Accelerated Rehabilitation After Arthroscopic Bankart Repair - A Prospective Randomized Clinical Study - (관절경적 Bankart 봉합술후 적극적 재활치료 - 전향적 임상연구 -)

  • Purpose: In this prospective, randomized study, we compared the results of early motion versus conventional immobilization after the arthroscopic Bankart repair. Materilal and Methods : We performed an arthroscopic Bankart repair using suture anchors in 62 patients with traumatic anterior shoulder instability and randomized them into two groups; Group 1 (n=28; mean age, 28 years) underwent three-week of immobilization and conventional rehabilitation program, while Group 2 (n=34; mean age, 29 years) underwent an accelerated rehabilitation program with staged range of motion and strengthening exercises starting from the immediate postoperative day. Selected patients were non-athletes with a classic Bankart lesion and a robust labrum. Analysis of outcome included pain scores (6-week and follow-up: 31(9 months), range of motion, return to activity, recurrence, patients’ satisfaction with each program, and shoulder scores (ASES, UCLA, and Rowe). Results : The recurrent rate was not different between the two groups (2 anterior apprehension from each group) (p=0.842). Patients with accelerated rehabilitation resumed functional range-of-motion faster and returned earlier to the functional level of activity (p<0.05). Accelerated rehabilitation decreased postoperative pain and more patients were satisfied with this program (p<0.05). No differences were found between the two groups at the follow-up with regards to the shoulder scores, return to activity, pain score, and the range-of-motion. Conclusions : Early mobilization after arthroscopic Bankart repair does not increase the recurrence rate in selected patients. Although the final outcomes are similar in both groups, the accelerated rehabilitation program promotes functional recovery and reduces postoperative pain, which enables patients an early institution of desired activities.

Evaluation of Fate of $NH_4{^+}$ of Condensed Molasses Solubles(CMS) in Soil Using by $^{15}N$-Tracer Method (중질소 추적자법을 이용한 Condensed Molasses Solubles(CMS) 함유 $NH_4{^+}$의 토양 중 행동 연구)

  • To study the behavior of $NH_4{^+}$ of CMS (condensed molasses solubles) in soil, a laboratory incubation experiment was conducted during a period of up to 21 days at $25^{\circ}C$. The $NH_4{^+}$ of CMS was labeled with $^{15}N$ and was applied to water-unsturated and water-saturated conditions. Soil pH was gradually decreased from 6.1 to 5.4 under unsaturated condition. However, soil pH was increased to 6.5 within 2 days under saturated condition and then was constant. The concentration of ammonium was decreased 3 times faster under unsaturated condition than under saturated condition. The concentration of nitrate was increased from 17.4 to $155.4mg\;kg^{-1}$ under unsaturated condition. But concentration of nitrate was kept with low(below $8.0mg\;kg^{-1}$) under saturated condition. During the incubation, 52.4% of $^{15}NH_4{^+}$ applied was existed in the form of $NO_3{^-}$ by nitrification under unsaturated condition. Most of applied nitrogen was immobilized within 4 days of incubation. On 21 days of the incubation the percentage of immobilized nitrogen derived from $^15NH_4{^+}$(NDFA) was 19.6% under unsaturated condition and 17.0% under saturated condition. The percentage of unaccounted N, which was lost by denitrification, was 28.4% under unsturated condition and 67.6% under saturated condition.

Decay Rate and Nutrients Dynamics during Decomposition of Oak Branches (상수리나무 가지의 분해 및 분해과정에 따른 영양염류 변화)

  • Decay rate and nutrient dynamics during decomposition of oak (Quercus acutissima) branches were investigated for 33-months in Kongju, Korea. After 33-months, remaining weight of B₁, B₂ and B₃ was 44.5%, 58.5% and 55.37%, respectively. Decomposition constant (k) for B₁, B₂ and B₃ was 0.294/yr, 0.195/yr, 0.215/yr, respectively. N concentration in decomposing oak branches increased in all diameter classes. After 33-months, remaining N in B₁, B₂ and B₃ was 101.2%, 91.9%, 104.4%, respectively. P concentration in decomposing oak branches increased in B₁ and B₂, and there was no immobilization period. After 33-months, remaining P in B₁, B₂ and B₃ was 57.2%, 74.4%, 53.9%, respectively. K concentration in decomposing oak branches decreased significantly. Remaining K in B₁, B₂ and B₃ was 7.7%, 17.1% and 17.2%, respectively, which was significantly lower than other nutrients. Ca concentration in decomposing oak branches increased in B₂ and B₃. After 33-months, remaining Ca in B₁, B₂ and B₃ was 58.5%, 47.8% and 75.2%, respectively. Initial concentration of Mg in oak branch was higher in smaller diameter class. After 33-months, remaining Mg in B₁, B₂ and B₃ was 44.3%, 57.9% and 47.7%, respectively.

