음운 환경이 비음치에 미치는 영향

  • Nasometer를 통해 얻은 비음치(nasalance score)는 객관적일 뿐만 아니라 훈련된 언어임상가가 청지각적으로 판단한 비음도와 상관관계가 높기 때문에, 공명장애 진단의 보조자료로 유용하다. 그러나 비음치는 자극어에 따라 다양한 수치를 보이므로 공명장애의 진단을 위해서는 타당하고 신비로운 자극어의 개발이 요구되고 있다 따라서 본 연구는 비음치에 영향을 미치는 주요한 음운 환경이 무엇인지 살펴보고, 무의미 1음절어와 유의미 문장 자극어 중 어떤 것이 더 신뢰로운지 알아보아, 비음치 측적을 위한 자극어 개발에 기초를 마련하고자 하였다. PHYSIOANATOMY OF NASOPHARYNGEAL SPACE AND HYPERNASALITY IN CLEFT PALATE (구개열에서 비인두강의 생리해부학적 구조와 과비음과의 연관성 연구)

  • Velopharyngeal closure is a sphincter mechanism between the activities of the soft palate, lateral pharyngeal wall and the posterior pharyngeal wall, which divides the oral and nasal cavity. It participates in physiological activities such as swallowing, breathing and speech. It is called a velopharyngeal dysfunction when this mechanism malfunctions. The causes of this dysfunction are defects in (1) length, function, posture of the soft palate, (2) depth and width of the nasopharynx and (3) activity of the posterior and lateral pharyngeal wall. The purposes of this study are to analyze the nasopharynx of cleft palate patients using cephalometry and to evaluate the degree of hypernasality using nasometry to find its relationship with velopharyngeal dysfunction. The following results were obtained : 1. In cephalometry, there were significant differences in soft palate length, soft palate thickness, nasopharyngeal depth, nasopharyngeal area, and adequate ratio between two groups. 2. In nasometry, there were significant differences between two groups in vowel /o/ and sentences including oral consonants. 3. In cleft palate patients, though no general correlation was found between Anatomic VPI and nasalance scores, vowel /i/ and sentences including oral consonants were slightly correlated. In conclusion, cephalometry and nasometer results were significantly different between the two groups. Though in the cleft palate group, Anatomic VPI and nasalance scores, which are indices for velopharyngeal closure, excluding the vowel /i/ and sentences including oral consonants show generally no significance.

An Aerodynamic and Acoustic Study of Nasalization in Cleft Palate Speakers. (구개열 언어의 비음화에 관한 공기역학 및 음향학적 연구)

  • Cleft palate patients have general speech problems with resonance disorders and articulation disorders. The aim of this study is to find the aerodynamic and acoustic characteristics of the nasalization in cleft palate speakers. Thirteen control groups and three cleft palate patients pre- and post operation were selected for these studies. The test words are composed by polysyllabic words: consonants between high vowel /i/ analysis. The cleft palate patients repeated test words pre- and post-operation from one, three and six month periods. The subjects repeated test words on Macquirer and on Nasometer Model 6200-3. The aerodynamic and acoustic results of nasalization show as follows: (1) The nasal rate in overall airflow of aspirated consonant for cleft palate patients shows higher levels than that of the control group. It had decreased since one month after operation. (2) The overall airflow of cleft palate patients is higher than in the control group, however oral air pressure is lower than control group. (3) The nasal airflow and the nasal rate in overall airflow of cleft palate patients has higher than the control group, however its decreased after operation. (4) The nasalance scores of cleft palate patients were 40% higher than that of the control group. The scores did not decrease after operation. The nasalance score of lateral and fricative sounds did not decrease after operation.

Phonatory Caracteristics of Vwels and Resonant Consonants using the Electroglottography (전기성문파형검사를 이용한 모음과 공명 자음의 발성특성)

