Comparison of Spatial Neurocognitive Function between Insomnia Disorder Patients and Normal Sleeping Control : Pilot Study (불면장애 환자와 정상대조군간의 공간인지기능 비교 : 예비연구)

  • Objectives: Chronic insomnia disorder is a common and one of the most distressing sleep disorders. This pilot study was conducted to compare the spatial function between insomnia disorder patients and good sleeping control. Methods: We enrolled the 22 patients with chronic insomnia during over one year who met the DSM-5 diagnostic criteria of insomnia disorder and 27 normal sleeping controls. The Cambridge Neuropsychological Test Automated Battery (CANTAB) has been performed to compare the spatial cognitive function between insomnia disorder patients and good sleeping controls. Results: The CANTAB results showed significant differences in the problems solved in minimum moves of Stockings of Cambridge test (t = -2.499, p = 0.017). The significant difference between two groups remained after controlling age, sex, and Beck Depression Index non-sleep scores (F = 5.631, p = 0.022). Conclusion: This study suggests that the patients with insomnia disorder have poor spatial planning function.

5th and 6th Grade Korean Students' Proportional Reasoning Abilities (초등학교 5학년과 6학년의 비례 추론 능력 분석)

  • This research analyzed proportional reasoning abilities of the 5th grade students who learned only the basis of ratio and rate and 6th grade students who also learned proportion and cross product strategy. Data were collected through the proportional reasoning tests and the interviews, and then the achievement of the students and their proportional reasoning strategies were analyzed. In the light of such analytical results, the conclusions are as follows. Firstly, there is not much difference between 5th and 6th grade students in the achievement scores. Secondly, both 5th and 6th graders are less familiar with the geometric, qualitative and comparisons tasks than the other tasks. Thirdly, not only 5th graders but also 6th graders used informal strategies much more than the formal strategy. Fourthly, some students can't come up with other strategies than the cross product strategy. Finally, many students have difficulties in discerning proportional situation and non-proportional situations. This study provided suggestions for improving teaching proportional reasoning in elementary schools in Korea as follows: focusing on letting students use their informal strategies fluently in geometric, qualitative, and comparisons tasks as well as algebraic, quantitative, and missing value tasks focusing on the concept of ratio and proportion instead of enforcing the formal strategy.

Effects of Maternal Role Education Program on the Mother-Infant Interaction and Infant Development (영아기 어머니역할 교육 프로그램이 모아상호작용과 영아발달에 미치는 효과)

  • The impact of childhood experience has lifelong significance on subsequent health and development. Especially, the experience of infant is mostly affected by the quality of parental care and rearing environment. But the new mothers usually do not know what to do because of the lack of experience in these days. Therefore, an educational program regarding maternal role would be necessary. This study was conducted to evaluate the effectiveness of the maternal role education program for mother-infant interaction, child-rearing environment, and infant development. Non-equivalent control group time-series design was used, and Barnard's mother-infant interaction model was used as a conceptual framework of this study. The subjects were the healthy infants weighing over 2,500gm at birth, whose gestational age was more than 37 weeks, and their mothers. The final sample consisted of 19 mother-infant dyads for intervention group and 18 dyads for control group. Data were collected from March 15th to September 3rd in 1999. For the intervention group, programmed education which focused on mother-infant interaction, breast feeding, and infant care was provided before discharge. Telephone counselling was provided within one week after discharge. Home visiting for maternal role education was provided twice, one month and three months postpartum. For the control group, home visiting was also conducted but only for data collection. The data were analyzed using chi-square test and t-test to test the equivalence of two groups, and the effectiveness of intervention program was determined with repeated measure ANCOVA and t-test. The results were as follows: 1. Significant differences were found in mother-infant interaction between two groups(p=.000). It indicates that intervention program was effective in improving mother- infant interaction. In subscale analysis, four out of six subscale showed significant differences between the groups: sensitivity to cues (p=.000), social-emotional growth fostering (p=.000), cognitive growth fostering(p=.000) in mothers, and responsiveness to caregiver (p=.019) in infants. 2. The difference in the mean score of childrearing environment (HOME) between the intervention group and control group was significant(p=.003). When each subscale of HOME was examined individually, intervention group showed significantly higher scores in the diversity of stimulation(p=.000), and mother's involvement(p=.001). 3. Three-month-Infants of the intervention group showed higher GQ in the Griffiths mental development scale(p=.026). In subscale analysis, significant differences were found in the personal-social(p=.005), and the hearing and speech(p=.003). In conclusion, the maternal role education program proved to be effective in promoting the mother-infant interaction, organizing the childrearing environment, and fostering the infant development. These results are very meaningful that we found maternal role education necessary for normal infants' mothers, and that nurses can make a great contribution in promoting health of infants and mothers.

