A Case of Epidermal Cyst Occurred in the Bony External Auditory Canal Misdiagnosed as External Auditory Canal Carcinoma (외이도 암으로 오인된 외이도 골부에 발생한 표피 낭종 1예)

  • Epidermal cysts are generally benign tumors that usually originate from the skin caused by inflammation of hair cortex and proliferation of epidermal cells within the dermis; however, for these cysts to occur in the bony external auditory canal (EAC) is rare. They are often present as a solitary, painless lesion and usually asymptomatic and the diagnosis depends on the results of the histological examination. In treatment, the cyst wall must be completely removed surgically. We recently encountered a 82-year-old male with a mass in the right EAC. An otoscopic examination showed a polypoid mass on the bony EAC, which was finally diagnosed as epidermal cyst after an initial misdiagnosis as EAC carcinoma. We report the rare, unique case with literature review.

Simultaneous Occurrence of Papillary and Follicular Thyroid Cancer - A Case Report - (서로 다른 분엽에 동시에 발생한 유두성과 여포성 갑상선암 -증례보고-)

  • We present a case of a mixed papillary and follicular thyroid cancer in a 45-year-old female presented with palpable mass on anterior neck area for 1 week ago. Neck CT and ultrasonogram revealed small solid masses in the both lobes of the thyroid gland. Thyroid scintigraphy presents as a cold nodule in the right lobe of the thyroid gland and FNA cytology demonstrated papillary thyroid cancer. At the time of operation, small sized solid masses were detected in the both lobes, and no cervical lymph nodes enlargement along the mass. Biopsies of the both mass demonstrated papillary cancer on right lobe and follicular cancer on left lobe. Simultaneous papillary and follicular thyroid cancer is an extremely rare clinical entity. We experienced a case of simultaneous papillary and follicular thyroid cancer, so we report it with a review of some articles.

Case Report: Intracapsular Carcinoma Ex Pleomorphic Adenoma of Parotid Gland (증례보고: 이하선에 발생한 다형선종 유래 피막내 암종 1예)

  • Among a variety of malignant types for parotid gland tumors, intracapsular carcinoma ex pleomorphic adenoma which is classified as a non-invasive tumor has been reported rarely. We report a case of a 69-years old patient, who presented with a left parotid mass that was detected 30 years ago. Fine needle aspiration biopsy result of the mass was "suggestive of pleomorphic adenoma". Superficial partial parotidectomy was performed for the mass and the permanent pathologic finding was "intracapsular carcinoma ex pleomorphic adenoma" which was a salivary ductal carcinoma with well-preserved myoepithelial cells surrounding the malignant epithelial cell clusters. Surgical resection is the main treatment modality for the treatment of intracapsular carcinoma ex pleomorphic adenoma. Herein, we present the case with a review of literature.

  • Liposarcoma is the second most common soft tissue sarcoma in adult. The incidence of liposarcoma in the head and neck area is very uncommon. Liposarcoma of the neck usually express non-tender palpable mass. The main treatment of cervical liposarcoma is surgical excision and adjuvant radiation therapy. Herein, we report a case of cervical liposarcoma with the review of literature.

  • Liposarcoma is a malignant tumor originated from adipose tissue, accounting for 20% of all soft tissue sarcomas. A 69-year-old man with anterior and posterior neck mass visited to our department and we performed an excisional biopsy in level I. Histopathological examination revealed liposarcoma, so we performed wide excision in anterior and posterior neck, and adjuvant radiotherapy. We report a unique case of well-differentiated liposarcoma on anterior and posterior neck with literature review. To our best knowledge, such case has not been reported earlier in Korea previously.

