  • Botulinum toxin type A(BTX-A) has been applied successfully to treat masseteric hypertrophy. However it can cause muscle weakness. This study was designed to measure the change in maximum bite force(MBF) after BTX-A injection into human masseter muscle and to evaluate the influence of a booster(repeated) injection. Thirty volunteers completed 18-week follow-up and MBF was measured. At 18 weeks after the first injection, a booster injection was given to 14 patients and they were followed up until 18 weeks from the booster injection. The mean MBF was approximately 20% lower at 2 weeks than before the injection, and it recovered gradually after 4 weeks to return to the preinjection level at 12 weeks. The MBF differed significantly between before the injection and at 2, 4, and 8 weeks after the injection(p<0.05). In booster injection group(n=14),the MBF decreased markedly at 6 weeks(p<0.05),and it recovered gradually in 12 weeks. The MBF was significantly reduced after booster injection of BTX-A into the human masseter muscle. The degree of discomfort experienced by the subjects had little effect on normal mastication.

  • The first molar is important for mastication and also it plays roles to formation of vertical occlusion and growth of jaw bone after mixed dentition. Impaction of mandibular first molar can result in a short lower facial height, formation of a follicular cyst, pericoronal inflammation, resorption of the roots of neighboring teeth and malocclusion. The options of treatment plans are as follows; observation, surgical exposure, orthodontic traction, surgical relocation and extraction. Surgical exposure could be considered as a basic treatment plan. For surgical exposure it is important to maintain patent channel between the crown and the normal eruptive path into the oral cavity, many techniques including cementation of a celluloid crown, packing with zinc oxide-eugenol surgical pack are used. In these cases, we could observe spontaneous eruption of mandibular first molar using surgical exposure with or without removal of odontoma. Also we could obtain the main patency effectively and conveniently by using surgical pack and translucent retainer.

  • This Study was designed to investigate the effect of Sunkihwalhyul -Tang extract(SHT) on the change of cerebral hemodynamics [regional cerebral blood flow(rCBF), pial arterial diameter(PAD) and mean arterial blood pressure(MABP)] in normal and cerebral ischemic rats, and further to determine the mechanisms of action of SHT on hemodynamics. In addition, this study was designed to investigate whether SHT inhibits lactate dehydrog enase(LDH) activity in neuronal cells and cytokines production in serum of cerebral ischemic rats. The results were as follows 1. SHT significantly increased rCBF and PAD in a dose-dependent manner, but MABP was not changed by injecting SHT. These results suggest that SHT significantly increases rCBF by dilating PAD. 2. The SHT-induced increase in rCBF was significantly inhibited by pretreatment with indomethacin(IDN, 1 mg/kg, i.p.), an inhibitor of cyclooxygenase and methylene blue(MTB, $10{\mu}g/kg$, i.p.), an inhibitor of guanylate cyclase. 3. The SHT-induced dilation in PAD was significantly inhibited by pretreatment with IDN and MTB. 4. The SHT-induced some increase in MABP was significantly increased by pretreatment with IDN. These results suggest that the mechanism of action of SBT is mediated by guanylate cyclase. 5. Both rCBF and PAD were significantly and stably increased by SHT(10 mg/kg, i.p.) during the period of cerebral reperfusion, which contrasted with the findings of rapid and marked increase in control group. 6. SBH significantly inhibited LDH activity in neuronal cells. These results suggest that SHT prevents the neuronal death. 7. In cytokine production in the senlm drawn from femoral artery 1 hr after middlecerebral arterial occlusion, sample group showed significantly decreased production of IL-1$\beta$ production, decreased production TNF-$\alpha$ and increased Production of IL-10 compared with control group. 8. In cytokine production in the serum drawn femoral artery 1 hr after reperfusion, sample group showed significantly decreased production of IL-1$\beta$ and TNF-$\alpha$ as wellas significantly increased production of IL10 compared with control group. These results suggest that SHT mediated by guanylate cyclase has inhibitive effect on the brain damage by inhibiting LDH activity, IL-1$\beta$ and TNF-$\alpha$ production, and by accelerating IL-10 production. The present author thinks that SHT has an anti-ischemic effects through the improvement of cerebral hemodynamics and inhibitive enects on the brain damage.

  • The human temporomandibular joint as a ginglymoarthrodial one has much in common with the other synovial joints of the body, but it does possess an unique charachteristic in that it must accomodate the various occlusal relations of dentition during an end point of closure. For that reason, the movement of the condyle in the temporomandibular joint is susceptible to influences from the nature of occlusion. Undue loading to the temporomandibular joint can be applied on the occasion of premature contacts in centric relation, balancing side interferences, change of occlusal surfaces due to excessive attrition, loss of tooth. Such occlusal disharmonies in association with the systemic and emotional factors may give rise to the temporomandibular disorder. On the other hand, the changes of occlusal patterns in the growing body can also have an effect on the growth of the temporomandibular joint through the alterations of functional stresses. The purpose of this study was to observe histopathologic response of the temporomandibular joint in unilateral chewing on one side exclusively for 10 months. Three dogs showing normal masticatory function were chosen. One dog aged about 12 months was for control, two dogs for experimental specimens were about 12 and 18 months old respectively. For chewing on the left side only, unilateral lower right premolar and molar were extracted in two experimental specimens. And then three dogs were sacrificed 10 months later. Frontal histologic sectioning of joints were done for the observation of the effects of one-side chewing. 24 specimens from three dogs were obtained and fixed in 10% formalin and routinly processed with H-E staining for histologic examination. The light microscopic findings were interpretated as follows: 1. Experimental specimen 1 aged about 22 months: In comparison with control and right non-chewing side, the proliferative and hypertrophic zone were increased at the mesial and lateral part of left chewing side condyle. There was no change of the articular tissue of temporal bone. 2. Experimental specimen 2 aged about 28 months: The articular tissues of adult joint were observed. The differences between the chewing and non-chewing side were not seen in the articular tissues of condyle and temporal bone.

