A comparison of Multilayer Perceptron with Logistic Regression for the Risk Factor Analysis of Type 2 Diabetes Mellitus (제2형 당뇨병의 위험인자 분석을 위한 다층 퍼셉트론과 로지스틱 회귀 모델의 비교)

  • The statistical regression model is one of the most frequently used clinical analysis methods. It has basic assumption of linearity, additivity and normal distribution of data. However, most of biological data in medical field are nonlinear and unevenly distributed. To overcome the discrepancy between the basic assumption of statistical model and actual biological data, we propose a new analytical method based on artificial neural network. The newly developed multilayer perceptron(MLP) is trained with 120 data set (60 normal, 60 patient). On applying test data, it shows the discrimination power of 0.76. The diabetic risk factors were also identified from the MLP neural network model and the logistic regression model. The signigicant risk factors identified by MLP model were post prandial glucose level(PP2), sex(male), fasting blood sugar(FBS) level, age, SBP, AC and WHR. Those from the regression model are sex(male), PP2, age and FBS. The combined risk factors can be identified using the MLP model. Those are total cholesterol and body weight, which is consistent with the result of other clinical studies. From this experiment we have learned that MLP can be applied to the combined risk factor analysis of biological data which can not be provided by the conventional statistical method.

Effects of Conjugated Linoleic Acid and Stearic Acid on Apoptosis of the INS-1 β-cells and Pancreatic Islets Isolated from Zucker Obese (fa/fa) Rats

  • To determine whether dietary fatty acids affect pancreatic $\beta$-cell function, the INS-1 $\beta$-cells and the pancreatic islets isolated from Zucker obese (fa/fa) rats were cultured with stearic acid and conjugated linoleic acid (CLA). As a result, DNA fragmentation laddering was substantially decreased in the INS-1 $\beta$-cells and the isolated pancreatic islets cultured with 2 mM CLA compared to those cultured with stearic acid. To investigate the mechanism by which CLA alleviates cell apoptosis under DNA fragmentation assay, we examined mRNA expressions of apoptosis-related proteins including Bax and Bcl-2 associated with cell death agonist and antagonist, respectively, in both INS-1 cells and islets cultured with 2 mM fatty acids. Bax mRNA expression was not altered by either stearic acid or CLA, whereas Bcl-2 mRNA expression was enhanced by CLA when compared to the stearic acid cultures. However, there were no changes in cell apoptosis and apoptotic-regulating gene products in either INS-1 cells or isolated islets treated with or without 2 mM CLA. It is concluded that CLA maintains $\beta$-cell viability via increased Bcl-2 expression compared to the stearic acid cultures, which may help to alleviate, at least somewhat, the onset of NIDDM in the physiological status. More detailed study is still needed to elucidate the effect of CLA on the prevention of fatty acid-induced $\beta$-cell apoptosis.

Nutrition Contents and Health Status, Blood Glucose Response Effect for NIDDM Patients of Inonotus Obliquus in Culturing Method by Using Unpolished Rice (현미에서 배양된 차가버섯 분말의 영양성분과 제2형 당뇨환자의 건강지표 및 혈당관련인자에 미치는 영향)

  • The purpose of this study was to investigate the possibility of Inonotus obliquus as an functional resource. To assess the effects of Inonotus obliquus in culturing method by using unpolished rice in 15non-insulin dependent diabetes mellitus persons, we examined changes of hematological and chemical variables, DM indices content in serum during the Inonotus obliquus supplementation. GOT, GPT concentrations were significantly decreased. But TIBC content was significantly increased. Fasting glucose level during Inonotus obliquus supplementation was decreased respectively. These results show that modest dose of Inonotus obliquus in culturing method by using unpolished rice supplementation can decrease fasting blood glucose level without any changes in health indices and nutrition status of the non-insulin dependent diabetes mellitus person. These results indicated that Inonotus obliquus in culturing method by using unpolished rice diet is effective therapeutic regimen for the control of metabolic derangements in diabetes mellitus. Also, the results imply that Inonotus obliquus in culturing method by using unpolished rice can be used as possible functional food materials. However, large amounts of Inonotus obliquus in culturing method by using unpolished rice showed be used cautiously application.

