Effects of nasopharyngeal microbiota in respiratory infections and allergies

  • The human microbiome, which consists of a collective cluster of commensal, symbiotic, and pathogenic microorganisms living in the human body, plays a key role in host health and immunity. The human nasal cavity harbors commensal bacteria that suppress the colonization of opportunistic pathogens. However, dysbiosis of the nasal microbial community is associated with many diseases, such as acute respiratory infections including otitis media, sinusitis and bronchitis and allergic respiratory diseases including asthma. The nasopharyngeal acquisition of pneumococcus, which exists as a pathobiont in the nasal cavity, is the initial step in virtually all pneumococcal diseases. Although the factors influencing nasal colonization and elimination are not fully understood, the adhesion of opportunistic pathogens to nasopharyngeal mucosa receptors and the eliciting of immune responses in the host are implicated in addition to bacterial microbiota properties and colonization resistance dynamics. Probiotics or synbiotic interventions may show promising and effective roles in the adjunctive treatment of dysbiosis; however, more studies are needed to characterize how these interventions can be applied in clinical practice in the future.

비강질환에 대한 향기흡입과 He-Ne 레이저 병행 치료의 효과 및 예후 (Effect and Prognosis of Combined Essential Oil Inhalation and He-Ne Laser Treatment of Nasal Diseases)

  • Objective : Essential oil inhalation and He-Ne laser treatment has become increasingly available in children with nasal diseases. But the reports on the effect and prognosis have been poor. In this study, we analyzed the treatment effect, recurrence rate and the degree of recurrent symptoms. Materials/Methods: 23 children(2-10 year-old) with nasal diseases, who received essential oil inhalation with He-Ne laser treatment at least 10 times in our hospital from January 1st 2001 to December 31th 2002 were studied. Result : Ten cases are male & thirteen cases are female. The average of age was 4.91 year-old. The most frequency symptoms are nasal discharge (82.6%), nasal obstruction (78.3%), cough (47.8%), secretion sound (39.1%), sneezing (17.4%). Average numbers of treatment period, interval and treatment times are 71.79 days, 4.06 days, 18.13 times respectively. Of the 23 children, 14 children were in 'all symptom improved' at the remedial ends, and 22 children relapsed. In the degree of recurrent symptoms, 15 children were in 'more improved then before-treatment and not equal to directly remedial ends'. No case was in 'worse than before-treatment'. The final result of treatment, 6 children keep the degree of remedial ends, on the other hand 17 children are not. Conclusions : Even though, the recurrence rate was high, this combined treatment improve nasal symptoms. The final assessment is good, when the treatment-result is good, when the treatment interval is regular and when parent's are cooperative. If the symptoms continue a whole year, the result is not so good.

Investigation on Airflows in Abnormal Nasal Cavity with Adenoid Vegetation by Particle Image Velocimetry

  • Knowledge of airflow characteristics in nasal cavity is essential to understand the physiological and pathological aspects of nasal breathing. Several studies have utilized physical models of the healthy nasal cavity to investigate the relationship between nasal anatomy and airflow. Since the final goal of these works is their contribution to the diagnosisand treatment of nasal diseases, the next step on this topic is naturally studies for disordered nasal cavities. In this paper, as the first application, airflows in the normal and abnormal nasal cavities with adenoid vegetation are investigated experimentally by PIV, and comparisons of both cases are appreciated. Dense CT data and careful treatment of model surface under the ENT doctor's advice provide more sophisticatedcavity model. The CBC PIV algorithm with window offset is used for PIV flow analysis. Average and RMS distributions are obtained for inspirational and expirational nasal airflows. Airflow characteristics that are related with the abnormalities in nasal cavity are presented.

