Sodium Intake and preference for Salty Taste in elementary School Children Residing in Rural and Urban Areas of Korea (우리나라 농촌과 서울 아동의 Na 섭취 및 짠맛에 대한 기호도 비교 연구)

  • To compare Na intake and excretion in elementary school children in rural and urban areas of Korea, dietary intake record, urinary Na and K excretion and preference for salty taste were measured in 39 children in 4th grade in Seoul and 36 children in 4th and 5the grades in Kochang-gun)farming area) and their mothers. mean daily urinary Na excretions were 48.8mEq in children and 62.3mEq in mothers. Mothers showed significantly higher preference for salty taste compared to children(X2=16.09, p<0.01). Among the children, those in Seoul had higher preference for salty taste than those in Kochang-gun(X2=8.32, p<0.05). Since the children showed significantly lower preference for salty taste than mothers, more researches and efforts should be directed to prevent the shifts of the taste of the children in order to decrease their Na intake and the risk of hypertension later in life.

The Effect of Dietary Sodium on Calcium Metabolism in Healthy Young Adult Women (한국 성인 여자에 있어서, 나트륨 섭취 수준이 체내 칼슘대사에 미치는 영향)

  • The present study was conducted to examine the effect of an increased level of dietary sodium on calcium excretion in 8 health young adult Korean women on a controlled diet. After adaptation period of 2 days, each subject received 2811.8$\pm$68.1 mgNa(day during the initial period of 5 days (low sodium period) and 6417.1$\pm$248.6mgNa(day during the following period of 5 days (high sodium period). Calcium intake was 593.7$\pm$15.7 mg Ca/day during the low sodium period of 596.1$\pm$25.1 Ca/day duing the high sodium period. When the low sodium period is compared with the high soidum period, the results were as following. 1) Mean urinary sodium excretion was significantly higher during the high sodium period (5760.1$\pm$156.5mg0 than during the low sodium period (2272.2$\pm$108.6mg)(P<0.001). Fecal sodium excretion of the high sodium period was also significantly higher than that of the low sodium period(P<0.001). Mean value of sodium balance during the high sodium period was higher than that of the low sodium period . However, the difference was not significant. 2) Mean urinary calcium excretion was significantly higher during the high sodium period than during the low sodium period ; mean value of the low sodium period was 124.7$\pm$11.3mg and that of the high sodium period was 202.6$\pm$17.2mg)P<0.001). Fecal calcium excretion was higher during the high sodium period (284.9$\pm$31.0mg) than during the low sodium period (253.9$\pm$15.3mg0, but there was no significance. Mean value of calcium balance during the high sodium period was significantly lower than that of the low sodium period(P<0.001). The above results show that high sodium intake increases calcium excretion as well as sodium excretion.

Renin Activity, Habitual Ca, Na Intake and Hormonal Effect on Hypertension (혈압조절에 있어서 Renin 활성도의 차이와 Ca, Na 섭취습관 그리고 호르몬 간의 관련성)

  • Twenty two hypertensive and thirty normotensive in-patients were participated in this study to investigate the relationship between plasma renin activity and metabolism of Ca and Na, Prior to pharmacological treatments, renin activity, aldosterone and parathyroid hormone(PTH) levels were measured from the fasting blood samles. Twenty four hour urine samples were collected to analyze urinary levels of creatinine, Ca, Na and K. Habitual intake of Na and Ca were also measured for hypertensive and normotensive patients. Hypertensive subjects were classified into higher reinin hypertensive (HH), medium renin hypertensive(MH) and low renin hypertensive (LH) group according to their renin activities. PTH level of LH group was the highest among three hypertensive groups. It appeared that aldosterone levels of HH group were significantly higher than LH or MH groups(p<0.05). However there were no significan시 differences in aldosterone level between LH group and normotensive group. Habitual intake of Na and Ca were highest in LH group but lowest in HH group, however, they were not statistically different. Positive correlations of systolic blood pressure with PTH(r=0.2597) and aldosterone(r=0.26480existed(p<0.05). Urinary Ca level was positively correlated with urinary Na(r=0.5619), K(r=0.4533) and habitual Na intake(r=0.3253). Above results suggested the possible relationships among renin activity, habitual Ca intake and Na intake and suggested a further study on the interrelationship between the hormonal control of Ca and Na metabolism and blood pressure in hypertension.

