우울증 시조치료 방법론 모색 -이지엽 시조를 중심으로- (A Study on the Existential Reflection -A Study on the Yi Ji-yeop Sijo-)

  • 본 연구는 시조치료 관점과 의힉적 측면에서 우울증 극복을 위한 존재 성찰과 초월성을 탐색하는 것이다. 우리나라 자살률은 OECD 국가 중 1위(2006년~2019년)인데 최근에는 우울증이 더욱 확산되는 추세에 있다. 우울증은 급격한 산업사회의 경쟁 속에서 물질만능주의 폐단과 연결된 문명병이며 약물치료만으로 우울증을 극복하기에는 한계가 있다. 이 연구는 우울증을 개인사적으로 규정하고 우울증을 문학치료학의 일환으로 우울증을 새롭게 인식하고 그 극복 방안을 탐색하는 것을 목표로 한다. 이에 본 연구는 한국문학치료학의 '자기서사'를 활용하고 논증들을 시조치료시각에서 재조명하려는 시도다. 시조치료란 시조작품을 매개로 인간 내면에 억압된 감정을 끌어내는 것이다. 이는 분열된 자아를 통합하고 억압된 감정에서 해방하여 자유로운 삶을 추구하는 데 의의가 있다.

Gosijo's Literature Physiology Formed by Question

  • Sometimes, literature therapy is done by literature question. Participants naturally get the effect of literature therapy depending on when and what questions we ask. This study aims to lead the discussion of Gosijo's literature physiology ignited by the question. Gosijo, the subject of the study, described the depressed present state of the poetic narrator in the first and second line. By the way, poetic narrator asked a question in the first phrase of the last line and led the action potential. And in the second phrase of the last line, the poetic narrator called the code of sadness and the sadness code came. We have plotted this as Emotion Codon. The result of Emotion Codon at this time was that the narrative of Gosijo ignites the literature therapy mechanism through sadness.

Effective and appropriate use of weight loss medication in pediatric obesity: a narrative review

  • Over the past few decades, there has been a notable increase in the incidence of pediatric obesity, which is a significant public health concern. Children who are obese have a greater risk of type 2 diabetes, hypertension, dyslipidemia, polycystic ovary syndrome, obstructive sleep apnea, and adult obesity. Lifestyle modification therapy is typically the initial approach to treat pediatric obesity. For patients who do not achieve success with lifestyle modification therapy alone, pharmacotherapy is the next logical treatment option. When selecting an anti-obesity medication (AOM), it is essential to first ascertain the medical background of the patient, including current medications and obesity-associated comorbidities. Evaluation of obesity phenotypes in patients may also be beneficial. AOMs for pediatric obesity include metformin, orlistat, glucagon-like peptide 1 agonists, phentermine, and the phentermine/topiramate combination. Sufficient lifestyle modification therapy should be administered before considering pharmacotherapy and continued after the initiation of AOM. To ensure healthy development, monitoring growth and puberty development during anti-obesity treatments is essential.

GIM(Guided Imagery and Music)에서 GIM 치료사의 음악 선택에 관한 내러티브 문헌 고찰 (Music Choices of GIM(Guided Imagery and Music) Therapist in GIM: Narrative Literature Review)

  • 본 연구는 GIM 음악치료사의 GIM 치료에서의 음악선택에 대하여 고찰하고자 하는 것이다. 학술지 데이터베이스 7개와 음악치료 전문학술지 9개를 대상으로 1964년부터 2013년까지의 연구를 검색하였다. 검색어는 "Guided Imagery and Music", "GIM", "Helen Bonny", "Bonny method", "Music and Imagery", "BMGIM"였다. 초기 검색된 모든 연구의 초록을 확인하여 1차 선정된 126개의 연구에서 음악선택이 명시된 12개의 연구를 최종 선택하여 내러티브 문헌 고찰을 하였다. 12개의 연구에 대한 문헌고찰 결과 GIM 치료사들은 내담자의 이슈와 관련된 기분, 정서, 느낌의 특색(quality), 치료에서 다루어지는 내담자의 주제(theme)와 연관성이 있는 음악을 선택하였다. 이중 특히 5개의 연구에서는 내담자의 상태(state)와 연관 지을 수 있는 음악을 여러 음악요소를 분석적으로 고려하여 선택함으로써 보다 개연성을 가진 음악선택의 이유를 확인할 수 있었다. 문헌고찰을 위해 선택된 126개의 연구 대부분은 사례연구로 연구자 고유의 관점으로 질적으로 기술하고 있어 연구들 간의 공통점을 도출하는 것에는 한계가 있었다. 본 문헌고찰 연구를 통해 음악중심의 심층 음악심리치료기법인 GIM에서 음악선택에 대한 임상적 근거 마련을 위해 보다 기초적인 연구가 필요함을 확인할 수 있었다.

