelling, Tenderness, Exudation, Bruise, Medial/Lateral. stability test, Anterior drawer test, Squeeze test, Bearing weight, Pressure Algometer, Walking state and VAS. Results : Although at the end of the treatment there was no evidence of significant statistical differences between the two different groups, Pain reduction(VAS) was greater in the group treated with deep acupuncture. A statistical difference existed between the two groups at the 1 week follow up, with a better result in the deeply stimulated group in VAS. Conclusion : Clinical results show that deep stimulation has a better analgesic effect when compared with superficial stimulation in pain reduction(VAS, Visual analogic scale).

수술후 오심구토에 대한 이침요법의 임상적 연구 (Effect of Auricular acupuncture on Postoperative Nausea and Vomiting)

  • 본 논문은 수술후 오심, 구토증에 대한 이침요법의 효과를 연구하기 위하여 시도되었다. 경희대학교 의과대학 부속병원에서 전신마취하에 복식 자궁절제술을 받기로 예정된 100명의 환자를 두 군으로 나누어 이침요법을 하지 않은 대조군과 이침요법을 시행한 실험군을 대상으로 연구를 하였다. 대상환자의 연령, 체중, 신장, 마취시간은 두 군간에 통계적으로 유의성있는 차이는 없었다. 수술후 12시간이내에 한번이라도 오심, 구역 및 구토가 발생된 환자는 대조군이 68%, 실험군이 30%으로 실험군이 대조군에 비하여 유의성있는 효과를 보였다. 이침요법에 의한 부작용은 나타나지 않았다. 이침요법은 복식 자궁절제술 후 오심, 구토를 부작용없이 예방하는데 상당한 효과가 있는 것으로 사려된다.

Comparative evaluation of pain perception following topical application of clove oil, betel leaf extract, lignocaine gel, and ice prior to intraoral injection in children aged 6-10 years: a randomized control study

  • Background: In the field of dentistry, topical anesthetics play an important role in reducing pain during needle pricks. The anesthetic property of betel leaves remain unexplored, even though they have been widely used for the treatment of various ailments. The purpose of this study was to compare and evaluate pain perception following topical application of lignocaine gel, clove gel, ice, and newly developed betel leaf extract gel during intraoral injection in children. Methods: Sixty children aged 6-10 years who met the inclusion criteria were divided into four groups. Topical anesthetic agents, 2% lignocaine (Lox-2% Jelly, Neon, Mumbai, India), 4.7% clove gel (Pain Out Dental Gel, Colgate Palmolive India Ltd, Solan, India), 10% betel leaf extract gel, and ice were applied to each group for one min, followed by administration of infiltration anesthesia. Pain perception was analyzed during needle insertion. The Wong Bakers FACES pain rating scale (WBFPRS) was used for subjective assessment and the Sound, Eye, Motor (SEM) scale for objective assessment. Recorded values were tabulated and subjected to appropriate statistical analysis using SPSS software with a P value set at 0.05. Results: The clove oil and betel leaf groups demonstrated the highest WBFPRS scores, followed by the ice and lignocaine groups. The clove, betel leaf extract, and ice groups showed equal and highest SEM scores, followed by the lignocaine group. The mean WBFPRS and mean SEM scores were statistically significant. Conclusion: Betel leaf extract gel is effective in reducing pain and can act as an alternative topical anesthetic agent.

  • Objectives : A previous study demonstrated that the connecting points of three yang meridians attenuated changes of nNOS, and Norepinephrine(NE) in rats. The current study investigated the changes in nNOS, NO and NE upon the needle insertion at varying depths at the connecting point of three yin meridians of the hand. Methods : Needles were inserted into rats, on both left and right sides of the connecting point, including the LU7, HT5 and PC6 acupoints which are three yin meridians of the hand. After insertion, needles were retained for five minutes. Each acupuncture groups were treated acupuncture at each acupoint and at the depths of superficial, middle and deep layer. After the retention, blood was drawn via cardiac puncture, and tissues of each point near meridian vessel was extracted to examine the changes in the expression of nNOS, NO and NE. Results : Compared with the superficial layer group, nNOS production significantly decreased in the middle and deep layer at LU7 acupoint group and in the deep layer at HT5, PC6 acupoint group. The tissue NE decreased in the deep layer on PC6 acupoint and the plasma NE increased at the middle layer at LU7 acupoint group but decreased at the deep layer on at LU7 acupoint group. Conclusions : Acupuncture at connecting points of three yin meridians of the hand can regulate the activities of nNOS, and NE.