Group Lunch Visits at the Public Health Center Improve Glycemic Control in Older Adults with Type 2 Diabetes Mellitus (보건소 중식실습교육이 제 2 형 당뇨병환자의 혈당개선에 미치는 영향)

  • This study was carried out to investigate the effect of nutrition education program for diabetic patients on the glycemic control at the public health center. The study subjects, aged 61.7 $\pm$ 9.4 years, were 93 sex-and age-matched patients with type 2 diabetes mellitus. They were divided into three groups: nutrition education & diet practice group (EDG), nutrition education-only group (EG), and the control group (CG). Height, weight, and the postprandial 2 hour blood glucose (PP2) were measured at baseline, and 4, 6 and 8 week after the diabetic nutrition education program. At baseline there were no differences in height, weight, and blood glucose levels among the three groups. Nutrition education programs, especially that with group lunch practice sessions were found to be effective in lowering the blood glucose levels in patients with NIDDM patients. At 4 week blood glucose levels were decreased by 40.6% and 19.6% in EDG and EG, respectively, which was further dropped by 50.2% and 35.1% at 8 week, as compared to the CG group. For the EDG group, the total energy intake, which was 162.3% of the prescription before the diet counselling session, was decreased to 113.6% of the prescription after the lunch visit, with most decrease coming from the reduction in carbohydrate and fat intake. Multiple stepwise regression analysis revealed that the total energy intake explained 47.9% and 57% of blood glucose changes for men and women, respectively, and that percent energy intake from protein explained 15.8% for women. These results demonstrate that the public health center nutrition education programs for diabetic patients, especially that with group lunch practice sessions are very effective for the glycemic control in patients with diabetes mellitus.

Effects of Ojung-hwan on Blood Glucose, Hyperlipidemia, Polyol Pathway and Antioxidative Mechanism in Ob/ob Mouse (Ob/ob mouse에서 오정환(五精丸)이 혈당, 고지혈증, Polyol Pathway 및 항산화작용에 미치는 영향)

  • Objectives : Diabetes is a disease in which the body does not produce or properly use insulin. Etiological studies of diabetes and its complications have shown that oxidative stress might play a major role. Therefore, many methods have been tried to regulate free oxygen radicals for treating diabetes and its complications. Ojung-hwan, composed of five crude herbs, has been considered effective for treating symptoms of aging. In male ob/ob mouse of severe obesity, hyperinsulinemia and hyperlipidemia, which are features of NIDDM, the hyperglycemic activities and mechanisms of Ojung-hwan were examined. Methods : Mice were grouped and treated for 5 weeks as follows. Both the lean (C57/BL6J black mice) and diabetic (ob/ob mice) control groups received standard chow. The experimental groups were fed a diet of chow supplemented with 30 and 90 mg Ojung-hwan per 1 kg of body weight for 14 days. The effects of Ojung-hwan extract on the ob/ob mice were observed by measuring the serum levels of glucose, insulin, lipid components, and the kidney levels of superoxide anion radical (${\cdot}\;O{_2}{^-}$), MDA+HAE, GSH/GSSG ratio, and also the enzyme activities involved in polyol pathway. Results : Ojung-hwan lowered the levels of serum glucose and insulin in a dose-dependent manner. Total cholesterol, triglyceride and free fatty acid levels decreased, while the HDL-cholesterol level increased, in Ojung-hwan treated groups. Renal aldose reductase and sorbitol dehydrogenase activities increased in the ob/ob mice, whereas they were inhibited in the Ojung-hwan treated groups. Ojung-hwan inhibited the generation of ${\cdot}\;O{_2}{^-}$ in the kidney. Finally, MDA+HAE levels increased and GSH/GSSG ratio decreased in the ob/ob mice, whereas they improved in the Ojung-hwan treated groups. Conclusions : Ojung-hwan showed antidiabetic and antihyperlipidemic activities by regulating theactivities of polyol pathway enzymes, scavenging reactive oxygen species and reducing the MDA+HAE levels in the ob/ob mice.