Comprehensive Physiotherapy Approaches for Children With Cerebral Palsy: Overview and Contemporary Trends

  • Cerebral palsy (CP) is a prevalent neurodevelopmental disorder characterized by motor and postural impairments caused by central nervous system dysfunction. It significantly impacts children's daily functioning and quality of life. Physical therapy is a crucial intervention for children with CP that aims to improve motor skills and functional abilities. This study aimed to provide a comprehensive overview of holistic physical therapy approaches methods specifically designed for children with CP and examine recent research trends and their implications for optimizing outcomes in this population. This study employed a narrative review approach, conducting a comprehensive examination of the current literature pertaining to physical therapy methods for children with CP. The review encompassed studies exploring assessment techniques, evidence-based interventions, and innovative approaches in the field. It was discerned that encompassing physical therapy strategies, which encompass individualized treatment plans, evidence-based interventions, and the integration of innovative techniques, yield a favorable influence on the motor skills and functional capacities of children with CP. This review synthesizes the current knowledge on effective physical therapeutic strategies for children with CP. Furthermore, this review highlights the need for continued research and innovation in the field of pediatric physical therapy for CP.

Current diagnosis and treatment of vestibular neuritis: a narrative review

  • Vertigo is the sensation of self-motion of the head or body when no self-motion is occurring or the sensation of distorted self-motion during an otherwise normal head movement. Representative peripheral vertigo disorders include benign paroxysmal positional vertigo, Ménière disease, and vestibular neuritis. Vestibular neuritis, also known as vestibular neuronitis, is the third most common peripheral vestibular disorder after benign paroxysmal positional vertigo and Ménière disease. The cause of vestibular neuritis remains unclear. However, a viral infection of the vestibular nerve or ischemia of the anterior vestibular artery is known to cause vestibular neuritis. In addition, recent studies on immune-mediated mechanisms as the cause of vestibular neuritis have been reported. The characteristic clinical features of vestibular neuritis are abrupt true-whirling vertigo lasting for more than 24 hours, and no presence of cochlear symptoms and other neurological symptoms and signs. To accurately diagnose vestibular neuritis, various diagnostic tests such as the head impulse test, bithermal caloric test, and vestibular-evoked myogenic potential test are conducted. Various treatments for vestibular neuritis have been reported, which are largely divided into symptomatic therapy, specific drug therapy, and vestibular rehabilitation therapy. Symptomatic therapies include generalized supportive care and administration of vestibular suppressants and antiemetics. Specific drug therapies include steroid therapy, antiviral therapy, and vasodilator therapy. Vestibular rehabilitation therapies include generalized vestibular and customized vestibular exercises.

Clinical Consideration of Trigger Point Injection/Dry Needling Therapy: A Narrative Review

  • Myogenous temporomandibular disorder is a collective term for pathologic conditions of the masticatory muscles, mainly characterized by pain and dysfunction associated with various pathophysiological processes. Among the subtypes of myogenous temporomandibular disorder, myofascial pain is one of the most common muscle disorders, characterized by the presence of trigger points (TrPs). Various modalities, such as ultrasound, manipulative therapy, spray-and-stretch technique, transcutaneous electrical nerve stimulation, injection/dry needling, and low-level laser therapy are used to inactivate TrPs. Needling/injection on the TrPs is one of the most common treatments for myofascial pain. Despite the evidence, there is continued controversy over defining the biological and clinical characteristics of TrPs and the efficacy of injection/dry needling. This review discusses the current concept of injection/needling to relieve TrPs.

