This retrospective study reports the effects of combined traditional Korean treatment of cervical radiculopathy in patients who underwent ineffective epidural steroid injection treatment. This study analyzed cervical radiculopathy in patients who visited traditional Korean medicine hospital following an ineffective epidural steroid injection. There were 29 cases included in this study. Scores for Visual Analog Scale (VAS) and Neck Disability Index (NDI) were measured before and after combined treatment with acupuncture, herbal medicine, and chuna therapy. The results of this study showed that patient VAS scores for neck and shoulder pain were significantly reduced (p < 0.001) when comparing scores before treatment ($6.03{\pm}2.04$) with after treatment ($2.14{\pm}1.27$). In addition, the VAS score for radiating pain before treatment ($6.67{\pm}1.44$) compared with after treatment ($2.89{\pm}1.83$) showed a significant reduction (p < 0.001). Furthermore, the NDI score before treatment ($25.85{\pm}6.33$) compared with after treatment ($11.33{\pm}7.47$), also showed a significant reduction (p < 0.001). The results in this study showed the positive effects of combined traditional Korean medicine treatment in significantly reducing pain for patients with cervical radiculopathy, who had ineffective anesthesia.
PURPOSE: The main objective of this study is observing the effects of the functional postural trunk exercise (FPTE) on deep cranio-cervical flexor (CCF) muscle endurance applied on neck pain patients suffering from the acute whiplash-association disorder (WAD). METHODS: The study was tested with 47 patients with neck pain. All patients were equally treated with the ordinary therapy, only experimental group (n=23) was treated with FPTE: assumption of an upright lumbo-pelvic and spinal postural position, adding a neck lengthening manoeuvre in addition. Patients attended physical therapy for 3 times a week, for 4 weeks. Visual analogue scale (VAS) for pain, neck disability index (NDI), range of motion (ROM), were recorded both before and after the intervention. Also muscle activity in the CCF test was employed to analyze the changes between before and after. RESULT: After 4 weeks of training intervention, a remarkable increase in muscle endurance, rotation, extension ROM (p<.05) and decrease in pain and NDI (p<.05) in both groups. Further, the differences between groups were muscle endurance (F=60.350, p<.01), pain, and the left rotation ROM (p<.05). CONCLUSION: From the results, the experimental group had significant the increase in muscle endurance, and the significant decrease in pain. Whilst further research in this category is necessary, these observations suggest that applying exercise in early diagnosis can be of help to treat the neck pain patients suffering from the WAD.
Purpose : VDT (visual display terminal syndrome) can affect individuals who monitor or who work or play using video screens, including those of smartphones. In general, headache symptoms from overuse of these screens can appear due to eye fatigue, muscle pain in the joints of wrists or fingers, and muscle pain in the neck or shoulders. Many studies in the literature have supported standards that seek to prevent these symptoms. The incidence of musculoskeletal diseases caused by the use of smartphones is expected to increase rapidly, particularly among children and young adults, and these diseases are expected to develop into a societal problem. Therefore, in this study we investigate whether tension headaches that develop from smartphone usage can affect forward neck posture, neck pain, and quality of life. Methods : A total of 93 students from University participated in this study. We divided participants into two groups, those with tension headaches (n = 25) and those without (n = 68) and took forward neck measurements. Headaches were classified according to criteria from the International Headache Society and involved bilateral headache position, quality of pressing or tightening pain, mild or moderate pain intensity, and none due to daily physical activity. We surveyed participants using the smartphone addiction diagnosis questionnaire, the Neck Disability Index (NDI), the Headache Impact Test (HIT-6), and the Quality of Life Questionnaire. Results : Although we found no significant differences in tension headaches due to smartphone addiction diagnosis (p = 0.25), SF-36 life quality assessment (p = 0.06), and cranio-vertebral angle (p = 0.07), we found significant differences from the HIT-6 and the NDI (p <.05). Conclusion : Tension headaches are not correlated with smartphone addiction, quality of life, and forward neck angle but do have a correlation with the degree of cervical dysfunction and the effects of the headaches.