Plasma total homocysteine and macrovascular complications are associated with food and nutrient intake in patients with Type II diabetes mellitus

  • The present study was conducted to document the association between plasma homocysteine levels and the presence of macrovascular angiopathy with food and nutrient intake patterns among patients with Type II diabetes mellitus in Korea. Plasma total homocysteine concentration was analyzed by HPLC-fluorescence detector method in 127 patients with non-insulin dependent diabetes mellitus. Logistic regression analyses were performed respectively to study the association of plasma homocysteine levels with clinical and dietary characteristics and macroangiopathy (MA). The average plasma homocysteine level of patients with MA was $14.2{\mu}mol/l$, which was significantly higher than that of patients without MA ($11.4{\mu}mol/l$). The proportions of patients with MA showed a significant difference, being 32.3% in hyperhomocysteinemic ($>14.0{\mu}mol/l$) patients and 13.5% in others with homocysteine levels lower than $14.0{\mu}mol/l$. Odds ratios for macroangiopathy by tertile increase of plasma homocysteine concentration were 1.633 ($Q_2$) and 4.831 ($Q_3$), when adjusted for age, sex, and cigarette smoking. Patients with MA consumed reduced amounts of vitamin $B_1,\;B_2$, and folate. The results indicate that the plasma homocysteine levels are significantly increased in NIDDM patients who have macroangiopathy. Dietary management such as increased fruits and vegetables and decreased potatoes and starches might be beneficial for the prevention of macroangiopathy in diabetic patients.

A Study of the Relationship among Self-Efficacy, Sick-Role Behavior and Metabolic Control in Diabetic Patients (당뇨병 환자의 자기효능, 환자역할행위 이행 및 대사조절간의 관계)

  • The purpose of this study was to identify the degree of self-efficacy, sick-role behavior and metabolic control in patients with diabetes and to identify the relationships among those variables. The subject of the study were composed of 204 NIDDM patients from 2 hospitals, who were visiting the outpatient clinic in Taegu. Data were collected from Sep. 18th to Oct. 5th, 1996. In data analysis, an SPSS/PC+ program was utilized for descriptives correlation, T-test, and ANOVA. The results are as follows. 1) The mean scores for self-efficacy were 65.74 (range 30.83-90.68), on a 100 point scale. 2) The mean scores for sick-role behavior were 3.36(range 1.80-4.67), on a 5 point scale. 3) The mean score for metabolic control was 7.63 (range 5.0-13.2). 4) Self-efficacy was significantly correlated with sick-role behavior(r=.3614, p<0.01) but there were no significant correlations with self-efficacy and metabolic control (r=-.1045, p>0.01), sick-role behavior and metabolic control (r= - .1288, p>0.01). 5) There were significant differences in self-efficacy according to sex(t=4.47, p=0.000) and education(F=8.27, p=0.000). 6) There were no significant differences in sick -role behavior according to demographic characteristics. 7) There were significant differences in metabolic control according to the duration of diagnosed diabetess(F=4.13, p=0.007) and complication (t=2.47, p=0.014). These results suggest that for improvement in sick-role behavior nursing intervention needs to be directed at promoting self-efficacy.