  • Effect of cadmium (Cd) intoxication on renal acid-base regulation was studied in adult male Sprague-Dawley rats. Cd intoxication was induced by subcutaneous injections of $CdCl_2$ at a dose of 2 mg Cd/kg/day for $3{\sim}4$ weeks. In Cd-intoxicated animals, arterial pH, $PCO_2$ and plasma bicarbonate concentration decreased, showing a metabolic acidosis. Urine pH and urinary bicarbonate excretion increased and titratable acid excretion decreased with no change in ammonium excretion. In renal cortical brush-border membrane vesicles derived from Cd-exposed animals, the $Na^+/H^+$ antiporter activity was significantly attenuated. These results indicate that chronic exposures to Cd impair the proximal tubular mechanism for $H^+$ secretion (i.e., $Na^+/H^+$ antiport), leading to a metabolic acidosis.

  • To investigate a possible effect of pectinate(apple and carrot) and alginate (tangle or green laver) on blood pressure and sodium retention, male spontaneous hypertensive rats (SHR) were assigned to 5 different experimental groups and fed diets containing 5% dietary fiber and 1% NaCl for 3 weeks. Dietary fibers were provided from one of the followings : cellulose, freeze dried apple, carrot, tangle(Undardia pinnatifda) or green laver(Monostroma nitidium). Fecal sodium excretion did not change significantly among the groups, however, urinary sodium excretion was increased in groups fed either carrot. tangle or green laver compared to group fed cellulose. Sodium balance was also negative in groups fed either carrot, tangle or green laver. most effectively in green laver group. Blood pressure of groups fed apple. carrot. tangle or green laver were decreased, especially those of group fed tangle, compared to those of group fed cellulose. Dietary fiber containing alginate. such as tangle and green laver, might have cellular binding capacity to sodium so that increased urinary sodium excretion and decreased blood pressure occur.

Correlation between Dietary Sodium and Electroecardiographic Left Ventricular Hypertrophy Among Hypertensives

  • In hypertensives, electrocardiographic left ventricular hypertrophy (LVH) is an independent risk factor for cardiovascular disease. Animal and human studies suggest an association between left ventricular mass and dietary sodium (Na) intake. This study determined if dietary Na intake in a homogenous ethnic population is associated with electrocardiographic LVH (S in Vl + R in $V5{\geq}5.5 mV$). Blood pressure (BP), body mass index, EKG, and 24 hour urine Na and potassium (K) excretion were determined in 40 otherwise healthy Korean patients with untreated essential hypertension on the standard Korean diet. Among these variables, only Na excretion (mmol/day) was significantly different between those with and without LVH $[LVH+:357{\pm}50,\;LVH-;\;268{\pm}25(p=0.04]$. Thus, dietary sodium intake may be predictive of electrocardiographic LVH.

The Relationship of Renin Activity, Hormonal Na, Ca and Habitual Na, Ca Intake in Hypertension (정상생활을 하는 고혈압 환자에서 Renin활성도의 차이와 Na, Ca 조절호르몬 및 Na, Ca 섭취습관과의 관련성)

  • In order to investigate the relationship between plasma renin activity and metabolism of Ca and Na in blood pressure, the habitual dietary intakes of Na and Ca urinary excretion of Ca, Na and K, and plasma level of renin activity, aldosterone, and indices of Ca metabolism were measured in 27 untreated hypertensive women and 30 age-matched normal women on a free diet. Hypertensive subjects were classified into high renin hypertensive (HH), medium renin hypertensive(HM) and low renin showed no significant difference among normotensive, LH, MH and HM groups. It appeared that 25-(OH) Vit D3 level of HH group was significantly higher than LH group(p<0.05). There was significant difference in habitual intake of Ca between normotensive and LH groups. However, habitual intake of Na showed no significant difference among normotensive, LH, MH and HH group. Positive correlation of systolic and diastolic blood pressure with PTH(r=0.324, r=0.375) and urinary Ca(r=0.496, r=0.278) and a negative correlation of systolic blood pressure with habitual Ca intake(r=-0.371) existed(p<0.05). A relative magnitude of factors affecting hypertension was analyzed by multiple regression analysis. Overall results about relative influence of independent variables to dependent variable (systolic blood pressure) indicated that urinary Ca was the higher correlation in all subjects(p<0.0001), followed by age and aldosterone. PTH showed a significant correlation for relative influence on diastolic blood pressure in all subjects. The above results indicated that renin-aldosterone system and Ca regulating hormone had a mutual relationship in hypertension.

