Objective: The aim of this study is to develop a farming shovel to reduce workload, which helps farmers lower the risk of musculoskeletal disease. Background: Most of work using farming tools including shovels requires repetitive works and awkward postures on body parts, and it could possibly cause work-related musculoskeletal disorders. It is necessary to develop and distribute farm equipment and tools in order to reduce physical workload. Method: To improve the most uncomfortable task perceived by ten farmers during the work with a shovel, the improved shovel was designed and made as a prototype for experiment for the comparison of the existing and improved shovels. Twenty males were recruited for this experiment, and muscle activity (%MVC) of six body parts and subjective discomfort ratings by body parts while working with a shovel were measured. A paired t-test was performed to compare physical workload between the existing shovel and the developed one. Results: A shovel applying foothold tied between shaft and blade was designed, which can help workers reduce repetitive bending of back and pressures for upper limbs while digging soil. According to compared evaluation of the developed shovel and the existing shovel, the developed shovel's %MVCs in all experimental muscles were significantly lower than those of the existing shovel. The developed shovel showed the biggest drop in perceived subjective discomfort rating of back, followed by arm and neck, compared to the existing shovel. Conclusion: It was confirmed that attaching a foothold to a shovel was an effective way of reducing workload in back and upper limbs during digging. Application: In the near future, if we put the prototype of developed shovel to practical use after making up for defects, it will help farm work environment be healthier and safer.
Objectives The purpose of this study is to investigate the efficacy of E10 on the relief of cervical myalgia. Methods This study is a one group pre-post, pilot trial test. After the approval of institutional review board (IRB), we have recruited 25 patients suffering from cervical myalgia. 23 patients (2 patients dropped out) was treated by using E10 on the cervical muscle and acupuncture points, three times a week for a total of two weeks from February 2016 to April 2016. Evaluations of VAS for bothersomeness of neck pain (VASB), VAS for pain intensity (VASP), NDI, SF-36, X-ray, thermography, etc. before and 1 week after treatment were carried out. The primary outcome measure was the VASB, measured 1 week after the end of the therapy. Results VASB scores was significantly decreased from $72.5{\pm}9.9$ to $50.4{\pm}9.9$. VASP scores was significantly decreased from $71.5{\pm}10.5$ to $48.5{\pm}18.3$. Results showed significant improvements in VASB, VASP, NDI, SF-36 from the baseline. However, in X-ray, thermography, there was no singnificant difference. Conclusions This study demonstrates the effectiveness of E10 treatment for relieving cervical myalgia.

Traumatic ulcerative granuloma with stromal eosinophilia (TUGSE) is an ulcerative lesion on oral mucosa featuring as a benign mass of self-limiting growth. It can be easily misdiagnosed as squamous cell carcinoma (SCC) due to its long healing period and elevated or rolled-up margin. A 57-year old male patient who visited our department was diagnosed as SCC according to the clinical features, results of positron emission tomography (PET) and magnetic resonance imaging (MRI) of the oral lesion. However, after performing incisional biopsy, histopathologically, there were no atypical cells, but eosinophil and CD 30+ T-cells were clustered in subcutaneous and muscle tissue. It is very significant to consider this reactive lesion in the field of oral and maxillofacial surgery because of its possibility of mistaken as malignant disease. Here, we will report a couple of cases of TUGSE with references and limit of radiographic tools used for diagnosis of head and neck cancer.