삼대록계 국문 장편소설에 나타난 가족치료양상 연구 - 보웬의 이론에 근거하여 - (Three-generation stories of the Joseon Dynasty, A Study on the Aspects of Family Therapy)

  • 이 논문에서는 보웬의 가족치료 이론의 관점에서 삼대록계 국문 장편소설의 서사를 분석하였다. 보웬은 가족 내에서 문제를 일으키는 개인의 행동을 설명함에 있어 자기분화와 불안이라는 두 변인을 사용하였는데 이들 작품에서 문제나 갈등이 표출된 가정의 경우, 문제의 중심에 자아 미분화된 인물이 존재하였다. 자아가 미분화된 인물들은 적장자 위주의 가족 관계에서 소외될 때 극도로 불안감을 느꼈으며, 불안을 해소하고 가족 관계에서 자신의 입지를 확고히 하기 위해 자아 미분화된 인물들끼리 연대의식을 강하게 발휘하였다. "소현성록"과 "조씨삼대록" 연작의 가정 내 문제 발생의 구조와 양상은, '자아 미분화된 어머니와 자아 분화된 아들, 자아 분화된 어머니와 자아 미분화된 아들, 자아 미분화된 어머니와 자아 미분화된 아들'로 세분화할 수 있다. 서사 전개 방식에 있어 이들 간의 공생 관계 성립과 미성립, 치료자의 존재 양상 등이 달랐으며, 그에 따라 자결하는 아들, 처형당하는 아들, 회개를 통해 거듭나는 아들로 나뉘어졌다. 서술상의 전개나 구조는 무엇보다 치료자 존재 유무에 따라 달라졌다. 자아 미분화된 어머니와 자아 미분화된 아들의 경우, 가족 내에 치료자가 존재함으로써 가족문제는 해결되고, 문제를 일으킨 아들은 회개하여 거듭나는 아들이 된다. 그러나 자아 미분화된 어머니와 자아 분화된 아들, 자아 분화된 어머니와 자아 미분화된 아들은 가정 내 치료자 부재로 각각 아들이 자결하고 처형되는 비극이 빚어졌다. 이처럼 보웬의 가족 치료 이론을 적용하여 삼대록계 국문 장편소설을 분석했을 때 가정에서 발생한 갈등이나 문제에 대해 개인의 차원이 아니라 가족 간의 관계 차원에서 조망이 가능하였다. 즉 가정 내의 심각한 갈등과 문제는 어느 한 개인의 인성이나 욕망이 아니라 가족 제도 자체에 근본 원인이 있었다. 그리고 가족 치료자의 유무에 따라 서사의 전개 양상이 크게 달라졌는데 그의 역할에 따라 나머지 가족의 안위와 평화 여부가 결정됨으로써 삼대록계 국문 장편소설에서 남성의 수신은 중요한 서사축을 형성하고 있음도 알 수 있었다. 이처럼 삼대록계 국문 장편소설 속의 문제 상황을 한 개인의 욕망 혹은 인성에 국한하지 않고 가족 전체의 차원에서 살피고 심리학적 외재적 접근을 통하여 삼대록계 국문 장편소설의 주요 서사인 여성수난담의 배경에 대해 접근한 것에 의의를 두고자 한다.

Effectiveness of Cognitive Behavioral Therapy Techniques for Control of Pain in Lung Cancer Patients: An Integrated Review