  • Progressive transformation of germinal centers(PTGC) is a rare benign lymph node enlargement characterized by chronic lymphadenopathy and the presence of germinal centers with fused and expanded mantles within a lymph node that are 3-5 times the size of a typical reactive follicle. Although PTGC is reported in 3.5-10% of chronic lymphadenopathy, head and neck surgeons do not often experience this condition and its clinical implications. Althoug PTGC is benign disease, it could not be excluded the association with other malignant lymphoid diseases. Therefore, periodic follow up of PTGC patient could be required. We report two cases at this study ; a 16 years old man who visited out-patient clinic due to palpable mass of right cervical area, several months ago, and a 35 years old man who visited out-patient clinic for evaluating right cervical mass. Both of two patients were diagnosed with PTGC. In this study, we focus on the clinic-pathologic features of this uncommon disease along with other literature reviews.

Fine Needle Aspiration Cytology of the Branchiogenic Carcinoma -Report of A Case- (아가미 암종의 세침흡인 세포학적 소견 - 1예 보고-)

  • Development of an invasive squamous cell carcinoma within a branchial cleft cyst (branchiogenic carcinoma) is very rare and this case is the first description of cytologic findings in Korea. A 62-year-old man presented with a 5-month history of an enlarged right neck mass. Fine needle aspiration cytology of the mass showed low cellularity and the background containing proteinaceous debris, inflammatory cells, and histiocytes suggested a cystic nature. Abundant lymphocytes, a few anucleated squames, and rare atypical squamous cells with pyknotic nuclei and abnormal keratinization were also noted. After excision of the mass, histologic findings were consistent with a branchiogenic carcinoma. We recommend to raise the possibility of carcinoma (primary or metastatic) when we observe a few dyskeratotic cells in a cystic mass within the neck area, especially in an old male.

A Case Report of Accessory Cervical Thymus (경부 이소성 흉선종 1예 보고)

  • Accessory cervical thymic tissue has been rarely reported. However, it should be included in the differential diagnosis of neck masses in children, along with branchial anomalies, lymph nodes and other tumors. This lesion occurs along the descending line of the thymus between the angle of mandible and superior mediastinum. A 2-month-old infant presented with an asymptomatic left sided neck mass. MRI revealed a well defined homogeneous mass in the deep lobe of left parotid gland. At operation, an easily identified soft tissue mass was found in the left submandibular area, measuring $3{\times}1.5cm$. It was completely excised. Microscopic examination revealed normal thymic tissue.

A Case of Solitary Fibrous Tumor of the Chin (하악부에 발생한 고립성 섬유종 1예)

  • Solitary fibrous tumor (SFT), which was initially believed to be a subtype of mesothelioma, has been reported to occur outside the pleura. In the head and neck region, it primarily manifests in the oral or nasal cavity, with rare occurrences in the facial region. A 40-year-old woman visited our hospital with a mass on her chin. Prior to surgery, involuntary movement was observed in the ipsilateral corner of the mouth upon palpation of the mass. Special care was taken during the surgical procedure to avoid damaging the facial nerve. The mass was excised, and histological examination and immunohistochemical analysis confirmed the diagnosis of an SFT. Here, we present the first reported case of an SFT diagnosed in a jaw mass in Korea. The objective of this study was to highlight the importance of the diagnostic accuracy of SFTs in lower jaw masses.

Co-Occurrence of Papillary Thyroid Carcinoma and Primary Thyroid Lymphoma in a Patient with Tuberculous Lymphadenitis (결핵성 림프절염을 주소로 내원한 환자에서 갑상선 유두상암과 원발성 갑상선 림프종의 동시발생 1예)

  • Primary thyroid lymphoma is rare and accounts for less than 5% of all thyroid malignancy. The clinical presentation includes a rapidly enlarging neck mass, associated with dysphagia, dyspnea or hoarseness. The most common histologic type is diffuse large B cell non-Hodgkin's lymphoma. Papillary thyroid carcinoma is the most common type of thyroid malignancy. Co-occurrence of papillary thyroid carcinoma and primary thyroid lymphoma is very rare. Recently, we experienced a case with co-occurrence of papillary thyroid carcinoma and primary thyroid lymphoma in a 79-year-old woman, who had tuberculous lymphadenitis presented as lateral neck mass. We present this case with a review of the literature.
