Drug Resistance and R-Plasmids of Shigella Strains Isolated from Humans, Korea (Shigella균속의 항균제내성 및 전달성 R-Plasmid에 관한 연구)

  • Shigella remains to be an important enteric pathogen in this country for the present. Moreover, most of the isolates have become multiple resistant to various antibiotics which used to be drugs of choice for shigellosis. This study was made as an attempt to assess the present stage of antibiotic resistance and the incidence and transferability of R factors of Shigella. A total of one hundred and seventeen strains of Shigella isolated from patients in Seoul and provincial area between 1982 and 1983 were tested for their resistant to antimicrobial agents and transmission of R-plasmid. Antibiotic susceptibilities were determined by an agar dilution method. Muller hinton agar were used for the assay of drug resistance and tryptic soy broth were used for propagating medium for conjugation. Shigella isolated found to be one or more antibiotics were considered potential donor of R-plasmid. The following results were obtained. 1. Among 117 strains of Shigella isolated, 111 strains(94.9%) were found to be resistant to one or more drugs tested and 97.3% of these resistant strains were multiply resistant, indicating the multiply resistant strains were more than the single resistant strains. Only six strains were susceptible to all drugs tested. 2. Among 117 strains of Shigella isolated, 107 strains(91.5%) were resistant to Tetracyclin(Tc), 106 strains(90.6%) to Chloramphenicol(Cp) and Streptomycin(Sm), 97 strains(82.9%) to Ampicillin(Ap), 68 strains(58.1%) to Cephaloridine(Cr), 10 strains(8.5%) to Nalidixic acid(Na), 5 strains(4.3%) to Kanamycin(Km) and 2 strains(1.7%) to Rifampicin. No strain was resisfant to Amikacin(Ak) and Gentamicin(Gm). 3. All drug-resistant Shigella strains, except three, were multiply resistant to two or more drugs. Fifty eight strains were resistant to five drugs, followed by 26 strains resistant to dour drugs, 12 strains resistant to three drugs and 11 strains resistant to six drugs. 4. The 73% of multiply drug-resistant Shigella transferred their resistance to E. coli by conjugation and the resistance was considered to be mediated by R-plasmid. Resistance to Nalidixic acid and Rifampicin were not transferred by conjugation to recipient. As for the transferability of resistance to each seperate drug, Ap resistance was transferred with 73.2% frequence and Cm and Tc resistance were transferred with approximately 50-60% frequence whereas Sm and Cr resistance were transferred in 19.1-21.4% The other four drugs resistant failed to transfer their resistance to recipient. 5. As for the incidence and transferability of resistance to each seperate drug, the strains resistant to Tc and Cm were encountered most frequently with the rate of 91-92%, whereas transfer of Tc and Cm were low, 51-52%. The incidence of Sm resistance was very high(90.6%) but transferability of drugs resistance was much lower(25.4%). Though the incidence of Km reristance was much lower(4.3%) transferability of Km resistance was considerably higher(60%). 6. The greater the multiplicity of resistance, the greater was the likelihood that part of all of the resistance markers would be transferable.

Molecular Typing of Salmonella enterica serovar Typhi Strains Isolated in Busan by Pulsed-Field Gel Electrophoresis (부산지역에서 분리된 Salmonella enterica serovar Typhi균에 대한 PFGE를 이용한 Molecular typing)

  • We obtained 424 Salmonella enterica serovar Typhi isolates from sporadic cases of infection in Busan during 1996 to 2005. We investigated the trend of antimicrobial resistance and molecular typing by pulsed-field gel electrophoresis (PFGE). Of the total 424 isolates, 6 strains (1.4%) were multidrug-resistant (MDR) S. enterica serovar Typhi isolates, 2 strains (0.5%) were resistant to only nalidixic acid, and the remaining 416 strains (98.1%) were fully susceptible to the 18 antimicrobial agent. PFGE of XbaI-digested chromosomal DNA was performed on 50 sporadic S. enterica serovar Typhi isolates with the objective of investigating the extent of genetic diversity of these isolates in our region. We could find that these isolates were much more heterogeneous and at least 32 different PFGE patterns were generated according by dice coefficient, between 0.69 and 1.0. Restriction fragment patterns consisted of 13 to 18 fragments ranged in size from 20 to 630 kb. The results confirmed that PFGE would be an useful tool for investigating surveillance of sporadic or outbreak case and assessing clonality for S. enterica serovar Typhi in Busan area. Our finding will be valuable in developing rational strategies to control this pathogen and setting the basis of an effective PulseNet system in Korea.

