Objective: Several muscles surrounding neck are vital not only for neck motion, but for upper extremities motions as well. Neck pain would affect neck and shoulder disability. The Neck Disability Index (NDI), Shoulder Pain and Disability Index (SPADI), and Visual Analogue Scale (VAS) are increasingly used to evaluate treatment effectiveness after chronic neck pain. The purpose of this study was to analyze the correlation of neck pain, shoulder pain, and quality of life in patients with chronic neck pain. Design: Cross-sectional study. Methods: Forty-three patients with neck pain participated in this study. Participants were instructed to place a mark on each item of three clinical measures that best represented their experience of his/her neck and shoulder problem over the last week. The outcome measures of the study were NDI, SPADI, and VAS for neck and shoulder pain and disability. Results: The NDI was significantly correlated with the VAS-Neck, SPADI-Pain, SPADI-Disability, SPADI-Total, and VAS-Shoulder (p<0.05). The VAS-Neck was significantly correlated with the VAS-Neck, SPADI-Pain, SPADI-Disability, SPADI-Total, and VAS-Shoulder (p<0.05). The SPADI-Total was significantly correlated with the NDI, VAS-Neck, SPADI-Pain, SPADI-Disability, and VAS-Shoulder (p<0.05). The SPADI-Pain was significantly correlated with the NDI, VAS-Neck, SPADI-Disability, SPADI-Total, and VAS-Shoulder (p<0.05). The SPADI-Disability was significantly correlated with the NDI, VAS-Neck, SPADI-Total, SPADI-Pain, and VAS-Shoulder (p<0.05). The VAS-Shoulder was significantly correlated with the NDI, VAS-Neck, SPADI-Pain, SPADI-Disability, and SPADI-Total (p<0.05). Conclusions: Neck disability in patients with chronic neck pain should be considered with disability and pain of the shoulder.
Background: The prevalence of neck pain and neck dysfunction is high in general population. However, there is little literature on the relationship and factors affecting neck pain and neck dysfunction. Objective: To investigate the correlation between neck and shoulder pain, headaches, neck and shoulder dysfunction, and sleep quality in adults with chronic neck pain, and factors affecting neck pain and neck dysfunction. Design: Cross-sectional study Methods : The sample included 114 subjects, who had complained of chronic neck pain for more 12 weeks. We conducted a Pearson's correlation between neck and shoulder pain, headaches, neck and shoulder dysfunction and sleep quality and a regression analysis of the related variables, thereby analyzing factors affecting neck pain and neck dysfunction. Results : In the present study, in adults with chronic neck pain, neck pain was positively correlated with the Neck Disability Index (NDI), Shoulder Pain and Disability Index (SPADI)-Pain score, and SPADI-Total score (p<.05). The NDI was positively correlated with neck pain, SPADI-Pain score, and SPADI-Total score, as well as with Pittsburgh Sleep Qulity Index(PSQI-K) (p<.05). Among the factors affecting neck pain, shoulder disability as assessed by the SPADI was a significantly associated with neck pain, while shoulder pain and shoulder disability determined by the SPADI were identified as significant variables among the factors affecting neck disability. Conclusion : These results indicated that as neck pain worsened, shoulder pain and neck and shoulder dysfunction also increased, which suggested that shoulder disability affected neck pain. In addition, as the neck dysfunction increased, neck pain and shoulder pain and shoulder dysfunction increased, and sleep quality deteriorated, which suggested that shoulder pain and shoulder disability affected neck disability.
Purpose: The purpose of this study was to evaluate the impact of active vibration exercise on the neck pain, disability index, and muscle activity of patients with forward head posture. Methods: A total of 24 patients were randomly assigned to an experimental group or a control group (n=12 each). The experimental group performed active vibration exercise using a flexi-bar for 20 minutes a day, five times a week for four weeks. The study measured patient neck pain using a visual analog scale, neck pain related disability using the neck disability index, and muscle activity using electromyography. Results: The intragroup comparison showed significant differences in the visual analog scale score, neck disability index score and upper trapezius, lower trapezius and serratus anterior muscle activity values among patients in the experimental group. The intergroup comparison showed that differences in the visual analog scale score, neck disability index score and upper trapezius, lower trapezius and serratus anterior muscle activity values in the control group. Conclusion: This study showed that active vibration exercise was effective in improving the neck pain, disability index, and muscle activity of patients with forward head posture.