Insomnia in Relation to Suicide and Serotonin in Patients with Major Depressive Disorder (우울증 환자에서 불면과 자살, 세로토닌의 연관성)

  • Objectives: The aim of current study is to evaluate the relationship between sleep, suicide and serotonin using some scales and loudness dependence of auditory evoked potentials (LDAEP). Methods: Total 65 patients who met the criteria for major depressive disorder were enrolled in current study. The patients were divided into two subgroups according to their insomnia and a history of suicide attempts. The auditory event-related potentials were measured to evaluate LDAEP before beginning antidepressants. Results: The scores of total Beck Depression Inventory (BDI) and BDI item 9 (suicide) were higher in insomnia subgroup than non-insomnia subgroup (respectively, p=0.0033 and p=0.03). However, LDAEP did not differ each other. The subgroup with a history of suicide attempts had a higher score of BDI item 9 than the subgroup without a history of suicide attempts (p=0.00012). There was a tendency for the LDAEP to be higher in the subgroup with a history of suicide attempts ($1.39{\pm}0.94{\mu}V$) than the sub-group without a history of suicide attempts ($1.05{\pm}0.75{\mu}V$), although the difference was not statistically significant (p=0.078). Conclusion: Suicidality was related to insomnia. In addition, there was a tendency for serotonin activity to be lower in the subgroup with a history of suicide attempts. In future, more studies are needed.

Reliability and Validity of Korean Version of the SWAL-QOL (한국판 SWAL-QOL의 신뢰도와 타당도)

  • The Purpose of the this study was to identify reliability and validity of the Korean version of the Swallowing Quality of Life questionnaire(KSWAL-QOL). The study was performed in 71 patients diagnosed dysphagia by videofluoroscopy and 80 healthy swallowers. The reliability was good with a Cronbach's ${\alpha}$ and intraclass correlation coefficient of .86~.96 and .80~.93, respectively. The Pearson product moment correlation coefficients between KSWAL-QOL scales ranged from .17~.74 which was showed significant correlation. Healty swallowers scored higher than dysphagic patients on all scales and statistically significant differences were observed across all the scales between healthy swallowers and dysphagic patients(p<.01). Tube feeders scored lower than non-tube feeders on all scales and statistically significant differences were observed in all the scales except sleep(p<.05). There are significant difference between diet steps in all scales except eating desire, communication, fear and people on diet fourth step feeding had the highest scores on the all scales(p<.05). Because KSWAL-QOL seems to be a reliable and valid tool, it is considered to be appropriate as a tool to measure quality of life of patient with swallowing disorder.

Broiler responses to dietary 3,4,5-trihydroxybenzoic acid and oregano extracts under Eimeria challenge conditions

