The Effect of Active Neck Muscle Training in the Treatment of Chronic Neck Pain (만성 경통 환자의 치료에 있어서 능동적 경부 근육 훈련의 효과)

  • The purposes of this paper were to investigate the effect of active treatment compared with a conservative treatment and to provide the information for physical therapy in patients suffering from chronic neck pain. Forty female subjects who were diagnosed with cervical radiculopathy participated in this study and were divided into the conservative and active treatment groups. The active treatment group consisted of 20 patients who were treated with therapeutic modalities and active neck exercise program during the admission (15 days) and one month after discharge. The conservative treatment group consisted of 20 patients who were not received with active neck exercise program. The assessment tools were made using visual analogue scale(VAS), neck disability index(NDI) and modified Zung depression scale(MZDS). All subjects were measured three times: before the admission, at discharge, and at one month after discharge. Data were compared by groups using independent t-test. VAS, NDI and MZDS scores measured at admission and discharge were not significantly different between the groups. On the assessment performed one month after discharge, VAS and NDI scores were significantly lower for the active treatment group compared with those of the conservative treatment group(p<0.05), but MZDS score was not significantly different between the groups. In the comparison of two treatment methods for chronic neck pain, active treatment was more effective than conservative treatment. The findings of the study indicate that active treatment in chronic neck pain has a positive effect in relieving pain and restoring neck function.

The Study about the Correlation between Cervical Hypolordosis and Neck Pain (경추의 전만 감소와 경항통의 상관관계 연구)

  • Objectives : The objective of this study was to observe the correlation between Cervical hypolordosis and Neck pain. Methods : Cervical lordosis(Cobb method[C1-C7], Cobb method[C2-C7], Ishihara index) was measured using the sagittal radiograph of the cervical spine and Neck pain(VAS scale, Neck Disability Index) was evaluated. Based on Cobb method[C1-C7], 48 subjects(Cobb method[C1-C7]${\leq}45^{\circ}$) were divided into Hypolordosis group and Normal group. Window version SPSS 12K was used for statistical analysis about correlation between Cervical lordosis and Neck pain, also about difference of the Neck pain between Normal and Hypolordosis group. Results : It was as follows, 1. Cobb method[C1-C7] and Neck pain(VAS scale, NDI) showed a significant correlation coefficient (p<0.05). Ishihara Index and NDI also showed a significant correlation coefficient of -0.297(p=0.040). 2. The VAS scale in the hypolordosis group was $5.62{\pm}2.34$ and that of the normal group was $3.41{\pm}2.01$(p=0.001). The NDI in the hypolordosis group was $2.30{\pm}0.66$ and that of the normal group was $1.81{\pm}0.51$(p=0.007). There was no significant difference in Age, Sex between both group. Conclusions : There was a significant correlation between Cervical hypolordosis and Neck pain. Hypolordosis group complained a severe neck pain.

The Effects of Kinesio Taping and Neuromuscular Rehabilitation Exercise for Patients with Acute Whiplash-Associated Disorder (급성 편타성-관련손상 환자에게 키네지오 테이핑과 신경근 재활 운동이 미치는 영향)

  • Background: The purpose of this study was to investigate the effects of kinesio taping and neuromuscular rehabilitation exercise for patients with acute whiplash-associated disorders. Methods: Twenty acute whiplash-associated disorders patients were recruited for the study. Subjects were randomly allocated into two groups. Neuromuscular rehabilitation group (NRG) received neuromuscular rehabilitation exercise, kinesio taping group (KTG) received kinesio taping. All subject was evaluated before and after intervention by their range of motion (ROM), neck disability index (NDI), numeric pain rating scale (NPRS) and fatigability. Results: First, the ROM was significantly increased in all group (p<.05). The NDI, NPRS and fatigability were significantly decreased in all group (p<.05). Secondly, NRG had significantly more increased ROM (flexion, both rotation) than KTG (p<.05). NRG had significantly more decreased NDI, NPRS and fatigability than KTG (p<.05). Conclusion: According to the results above, increased in range of motion in the neck, decreased in NDI, NPRS, fatigability were more effective in the NRG that received neuromuscular rehabilitation exercise than the KTG that received kinesio taping.

