하비갑개 수술 후 비강 모델 내의 세부 유동장의 실험 및 전산 유동가시화 (Experimental and Numerical Flow Visualization on Detailed Flow Field in the Post-surgery Models for the Simulation of the Inferior Turbinectomy)

  • Three major physiological functions of nose can be described as air-conditioning, filtering and smelling. Detailed knowledge of airflow characteristics in nasal cavities is essential to understanding of the physiological and pathological aspects of nasal breathing. In our laboratory, a series of experimental investigations have been conducted on the airflows in normal and abnormal nasal cavity models by means of PIV under both constant and periodic flow conditions. In this work, more specifically experimental and numerical results on the surgically modified inferior turbinate model were presented. With the high resolution CT data and a careful treatment of the model surface under the ENT doctor's advice yielded quite sophisticated cavity models for the PIV experiment. Physiological nature of the airflow was discussed in terms of velocity distribution and vortical structure for constant inspirational flow. Since the inferior and middle turbinate are key determinants of nasal airflow, the turbinectomy obviously altered the main stream direction. This phenomenon may cause local changes in physiological function and the flow resistance.

비강 내 약물 도포가 병원 감염 예방에 미치는 효과 (The Effects of a Mupirocin Smear in the Nasal Cavity Against Nosocomial Infections)

  • This study was done to analyze the effects of a smear in the nasal cavity against nosocomial Infection. The smear used was mupirocin, and the study centered on infection which constitutes the majority of nosocomial infections called MRSA. The data were collected between March 23 1998 and June 31 of the same year in a university hospital in the Kyongi Province, and is made up of a experimental group of 14 patients who were given nasal cavity smears and a control group of 16 patients who were not given nasal cavity smears. The data were analyzed through frequency and the Chi-square tests and gave forth these results. 1. Of the experimental group 28.6% developed nosocomial infections while 62.5% of the control group developed infections. This difference was significant. 2. In the experimental group, all of the patients developed infections within the first week in the ICU, while 80% of the control group developed infections in the first week and 20% in the second week. The difference was not as marked here. 3. In the experimental group the DM group 66.7% contracted MRSA while 18.2% developed it in the non-DM group showing that the DM group had infection rate was 3.7 times higher than the non-DM groups. In the control group the DM group had a 100% infection rate while 50% of the non-DM group developed it. Overall the DM group's rate infection was 2.4 times higher than the non-DM group. 4. In the experimental group, 37.55% of the patients who had a tracheostomy developed it while 16.7% of the patients who did not have a tracheostomy developed infections. In the control group, 62.5% of the patients who had tracheostomy, and 37.5% of the patients who did not have tracheostomies developed infections. Those who had tracheostomies, and the control group had double the rate contracting infections. From these results we can see that nasal cavity smears are effective against nosocomial infections. In spite of the smears, patients with the diabetes mellitus had a high MRSA infection rate, which requires new alternative treatments.

비강 내 이물에 의한 만성 비염 치료를 위해 비강절개술 적용 증례 (Rhinotomy for Chronic Rhinitis by Nasal Foreign Body in a Dog)

  • 5 year-old female Siberian husky which was 27 kg had presented with a recurrent rhinitis with chronic discharge and cough. A nasal foreign material had been suggested by a finding of a bone density ($0.3{\times}0.3$ cm) in the left nasal cavity on X-ray and CT-scanning. Soft tissue opacity in frontal sinus and nasal cavity was increased and foreign material was located beside turbinate bone in the left nasal. We found that there was the increase in the number of eosinophil and mast cell by the nasal cytology test. These results mentioned above indicated that the rhinitis by nasal foreign body was suspicious. We decided that the transfrontal rhinotomy could be the proper procedure to approach the material in this case. After rhinotomy, the foreign body and severe sticky discharge were removed. Drain was placed through the hole and into the frontal sinus and nasal cavity which were flushed two times a day for 7 days. The clinical signs such as cough and nasal discharge were shown to be improved in the every visiting for the re-check. On the $40^{th}$ day after surgery, we could confirm that the most of soft tissue density in the frontal sinus and nasal cavity was decreased by CT-scanning. However, foreign body was not identified by histological examination. For the treatment of chronic rhinitis caused by foreign body, the surgical method such as rhinotomy can be applied, when it is difficult to remove it in the guide of the nasal endoscope.