  • This study was conducted to evaluate the efficacy of a combination 3,4,5-trihydroxybenzoic acid (THB) and oregano extracts (i.e., Carvacrol and Thymol) at intake/dietary different levels on growth performance, intestinal health indicators, immune responses and fecal oocyst shedding in broiler chickens under Eimeria challenged condition. A total of 336 one-day-old broilers were randomly assigned to one of six dietary treatments with seven replications per treatment. Dietary treatments were: i) Non-challenged bird without any dietary treatment (NCNT), ii) Challenged bird without any dietary treatment (CNT), iii) Challenged birds fed a THB diet (0.1 g/kg, THB), iv) Challenged birds fed a combination of THB and oregano extracts diet (0.1 g/kg, COM 100), and a gradual increase of combination of THB and oregano extracts likely v) 0.15 g/kg (COM 150), and 0.2 g/kg (COM 200). On day 14, all groups except for NCNT have orally challenged with a 10-fold dose of Livacox® T anticoccidial vaccine to trigger coccidiosis. The results indicated that Eimeria-challenged broilers fed COM 100 and COM 200 diets increased (p < 0.05) body weight than CNT diet on day 35. Furthermore, birds fed COM 100 and COM 200 diets increased (p < 0.05) average daily gain compared to those fed CNT diets for the entire experimental period. There is no significant (p > 0.05) in average daily feed intake, feed efficiency between NCNT and birds fed with combined THB and oregano extracts for the entire experimental period. A combination of THB and oregano extract regardless of concentration levels or THB alone reduced (p < 0.05) lesion score in ileum compared to the CNT diet for 7 days post-infection (dpi). Birds fed COM 100 diet had lower (p < 0.05) intestinal lesion scores in jejunum and caeca on 7 dpi compared to those were in the CNT diet. No (p > 0.05) difference was observed in the oocysts per gram of feces count, intestinal morphology, carcass traits and blood cytokine concentration among the infected treatments. Collectively, we conclude that birds fed with a combination of THB and oregano extracts regardless of the ratios that were used demonstrated better recovery of health after the coccidial challenge than using only THB alone.

Development of Korean Adult Patients Delirium Screening Tool (한국형 성인 환자 섬망 선별 도구 개발)

  • Purpose: The purpose of this study was to develop a Korean Adult Patients Delirium Screening Tool (K-APDS) for those admitted to general wards, and to verify its reliability and validity. Methods: For the development of the tool, 12 items were derived through the results of literature review and focus group interviews with general ward nurses, and the content validity was confirmed by experts. To verify the reliability and validity of the developed tool, 317 adult patients who were admitted to general wards of three tertiary general hospitals from October to November 2022 were evaluated by the attending nurse and data were collected. Results: After factor analysis for construct validity verification, two factors were extracted, which explained 60.1% of the total variance. After the validation of the control group, the difference in the delirium incidence scores calculated using the K-APDS between the delirium group and non-delirium group was very significant (Z=-10.82, p<.001). To verify the criterion validity, K-APDS, Delirium Observation Screening, and Pearson's correlation coefficient were checked and found to be .94 (p<.001). The predictive validity test reported that the sensitivity was 91.1%, specificity was 82.4%, positive predictive value was 52.6%, and negative predictive value was 97.8%. The reliability of K-APDS was found to be high with Cronbach's ⍺=.91. Conclusion: K-APDS can screen for delirium with 2 or more points, excellent validity and reliability have been verified. Therefore, this tool could be applied immediately in the clinical field, and will contribute to the early detection of delirium, enabling rapid interventions.

Effects of Cooking Method and Temperature on the Lipid Oxidation of Electron-Beam Irradiated Hanwoo Steak. (가열방법 및 온도가 전자선 조사한 한우 steak의 지질산화에 미치는 영향)

  • This study was carried out to investigate the effect of electron beam irradiation and cooking temperature on physico-chemical characteristics and lipid oxidation of beef. A total of six beef carcasses ($280\∼300 kg$) that were quality grade $1^{+}$(marbling score No.7, meat color No.4, maturity No.1, texture No.1) was purchased at the commercial slaughter house. The carcasses were transported and washed using high pressure water, and pasteulized with $ 50\% $ ethyl alcohol in the laboratory. After the carcasses were deboned and trimmed, loin and round were taken out to make steak (1.5cm thickness) or ground beef respectively. Samples were wrap or vacuum packaged and irradiated with 0, 3, 4.5, 6 and 7.5 kGy using electron-beam accelerator at Samsung Heavy Industries Ltd. Co. (in Taejun). Irradiated samples were cooked with different methods(electronic pan and gas oven) and temperatures ($ 60^{\circ}C, 70^{\circ}C and 80^{\circ}C$) and used to measure fatty acid composition, TBARS, cholesterol oxide products and panel test scores. The content of saturated fatty acids increased by increasing heating temperature in oven boiling steak (OBS) and pan boiling steak (PBS), and there was no difference by electron-beam irradiation. Both irradiated and non-irradiated treatment were high as the heating temperature increased in TBARS by heating temperature in PBS (p < 0.05) and the amount of Malonaldehyde (MA), standard of fat deterioration, was increased in OBS (p < 0.05). Non-irradiated and 3, 6 kGy treatment produced about 2 fold amount of MA at $ 60^{\circ}C $ compared with $ 80^{\circ}C $. In comparison with PBS, OBS produced much amount of MA and a bit different from non-irradiated treatment but did not show no tendency. As irradiation levels and heating temperature increased, the amount of cholesterol oxides products was increased and also pan-heating method, direct heating method, significantly increased the degree of oxidation compared with oven-heating method, indirect heating method (p < 0.05).

