Acute Hyponatremia in Pneumonia and CNS Infections of Children (소아의 폐렴과 중추신경계 감염에서 급성 저나트륨혈증의 발생 양상)

  • Purpose: The option of selecting isotonic rather than hypotonic fluids for maintenance fluid in children has been advocated by some authors. Pneumonia and CNS infections are frequent clinical settings for acute hyponatremia because of nonosmotic anti-diuretic hormone stimuli in children. We conducted the present study to identify the incidence of hyponatremia in pneumonia and CNS infection of children and to determine the importance of maintenance intravenous fluid therapy regimen and other related factors. Methods: The study included 1,992 patients admitted to the Department of Pediatrics at Pusan National University Children's Hospital between November 2008 and August 2011, who were diagnosed with pneumonia or CNS infections and checked for serum sodium concentration. Their clinical data including laboratory findings were reviewed retrospectively. Results: During the study period, 218 patients were identified to have acute hyponatremia among 1,992 patients. The overall incidence of hyponatremia was 10.9%. The incidence of hyponatremia in encephalitis (37.3%) was highest and the incidence in bacterial meningitis (27.4%), viral meningitis (20.0%), bacterial pneumonia (11.1%), mycoplasma pneumonia (9.2%), and viral pneumonia (6.8%) were in descending order. The mean age was higher in hyponatremic patients than in isonatremic patients. The incidence of hyponatremia was higher in who had 0.18% NaCl in 5% dextrose (D5 0.18% NS) than 0.45% NaCl in 5% dextrose infusion (D5 1/2NS) (9.0% vs. 2.2%). SIADH was identified in 20.5% among hospital acquired hyponatremic patients after adequate evaluation for SIADH. Conclusion: We recommend D5 1/2NS rather than D5 0.18% NS as the maintenance fluid given to children with pneumonia or infectious CNS diseases.

  • This paper describes a 3.3V 8bit CMOS digital to analog converter (DAC) with two state current cell metrix architecture which consists of a 4 MSB and a 4 LSB current matrix stage. The symmetric two stage current cell matrix architecture allow the designed DAC to reduce hot only a complexity of decoding logics, but also a number of wider swing cascode curent mirros. The designed DAC with an active chip area of 0.8 mm$_{2}$ is fabricated by a 0.8 .mu.m CMOS n-well standard digital process. The experimental data shows that the rise/fall time, the settling time, and INL/DNL are6ns, 15ns, and a less than .+-.0.8/.+-.0.75 LB, respectively. The designed DAC is fully operational for the power supply down to 2.0V, such that the DAC is suitable for a low voltage and a low power system application. The power dissipation of the DAC with a single power supply of 3.3V is measured to be 34.5mW.

Genogroup position of aquabirnavirus GC-1 isolated from rockfish Sebastes schiegeli in Korea

  • The cDNA of the aquabirnavirus, GC-1 isolated from rockfish Sebastes schlegeli in Korea, was synthesized using the reverse transcriptase-polymerase chain reaction. The nucleotide and deduced amino acid sequences were determined from cDNA of the VP2-NS-VP3 coding region of genome segment A. The nucleotide sequences of the segment A were 3,086 base pairs (bp) in length and contained large open reading frame (ORF) and terminal sequences. The large ORF was comprised of 2,916 bp nucleotides and composed of 972 deduced amino acid sequences. Pairwise comparisons were made with other aquabirnavirus sequences published previously. The study of genetic relationships between GC-1 and aquabirnaviruses in the large ORF and VP2 coding regions demonstrated that the GC-1 has the nearest genetic relationship with the marine birnaviruses (MABV strains), and the GC-1 and MABV strains can be clustered as the same genogroup. GC-1 can be included in MABV, which is the 7th genogroup of family Aquabirnaviridae.

Performance Analysis of Traffic Information Service Based on VANET (VANET기반 교통정보 서비스 방식 성능분석)

  • We propose a traffic information service for which traffic data are collected over ad-hoc networks from neighbor vehicles, processed to minimize the data size, and eventually provided to its destination. The proposed scheme simply relies on the existing navigtion systems in vehicles and wireless communication devices for vehicle-to-vehicle communication, rather than on a separately established server. It allows collecting and analyzing traffic status of large areas without incorporating separated monitoring systems, e.g., probe cars and enables to provide accurate traffic information to drivers in timely manner. We also evaluate its performance by ns-3 simulation.

A PIN Diode Switch with High Isolation and High Switching Speed (높은 격리도와 고속 스위칭의 PIN 다이오드 스위치)

  • The isolation of the series PIN diode switch is restricted by the parallel capacitance of PIN diode and the switch driver circuit limits switching speed of PIN diode switch. To overcome these problems, a high isolation and high switching speed Pin diode switch is proposed adapting the parallel resonant inductance and TTL compatible switch driver circuit. The measurement results of the 3 GHz PM diode switch show 1 GHz frequency band, less than 1.5 dB insertion loss, 65 dB isolation, more than 15 dB return loss and less than 30 ns switching speed. In particular the 3 GHz PIN diode switch using the parallel resonant inductance exhibits the improvement of isolation by 15 dB.

