치과위생사의 재가방문 구강건강관리 중재 활동을 위한 프로토콜 고찰 (Oral health care intervention protocol for older adults at home in dental hygienists: a narrative literature review)

  • Objectives: This study aims to propose a standard protocol for oral health care intervention activities by dental hygienists. Methods: A narrative literature review of home visit oral health care intervention activities reported in Cheonan, South Korea was conducted to enable the proposal of a standard home visit protocol for dental hygienists in the context of community care. Results: Oral health management intervention activities contributed to improving the quality of life, as well as the oral health, of older adults living at home. This was a result of applying a protocol consisting of oral observation, oral massage, expert oral hygiene management, oral muscle function training, and final stages. Conclusions: The visiting oral health intervention protocol was effective in resolving oral health problems of older adults. In the future, customized programs and reimbursement systems should be developed to promote oral health care for older adults that can be provided at home.

  • In adult spinal deformity (ASD) surgery, mechanical failure (MF) has been a significant concern for spine surgeons as well as patients. Despite earnest endeavors to prevent MF, the absence of a definitive consensus persists, owing to the intricate interplay of multifarious factors associated with this complication. Previous approaches centered around global spinal alignment have yielded limited success in entirely forestalling MF. These methodologies, albeit valuable, exhibited limitations by neglecting to encompass global balance and compensatory mechanisms within their purview. In response to this concern, an in-depth comprehension of global balance and compensatory mechanisms emerges as imperative. In this discourse, the center of gravity and the gravity line are gaining attention in recent investigations pertaining to global balance. This narrative review aims to provide an overview of the global balance and a comprehensive understanding of related concepts and knowledge. Moreover, it delves into the clinical ramifications of the contemporary optimal correction paradigm to furnish an encompassing understanding of global balance and the current optimal correction strategies within the context of ASD surgery. By doing so, it endeavors to furnish spine surgeons with a guiding compass, enriching their decision-making process as they navigate the intricate terrain of ASD surgical interventions.

Functional Anatomy of the Temporomandibular Joint and Pathologic Changes in Temporomandibular Disease Progression: A Narrative Review

  • The temporomandibular joint (TMJ) is one of the most unique joints in the human body that orchestrates complex movements across different orthogonal planes and multiple axes of rotation. Comprising the articular eminence of the temporal bone and the condylar process of the mandible, the TMJ integrates five major ligaments, retrodiscal tissues, nerves, and blood and lymph systems to facilitate its function. Cooperation between the contralateral TMJ and masticatory muscles is essential for coordinated serial dynamic functions. During mouth opening, the TMJ exhibits a hinge movement, followed by gliding. The health of the masticatory system, which is intricately linked to chewing, energy intake, and communication, has become increasingly crucial with advancing age, exerting an impact on oral and systemic health and overall quality of life. For individuals to lead a healthy and pain-free life, a comprehensive understanding of the basic anatomy and functional aspects of the TMJ and masticatory muscles is imperative. Temporomandibular disorders (TMDs) encompass a spectrum of diseases and disorders associated with changes in the structure, function, or physiology of the TMJ and masticatory system. Functional and pathological alterations in the TMJ and masticatory muscles can be visualized using various imaging modalities, such as cone-beam computed tomography, magnetic resonance imaging, and bone scans. An exploration of potential pathophysiological mechanisms related to the TMJ anatomy contributes to a comprehensive understanding of TMD and informs targeted treatment strategies. Hence, this narrative review presents insights into the fundamental functional anatomy of the TMJ and pathological changes that evolve with TMD progression.

의과대학생을 위한 죽음학 수업 (Course on Death and Dying for Medical Students)

  • The aim of modern medicine is to prolong life by fighting death. Doctors have traditionally believed that this was an ethical good deed. The negative connotation surrounding death has led to the avoidance of terminally ill patients. But in a modern society where death is medicalized, doctors have to see dying patients every day and are in a state of guilt from implementing meaningless life-sustaining treatments. Therefore, medical schools should allow medical students to embrace a new perspective through death education. Yonsei University Medical College has implemented death education since 2017 as an optional class for first and second year medical students. Students watch videos related to death once a week for 6 weeks and submit their reflections by e-mail. The professor reads the students' reflections and gives them weekly feedback. Through this coursework, students realize that death is not a medical event, but rather a part of life and completion. The ultimate purpose of death education is to transform blind life-absolutist identity into narrative identity.

  • 이 논문은 그레마스의 기호학 방법을 이용해 <겨울왕국>의 서사 분석을 한 기존 논문들의 오류를 지적하고 <겨울왕국>의 심층 의미를 밝히는 것을 목적으로 한다. 기존 연구들은 행위소 모델과 기호사각형에 대한 부정확한 이해를 바탕으로 잘못된 분석을 했다. 그레마스의 방법을 엄밀히 이용한다면 <겨울왕국>은 기존의 연구들에서 주장된 것과는 달리 엘사가 아닌 안나의 영웅 서사임이 드러난다. 서사구조 안에서 안나와 엘사는 서로 대립하는 요소들이기 때문에 안나의 성공은 사실상 엘사의 실패이다. 따라서 <겨울왕국>은 기존의 연구들에서 주장하듯이 남성 지배적 담론에 저항하는 독립적이고 능동적인 여성의 모습을 보여 줌으로써 젠더 재현에 있어서 전복적인 메시지를 전달하는 것이 아니다. <겨울왕국>은 가족 안에서의 사랑을 최고의 가치로 내세우고 공동체가 부여한 역할을 받아들이는 것이 최선이라고 말하면서 기존의 체제 순응적 이데올로기를 재생산하고 있다.