Nasal airflows in deformed nasal cavity models

  • Several studies have utilized physical models of the healthy nasal cavity to investigate the relationship between nasal anatomy and airflow. With our experiences of experimental investigations on nasal airflows in normal and abnormal nasal cavity models, we are going to deal with the topic that may contribute to the diagnosis and treatment of nasal diseases. In this paper, airflows in the normal and artificially deformed models, which simulate surgical treatment, are investigated experimentally by PIV. High-resolution CT data and careful surface rendering of computational model with the help of the ENT doctor provide more sophisticated nasal cavity models. The CBC PIV (Correlation Based Correction PIV) algorithm with window offset is used for PIV flow analysis. Average and RMS distributions in sagittal and coronal sections are obtained for inspiratory and expiratory nasal airflows. Comparisons in nasal airflows for both normal and deformed cases are also appreciated. In case of simulations of surgical operations, velocity and RMS distributions in coronal section changes locally, this may cause some difficulties in physiologic functions of noses and may hurt mucosal surface.

코질환과 수면무호흡증 (Nasal Diseases and Its Impact on Sleep Apnea and Snoring)

  • Nasal congestion is one of the most common symptoms of medical complaints. Snoring is caused by vibration of the uvula and the soft palate. Nasal obstruction may contribute not only to snoring and obstructive sleep apnea (OSA) but also impair application of continuous nasal positive airway pressure (CPAP), which is the most widely employed treatment for OSA. Total or near-total nasal obstruction leads to mouth breathing and has been shown to cause increased airway resistance. However, the exact role of the nasal airway in the pathogenesis of OSA is not clear and there is no consensus about the role of nasal obstruction in snoring and sleep apnea. Some reports have failed to demonstrate any correlation between snoring and nasal obstruction. On the other hand, opposing reports suggest that nasal disease may cause sleep disorders and that snoring can be improved after nasoseptal surgery. Reduced cross-sectional area causes increased nasal resistance and predisposes the patient to inspiratory collapse of the oropharynx, hypopharynx, or both. Discrete abnormalities of the nasal airway, such as septal deformities, nasal polyps, and choanal atresia and with certain mucosal conditions such as sinusitis, allergic rhinitis and inferior turbinate hypertrophy can cause snoring or OSA. Thus, these sources of nasal obstruction should be corrected medically or surgically for the effective management of OSA and adjunctive for CPAP.

소아 알레르기 비염으로 인한 구호흡이 악안면 성장에 미치는 영향과 이에 대한 치과적 고려사항 (The effects of nasal breathing due to pediatric allergic rhinitis on dentofacial growth and its dental considerations)

  • Allergic rhinitis is a specific IgE mediated inflammatory disease of the nasal mucosa, characterized by symptoms such as rhinorrhea, nasal congestion, nasal obstruction, nasal and eye itching, and sneezing. The prevalence of allergic rhinitis varies according to country, age, and surveying methods, but it seems to increase worldwide, also in Korea. Prolonged mouth breathing caused by allergic rhinitis can produce muscular and postural alterations, causing alterations on the morphology, position, growth direction of the jaws, and malocclusion. Also, mouth breathing leads to dryness of the mouth, causing various oral diseases; gingivitis, halitosis, inflammation of tonsil, increased risk of dental caries and dental erosion. In dental clinic, using rapid maxillary expansion to persistent allergic rhinitis patients with narrow maxilla can enlarge maxillary dental arch and nasal cavity anatomy, improving nasal breathing and reducing nasal cavity resistance. However, it is desirable to use along with otolaryngologic treatment. Dentists should be aware of the characteristics of allergic rhinitis and its effects on patients, and consider when planning dental treatment.

알레르기 치료를 이용한 비용증을 동반한 부비동염의 치험 2례 (Two Cured Cases of Nasal Polyposis Combiend with Chronic Sinusitis by Allergy Therapy)