  • Objectives & Methods : This present study was performed to investigate the effect of Hwadamtongrak-Tang extract (HTT) on the regulation of cerebral hemodynamics in terms of regional cerebral blood flow (rCBF) and mean arterial blood pressure (MABP)] in normal and cerebral ischemic rats. Also the effects of HTT on changes in local blood flow, inhibition of LD H activity in neuronal cells, and levels of cytokine production in the serum were determined in the ischemic rat model. The major findings are summarized below. Results : 1. HTT significantly increased rCBF in a dose-dependent manner, but MABP was not changed by HTT treatment. These results suggest that HTT may increase rCBF by dilating cerebral arterial diameter. 2. HTT-induced increase in rCBF was blocked by pretreatment with cyclooxygenase inhibitor indomethacin (IDN, 1 mg/kg, i.p.) and MABP was significantly increased by ID N. 3. Pretreatment of methylene blue $(MTB,\;10\;{\mu}g/kg,\;i.p.)$, an inhibitor of guanylate cyclase, significantly decreased both rCBP and MABP in HTT-treated rats. 4. HTT treatment significantly increased rCBP to a stable level during the period of cerebral reperfusion. 5. HTT significantly inhibited LD H activity in neuronal cells, suggesting a neuroprotection by HTT. 6. Serum interleukin $(IL)-1{\beta}$ and tumor necrosis factor $(TNF)-{\alpha}$ levels were significantly decreased in the femoral artery 1 hr after middle cerebral arterial occlusion in HTT-treated rats. IL-10 levels in the serum were significantly increased by HTT treatment whereas transforming growth factor $(TGF)-{\beta}$ levels were similar between HTT-treated and control groups. 7. Serum interleukin $(IL)-1{\beta}$ and tumor necrosis factor $(TNF)-{\alpha}$ levels were significantly decreased in the femoral artery 1 hr after reperfusion in HTT-treated rats. Serum IL-10 levels were significantly decreased in HTT-treated rats compared with the control group, and no significant changes in $(TGF)-{\beta}$ in the serum were observed by HTT treatment. Conclusions: The present data suggest that HTT may have an anti-ischemic effect via the improvement of cerebral hemodynamics and thus protect the brain from ischemic damage.

  • Craniofacial growth pattern is an important diagnostic data in the course of orthodontic diagnosis and treatment planning ; it also has great influence in the establishment of occlusion as well as shaping and development of face. There have been many studies to classify different craniofacial growth patterns and attempts to predict growth patterns. This study aimed to correlate craniofacial growth pattern and symphysis morphology. 120 adult patients with age from 19 to 39 (mean age : 23.1) were chosen as subjects , using lateral cephalometric films. their anterior to posterior facial height ratios were calculated. They were divided into 3 groups - clockwise growth pattern with $56\%-62\%$(36subjects), counter-clockwise growth pattern group with $65\%$-80\%$(43subjects) and normal growth pattern group with $62\%-65\%$(41subjects). Symphysis morphology and Prominence evaluation in each subject were studied and the following conclusions were drawn : 1. In comparison of symphysis morphology between the sex groups, men showed large symphysis height and prominence. 2. Concerning the symphysis morphology, the clockwise growth pattern group showed larger height, H/D ratio and actual length but smaller depth, angle, effective length and E/A ratio compared to the counter -clockwise growth pattern group. 3. Those with smaller prominance of symphysis showed clockwise growth tendency and those with larger prominance showed counter-clockwise growth tendency.

  • This study was conducted to examine the growth change in external nose according to age which plays an important role in determining the facial profile, by analysing the biennial serial lateral cephalograms of 20 males and 20 females of mean 8.5 years old to 16.5 years old who have a normal occlusion. The results of this study were summarized as follows. $\cdot$ Both mean and standard deviation were obtained according to each item, sex and age for the male and female. $\cdot$ Nose heights of the male and female increased with increase in their age. The upper to lower nose height ratio somewhat decreased for both male and female with increase of age. The mean ratio was 3.69 : 1 for the male, respectively, and 3.83 : 1 for the female during this study.$\cdot$ The nose depth indicated the most significant increase of all items according to the increase of age, while the sagittal depth inicated higher value for the female than that of the male in the most age groups. $\cdot$ The nose inclination did not show a remarkable change during this study. $\cdot$ The proportions of growth completed at the earliest age showed the highest percentage for sagittal depth(male : $87.14\%$ & female : $91.29\%$). Also, the male indicated the lowest percentage in lower nose height $(73.04\%)$, while the female did the lowest in nose depth$(79.09\%)$. $\cdot$ There was shown the variable of the greatest correlation between the upper nose inclination and nose depth. $\cdot$ From this study, the whole external nasal growth was occurred downward and forward which was more frequent for forward growth than vertical growth.