PURPOSE: The aim of this study is to investigate how upper thoracic manipulation and cervical stability training affects cervical range of motion and neck disability index of patients with chronic mechanical neck pain. METHODS: 30 patients with chronic mechanical neck pain, and randomly divided into the upper thoracic manipulation and the cervical stability training group. Upper thoracic manipulation group was conducted to the upper thoracic manipulation, and cervical stability training was conducted to the cervical stability training. Intervention period was 6 weeks, and 3 sessions, each of which was run for 5~10 minutes. The subjects were measured neck range of motion before and after intervention by electro-goniometer. Neck disability index was used to measure neck disability index Korean version. RESULTS: Comparison within groups, there were significant difference in neck range of motion before and after intervention, and Neck disability index significantly reduced in the cervical stability training group. The comparison between groups, there were no significant difference in neck range of motion and neck disability index. CONCLUSION: Upper thoracic manipulation and cervical stability training to the patients with chronic neck pain was helpful to improve neck range of motion and cervical stability training was helpful to improve neck disability index.
Purpose: The purpose of this study is to examine the effects of proprioceptive neuromuscular facilitation (PNF) exercises on the neck disability index and deep neck flexor endurance of acute whiplash injury patients and to provide basic data for PNF exercises for musculoskeletal system disorder patients. Methods: Twenty acute whiplash injury patients were randomly assigned to an experimental group (n=10) and were treated with PNF exercises whereas a control group (n=10) underwent general exercises. Each session lasted 15 minutes and was performed five times a week for two weeks. The degree of pain was assessed using a visual analogue scale (VAS) and the degree of neck disability was measured by the neck disability index (NDI). Craniocervical flexor endurance (CCFE) tests were conducted to measure deep neck flexor endurance. Results: In terms of the intragroup changes in VAS, NDI, and CCFE, there were significant decreases in both the experimental and the control groups. After intervention, there were significant differences between the experimental group and the control group in terms of intergroup changes in VAS, NDI and CCFE. Conclusion: Although the exercises that are generally applied to acute whiplash injury patients are effective on the whole, PNF exercises are considered to be beneficial, given the improvements in the neck disability index and deep neck flexor endurance.
The purpose of this study was to determine reliability and validity of the Neck Disability Index (NDI) now in use for the first time in neck pain patients of Korea. Fifty subjects (26 males and24 female) with neck pain enrolled in the study. They completed a standardized self-administered questionnaire that include pain intensity, personal care, lifting, reading, headaches, concentration, work, driving, sleeping and recreation. Reliability was determined by intra-class correlation coefficient (ICC) and Cronbach's alpha by internal consistency. Validity was examined by correlating the NDI scores to the Visual Analog Scale (VAS) score. The Test-retest reliability of the translated versions of the NDI was good ICC (2,1) = .90 (95%CI .85 .95). Cronbach's alpha value for NDI was found to be .95 and this was statistically significant (p<.05). The criterion-related validity coefficients was .72 (p<.01). We conclude that the Korean version of NDI has shown to be a reliable and valid instrument for the assessment of neck pain.