Transcranial magnetic stimulation devices has been used mainly for diagnostic purposes by measuring the functions of the nervous system rather than for treatment purposes, and has a problem of considerable energy fluctuations per repeated pulse. The majority of strokes are caused by ischemia and result in brain tissue damage, leading to problems of the central nervous system including hemiparesis, dysfunction of language and consciousness, and dysfunction of perception. Control is difficult and the size is large due to the difficulty of digitalizing the energy stored in a capacitor, and there are many heavy devices. In addition, there are many constraints when it is used for a range of purposes such as head and neck diagnosis, treatment and rehabilitation of nerve palsy, muscle strengthening, treatment of urinary incontinence etc. Output stabilization and minimization of the energy variation rate are required as the level of the transcranial magnetic stimulation device is dramatically improved and the demand for therapeutic purposes increases. This study developed a compact, low cost transcranial magnetic stimulation device with minimal energy variation of a high repeated pulse and output stabilization using a real time capacitor charge discharge voltage. Ischemia was induced in male SD rats by closing off the common carotid artery for 5 minutes, after which the blood was re-perfused. In the cerebrum, the number of PARP reactive cells after 24 hours significantly decreased (p < 0.05) in the TMS group compared to the GI group. As a result, TMS showed the greatest effect on necrosis-related PARP immuno-reactive cells 24 hours after ischemia, indicating necrosis inhibition, blocking of neural cell death, and protection of neural cells.
This study was designed to evaluate the analgesic effect of bee venom (BV) Acupuncture into different treatment points, Chok-samni (ST36) and blank loci of the gluteal muscle and back. We investigated neuronal activity in the spinal cord using the Fos immunohistochemical technique according to the pretreatment with different concentrations of BV, thirty minutes before the formalin injection. The results were summarized as follows: 1. The number of Fos-like immunoreactive (Fos-LI) neurons in L2 segment of the saline-formalin treated group was significantly increased in NECK and VENT of the spinal cord as compared with that of the room control group. However, there was no significant change in the number of the Fos-LI neurons in L2 segment of the BV-formalin treated group as compared with that of the room control group. 2. The number of Fos-LI neurons in L3-5 segment of the saline-formalin group was significantly increased in all the regions of 142 the spinal cord as compared with that of the room control group. However, the Fos-LI neurons in L3-5 segment of the BV-formalin treated group was dramatically decreased in all the regions of the spinal cord as compared with that of the saline-formalin group. Therefore, these results indicated that the BV acupuncture suppressed the nociceptive neuronal activities in L3-5 of the spinal cord induced by formalin injection. 3. There was a strong positive correlation between the formalin-induced pain behavior and the number of the Fos-LI neurons in L3-5 segment.
Background: Echocardiography involves strenuous postures of the upper limbs. This study explored the physical workload in the neck and upper limbs in sonographers performing echocardiography, and the extent to which the workload differs from than in other work tasks (other sonographic examinations, and nonsonographic tasks). Methods: The physical load was assessed by inclinometry, goniometry, and electromyography methods in 33 female sonographers during authentic work using three different echocardiography techniques and other work tasks. Results: Echocardiography was characterized by low velocities of the head, arms, and wrists, and a low proportion of muscular resting time in the forearms, in the transducer limb, and the computer limb. The transducer limb was more elevated in one of the techniques, but this technique also involved a higher proportion of muscular resting time of the trapezius muscle. We also found a high proportion of awkward wrist postures in the transducer wrist in all three techniques; in one due to prolonged flexion, and in the others due to prolonged extension. Other work tasks were less static, and were performed with higher upper arm and wrist velocities. Conclusion: None of the three echocardiography techniques was optimal concerning physical workload. Thus, to achieve more variation in physical load we recommend that the equipment be arranged so that the sonographer can alternate between two different techniques during the workday. We also propose alternation between echocardiography and nonsonographic tasks, in order to introduce variation in the physical workload. Clinical expertise should be used to achieve further improvements.
Purpose: Facial rejuvenating surgery has become a challenge to most plastic surgeons. Patients are demanding fewer complications, a prompt recovery, and more natural results. Current trend of the face lift surgery has been developed into less invasive procedures. Every aging patient in Asia wants to look younger without obvious evidence of surgical correction. Methods: The authors performed the limited dissection face lift with PDS quilting suture on twenty five patients. These five quilting sutures consist of sutures at 1~2 cm posterior to the prominent point of zygoma to the periosteum of the zygomatic arch, at the lateral border of Bichat's fat pad area to the zygomatic arch, at the lateral border of the orbicularis oculi muscle to the deep temporal fascia, at the upper lateral border of the platysma to the periosteum of the mastoid, and at the anterior lower margin of the earlobe to the deep temporal fascia by quilting suture technique to achieve rejuvenation. Results: These procedures could produce a balanced volumetric rejuvenation. This method gave considerable benefit of stable and satisfactory results. It provides reduced operative time, well corrected nasolabial fold and neck wrinkle, and swift recovery with minimal complications. Conclusion: Although it could not replace the classic facelift, this technique can be recommended as an option for patients who do not present with advanced facial aging or not want a more extensive procedure.