자침시(刺鍼時) 탄침(彈鍼)의 수기자극(手技刺戟)이 전위변화(電位變化)에 미치는 영향(影響) (A Study on the changes of electric change induced by Tanchim (彈鍼) manipulation during acupuncture therapy)

  • Objectives : To understanding biological mechanism of acupuncture therapy, we have proposed a hypothesis. First of all, there exists electric property in meridian and meridian point. Second of all, energy flowing in meridian is related with electric property. Third of all, there is electronic interaction between the operator who performs acupuncture therapy and the receiver who is given acupuncture therapy. Forth of all, acupuncture effects may depend on the electric capacity which is transferred between the operator and the receiver via acupuncture needle. Methods : Under the hypothesis, we studied the effects of electric change in $ST_{37}(+)\;and\;ST_{39}(-)$ generated by Tanchim (彈鍼) manipulation which was stimulated at $ST_{36}$ point. And compared with data on the changes of electric change from the hand of the operator during acupuncture stimulation. Electric charge induced via acupuncture needle from the operator may be important factor that causes the changes of electric change in meridian and acupoint in the receiver. Therefore we investigated the changes of electric charge induced by the operator using Maclab 400 by the following methods. The one was in stable electric circle condition and the other was in unstable electric circle condition. Results : In this experiments, the changes of electric change from the stimulation type of Tanchim manipulation performed in our lab condition in acupuncture therapy was induced at least three factor, one was the difference of bio-potentials between the operator and the receiver of acupuncture therapy, another was the depth of acupuncture insertion from the skin of the receiver, the other was an electromyogram of the receiver. Conclusion : The data imply that the first factor should make a capacitance current when the operator touched the acupuncture needle which was inserted in the receiver. Therefore, the results suggest that capacitance currents stimulus in electronic view may be important factor in acupuncture therapy between the operator and the receiver of acupuncture therapy.

A Pilot Clinical Study on the Accuracy and Safety of Ultrasound-guided Gyeontonghyeol (BP-LE6) Acupuncture: A Prospective Randomized, Single Blinded Crossover Study

  • Background: The purpose of this study was to assess the accuracy and safety of procedures using ultrasound equipment for acupuncture treatment. Methods: A pilot, prospective randomized, single blinded, crossover clinical study on the accuracy and safety of ultrasound-guided Gyeontonghyeol (BP-LE6) acupuncture treatment was conducted. Patients (n = 13) with shoulder pain were randomly divided into 2 groups. During Visit 1, ultrasound-guided BP-LE6 acupuncture was administered to the experimental group. In the control group, patients received BP-LE6 acupuncture (without checking ultrasound images) by manipulating the ultrasound probe as if administering ultrasound-guided acupuncture. Visit 2 was arranged within 7-14 days and the remaining procedures, other than those administered in Visit 1, were performed. In both the experimental group and control group, the number of needle insertions, and time required for the treatment to result in the patients feeling de-qi was recorded. The numeric rating scale (NRS) score for shoulder pain was recorded before and after the acupuncture treatment. Results: The number of needle insertions was $5.31{\pm}3.50$ times in the experimental group, and $6.62{\pm}3.38$ times in the control group, however, there was no statistically significant difference between the groups (p > 0.05). The mean time required to perform the procedure was $151.54{\pm}48.59$ seconds in the experimental group and $86.69{\pm}37.17$ seconds in the control group, which was statistically significantly different (p < 0.05). The changes observed in numerical rating scale scores between groups were not statistically significantly different. Conclusion: Although there was no statistically significant difference, administering acupuncture using ultrasound guidance may lead to accurate needling with a reduced number of needle insertion attempts. A large-scale clinical study of better design should be conducted in the future.
