흉벽 외측을 타고 내려온 비전형성 하행 경부 괴사성 근막염 1예 (Atypical Type of Descending Necrotizing Fasciitis Spreading into the Lateral Chest Wall: Case Report)

  • Cervical necrotizing fasciitis is a rare but well known life-threatening soft tissue infection primarily affecting the superficial and deep cervial fascial planes. It may rapidly spread into the mediastinum along fascial planes. So, if proper diagnosis and prompt surgical debridement is delayed, the infection may cause descending necrotizing mediastinitis with extensive necrosis of overlying skin, destroying fascia and muscle. Therefore, the key to the successful outcome is early recognition and rapid initiation of definitive radical debridement. The authors report, along with a literature review, a case of cervical necrotizing fasciitis with atypical spreading into the lateral chest wall.

  • 괴사성 임프절염(기구찌 병)은 원인이 알려지지 않은 새로운 질병으로서 주로 젊은 여자의 경부 임프절을 침범하여 임프절 비대를 일으키며, 대부분 수개월 내에 자연 소실되는 양호한 임상 경과를 보인다. 임상적으로 국소 임프절 비대가 특징적이며 때로는 압통, 발열, 체중 감소, 백혈구 수의 감소 또는 적혈구 침강 속도의 증가를 보이며 악성 임프종 및 결핵과의 감별을 요한다. 특징적인 조직 병리학적소견은 임프절의 괴사, 세포핵 붕괴 등을 보이고 형질세포의 침윤은 없다. 저자들은 1990년 3월부터 1993년 1월까지 괴사성 임프절염으로 확진된 19례를 검토한 후 본 질환의 보다 정확한 진단과 적절한 치료를 위하여 보고하는 바이다.

  • Mandible angle ostectomy is the most widely-operated facial contour surgery. We experienced a rare case of necrotizing fasciitis of the lower leg following mandible angle ostectomy. A 20 years old female visited our department 5 days after mandible angle ostectomy. At the time, she was diagnosed as sepsis and on her 2nd admission day, she was diagnosed as necrotizing fasciitis on her left leg. She was treated by fasciotomy and necrotized tissue removal. Necrotizing fasciitis is a rare complication of any kinds of operation but once it develops, it causes fatal results, especially to cosmetic surgeries. Early diagnosis and aggressive operation is known as only treatment for the disease.

  • Necrotizing sialometaplasia is a rare, benign, self-limiting inflammatory process, with most cases occurring in the palatal region. Although the pathophysiology of this condition remains unknown, it has generally been considered an ischemic reaction involving the minor salivary glands. Due to its clinical and histologic similarities to malignant salivary gland tumor, diagnostic biopsy is required. Careful monitoring is also recommended, and clinicians should provide sufficient information for pathologists to ensure the histologic diagnosis.

액와에 발생한 아급성 괴사성 림프선염 1례 (A Case of Subacute Necrotizing Lymphadenitis in Axillary Area)

  • Since 1972, a unique lymphadenitis called as subacute necrotizing lymphadenitis has been described in many Japanese literature. Originally described in Japan, it now appears worldwide. It mainly affects young women and usually manifests as fever and lymphdenopathy. The pathology is characterized by necrosis with loss of nodal architecture, infiltration with many histiocytes, and an absence of granulocytes. We report a case of subacute necrotizing lymphadenitis in axillary area. 4 10-year-old male child was admitted with a one-month history of fever, swelling in the axillary area. He treated with intravenous antibiotics. Despite treatment he remained febrile for the next ten days. 4 dissection of the axillary swelling was then performed, as was a diagnostic lymph node biopsy. The patient's condition resolved over several days and he was discharged.

Pug dog에서 발생한 괴사성 뇌막뇌염 증례보고 (Necrotizing meningoencephalitis in a pug dog in Korea)

  • A case of necrotizing meningoencephalitis was described in a 3-year-old female pug dog. Significant histopathological changes were confined to the brain. Histopathologically, brain lesion of the pug dog was characterized by (1) severe lymphocyte, plasma cell and macrophage infiltration in the meninges and gray and white matter of the cerebral hemisphere, (2) perivascular mononuclear cell cuffing and (3) focal polioencephalomalacia in the gray matter of cerebrum. Replicate sections of the brain and lung were negative for fungal infection, canine distemper and toxoplasmosis on PAS stain and immunohistochemistry, respectively. Based on clinical signs, histopathological and immunohistochemical studies, this case was diagnosed as necrotizing meningoencephalitis in pug dog that had not been preriously described in Korea.

  • Necrotizing lymphadenitis is a peculiar reactive condition with a predilection for cervical lymph nodes commonly in young women. It is characterized by persistent, painless cervical adenopathy with or without fever. Although the histologic features may be confused with those of malignant lymphoma, to our knowledge the natural history has been benign in all cases to date. The excised lymph nodes were moderately enlarged and typically showed focal, well-circumscribed, paracortical, necrotizing lesions, and abundant karyorrhectic debris, scattered fibrin deposits, aggregates of large mononuclear cells, and a paucity of plasma cells and neutrophils. Recently we experienced two female cases of necrotizing lymphadenitis and report with a review of literature.

하행 괴사성 종격동염 (Descending Necrotizing Mediastinitis)

  • A 44-year-old man was admitted to our hospital because of high fever, neck swelling, and dyspnea. He was diagnosed as descending necrotizing mediastinitis (DNM) extended from oropharyngeal infection. Descending necrotizing mediastinitis(DNM) is a lethal process originating from odontogenic. oropharyngeal, or cervical infections that descends along the fascial planes into the mediastinum. DNM is relatively rare, but it is lethal disease with high mortality. Immediate and sufficient mediastinal drainage is indispensable for the disease. The principles of the surgical approach for DNM are discussed, with a brief review of the literature following these case reports.

  • Statin is commonly used for lowering cholesterols and can be myotoxic to cause drug-induced necrotizing myopathy. Statin-induced myopathy ranges from asymptomatic hyperCKemia to lethal rhabdomyolysis but is usually reversed by withdrawal of causative drugs. The patient in this study presented with statin-induced necrotizing myopathy, which was finally improved with immunosuppressive therapy, but not just with drug withdrawal. Since statin can induce myopathy through autoimmune processes, we should consider using immunomodulating agents in cases with statin-induced myopathy, which is refractory to drug withdrawal.