  • There are several purpose to introduce microorganism into the Soil. The major purpose is to promote plant growth and inhibit plant pathogens. The model example is to put in nitrogen fixing symbiotic bacteria, Pythium and Rhizobium. In order to achieve the intended goal, the introduced microorganism should survive and colonize with sufficient density. The survival of introduced microorganism depend upon biotic and abiotic factors. Predation and competition are important among biotic factor. Water tension, organic carbon, inorganic nutrients(N, P), pH are important factor among abiootic factor. Soil texture and distribution of soil pore are also important in the survival and colonization of introduced microorganism. Selection by soil ecosystem for inoculant is a crucial factor for colonization. Good example are control of autochtonous microorganism and the introduction of surfactant biodegrading Pseudomonas. Sometimes, carriers such as peat and montmorillonite can be added to help colonization. Carriers can protect introduced microorganism by supplying protective microhabitat. Organic polymer is also used as a carrier to immobilize bacteria or industrial enzymes. Examples of these carrier are calcium alginate, agarose and k-carrageenan. The function of these carrier is to provide microhabitat and help colonization for introduced microorganism.

Stabilization of Pb Contaminated Army Firing Range Soil using Calcined Waste Oyster Shells (소성가공 굴껍질을 이용한 군부대 사격장내 고농도 납 오염토양의 안정화)

  • The objective of this study was to investigate the effectiveness of stabilization for army firing range soil highly contaminated with Pb (total Pb: 29,000 mg/kg) using calcined waste oyster shells. The calcination was conducted to activate quicklime from calcite. In order to evaluate the effectiveness of calcination, both natural oyster shells (NOS) and calcined oyster shells (COS) were applied to the Pb contaminated soil. Stabilization was conducted by mixing the contaminated soil with oyster shell media at 5-20 wt% and cured for 28 days. Following 28 days of curing, Pb leachability was measured based on the Korean Standard Test method (0.1 N HCl extraction). The treatment results showed that the COS treatment outperformed the NOS treatment. All of the NOS treatments failed to meet the Korean warning standard of 100 mg/kg. However, the Pb concentrations were significantly reduced to 47 mg/kg and 3 mg/kg upon 15 wt% and 20 wt% COS treatments, respectively which passed the Korean warning standard. Moreover, -#20 mesh materials were more effective than the -#10 mesh materials in effectively reducing Pb leachability. The scanning electron microscopy (SEM)-energy dispersive X-ray spectroscopy (EDX) results indicated that Pb immobilization was strongly linked to Al and Si.

Cycloaddition of Carbon Dioxide to Allyl Glycidyl Ether Using Silica-supported Ionic Liquid as a Catalyst (실리카에 고정화된 이온성액체를 촉매로 이용한 알릴글리시딜에테르와 이산화탄소의 부가반응)

  • In this study, imidazolium salt ionic liquid on amorphous silica was prepared and its catalytic performance in the cycloaddition of $CO_2$ with allyl glycidyl ether (AGE) to produce heterocyclic carbonate was investigated. The ionic liquid was generated on chloropropyl functionalized silica through the immobilization of imidazole. The prepared catalyst was characterized using a number of instrumental analysis including XRD, BET, $^{29}Si$ MAS-NMR and SEM. $^{29}Si$ MAS-NMR showed that the ionic liquid formed adduct with the chloropropyl groups attached to the silica surface. The immobilized ionic liquid showed very good catalytic activity for the cycloaddition of $CO_2$ with AGE, showing 55-61% of AGE conversion with over 85% of the carbonate selectivity at $80-120^{\circ}C$. Its AGE conversion and selectivity to the carbonate were even higher than the homogeneous analog, 1-n-butyl-3-methyl imidazolium bromide (BMImBr).

Analysis of an Immobilized β-Galactosidase Reactor with Competitive Product Inhibition Kinetics (경쟁적 저해를 갖는 고정화 β-galactosidase 반응기의 해석)

  • The present study deals with the immobilization of Kluyveromyces lactis ${\beta}$-galactosidase on a weak ionic exchange resin (Duolite A568) as polymer support. ${\beta}$-Galactosidase was immobilized using the adsorption method. A kinetic study of the immobilized enzyme was performed in a packed-bed reactor. The adsorption of the enzyme followed a typical Freundlich adsorption isotherm. The adsorption parameters of k and n were 14.6 and 1.74, respectively. The initial rates method was used to characterize the kinetic parameters of the free and immobilized enzymes. The Michaelis-Menten constant ($K_m$) for the immobilized enzyme (120 mM) was higher than it was for the free enzyme (79 mM). The effect of competitive inhibition kinetics was studied by changing the concentration of galactose in a recycling packed-bed reactor. The kinetic model with competitive inhibition by galactose was best fitted to the experimental results with $V_m$, $K_m$, and $K_I$ values of 46.3 $mmolmin^{-1}mg^{-1}$, 120 mM, and 24.4 mM, respectively. In a continuous packed-bed reactor, increasing the flow rate of the lactose solution decreased the conversion efficiency of lactose at different input lactose concentrations. Continuous operation of 11 days was conducted to investigate the stability of a long-term operation. The retained activity of the immobilized enzymes was 63% and the half-life of the immobilized enzyme was found to be 15 days.