Objective: The purpose of this study was to investigate the effects of myofascial decompressiontherapy using moving suction on body temperature, pain, neck disability index, and cervical rotation for young adult with nonspecific neck pain. Design: Two-group pretest-posttest design. Methods: The subjects were randomly assigned 22 patients with chronic cervical pain who met the study conditions to the experimental group (n=11) and the control group (n=11). In the experimental group, the myofascial decompressiontherapy (MDT) was performed for 10 minutes using moving suction withnegative 15 mmHg pressure from the insertion to the origin of the upper trapezius muscle, while the control group without negative pressure. In order to investigate the effects of the intervention, an infrared thermometer, a visual analogue scale, neck disability index, and goniometer were used. Results: As a result of comparing the pre- and post- changes in each group according to the intervention, skin temperature, pain, neck disability index, and cervical rotation in both the experimental and control groups were significantly improved (p<0.05). Comparison of pre- and post- changes between the experimental and control groups showed significant differences for pain and cervical rotation (p<0.05), but no significant difference was found in the body temperature and neck disability index. Conclusions: Based on the results, MDT using moving suction was effective in reducing pain and increasing of cervical rotation for young adult with nonspecific neck pain.
Purpose: The aim of the study was to investigate neck posture, range of motion, muscle endurance and self-report of pain and disability in smartphone users. Methods: Seventy-eight university student volunteers, aged between 18 and 30 years (mean age 23.2), were assessed for: a head-neck posture by measuring cranial vertical angle, neck range of motions using cervical range of motion device, and a deep neck flexor endurance using a stabilizer. Finally, subjects were asked about their neck pain and completed disability questionnaires, ie, Short Form McGill Pain Questionnaire, Neck Disability Index, and World Health Organization Disability Assessment Schedule 2.0. Results: Thirty-eight subjects experienced recurrent neck pain with/without upper limb pain (neck pain group) and 40 reported no current neck pain with/without upper limb pain (no neck pain group). Differences were found between groups on pain and disability questionnaires. Subjects with neck pain had significantly higher disability scores than those of no neck pain group. However, there were no differences observed between groups in a head-neck posture, neck range of motions, and deep neck muscle endurance time. The smartphone usage time was negatively correlated with neck pain intensity and disability score whereas it had positive relationship with flexibility and posture. Conclusion: Group differences were observed as lower capacity not only for neck specific daily activities but for general functioning in daily routine when the neck pain and no neck pain groups were compared. Therefore, functioning in daily activities should be investigated as prevention for further developing neck pain in smartphone users.
Purpose: The purpose of this study was to examine the functional category, the item structure and the model-data fit of the neck disability index (NDI) of neck pain subjects by performing a Rasch rating scale analysis. Methods: The data was obtained from the assessments of 71 college students (males: 27, females: 44) with neck pain. The data of the NDI was applied to the Rasch's rating scale model to estimate the difficulty of items, the goodness-of-fit of each item, the separation reliability and index, and the rating scale. Results: The 'sleep' item showed misfit and nine items were founds to be fits for self-reporting of disability due to neck pain. The most difficult item of the remaining 9 items was 'work' and the easiest item was 'headache'. The transformation formula score=(logit score+7.10)/(7.10+0.11)$\times$100. The 6 response levels of the NDI were validated according to the structure of the rating scale. The item and subject reliability of the separation reliability was 0.97 and 0.85, respectively. Conclusion: We proved that the NDI for self-reporting of disability of daily activities due to mild neck pain was valid and reliable. This study suggests that individuals with mild neck pain may be assessed by using the modified NDI that does not include the 'sleep' item in the 10 items of NDI.
Background: The purpose of this study was to determine the effects of cervical deep muscle flexion exercise (CCFE) on craniovertebral angle, pain, and neck disability for patients with chronic neck pain Methods: The subjects of this study were randomly divided into three groups of 30 patients with chronic neck pain: rectus abodominis functional massage (n=10), cervical deep muscle flexion exercises group (n=10), and the control group(n=10). To assess visual analog scale (VAS) was used to test the neck pain, To assess neck posture was used to craniovertebral angle, VAS was used to test the neck pain, neck disability index (NDI) was used to test the neck dysfunction. All measurements were performed before and after each intervention was applied 3 times a week for 4 weeks. Results: In the results of all measurements, 2 groups except for the control group showed a significant change in the recovery of posture, neck pain, neck disability index (p<.05). Conclusions: Our results of this study showed that applying cervical deep muscle flexion exercise and rectus abodominis functional massage to patients with chronic neck pain improved cervical posture, neck pain, neck disability.