Rhinolith: An incidental radiographic finding

  • Rhinoliths are foreign bodies composed of mineralised deposits in the nasal cavity that are rarely encountered in routine dental practice. The current report presents a case of a 77-year-old female patient who visited a dental treatment centre for a routine dental check-up and prophylaxis. On the clinical examination, the patient did not report any breathing difficulty, nasal obstruction, or any form of discharge. Panoramic radiography showed a horizontally magnified radiopaque structure in the right nasal fossa region. Cone-beam computed tomography (CBCT) revealed a hyperdense entity with a laminated presentation in the right nasal fossa. The hyperdense entity was diagnosed as a rhinolith based on the radiographic findings. Although the patient was asymptomatic, a referral was made to an ear, nose, and throat surgeon for a further evaluation. This report highlights the importance of CBCT imaging in the diagnosis of soft-tissue calcifications.

소아 알레르기 비염으로 인한 구호흡이 악안면 성장에 미치는 영향과 이에 대한 치과적 고려사항 (The effects of nasal breathing due to pediatric allergic rhinitis on dentofacial growth and its dental considerations)

  • Allergic rhinitis is a specific IgE mediated inflammatory disease of the nasal mucosa, characterized by symptoms such as rhinorrhea, nasal congestion, nasal obstruction, nasal and eye itching, and sneezing. The prevalence of allergic rhinitis varies according to country, age, and surveying methods, but it seems to increase worldwide, also in Korea. Prolonged mouth breathing caused by allergic rhinitis can produce muscular and postural alterations, causing alterations on the morphology, position, growth direction of the jaws, and malocclusion. Also, mouth breathing leads to dryness of the mouth, causing various oral diseases; gingivitis, halitosis, inflammation of tonsil, increased risk of dental caries and dental erosion. In dental clinic, using rapid maxillary expansion to persistent allergic rhinitis patients with narrow maxilla can enlarge maxillary dental arch and nasal cavity anatomy, improving nasal breathing and reducing nasal cavity resistance. However, it is desirable to use along with otolaryngologic treatment. Dentists should be aware of the characteristics of allergic rhinitis and its effects on patients, and consider when planning dental treatment.

개와 고양이에서 컴퓨터단층촬영술을 이용한 비강의 염증성 질병 평가 (Evaluation of Inflammatory Disease in the Nasal Cavity and Paranasal Sinuses using CT in Dogs and Cats)

  • 만성의 비강 삼출물 및 비출혈로 내원한 5마리의 개와 2마리의 고양이가 내원하여 일반 방사선 검사 및 컴퓨터단층촬영 (CT), 비강의 세포학적 검사와 미생물학적 검사를 실시하여 비염으로 진단한 증례를 선정하여 이의 CT 영상의 특징을 비교 평가하였다. 비강의 일반 방사선 검사에서 3 증례에서만 비강의 비정상 소견을 확인하였다. CT 검사 결과, 비강 (7증례) 및 전두동 (3 증례), 비인두 (2증례)에서 공동을 갖는 isodense한 병변을 확인하였으며 비중격의 소실 (4 증례), 비갑개의 파괴 (5 증례), 상악 (3 증례) 또는 경구개 (3 증례)의 골융해 소견이 관찰되었다. 병변의 조영증강효과는 5 증례에서 확인하였다. CT는 비강의 질병을 평가하고 특히 병변의 범위를 결정하는데 매우 유용하다.