Purpose: Primary malignant lymphomas of the salivary glands are uncommon. The parotid gland was most frequently involved, followed by the submandibular gland, minor salivary gland and sublingual gland. The most common subtype is mucosa - associated lymphoid tissue(MALT) lymphoma. We experienced a case of salivary MALT lymphoma involving parotid gland duct, so report a case with a review of the literature. Methods: A 65 year old female presented with a palpable mass on the left side of her cheek. There was no clinical or laboratory evidence of pre - existing autoimmune disease. Preoperative facial and neck CT with contrast showed $2.1{\times}1.7cm$ sized, ill defined, homogeneous low density mass near left masseter muscle, and no evidence of other enlarged lymph nodes. Results: At operation, a yellowish oval shaped mass was found slightly adhered to middle portion of the parotid gland duct, meaduring $2{\times}1.5{\times}0.7cm$. Microscopic finding showed that centrocyte - like cells, monocyte B cells and plasma cells were diffusely infiltrated. Immunophenotyping was preformed on fixed section. The majority of the small cells were immunoreactive for the B cell marker CD20. Based on the typical histological findings supported by immunostaining, the mass was defined as MALT lymphoma. Conclusion: We report that very rare case of MALT lymphoma involving parotid gland duct in 65 year old female patient was experienced with clinical characteristics, histologic features and references.
Rhabdomyosarcoma is a highly malignant soft tissue sarcoma that can arise in any site of the body containing striated muscle or its mesenchymal analgae. It is the most common childhood sarcoma with two peak age frequencies, one at ages 2 to 6, and one in the adolescence. The site, stage and extent of disease, and pathologic characteristics of the tumor contribute to prognostic factors that influence therapeutic decisions. The results of treatment of 52 patients with rhabdomyosarcoma, who were treated at Department of Radiation Oncology, Yonsei University College of Medicine, Yosei Cancer Center from 1976 to 1987 were retrospectively analyzed. The most frequent clinical group and primary sites were IRS group III (57.7%) and head and neck (42.3%) including orbit (11.5%) and parameningeal region(13.5%). The overall and disease free 5 year survival rate of eligible 41 patients were 31.7%, 29.3%, respectively. The complete remission (CR) rate was 50% in clinical group III and 0% in IV. Primary tumors of the orbit, clinical group 1 and embryonal subtype had the best prognosis. The Survival rate was improved by additiion of chemotherapy to operation and radiation therapy.
This study evaluated the preference and wearing characteristics of existing fitness compression wear for elderly women and identified the problems and improvements of existing fitness apparel. Preference and wearing characteristics for 6 types (A-F) of upper and lower fitness compression wear were evaluated. Photos and drawings as well as the preferred designs and details were investigated after presenting the evaluation clothing. Evaluations were made on a 5 point Likert scale for the fitness, allowance, pressure, ease of movement, fabric material, and overall satisfaction after wearing. The design preference indicated that B type (26.6%) and C type (23.4%) were preferred in the top with C, E and F type preferred to the same ratio of 19.4% in the slacks. The fitness and allowance amount of the top B type were the most appropriate (${\geq}4$), the C type was in close contact (2.86), and the F type was inadequate in the neck circumference (1.77). The feeling of pressure was high in the waist, abdomen, thighs, and knees. The area where the elderly people want to improve their strength was the legs; in addition, a pattern design was needed to strengthen leg muscles when designing fitness compression wear. However, a design with excessive adhesion due to a muscle support band was shown to be not preferred. Therefore, depending on the activity, it is necessary to develop a fitness compression wear that applies an optimum stretching ratio of fabric based on body surface area changes.