The Clinical Study of 29 Cases on Effects of Cervical Hyeopcheok(Jiaji) Point Treatment for Neck Pain (경항통 환자에 대한 경추 협척혈 치료 29례)

  • Objectives : The aim of this study is to evaluate the effect of cervical Hyeopcheok(Jiaji) point for neck pain. Methods : The twenty-nine patients who feel neck pain, are chosen from chart review, received acupuncture treatment on cervical Hyeopcheok(Jiaji) point. The evaulation of progress was checked by NDI(neck disability index), and VAS(visual analog scale). Data were analyzed by Friedman two way analysis of variance and Wilcoxon matched pairs signed rank test. Results : NDI value(mean${\pm}$SD) was $17.76{\pm}8.26$(n=29) at the first visit, $13.41{\pm}8.04$(n=29) at 3days after treatment, $10.85{\pm}8.46$(n=26) at 5days, $11.00{\pm}7.80$(n=13) at 9days, $7.22{\pm}4.76$(n=9) at 15days, $3.86{\pm}4.06$(n=7) at 20days, and $3.33{\pm}3.78$(n=6) at 30days. VAS value(mean${\pm}$SD) was $4.07{\pm}1.33$(n=29) at the first visit, $12.97{\pm}1.05$(n=29) at 3days after treatment, $2.15{\pm}1.12$(n=26) at 5days, $2.15{\pm}0.99$(n=13) at 9days, $1.56{\pm}0.73$(n=9) at 15days, $1.29{\pm}0.49$(n=7) at 20days, and $0.83{\pm}0.41$(n=6) at 30days. The value of NDI and VAS gradually decreased. Repeated measurement results(the difference between the first visit and each measurement day) and the difference between before and after the interval(except between 5 and 9days, between 20 and 25days) were statistically significant. Conclusions : This result was shown that treatment of cervical Hyeopcheok(Jiaji) acupuncture point for neck pain can be effective. Further systematic research will be needed.

  • Objectives : The study on Acupotomy therapy has recently become a popular academic field as proven to be an great alternative to the limitation of Western medical treatment. However, there has been little study examining acupotomy therapy on treating Cervical Herniated Intervertebral Disc. Thus, this study aims to investigate the sense of improvement and satisfaction from the Cervical Herniated Intervertebral Disc patients treated with acupotomy therapy. Methods : A total of 23 subjects that consists of Cervical Herniated Intervertebral Disc patients were participated in this study. They were treated with acupotomy therapy and acupuncture. All the subjects were asked to answer Odom's degree, VAS(visual analogue scale), NDI(neck disability index) and JOA score before and after the treatment. Results : The results of the Odom's degree, VAS, NDI and JOA score of analysis groups, show that the improvement of neck pain in acupotomy and acupuncture treatment group is significantly effective. Conclusions : It is suggested that acupotomy therapy has development-effectiveness on Cervical Herniated Intervertebral Disc.

  • Background: This study examined the effects of the muscle energy technique and cervical traction after applying conservative physical therapy to patients with cervical radiculopathy. Methods: Twenty-six patients with cervical radiculopathy were randomized into two groups: the experimental group (n=13) and the control group (n=13). The two groups performed the muscle energy technique and for 4 weeks, 3 times a week, in 70-minute sessions. Pain, range of motion, function, and muscle performance were assessed using the visual analogue scale (VAS), cervical range of motion (CROM) goniometer, neck disability index (NDI), and stabilizer pressure biofeedback, before and after training. Results: After four weeks of therapy, the VAS (p<.001), NDI (p<.01), and the maximum strength of the deep neck flexion muscles significantly decreased (p<.01) and CROM significantly increased in both groups (p<.05). Conclusion: The muscle energy technique and therapeutic modalities such as cervical traction are effective in reducing VAS and NDI and increasing CROM and muscle performance in patients with cervical radiculopathy.