두개강 공명의 영상해부학적 고찰 (The imaging Anatomical Consideration of the Resonance of the Cranial Cavity)

  • Because vocal technique is the basis of singing a song beautifully, so this study was undertaken to use the scientific and correct technique in order to get much better musical expressions. Shimadzu X - ray remote control TV system was used for checking the supporting state of diaphragmatic respiration after 5, 10, 15, 20, 25 and 30 sec during phonation in the state of full inhalation between professional singer and non - professional singer. Shimadzu Magnet $nex-{\alpha}$(SMT -50CX/H) was used for examining the scattering state from the resonance of nasal cavity to that of cranial cavity on the basis of diaphragmatic respiration. The results obtained were summerized as follows : 1. The resonance of cranial cavity must be scattered by the energy of diaphragmatic respiration after gathering the foci of the fundamental 5 vowels. 2. while raising the epiglottis, the resonance of nasal cavity must be clearly in order to maintain the resonance of cranial cavity beautifully. 3. We can get musical expressions by maintaining the elasticity of diaphragmatic respiration.

전산화단층영상에서 절치관 및 순측 치조골 폭경에 관한 연구 (The width of the incisive canal and labial alveolar bone of the incisive canal: an assessment on CT images)

  • Purpose : To assess the width of the labial alveolar bone of the incisive canal and the width of the incisive canal on spiral computed tomographic images of the anterior portion of the maxilla. Materials and Methods : Study materials included 38 CT scans taken for preoperative planning of implant placement. Axial cross-sectioned image entirely showing the incisive canal was selected and scanned with 600 DPI resolution. The width of the labial alveolar bone of the incisive canal at an orifice to the oral cavity, middle portion, and an orifice to the nasal cavity and the diameter of the incisive canal at the middle portion were determined by two specialist using Digora for Windows 2.1 The statistical analyses were carried out using SPSS 12.0.1. Results : When the maxillary central incisors remained, the mean labial alveolar bone width were $6.81{\pm}1.41mm,\;6.46{\pm}1.33mm$, and $7.91{\pm}1.33mm$. When the maxillary central incisors were missed the mean width were $5.42{\pm}2.20mm,\;6.23{\pm}2.29mm$, and $7.89{\pm}2.13mm$. Conclusions : The labial alveolar bone width at middle portion and an orifice to the nasal cavity were of no statistical significant difference according to presence of the maxillary central incisors (P>0.05). The width between oral cavity and nasal cavity, middle portion and to nasal cavity revealed statistically significant difference (P<0.05).

두경부에 다중전이된 신세포암 1례 (A Case of Multiple Head and Neck Metastases of Renal Cell Carcinoma)

  • Renal cell carcinoma is the third most common metastatic tumor to the bone and soft tissues of the head and neck. The common sites of metastatic renal cell carcinoma in head and neck region are nasal cavity, paranasal sinuses, oral mucosa, gingiva, tongue, palate, lip as the favored site. The present paper deals with one patient with metachronous oral tongue, nasal cavity and suspicious brain metastases after 2 years of renal cell carcinoma nephrectomy. Also, the patient had history of total thyroidectomy for thyroid follicular carcinoma. Total excision of nasal cavity and tongue mass were performed. Therapeutic aspects are briefly reviewed in literature.

A Case of Nasal Diffuse Large B-Cell Lymphoma in a Cat

  • A 6-year-old spayed female Turkish Angora cat presented with sneezing, nasal discharge, and decreased appetite lasting for 21 days. Skull radiography revealed slightly increased density of soft tissue in the left nasal cavity. Computed tomography (CT) scan revealed an extensive mass with nasal septum destruction and moderate contrast enhancement in the left nasal cavity. After surgical biopsy, histopathological examination confirmed that the mass was an infiltrative round cell neoplasm, composed of sheets of large neoplastic cells. Immunohistochemical analysis revealed that most of the neoplastic cells were strongly positive for CD79a and weakly positive for PAX5. Additionally, numerous mature lymphocytes were found to be positive for CD3. This is the first reported case of nasal diffuse large B-cell lymphoma (DLBCL) in a Turkish Angora cat in Korea.