Plasmid profiling of multi-drug resistant Vibrio sp. isolated from influent and effluent water samples of fish farms in Jeju, South Korea (제주 양식장 유입수과 방출수에서 분리한 다제내성 Vibrio균 플라스미드 프로파일링)

  • The objective of this study was to investigate the plasmid profiling of multi-drug resistant (MDR) Vibrio in influent (inflow) and effluent (discharged) water samples of fish farms in Jeju, South Korea. MDR isolates identified through disc diffusion susceptibility tests, were subjected to plasmid profiling. One hundred fifty Vibrio isolates were obtained from each influent and effluent water sample. All MDR isolates were subjected to plasmid profiling. Greater number of bacteria were enumerated from effluents (61%) comparing to influents (39%). High incidence of neomycin, sulfamethoxazole, amoxicillin and oxytetracycline resistance was observed among the isolates, which was higher in effluent samples. In contrast, Vibrio isolates were more susceptible to florfenicol, chloramphenicol, ciprofloxacin, and nalidixic acid. Among 99 (influent 39 and effluent 60) MDR isolates, a total of 58 (influent 38 and effluent 20) were found to bear plasmids ranging from 1.7 kb to >10 kb and showed 19 different antibiograms according to the size of plasmids. MDR isolates showed six and four distinct plasmid profiles in influent and effluent, respectively. Effluent samples contained more plasmid-carrying MDR Vibrio isolates with more diverse plasmid profiles and antibiograms, suggesting that fish farm tanks may serve as a reservoir of antibiotic resistance genes. The presence of plasmid-carrying MDR Vibrio isolates in fish farm effluent water may contribute to the dissemination of antibiotic resistance genes to the environments, which ultimately poses threat to human health.

Comparison of Methods for Detection of Escherichia coli O157:H7 in Ground Beef and Radish Sprouts

  • Escherichia coli O157:H7 is a food-borne pathogen that causes bloody diarrhea, hemorrhagic colitis, and hemolytic uremic syndrome (HUS). We compared three selective media and evaluated the performance of immunomagnetic separation (IMS) for the detection of low levels of E. coli O157:H7 in ground beef and radish sprouts with different levels of background flora. Bulk food samples (500 g for each trial) were artificially inoculated with nalidixic acid-resistant E. coli O157:H7 at the lowest dose that would generate 20 partial-positive samples of 25 g each. All samples were homogenized in mTSB (225 mL) and incubated overnight at $37^{\circ}C$. IMS was performed using the enriched mTSB samples (1 mL) along with conventional spreads plated onto three different selective media: Sorbitol MacConkey agar (SMAC), Sorbitol MacConkey agar with cefixime and tellulite (CT-SMAC), and Sorbitol MacConkey agar with nalidixic acid (NAL-SMAC) as the gold standard. Two suspicious colonies from each medium were selected and confirmed usinga serological test after transfer to tryptic soy broth with yeast extract (TSAYE). CT-SMAC was better than SMAC for detecting E. coli O157:H7 in all food types. Although there was no statistical difference in the number of positive samples when using IMS vs. non-IMS techniques, more positive samples were detected when IMS was used in both ground beef and radish sprouts. It appears that the improvement was more significant in radish sprouts, which had a higher level of background flora than ground beef. The results also suggest that the combination of CT-SMAC and IMS is sufficient to recover low levels of E. coli O157:H7 in high background flora food samples.

Inhibition Effect on Pathogenic Microbes and Antimicrobial Resistance of Probiotics (Probiotics의 병원성 미생물에 대한 억제효과와 항균제 내성)

  • To investigate the inhibition effect on pathogenic microbes and the antimicrobial resistance of probiotics, a total of 140 probiotics were isolated from 35 kinds of Korean commercially available Kimchi. Of those, L. plantarum was identified from 53 strains (37.9%), E. faecium from 27 strains (19.3%), and L. rhamnosus from 7 strains (5.0%) using 16S rRNA gene sequencing. Sixty nine strains (49.3%) showed overall antimicrobial activity against pathogenic microbes, namely S. Typhi, S. Enteritidis, E. coli O157:H7, S. flexneri, NAG Vibrio, Listeria monocytogenesis, Y. enterocolitica, S. aureus, S. pyogenes, G. vaginalis, C. albicans, and P. acne. The proportions of L. plantarum, E. faecium, and L. rhamnosus strains to pathogenic microbes were 75.5%, 40.7%, and 28.6%, respectively. In addition, a resistance test with 18 antimicrobial agents using a disk diffusion assay revealed a resistance incidence of 98.6% for nalidixic acid, 83.6% for streptomycin, 75.7% for gentamicin 73.6% for vancomycin, 72.1% for norfloxacin, and 67.9% for ciprofloxacin. In conclusion, L. plantarum, L. sakei, and E. faecium strains with various antimicrobial activities and broad antibiotic resistance are useful for treating diarrhea in long-term inpatients and for the alternative use for treating Candida species female vaginitis.