Design of a $54{\times}54$-bit Multiplier Based on a Improved Conditional Sum Adder (개선된 조건 합 가산기를 이용한 $54{\times}54$-bit 곱셈기의 설계)

  • In this paper, a $54{\times}54$-bit multiplier based on a improved conditional sum adder is proposed. To reduce the multiplication time, high compression-rate compressors without Booth's Encoding, and a 108-bit conditional sum adder with separated carry generation block, are developed. Furthermore, a design technique based on pass-transistor logic is utilized for optimize the multiplication time and the power consumption by about 5% compared to that of conventional one. With $0.65{\mu}m$, single-poly, triple-metal CMOS process, its chip size is $6.60{\times}6.69\;mm^2$ and the multiplication time is 135.ns at a 3.3V power supply.

Growth and Properties of GaN by Vapor Transport Epitaxy (Vapor Transport Epitaxy에 의한 GaN의 성장과 특성)

  • Highly c-axis oriented poly-crystalline GaN with a dimension of $1{\sim}3\;{\mu}m$ was deposited on $c-Al_2O_3$ substrate by vapor transport epitaxy (VTE) method at the temperature range of $900{\sim}1150^{\circ}C$. XRD intensities from (00'2) plane of grown GaNs were increased with reaction conditions which indicate the improvement of the crystal quality. In the PL spectra measured at 10 K, the spectrum composed with the neutral-donor bound exciton-related emission at 3.47 eV, crystal defect-related emission band at 3.42 eV and with its phonon replicas. The fact that intensity of $I_2$ were increased and FWHM were decreased with growth conditions means that the quality of GaN crystals were improved. With this simple VTE technology, we confirm that the GaNs were simply deposited on sapphire substrate and crystal quality related to optical properties of GaN grown by VTE were relatively good. PL emission without deep level emission in spite of polycrystalline structure can be applicable to the fabrication of large area and low cost optical devices using poly-GaN grown by VTE.

Performance Analysis of Differentiated Services by Classifying Traffic (트래픽 구분을 통한 차등화 서비스의 성능 분석)

  • This paper examined the performance of differentiated services applied to each different traffic. For this purpose, we used three traffics which have a different quality of service and generate packets from a specific source to destination. To allocate a proper code point according to QoS, a common policy which is the simple policy model of time sliding window with 3 color marking (TSW3CM) was established to each traffic. To evaluate the performance, we investigate and compare code points to each traffic. Simulation results using NS-2 showed that the code points 10, 11, and 12 can be differently allocated to each traffic according to its sending rates 1Mbps, 2.5Mbps, and 4Mbps.

A New Scale(NS) Score System to Predict Outcome of Intracranial Aneurysm Using TCD (TCD를 이용한 두개강내 동맥류의 예후 예측 가능한 New Scale(NS) Score System)

  • Objective : By conducing a review of clinical outcomes for patients with aneurysm treated using current microneurosurgical techniques and intensive care unit management, we speculated that grading systems based only on clinical condition or CT finding after admission failed to provide a significant stratification of outcome between individual grades of patients, because these systems did not include the factor for postoperative vasospasm. We hypothesized that postoperative blood flow velocity could have a significant impact on outcome prediction for patients surgically treated for intracranial aneurysms. Methods : We conducted a analysis on patient- and lesion-specific factors that might have been associated with outcome in a series of 55 aneurysm operations performed with measurements of blood-flow velocity with transcranial Doppler ultrasonography(TCD). In the new scale(NS) score system, 1 point is assigned additionally for the case with Hunt and Hess(H-H)/World Federation of Neurological Surgeons(WFNS) Grade IV or V, Fisher Scale(FS) score 3 or 4, aneurysm size greater than 10mm, patient age older than 60 years, blood-flow velocity higher than 120cm/sec, and posterior circulation lesion. By adding the total points, a 6-point scale score(score 0-6) is obtained. Results : Age of patient, size of aneurysm, clinical condition(H-H grade and WFNS), FS score, and blood flow velocity(TCD 1day after operation) were independently and strongly associated with long-term outcome. When NS scores were applied to 55 patients with at least 6 months follow-up, the correlation of individual scores with outcome was strongly validated the retrospective findings. Conclusion : It was speculated that TCD could be used to assess postoperative vasospasm and to monitor noninvasively the patients with aneurysmal SAH. This NS score system is easy to apply, divide patients into groups with different outcome, and is comprehensive, allowing for more accurate prediction of surgical outcome.

Characterization of gene expression and genetic variation of horse ERBB receptor feedback inhibitor 1 in Thoroughbreds

  • Objective: This study aimed to test the expression patterns of ERBB receptor feedback inhibitor 1 (ERRFI1) before and after exercise and the association of non-synonymous single-nucleotide polymorphisms (nsSNPs) of horse ERRFI1 with racing traits in Thoroughbreds. Methods: We performed bioinformatics and gene expression analyses for horse ERRFI1. Transcription factor (TF) binding sites in the 5'-regulatory region of this gene were identified through a tool for prediction of TF-binding site (PROMO). A general linear model was used to detect the association between the nsSNP (LOC42830758 A to G) and race performance. Results: Quantitative polymerase chain reaction analysis showed that expression level of ERRFI1 after exercise was 1.6 times higher than that before exercise. Ten transcription factors were predicted from the ERRFI1 regulatory region. A novel nsSNP (LOC42830758 A to G) was found in ERRFI1, which was associated with three racing traits including average prize money, average racing index, and 3-year-old starts percentile ranking. Conclusion: Our analysis will be helpful as a basis for studying genes and SNPs that affect race performance in racehorses.