Objectives : This study was aimed at comparing clinical effectiveness among pharmacopuncture treatment, Chuna treatment, pharmacopuncture-Chuna cooperative treatment for neck pain caused by traffic accidents. Methods : This study was carried out on the 87 patients with neck pain caused by traffic accidents. The patients were divided into 3 groups ; group A was treated with pharmacopuncture. group B was treated with Chuna treatment. group C was treated with pharmacopuncture and Chuna cooperative treatment one at a time. Also all groups were treated with general acupuncture and herb-medicine treatment. We measured visual analog scale(VAS) and neck disability index(NDI) before the treatment and 2, 4 weeks later. Results : In result each group had significant decrease in VAS and NDI after 4 weeks later. group C showed significant difference as compared to group A and B in NDI and VAS scores. In the first period(from pre to 2 weeks treatment) group C showed significant difference as compared to group A and B in VAS scores. Conclusions : According to the results of this study, it was suggested that chuna treatment and pharmacopuncture are effective in reducing neck pain caused by traffic accidents. Pharmacopuncture and Chuna cooperative treatment was more effective in reducing neck pain caused by traffic accidents. Especially in the first period(from pre to 2 weeks treatment) pharmacopuncture-Chuna cooperative treatment was more effective than pharmacopuncture treatment, Chuna treatment for neck pain caused by traffic accidents.
Objectives : The purpose of this study is to investigate the clinical application of chuna for temporomandibular joint(TMJ) in nuchal pain patients with temporomandibular joint disorder(TMD). Methods : Four patients were treated by chuna for TMJ to evaluate the effect of the treatment. The patient's symptoms were assessed by visual analogue scale(VAS), neck disability index(NDI), cervical lordotic curvature. Results : In all case, the pain was reduced according to VAS, NDI. cervical lordotic curvature of three cases were improved. Conclusions : These results suggest that chuna for TMJ might be an effective method to treat nuchal pain with TMD. But, it's necessary to have more observations and experiments.
Objective : To report long-term clinical and radiological outcomes of minimally invasive posterior cervical foraminotomy (MI-PCF) performed in patients with unilateral single-level cervical radiculopathy. Methods : Of forty-six patients who underwent MI-PCF for unilateral single-level radiculopathy between 2005 and 2013, 33 patients were included in the study, with a mean follow-up of 32.7 months. Patients were regularly followed for clinical and radiological assessment. Clinical outcome was measured by visual analogue scale (VAS) for the neck/shoulder and arm, and the neck disability index (NDI). Radiological outcome was measured by focal/global angulation and disc height index (DHI). Outcomes after MI-PCF were evaluated as changes of clinical and radiological parameters from the baseline. Mixed effect model with random patients' effect was used to test for differences in the clinical and radiological parameters repeat measures. Results : There were no complications and all patients had an uneventful recovery during the early postoperative period. VAS scores for neck/shoulder and arm improved significantly in the early postoperative period (3 months) and were maintained with time (p<0.001). NDI improved significantly post-operatively and tended to decrease gradually during the follow-up period (p<0.001). There were no statistically significant changes in focal and global angulation at follow-up. Percent DHIs of the upper adjacent or operated disc were maintained without significant changes with time. During the follow-up, same site recurrence was not noted and adjacent segment disease requiring additional surgery occurred in two patients (6%) on the contra-lateral side. Conclusion : MI-PCF provides long-term pain relief and functional restoration, accompanied by good long-term radiological outcome.