  • Background: Experience of lung cancer includes negative impacts on both physical and psychological health. Pain is one of the negative experiences of lung cancer. Cognitive behavioral therapy techniques are often recommended as treatments for lung cancer pain. The objective of this review was to synthesize the evidence on the effectiveness of cognitive behavioral therapy techniques in treating lung cancer pain. This review considered studies that included lung cancer patients who were required to 1) be at least 18 years old; 2) speak and read English or Thai; 3) have a life expectancy of at least two months; 4) experience daily cancer pain requiring an opioid medication; 5) have a positive response to opioid medication; 6) have "average or usual" pain between 4 and 7 on a scale of 0-10 for the day before the clinic visit or for a typical day; and 7) able to participate in a pain evaluation and treatment program. This review considered studies to examine interventions for use in treatment of pain in lung cancer patients, including: biofeedback, cognitive/attentional distraction, imagery, hypnosis, and meditation. Any randomized controlled trials (RCTs) that examined cognitive behavioral therapy techniques for pain specifically in lung cancer patients were included. In the absence of RCTs, quasi-experimental designs were reviewed for possible conclusion in a narrative summary. Outcome measures were pain intensity before and after cognitive behavioural therapy techniques. The search strategy aimed to find both published and unpublished literature. A three-step search was utilised by using identified keywords and text term. An initial limited search of MEDLINE and CINAHL was undertaken followed by analysis of the text words contained in the title and abstract, and of the index terms used to describe the article. A second search using all the identified keywords and index terms was then undertaken across all included databases. Thirdly, the reference list of all identified reports and articles were searched for additional studies. Searches were conducted during January 1991- March 2014 limited to English and Thai languages with no date restriction. Materials and Methods: All studies that met the inclusion criteria were assessed for methodological quality by three reviewers using a standardized critical appraisal tool from the Joanna Briggs Institute (JBI). Three reviewers extracted data independently, using a standardized data extraction tool from the Joanna Briggs Institute (JBI). Ideally for quantitative data meta-analysis was to be conducted where all results were subject to double data entry. Odds ratios (for categorical data) and weighted mean differences (for continuous data) and their 95% confidence intervals were to be calculated for analysis and heterogeneity was to be assessed using the standard Chi-square. Where statistical pooling was not possible the finding were be presented in narrative form. Results: There were no studies located that met the inclusion requirements of this review. There were also no text and opinion pieces that were specific to cognitive behavioral therapy techniques pain and lung cancer patients.Conclusions: There is currently no evidence available to determine the effectiveness of cognitive behavioural therapy techniques for pain in lung cancer patients.

봉침요법(蜂針療法)에 대(對)한 고찰(考察) -월간(月刊) $\ll$양봉계(養蜂界)$\gg$의 '봉침요법(蜂針療法)에 대(對)한 연구(硏究)'를 중심(中心)으로- (A Study on Bee Sting Therapy: Based on'Research on Bee Sting Therapy' of Monthly Yangbonggye)

  • Objectives : To survey concept, meaning, and problems of beekeeper's bee sting therapy by reviewing and summarizing outline, indications, and clinical cases of 'Research on Bee Sting Therapy' described in Monthly Yangbonggye. Methods : As a narrative review, literature researches were carried out based on 'Research on Bee Sting Therapy' in Monthly Yangbonggye in the aspects of outline, indications, and clinical cases. In the concrete, outline was just summarized and described, and indications were categorized by recent version of international statistical classification of diseases and related health problems(ICD) of WHO(World health organization), and clinical cases were collected and summarized from the viewpoint of acupoints and methods of therapy. Results : 1. Bee sting therapy is to inject venom into skin by stimulating affected lesions or acupoints on meridian with sting of bee alive for therapeutic purposes. It can be divided into two classes(direct stimulation(直針法) and indirect stimulation(拔針法)) by the differences of methods, and is different from bee venom acupuncture therapy in the aspects of the way of extraction and injection. 2. In this material, bee sting therapy has 83 indications classified into 17 of 22 chapters of ICD. Among clinical cases, cases on neoplasm, goitre, lump, and haemorrhoids by direct stimulation were especially outstanding. 3. The therapeutic acupoints selected in bee sting therapy are mainly Ashi points(阿是穴), and partly acupoints on meridian(經穴), Extra points(經外奇穴), and New points(新穴) with careful consideration of patients' condition. Conclusions : Although bee sting therapy has more or less controversial points of diagnosis, treatment, and management of side effects, it is thought that the accumulated and inherited experiences from old times can be used as meaningful material by further experimental and clinical researches.