Probiotic Properties of Pediococcus pentosaceus SH-10 Isolated from the Hard Clam Meretrix meretrix Shikhae (백합(Meretrix meretrix)식해에서 분리한 Pediococcus pentosaceus SH-10의 생균제적 특성)

  • This study examined the suitability of characteristics of potential strains of probiotic bacteria. Among 25 lactic acid bacteria isolated from Korean traditional fermented food, the Hard Clam Meretrix meretrix Shikhae, the SH-10 strain, which exhibited superior resistance to low pH and bile salts, was selected as a potential probiotic bacteria. By examining carbohydrate utilization, morphological properties, and the 16S rRNA gene sequence, the SH-10 strain was identified as Pediococcus pentosaceus (hereafter, P. pentosaceus SH-10). P. pentosaceus SH-10 was resistant to amikacin, cefotetan, ciprofloxacin, gentamicin, kanamycin, nalidixic acid, streptomycin, and vancomycin. Tests of antimicrobial activities against pathogens such as Bacillus cereus, Listeria monocytogenes, Salmonella choleraesuis, and Staphylococcus aureus, indicated that P. pentosaceus SH-10 inhibited the growth of pathogenic bacteria. These results suggest that P. pentosaceus SH-10 can be developed as a probiotic bacteria.

  • We report a new series of blue dopants composed of both electron donating and electron accepting moieties in one molecule, based on nalidixic acid. The EL device derived from the dopant exhibits pure blue light emission (0.15, 0.14) The current efficiency is estimated to be 3.88 cd/A at 100 $cd/m^2$, which shows remarkable enhancement, compared to that of the host itself (2.5 cd/A at 100 $cd/m^2$) under the same conditions. These results demonstrate that the incorporation of a proper guest into the host in a guest-host doped system improves not only the purity of the fluorescent blue emission but also elevates its quantum efficiency, thus improving the OLED performance.

Distribution and Antibiotic Resistance of Vibrio vulnificus isolated in Incheon Coastal Area (인천지역 해양환경에서 분리된 비브리오패혈증군의 분포 및 항생제 내성 양성)

  • We investigated the distribution and drug susceptibility test of V. vulnificus isolated from environmental sources in Incheon. In this survey, total 4,302 samples were obtained from different sites of the Incheon coastal area during the periods from march 2004 to November 2006. Among the 4,302 samples, 310 strains were isolated. The isolation rates of Vibrio vulnificus from fish, shellfish, estuarine water and sediment were 6.7%, 4.7%, 12.4% and 23.2%, respectively. The highest isolation rate was 14.6% in September and in spite of low temperature the isolation rate was 5.1% in November. The highest rate of antibiotic resistance was observed against streptomycin(15.3%), cefazolin(8.5%), cephalothin(8.3%), amikacin(8.3%), cefoxitin(6.7%) and nalidixic acid(6.7%). Seventeen percent of isolates were observed to be resistant to two or more of the antibiotics tested.

  • To investigate the epidemiological trait of intestinal diseases of animals caused by thermophilic Campylobacter spp., isolation of etiological agent was carried out. Isolated Campylobacter spp. were biotyped, serotyped and the susceptibility of the isolates to antimicrobial agents were examined. Th results were as follows. 1. Isolation rates of Campylobacter spp. from 649 fecal materials of 208 cattle, 300 pigs and 141 chickens were 25.5%, 23.7% and 38.3%, respectively. 2. The majority of the 130 isolates of C jejuni was classified as biotype I(50.6%) and biotype II (34.6%). Most of the 46 isolates of C coli were biotype I (71.7%). 3. Isolated C jejuni strains showed 14 different serotype, and serotype 4, 26, 36 were most frequent. Isolated C coli strains showed 5 different serotype and serotype 31 and 21 were relatively common. 4. Isolated Campylobacter spp. were highly susceptible to nalidixic acid, amikacin, gentamicin, colistin and chlorampehnocol.

Studies on Salmonella isolated from chicks (초생추 유래 Salmonella속균의 생물학적 특성)

  • This paper deals with the distribution, reservoir and mode of spread of infection on 6 hatcheries in Taegu, Kyungpook and 5 broiler farms in Kyungpook during the period from June 1991 to June 1992. Isolated Salmonella were examined for serotypes, biotyping of Salmonella(S) typhimurtum, antibiotic susceptibility and some biochemical characteristics. Forty two Salmonella strains were isolated from 42(2.7% of 1,577 caecal samples of chicks, and their serotypes were S typhimurium 10, S typhimurium var Copenhagen 5, S infantis 4, S thompson 3, and untypable 20. The isolation rate of Salmonella varied from 0 to 5.1% in 6 hatcheries and that of Salmonella from 5 broiler farms was 10. 5%. Biotypes of 10 S typhimurium and 5 S typhimurium var Copenhagen strains isolated from chicks of hatcheries and broiler farms were biotype 2(86.6%), 8(6.7%), and 10a(6.7%), and 26i(6.7%) according to Duguid's scheme. Antibiotic susceptibility test of Salmonella isolated were performed by agar dilution method, using 9 antibiotics as follows: ampicillin(Am), chloramphenicol(Cm), gentamicin(Gm), kanamycin(Km), nalidixic acid(Na), rifampicin(Rf), streptomycin(Sm), sulfadimethoxine(Su), and tetracycline(Tc), All the strains were sensitive to RF. But 8 strans(23.8%) were resistant to one or more drugs and the most common resistance patterns of transferred R plasmids were SmSuGm and SmSu. Among 42 isolates, one had transferable citrate utilizing plasmid. S typhimurium and S typhimurium var copenhagen strains were resistant to killing by 90% normal guinea pig serum.