  • Background and Objectives : Vowels and resonant including nasals and liquid are produced with vocal folds vibration have been used for voice therapy of hyperadduction patients. This study was conducted to investigate phonatory characteristics of vowels and resonant consonants through the EGG measures from Lx. Speech studio (Laryngograph Ltd, UK). Materials and Method : 7 male adults produced sustained vowel /a/, /i/, /u/, nasals /m/, /n/, /${\eta}$/and liquid /I/ and read the sentences (1nasals-liquid sentence, 1 non-nasals-liquid sentence) and tongue-tip trill and humming. Fx(Hz), Ox(%) were obtained of vowels, nasals, liquid and each of the posterior vowel /a/ of /ma/, /na/, /la/, /ha/ with same F0(around F#165Hz) and amplitude (75${\pm}$5db). And also DFx(Hz), DQx(%), CFx(%) and CAx(%) were obtained from reading two kinds of sentences. Results : Qx(%) was the highest in /u/ of vowels, and nasal/n/ of the resonant consonants and nasals-liquid sentence was higher Qx than non-nasals-liquid sentence but significant differences were not found. Qx(%) of the posterior vowel /a/ of nasal consonants/n/ was higher than in the isolated vowel/a/ and other posterior vowel of resonant consonants and fricatives /h/. Regularity or periodicity and higher Qx were observed in the nasals-liquid sentence than non-nasals-liquid sentence in graphs of QxFx & CFx produced by Quantiative analysis. In the nasalance score, /u/vowel was significant higher among the vowels and /I/ liquid was significant lower among the resonant consonants and nasals-liquid sentence is higher than non-nasals -liquid sentence. CQ(%) was not significantly correlated with nasalance(%). Conclusion : These findings might signify resonant phonation was not correlated with nasalance.

  • The communicative disorders in cleft palate patients have relationship with the acoustic and He physiological phenomena. Particularily hypernasality is a parameter of cleft palate speech that has been studied by many clinicians and speech pathologists. The degree of hypernasality has been assessed by the listener,s judgement, but perceptual assessements have poor scientific reliability, so objective instruments have been needed to test hypernasality with diagnostics accuracy. This study was analyzed the nasalance score using a Nasometer for cleft palate patients. The simple vowels /a/, /i/, /e/ and the approximants /j/, /w/ were tested for the degree of hypernasality after operation. The phrases containing long and short duration times were used in this study to asses hypeernasality. Fiberopic views shows the open velopharyngeal port that resulted in hypernasality of cleft palate patients. The authors assert the important of the management of cleft palate patients.

Three Cases of Submucous Cleft Palate with Hypernasality Treated with Double Opposing Z-plasty (과대비성을 동반한 점막하 구개열 환자에 대한 Double Opposing Z-plasty 3례)

  • Several kinds of surgical methods have been used to treat the submucous cleft palate with hypernasality. The purpose of this study was to evaluate the double opposing Z-plasty which is one of the newly established surgical methods by Dr. Furlow in three cases of submucous cleft palate with hypernasality operated in this department. There were a number of advantage of double opposing Z-plasty. The surgery is adaptable for use in early patient. Midfacial growth retardation and complication were not seen in all cases. Hypernasality was improved subjectively. Using the nasometer, the nasalance score of the postoperative period was compared with that of before surgery, much improvement were noted in 'Ah', 'PaPa'. And fiberoptic endoscope was used to evaluate the velopharyngeal closure pattern. Velopharyngeal closure pattern with velum was improved dramatically, Double opposing Z-plasty seem to be a good procedure for the treatment of submucous cleft palate with hypernasality.

Reconstruction of a Total Soft Palatal Defect Using a Folded Radial Forearm Free Flap and Palmaris Longus Tendon Sling

  • Background : The soft palate functions as a valve and helps generate the oral pressure required for normal speech resonance. Speech problems and nasal regurgitation can result from a soft palatal defect. Reduction of the size of the velopharyngeal orifice is required to compensate for the lack of mobility in a reconstructed soft palate. We suggest a large volume folded free flap for reduction of the caliber and a palmaris longus tendon sling for suspension of the reconstructed palate. Methods : Six patients had total soft palate resection for tonsillar cancer and reconstruction with a large volume folded radial forearm free flap combined with a palmaris longus sling. A single surgeon and speech therapist examined the patients with three standardized speech assessment tools: nasometer test, consonant articulation test, and speech acuity test performed for speech evaluation. Results : Mean nasalance score was 76.20% for sentences with nasal sounds and 43.60% for sentences with oral sounds. Hypernasality was seen for oral sound sentences. The mean score of the picture consonant articulation test was 84% (range, 63% to 100%). The mean score of the speech acuity test was 5.84 (range, 5 to 6). These mean ratings represent a satisfactory level of speech function. Conclusions : The large volume folded free flap with a palmaris longus tendon sling for total soft palate reconstruction resulted in satisfactory prognosis for speech despite moderate hypernasality.