  • E. N. T. Clinic에 내원하는 환자중 nasal symptoms을 호소하는 환자의 높은 비율에서 알레르기 질환이 있는 것을 느끼게 되며 특히 nasal cavity내에 오는 질환의 원인 중 알레르기가 놀라울 정도로 많은 것을 생각하면 이비인후과 영역에서의 알레르기에 대한 관심도 높다고 하겠읍니다. 일반적으로 비용증, 만성비염. 만성부비동염의 치료 방법으로는 약물요법과 수술요법을 병행하여 시행하는데 치료후 증상이 호전되다가 다시 계속적인 치료전의 증상이 재발되는 경우를 흔히 경험하게 된다. 특히 비용증 환자의 치료에서는 높은율의 재발을 경험하게 되는데 nasal symptoms을 일으키는 질환의 많은 원인이 allergy 라는데 착안하여 allegy symptoms은 호소하지 않았으나 비용증을 동반한 만성부비동염 환자에 수술 및 일반치료를 시행하였으나 얼마후 증상이 재현되어 Rinkels technique의 allergy test를 시행 하였다. 특정한 allergen에 양성을 보이지는 않았으나 일반적으로 nasal allergy의 많은 원인이 되는 House dust와 Mold groups의 allergen을 이용 (#2 ∼ #3 Solution)하여 계속적인 Desensitization을 시행함으로서 근치에 가까운 효과를 얻었기에 문헌고찰과 함께 보고하는 바이며 이러한 환자들에게 계속적인 allergy test상 양성반응을 보이지 않더라도 위의 방법을 적용하여 치료한 결과를 추후 발표드릴 것을 약속드립니다.

폐쇄형 수면무호흡증후군 환자에서 Nasal CPAP의 치료 효과 (The Effect of Nasal CPAP in Obstructive Sleep Apnea Syndrome)

  • 연구배경 : 수면무호흡증후군은 성인남자의 1~4%에서 발생되는 비교적 흔한 질환으로 적절한 치료가 이루어지지 않을시 고혈압, 야간 심부정맥, 주간과다졸음, 인지장애 등의 심각한 합병증을 일으킬 수 있다. 폐쇄형 수면무호흡증후군에 대한 치료로서 여러가지 방법들이 개발되어 왔는데 그중에서도 1981년 Sullivan 등에 의해 개발된 nasal CPAP이 세계적으로 널리 사용되어 현재 폐쇄형 수면무호흡증후군의 1차적 치료로 이용되고 있다. 그러나 이에 대한 국내보고가 미비해 저자들은 폐쇄형 수면무호흡증후군 환자에서의 nasal CPAP의 치료효과와 nasal CPAP 사용에 따르는 문제점을 알아 보고자 본 연구를 시행 하였다. 방법 : 수면검사를 통하여 폐쇄형 수면무호흡증후군이 확인된 20명의 환자를 대상으로 nasal CPAP 착용시의 수면검사를 실시하여 수면중의 호흡변화를 관찰하였고, 환자와의 면담을 통하여 nasal CPAP 착용에 따르는 문제점들을 알아 보았다. 결과: 1) 대상환자중 무호흡지수가 nasal CPAP 전보다 유의한 감소를 보인 환자가 17명(85%) 이었고, nasal CPAP 전보다 오히려 증가한 환자는 3명(15%) 이었다. 2) 대상환자의 평균 무호흡지수는 nasal CPAP 전의 $34.1{\pm}18.9$/시간에서 nasal CPAP 후의 $15.4{\pm}10.3$/시간으로 통계적으로 유의한 감소가 있었다(p<0.01). 3) 대상환자의 전체 수면시간에 대한 총무호흡시간은 nasal CPAP 전의 $28.5{\pm}16.0%$에서 nasal CPAP의 $11.9{\pm}9.3%$로 통계적으로 유의한 감소가 있었다(p<0.05). 4) 대상환자의 최장 무호흡시간과 무호흡중 최저산소포화도는 nasal CPAP 후에 의미 있는 변화가 없었다. 5) nasal CPAP의 부작용으로는 마스크착용에 대한 불편감이 16명(80%)으로 가장 많았고, 비강의 건조함이 13명(65%)으로 나타나 nasal CPAP의 장기적 사용에 있어 해결되어야 할 정도로 지적 되었다. 결론 : 이상의 결과로 보아 nasal CPAP이 폐쇄형 수면무호흡의 치료에 유용한 것으로 나타났으며, 향후 장기간의 추적을 통한 nasal CPAP 의 효과와 nasal CPAP의 부작용의 개선을 통한 환자의 순응도 제고에 대한 연구가 더 이루어져야 할 것으로 생각된다.