A Case Report of Warming Acupunture Therapy at Buyang, Golyun and Its Cotreatment with Traditional Korean Medicine in a Traffic Accident Patient with Neck Pain (교통사고 후 발생한 경항통 환자에 대한 부양, 곤륜혈의 온침 병행치료 증례보고)

  • Objectives : The aim of this study was to observe the clinical effect of warming acupunture therapy at Buyang(BL59), Golyun(BL60) in a traffic accident patient with neck pain. Methods : A 32-years-old, female patient with neck pain was treated by warming acupunture therapy at Buyang(BL59), Golyun(BL60) from April 28th to May 5th. The improvement of the patient's neck pain was evaluated by Visual Analog Scale(VAS), Pain Rating Score(PRS) and Neck Disability Index(NDI). Result : After treatment, flexion & extension VAS was decreased from 7.0 to 3.0 and lateral bending VAS was decreased from 7.5 to 4.0. PRS and NDI was improved 70 to 54, 41 to 35, respectively. Conclusions : The result suggests that warming acupunture therapy at Buyang(BL59), Golyun(BL60) may have therapeutic effect on neck pain patients.

Two Patients Who Were Hospitalized for Clavicle Fracture Caused by a Traffic Accident and Improved with Korean Medicine Complex Treatment (교통사고로 발생한 쇄골 골절로 입원하여 한방복합치료로 호전된 환자 2예)

  • To report improvement of 2 hospitalized patients who had clavicle fractures due to traffic accident with Korean medicine complex treatment. We collected a data from February 2020 to April 2022. A total of 2 patients were studied and we compared numeric rating scale (NRS), neck disability index (NDI), range of motion (ROM) at admission and discharge date of hospitalization. After receiving an average of 28.5 days of Korean medicine inpatient treatment, both patients had reduced NRS, NDI scores and increased ROM. After Korean medicine complex treatment, 2 patients with clavicle fracture showed to have been effective. But further studies need to be done.

A Clinical Study on Effect of Chuna Treatment for Neck Pain Patients Caused by Traffic Accident (자동차사고로 발생한 경항통 환자의 추나요법의 효과에 대한 임상적 연구)

  • Objectives : The purpose of this study is to investigate the effect of Chuna treatment for neck pain patients caused by traffic accident. Methods : 64 patients were divided into two groups, experimental group and control group, of 32 patients each. Control group was treated with general korean medical treatment three times per week for 2 weeks. Experimental group was treated with Chuna treatment and general korean medical treatment three times per week for 2 weeks. We evaluated the treatment effect of each group with the visual analog scale(VAS) and neck disability index(NDI). Results : 1. In both two groups, VAS were decreased significantly in statistics after treatment. 2. In both two groups, NDI were decreased significantly in statistics after treatment. 3. There were significant reduction of the VAS in experimental group than control group, but, there were not significant reduction of the NDI in experimental group than control group. Conclusions : We suggest that Chuna treatment could be recommended as a useful therapy to neck pain patients caused by traffic accident.

Effects of Therapeutic Exercise on Posture, Pain and Asymmetric Muscle Activity in a Patient with Forward Head Posture: case report (치료적운동이 전방두부자세 환자의 자세, 통증 및 비대칭적 근육활성에 미치는 영향: 증례보고)

  • PURPOSE: The purpose of this case report was to determine the effect of therapeutic exercise on posture, pain, and muscle activity in two patients with forward head posture (FHP). METHODS: A-31-year-old male (patient A) and a 19-year-old women (patient B) presented with FHP, neck pain, and headache. The therapeutic exercise program consisted of cervical mobilization, deep cervical flexors strengthening, and cervical extensors stretching, for 40 min/d, 2 d/week, for 8 weeks. Neck pain (VAS), neck disability (NDI), cervical range of motion (CROM), lateral view of cervical spine X-ray (indicating the FHP), and asymmetrical neck and shoulder muscular activity ratio were measured before, after 4 weeks, and after 8 weeks of corrective exercise. RESULTS: VAS and NDI decreased in patients A and B after exercise compared to before the program. CROM increased in patients A and B at flexion, extension, side bending, and rotation after exercise compared to before the program. FHP decreased in patients A and B at distance after exercise compared to before the program. In addition, asymmetrical neck and shoulder muscles activity ratio improved in patients A and B after exercise compared to before the program. CONCLUSION: We demonstrated in a case report that therapeutic exercise increases ROM, decreases pain and disability of neck, FHP, and asymmetry muscle activity ratio in patients with FHP. These finding have clinical implications for therapeutic exercise in patients with FHP.