GIM(Guided Imagery and Music)에서 GIM 치료사의 음악 선택에 관한 내러티브 문헌 고찰 (Music Choices of GIM(Guided Imagery and Music) Therapist in GIM: Narrative Literature Review)

  • 본 연구는 GIM 음악치료사의 GIM 치료에서의 음악선택에 대하여 고찰하고자 하는 것이다. 학술지 데이터베이스 7개와 음악치료 전문학술지 9개를 대상으로 1964년부터 2013년까지의 연구를 검색하였다. 검색어는 "Guided Imagery and Music", "GIM", "Helen Bonny", "Bonny method", "Music and Imagery", "BMGIM"였다. 초기 검색된 모든 연구의 초록을 확인하여 1차 선정된 126개의 연구에서 음악선택이 명시된 12개의 연구를 최종 선택하여 내러티브 문헌 고찰을 하였다. 12개의 연구에 대한 문헌고찰 결과 GIM 치료사들은 내담자의 이슈와 관련된 기분, 정서, 느낌의 특색(quality), 치료에서 다루어지는 내담자의 주제(theme)와 연관성이 있는 음악을 선택하였다. 이중 특히 5개의 연구에서는 내담자의 상태(state)와 연관 지을 수 있는 음악을 여러 음악요소를 분석적으로 고려하여 선택함으로써 보다 개연성을 가진 음악선택의 이유를 확인할 수 있었다. 문헌고찰을 위해 선택된 126개의 연구 대부분은 사례연구로 연구자 고유의 관점으로 질적으로 기술하고 있어 연구들 간의 공통점을 도출하는 것에는 한계가 있었다. 본 문헌고찰 연구를 통해 음악중심의 심층 음악심리치료기법인 GIM에서 음악선택에 대한 임상적 근거 마련을 위해 보다 기초적인 연구가 필요함을 확인할 수 있었다.

Effects of the Latest Robotic Horse-riding for Low Back Pain : Narrative Review

  • Purpose: The purpose of this study is to review health-care program using the recent robotic horse-riding technology and its clinical effects for chronic low back pain. Research design, data and methodology: Recent clinical articles were determined under three inclusion criteria for in-depth review: 1) article that is published within 1 year, 2) article that includes the detailed explanation of health-care program using robotic horse-riding, 3) the article that deals with chronic low back pain during more than 6 months. Results: As a result, the finally-determined two articles demonstrated the clinical effects of robotic horse-riding statistically on pain intensity, low back muscle strength, spinal alignment, and fear-avoidance belief. Conclusions: After in-depth review, I concluded that health-care program using robotic horse-riding for chronic low back pain needs to be provided at low-intensity (e.g. less than 6km/h horse walking program) in the beginning of health-care for improving their motor control ability, then, at the increased intensity for strengthening core muscles.

Narrative Review of Pathophysiology and Endoscopic Management of Basivertebral and Sinuvertebral Neuropathy for Chronic Back Pain

  • Chronic lower back pain is a leading cause of disability in musculoskeletal system. Degenerative disc disease is one of the main contributing factor of chronic back pain in the aging population in the world. It is postulated that sinuvertebral nerve and basivertebral nerve main mediator of the nociceptive response in degenerative disc disease as a result of neurotization of sinuvertebral and basivertebral nerve. A review in literature is done on the pathoanatomy, pathophysiology and pain generation pathway in degenerative disc disease and chronic back pain and management strategy is discussed in this review to aid understanding of sinuvertebral and basivertebral neuropathy treatment strategies.

새로운 유형을 포함한 갈락토스혈증의 이해 (The Narrative Review of Galactosemia Including a New Subtype)

  • Galactosemia is an inborn error disorder of carbohydrate metabolism, caused by metabolic disturbances at various stages of the Leloir pathway. In patients with galactosemia, accurate diagnosis and appropriate care are essential to avoid complications and unnecessary treatments. And a careful differential diagnosis of the type of galactosemia is crucial. Even with an appropriate galactose-restricted diet, long-term complications may occur, especially in patients with classic galactosemia. So new treatment options are being developed. In this review, we will review the new symptoms of each subtype that have been reported recently and GALM (Galactose mutarotase) deficiency, a new form of galactosemia, and treatment policies according to recent guidelines.

Role of gene therapy in treatment of cancer for craniofacial regeneration-current molecular strategies, future perspectives, and challenges: a narrative review

  • Gene therapy involves the introduction of foreign genetic material into host tissue to alter the expression of genetic products. Gene therapy represents an opportunity to alter the course of various diseases. Hence, genetic products utilizing safe and reliable vectors with improved biotechnology will play a critical role in the treatment of various diseases in the future. This review summarizes various important vectors for gene therapy along with modern techniques for potential craniofacial regeneration using gene therapy. This review also explains current molecular approaches for the management and treatment of cancer using gene therapy. The existing literature was searched to find studies related to gene therapy and its role in craniofacial regeneration and cancer treatment. Various databases such as PubMed, Science Direct, Scopus, Web of Science, and Google Scholar were searched for English language articles using the keywords "gene therapy," "gene therapy in present scenario," "gene therapy in cancer," "gene therapy and vector," "gene therapy in diseases," and "gene therapy and molecular strategies."