  • It was hypothesized that variations within the range of usual calcium(Ca) and sodium(Na) intakes of Korean influence bone mineral density(BMD) in healthy premenopausal women The relationship of nutrient intake urinary excretion physical activity and circulating IGF-1 level with spine({{{{ { L}_{2 } }}}}-{{{{ { L}_{4 } }}}}) and femur BMD was determined in 47 normal premenpausal women. There was a positive relationship between BMD of the lumbar spine and body weight. The BMD of femoral neck was positively correlated with Ca and protein intakes from animal source and circulating IGF-1 level. There was a negative relationship between femur BMD and both Na intake and urinary excretion. The complex interrelations between femur BMD regression analysis, From this analysis. Ca intake from animal origin was the only significnat Premenopausal women of femur BMD. In the basis of femur BMD three groups were divided Premenopausal women of femur BMD$\leq$0.84g/cm2 showed depressed Ca intake of animal origin in later and early life and enhanced urinary Na excretion compared to women of femur study suggests that dietary Ca is a major constituent affecting femur BMD because of a decrease in net Ca absorption and an increase in urinary Ca loss.

Influence of Intracerebroventricular Domperidone on Rabbit Renal Function (가토 신장기능에 미치는 뇌실내 Domperidone의 영향)

  • Dopamine when given icv induces antidiuresis along with transient natriuretic tendency, and it has been suggested that both subtypes of central dopamine receptors may influence renal function differentially. This study was undertaken to delineate the role of central $D_2$ receptors employing domperidone (DOM), a selective $D_2$ antagonist. DOM icv elicited antidiuresis and antinatriuresis in doses ranging from 15 to $135{\mu}g/kg$. GFR and RPF as well as sodium excretion decreased. Systemic blood pressure increased slightly. Intravenous DOM did not elicit significant changes in sodium excretion. Denervation of the kidney abolished the hemodynamic change induced by icv DOM, but sodium excretion decreased on both innervated and denervated kidneys. No diuretic tendency was uncovered by the denervation. Dopamine, $150{\mu}g/kg$ icv, produced antidiuresis along with decreases in hemodynamics. These effects were not affected by DOM-pretreatment, and no natriuretic tendency was unveiled. Bromocriptine, a $D_2$ receptor agonist, $200{\mu}g/kg$ icv, elicited marked diuresis and natriuresis, which were completely abolished by DOM-pretreatment. Apomorphine, another prototype of $D_2$ agonist, $150{\mu}g/kg$ icv, produced diuresis and natrituresis with increases in renal hemdoynamics, followed by decreases in all parameters. DOM-pretreatment did not affect the renal hemodynamic effects, wherease the increases in urine flow and sodium excretion were markedly reduced by DOM, Present study suggests that central $200{\mu}g/kg$ receptors mediate natriuretic and diuretic influence to the kidney, possibly through mediation of natriuretic humoral factor, and provide further evidence supporting the hypothesis that central $200{\mu}g/kg$ receptors mediate antidiuretic influence via nerve pathway, whereas natriuresis are brought about through mediation of central $200{\mu}g/kg$ receptors.

Potassium Intakes of Some Industrial Workers (일부 산업체 근로자의 Potassium 섭취에 관한 연구)

  • This study was undertaken to evaluate the potassium consumption and excretion in forty healthy male workers of a tire company in Seoul. Mean postassium intake for three days in the subject was $54.5\pm16.7mEq/day(2.13\pm0.64g)$ and urinary excretion of potassium in 24 hours was $45.9\pm10.5mEq(1.77\pm0.41g)$. Thus 83% of dietary potassium was excreted in the form of urine. Dietary ratio of Na to K was $4.15\pm0.58$ while urinary ratio of Na to K was $5.20\pm1.11$. The main food source of potassium was cooked rice with soybean in the rice group, potato with soybean paste soup in the part of soup group. and seasoned Spanish mackerel with raddish in the side dish group. There was a strong correlation between dietary protein and dietary potassium(r=0.694, p<0.001). Urinary sodium and potassium were also strongly associated with each other(r=0.647, p<0.001).