Outcomes for Patients with Submucous Cleft Palate Accompanying Hypernasality Treated with Double Opposing Z-plasty (과대비성을 동반한 점막하구개열 환자에 대한 Double Opposing Z-plasty를 통한 수술적 치료 결과)

  • Submucous cleft palate is a relatively uncommon congenital anomaly accompanying velopharyngeal incompetence(VPI). Double opposing Z-plasty has many advantages including prolongation of soft palate, normal midfacial growth, midline scar. We analyzed postoperative results comparing with those of preoperative evaluation by several variables(nasometer, endoscopy, satisfactory scale) in 14 patients treated with double opposing Z-plasty due to submcous cleft palate. Nasalance score in Ah sound, Ma phrase, and Pa phrase decreased 20.23%, 3.25%, and 23.26% in the average, respectively. As a result, hypernasality improved significantly. Closure rate in velum evaluated by endoscopy was increased from 0.44 to 0.76. In objective satisfactory scale checked by each patient's guardian at the postoperative period, much improved in 3, improved in 6, minimally improved in 1, and no difference in 1 was reported. (n=11 patients) Double opposing B-plasty is a good surgical modality in patients accompanying VPI with submucous cleft palate or incomplete cleft palate and will be used more usefully and widely.

The Phonetic Characteristics and Voice Handicap Index in Allergic Rhinitis Patients (알레르기성 비염 환자들의 음향음성학적 특성 및 음성장애지수)

  • Background and Objectives: There are few studies reported that specifically examine the phonetic characteristics and voice handicap index (VHI) in patients with Allergic Rhinitis. This study was designed to examine phonetic characteristics and VHI in adult patients with allergic rhinitis. Materials and Methods: Forty-two male patients diagnosed as allergic rhinitis were given skin-prick test and others, aged from 20 to 56 years, and were compared with a 16 male control group with no pathology and in the same age group. The VHI was used to measure the changes of patient's perception. Acoustic and aerodynamic analysis test were done, and a nasalance test performed to measure rabbit, baby, and mother passage. Acoustic rhionometry (AR) was performed to evaluate nasal volume and nasal crosssectional area. Statistical analysis was done using independent sample t-test. Results: VHI showed significantly different score in the studied group, higher than that of control group. AR graph showed that there was no significant differences of nasal volume and nasal cross-sectional area. The Shimmer and SFF value in the group of allergic patients were higher than in the control group. MPT value in the group of allergic patients was lower than in the control group. Nasalance in allergic patients showed hypernasality all passage. Conclusion: We suggest that patients with allergic rhinitis have considerable voice problems. Most of them have hypernasality, which may be a compensatory mechanism by nasal obstruction.

Research on Construction of the Korean Speech Corpus in Patient with Velopharyngeal Insufficiency (구개인두부전증 환자의 한국어 음성 코퍼스 구축 방안 연구)

  • Background and Objectives We aimed to develop a Korean version of the velopharyngeal insufficiency (VPI) speech corpus system. Subjects and Method After developing a 3-channel simultaneous speech recording device capable of recording nasal/oral and normal compound speech separately, voice data were collected from VPI patients aged more than 10 years with/without the history of operation or prior speech therapy. This was compared to a control group for which VPI was simulated by using a french-3 nelaton tube inserted via both nostril through nasopharynx and pulling the soft palate anteriorly in varying degrees. The study consisted of three transcriptors: a speech therapist transcribed the voice file into text, a second transcriptor graded speech intelligibility and severity and the third tagged the types and onset times of misarticulation. The database were composed of three main tables regarding (1) speaker's demographics, (2) condition of the recording system and (3) transcripts. All of these were interfaced with the Praat voice analysis program, which enables the user to extract exact transcribed phrases for analysis. Results In the simulated VPI group, the higher the severity of VPI, the higher the nasalance score was obtained. In addition, we could verify the vocal energy that characterizes hypernasality and compensation in nasal/oral and compound sounds spoken by VPI patients as opposed to that characgerizes the normal control group. Conclusion With the Korean version of VPI speech corpus system, patients' common difficulties and speech tendencies in articulation can be objectively evaluated. Comparing these data with those of the normal voice, mispronunciation and dysarticulation of patients with VPI can be corrected.