Changes in the Quality of Loin from Pigs Supplemented with Dietary Methyl Sulfonyl Methane during Cold Storage (식이유황(硫黃)을 급여한 돈육 등심의 저온저장 중 품질특성 변화)

  • This study was conducted to compare the quality of the pork from finishing pigs that were fed diets containing different levels of methyl sulfonyl methane (MSM). A total of 135 crossbred pigs $(Landrace{\times}Yorkshire{\times}Duroc)$ were fed either with a control commercial diet or the control diet supplemented with 300- and 500-ppm MSM for 158d. The pigs were slaughtered at approximately 110kg live weight and were transported to the local slaughterhouse for electrical stunning followed by exsanguination. After the slaughter, the pork muscles were dissected from each carcass, placed in wrap package bags, and stored for 8d at $4^{\circ}C$. The TEARS values of the pigs that were fed MSM diets were significantly lower (p<0.05) compared with those of the pigs that were fed with non-supplemented diets. The Na, Mg, and Ca contents of the dietary MSM were significantly lower (p<0.05) than those of the non-supplemented diets, but the Fe, Cu, and Zn contents of the dietary MSM were significantly higher (p<0.05) than those of the non-supplemented diets, and the increased level of MSM supplementation resulted in higher sulfur contents. There was no difference among the diets in terms of amino acid content. The dietary supplementation with MSM, however, led to increased saturated fatty acid and decreased unsaturated fatty acid (%) in the pork muscles (p<0.05). The sensory panelists recorded greater marbling and overall acceptability scores in the samples with 500-ppm-MSM dietary supplementation (p<0.05). These data suggest that supplementing pig diets with MSM can improve the quality of the pork and can enhance the eating quality because the sensory panels found that the pork from pigs that were fed an MSM-supplemented diet had better sensory characteristics.

Aggressive behavior of Male Rats following Hippocampal Ablation (뇌 해마를 떼어버린 흰쥐의 공격적 행동)

  • An experiment was designed to see if the hippocampus exerts any influence upon the aggressive behavior of male rats. Fighting between rats was observed for the estimation of aggressiveness. Seventeen rats in which the hippocampus was almost totally removed through a small hole with a diameter around 3 mm made in the neocortex at the boundary between the parietal and occipital lobes (hippocampal group), 8 rats with similar neocortical damage alone (operated control group), and 17 normal control rats (normal group) were prepared and subjected to the experiment 3 months after the operation. Applying electric shock of short duration to the feet in a box with grid floor, a fight was provoked between an animal belonging to the hippocampal group and one belonging to the nor-mal group, between a rat of the hippocampal group and one of the operated control group, and also between a rat of the operated control group and one of the normal group. Three observers judged the performance of each animal independently and described it as winning, defeated, tied, or non-fighting. Fifteen shocked trials were administered to each pair of animals with around 2 minutes' interval between each trial. An animal received a 'judging score' of 3 when it won more frequently than was defeated, a judging score of 2 when it won as frequently as was defeated, when all fights were tied, or when no fighting occurred, while it received a judging score of 1 when it won less frequently than was defeated. Group differences in performances were analyzed in terms of judging score using Kolmogorov-Smirnov test for one sample. The results obtained were as follows: 1. In the fights between the hippocampal and the normal groups, the hippocampal animals made significantly better judging scores than the normal animals did (Table 1). 2. There was no significant difference between the hippocampal and the operated control group as to the judging scores they made in the course of fights between the two groups. However, the hippocampal animals tended to dominate over the operated control group as judged by comparing the total 'winning' of the former (30) to that of the latter (14) (Table 2). 3. The total judging score made by the operated control group in the course of the fights against the normal group was not significantly superior to that made by the normal group (Table 3). It was inferred from the above results that, though inconspicuous, the hippocampus tended to exert an inhibitory influence upon the aggressive behavior of male rats.