Purpose : The purpose of this study was to investigate the effects of combined cervical stabilization exercise (CSE) and stretching exercise (SE) on office workers with forward head posture (FHP). Methods : A total of 32 office workers with forward head posture were randomly assigned to experimental (n=16) and control (n=16) groups. The experimental group underwent combined CSE and SE, and the control group underwent cervical self-myofascial release and SE. Both groups performed exercises for 40 min per day, thrice per week for a total of 6 weeks. Craniovertebral angle (CVA), respiration, disability, and joint range of motion (ROM) before and 6 weeks after intervention were measured and compared. Results : There was no significant between-group difference in the general characteristics (p>.05). The intra-group comparison showed significant differences in the visual analog scale (VAS) and neck disability index (NDI) of both groups post-intervention (p<.05). CVA and forced expiratory volume in 1 seconds (FEV1) were significantly improved post-intervention in the experimental group only (p<.05). In the experimental group, all ROM variables were significantly improved post-intervention. In contrast, in the control group, all ROM variables improved significantly post-intervention, except for extension (p<.05). The inter-group comparison showed significant differences in NDI, left lateral flexion, right lateral flexion, and left rotation between the two groups (p<.05). Conclusion : The combination of CSE and SE, which stabilizes the cervical spine, had positive effects on cranial rotation angle, respiration, disability, and joint ROM in office workers with forward head posture. Therefore, the combination of the two exercises may be an effective option to reduce symptoms and prevent postural problems in office workers with FHP.
Objectives : The purpose of this study is to report a case of Rhabdomyolysis that occurred after shock related to overworking, presenting myalgia(especially in the neck), muscular weakness, and dark urine. Methods : A patient diagnosed with Rhabdomyolysis received Korean medical treatment from November $17_{th}$ to $24_{th}$. Clinical improvement was evaluated using a numerical rating scale(NRS), neck disability index(NDI), and laboratory tests which included complete blood count, kidney function, liver function, serum eletrolytes and Creatinine kinase(CK). Results : After treatment, myalgia, muscular weakness and dark urine each improved. Laboratory results, including CK, had decreased to within normal range. Conclusions : Korean medical treatment could be effective for Rhabdomyolysis patients. Further extensive studies should be carried out.
Objectives: This case report presents the effects of Korean medicine treatment in a patient with a new daily, persistent headache. Methods: A 58-year-old female patient with a new daily, persistent headache was treated with Yonggakgyodaeyeong-tang and Kyoungbang Chungsanggyuntong-tang granules, acupuncture, pharmacopuncture, and chuna therapy for 68 days. Headache and neck disability were assessed using a numeric rating scale (NRS), the headache impact test-6 (HIT-6), and the neck disability index (NDI). Results: After 68 days of treatment, headache was relieved, as measured by an NRS decrease from 7 to 4. In addition, the HIT score decreased from 30 to 18, and the NDI score decreased from 42 to 20. Conclusion: These results show that a daily, persistent headache can be relieved with Korean medicine treatment. However, additional well-designed studies are required to confirm these findings.
Objectives: This study was performed to assess the effectiveness of Korean medicinal treatments including Antaeeum-gamibang on posterior neck pain and lower back pain caused by traffic accidents during pregnancy. Methods: Patients were treated by Antaeeum-gamibang, acupuncture and Chuna manual therapy. To measure treatment outcomes, Numerical rating scale (NRS), Oswestry disability index (ODI), Neck disability index (NDI), European Quality of Life-5 Dimensions (EQ-5d), and range of motion (ROM) were used. Results: After treatment, the average NRS of posterior neck pain significantly decreased from $4.20{\pm}1.30$ to $1.80{\pm}0.45$ and the average NRS of lower back pain significantly decreased from $6.40{\pm}1.14$ to $2.20{\pm}0.84$. ODI significantly decreased from $41.42{\pm}17.79$ to $28.29{\pm}14.69$, NDI significantly decreased from $34.93{\pm}9.85$ to $20.27{\pm}8.96$. EQ-5d and ROM improved at all cases. Conclusions: Korean Medicinal treatments including Antaeeum-gamibang may be effective in the treatment of posterior neck pain and lower back pain caused by traffic accidents during pregnancy. However, more case reports and studies are required to identify exactly what treatment is more efficient.