Pharmacokinetic-Pharmacodynamic Modeling for the Relationship between Glucose-Lowering Effect and Plasma Concentration of Metformin in Volunteers

  • Metformin is a biguanide antihyperglycemic agent often used for the treatment of non-insulin dependent diabetics (NIDDM). In this study, the pharmacokinetics and pharmacodynamics of metformin were investigated in Korean healthy volunteers during a fasting state for over 10 h. In order to evaluate the amount of glucose-lowering effect of metformin, the plasma concentrations of glucose were measured for a period of 10 h followed by the administration of metformin (oral 500 mg) or placebo. In addition, the concentration of metformin in blood samples was determined by HPLC assay for the drug. All volunteers were consumed with 12 g of white sugar 10 minutes after drug intake to maintain initial plasma glucose concentration. The time courses of the plasma concentration of metformin and the glucose-lowering effect were analyzed by nonlinear regression analysis. The estimated $C_{max}$, $T_{max}$, $CL_{t}$/F (apparent clearance), V/F(apparent volume of distribution), and half-life of metformin were 1.42$\{pm}$0.07 $\mu\textrm{g}$/mL, 2.59$\{pm}$0.18h, 66.12$\{pm}$4.6 L/h, 26.63 L, and 1.54 h respectively. Since a significant counterclock-wise hysteresis was found for the metformin concentration in the plasma-effect relationship, indirect response model was used to evaluate pharmacodynamic parameters for metformin. The mean concentration at half-maximum inhibition $IC_{50}$, $k_{in}$, $k_{out}$ were 2.26 $\mu\textrm{g}$/mL, 83.26 $H^{-1}$, and 0.68 $H^{-1}$, respectively. Therefore, the pharmacokinetic-pharmacodynamic model may be useful in the description for the relationship between plasma concentration of metformin and its glucose-lowering effect.

AN EXPERIMENTAL STUDY ON SUPEROXIDE DISMUTASE- AND CATALASE- ACTIVITY IN GINGIVAL TISSUES IN DIABETIC PATIENTS (당뇨환자의 치은조직내 Superoxide Dismutase와 Catalase의 활성도에 관한 실험적 연구)

  • Oxygen derived radicals($O_2\;^-$, $H_2O_2$, and $OH^-$) are thought to play a role in a lot of human diseases. And it has been believed that antioxidant enzymes such as superoxide dismutase(SOD) and catalase could protect the tissues from damage resulting from the oxygen derived free radicals. The purpose of this study was performed to investigate the activity of the SOD(CuZn- and Mn-SOD) and catalase in inflammatory gingival tissues and the correlation between boold glucose level and antioxidants and age in non-insulin dependent diabetes mellitus(NI- DDM) patients. For this study, the patients were classified into normal, inflammatory, and diabetic, and ten their papillary bleeding index(PBI) and gingival index were checked. Subjects consisted of 11 healthy patients with no inflammatroy gingiva, 20 adult periodontitis patients, and 8 diabetic patients, aged 33 to 66(average: 44.62). The blood glucose level of diabetic group was ranged from 120ml/dl to 160ml/dl(physical status 0 : averge : 135.67ml/dl). Gingival tissues were surgically obtained from the patients during periodontal surgery, extraction, and clinical corwn lenghening procedure. The activity of CuZn and Mn- SOD and catalase in the gingival tissues was measured by using UV-spectrophotometer by the same methods that Crapo et al. And Aebi did, respectively. The results were as follows : 1. The Mn-SOD activity was significantly lower in inflammatory group in comparison to normal group(P<0.05), and the activities of antioxidants in diabetic group were not significant in comparison to normal inflammatory group(P>0.05). 2. The activities of antioxidants showed little variation among individuals of different ages (P>0.05). 3. The higher blood glucose level was, the higher gingival index was(P<0.05). 4. There was no correlation between blood glucoe level and activity of antioxidant in inflammatory gingival tissues of NIDDM patients(P>0.05). In conclusion, these results, within the limits of the present experiment, suggest that the activity of Mn-SOD might reflect the inflammatory status of gingival tissue, and the activity of antioxidants was independent of blood glucose level of diabetic patients in physical status 0.