Effect of Impregnation and Modification on Activated Carbon for Acetaldehyde Adsorption (아세트알데하이드 흡착을 위한 활성탄의 첨착 및 개질 효과)

  • In this study, the acetaldehyde removal characteristics of activated carbon (AC) for air purifier filters were investigated using metal catalysts-impregnation and functional group-modification method. The AC with a high specific surface area(1700 m2/g) and micropores was prepared by KOH activation of coconut charcoal and the efficiency of catalyst and functional group immobilization was examined by varying the drying conditions within the pores after immersion. The physical properties of the prepared activated carbon were analyzed by BET, ICP, EA, and FT-IR, and the acetaldehyde adsorption performances were investigated using gas chromatography (GC) at various impregnation and modified conditions. As the concentration of impregnation solution increased, the amount of impregnated metal catalysts increased, while the specific surface area showed a decreasing trend. The adsorption tests of the metal catalyst-impregnated and functional group-modified activated carbons revealed that excellent adsorption performance in compositions MgO10@AC, CaO10@AC, EU10@AC, and H-U3N1@AC, respectively. The MgO10@AC, which showed the highest adsorption performance, had a breakthrough time of 533.8 minutes and adsorption capacity of 57.4 mg/g for acetaldehyde adsorption. It was found that the nano-sized MgO catalyst on the activated carbon improved the adsorption performance by interacting with carbonyl groups of acetaldehyde.

Evaluating the Usability of Medical Body Wrap in Whole Body Bone Scan (전신 뼈 검사에서 의료용 신체 고정구의 유용성 평가)

  • Purpose: When performing nuclear medicine examinations, body wraps or plastic supports are used to support and immobilize the patient's upper extremities to prevent patient safety accidents. However, the existing plastic supports compromised patient and staff safety, including finger entrapment and falls. Moreover, the body wrap provided by manufacturers compromised image quality such as upper extremities cutoff during whole body bone scan. Therefore, a new design of body wrap was developed to improve the issue, and this study aims to evaluate the usability of this medical body wrap. Materials and Methods: To evaluate the usability of the newly designed medical body wrap, a quality assessment of whole body bone scan images and a user satisfaction survey were conducted. Adult patients (male:female=129:152, age: 60.3±12.4 years, BMI: 24.0±4.2) aged 16 years or older who underwent a whole body bone scan during two periods: June to July 2022 (before improvement, n=139) and June to July 2023 (after improvement, n=142) were randomly selected for image quality evaluation. Five radiotechnologists visually evaluated the posterior view of the whole body bone image, including the left and right elbow (2 points), arm (2 points), whether the hand is extended (2 points), whether the hand is included (2 points), and the number of visible fingers (10 points), with a total of 18 points, which were converted to 100 points and analyzed for difference before and after improvement using an independent sample t-test. The user satisfaction questionnaire was evaluated using a 5-point Likert scale among 16 radiotechnologists from three general hospitals who experienced the new body wrap. Results: The image quality assessment was 82.0±13.8 before the improvement and 89.3±10.1 after the improvement, an average of 7.3 points higher, with a statistically significant difference (t=5.02, p<0.01). The user satisfaction survey showed an overall satisfaction rating of 4.1±0.8 for ease of use, 3.8±0.7 for scan preparation time, 3.9±0.7 for patient safety, 3.8±1.2 for scan accuracy, and 4.2±0.7 for recommendation (87.5% questionnaire response rate). Conclusion: The developed body wrap showed higher image quality and user satisfaction compared to the old method. Considering these results, it is deemed that the new body wrap may be more useful than existing methods.

Decomposition and Nutrient Dynamics of Leaf Litter of Camellia japonica L. in Korea (동백나무(Camellia japonica L.) 낙엽의 분해와 영양원소의 동태)

  • Litter fall is a source of nutrients and carbon transfer in terrestrial ecosystems. Litter decomposition provides nutrients needed for plant growth, sustains soil fertility, and supplies $CO_2$ to the atmosphere. We collected the leaf litter of evergreen broadleaf tree, Camellia japonica L., and carried out a decomposition experiment using the litterbag method in Ju-do, Wando-gun, Korea for 731 days from Dec 25, 2011 to Dec 25, 2013. The leaf litter of C. japonica remained 42.6% of the initial litter mass after experiment. The decay constant (k) of C. japonica leaf litter was $0.427yr^{-1}$. The carbon content of C. japonica leaf litter was 44.6%, and the remaining carbon content during the decomposition tended to coincide with the changes in litter mass. The initial nitrogen and phosphorus content was 0.47% and 324.7 mg/g, respectively. The remaining N in decaying litter increased 1.66-fold in the early decomposition stage, then gradually decreased to 1.18-fold after 731 days. The content of P showed the highest value (1.64-fold of initial content) after 456 days, which then fell to a 1.15-fold after 731 days. The remaining Ca, K, Mg and Na content in C. japonica leaf litter tended to decrease during decomposition. The remaining K showed a remaining mass of 8.9% as a result of rapid reduction. The initial C/N and C/P ratio of C. japonica leaf litter was 94.87 and 1368.5, respectively. However, it tended to decrease as decomposition progressed because of the immobilization of N and P (2.78 and 2.68-fold of initial content, respectively) during the leaf litter decaying. The study results showed that N and P was immobilized and other nutrients was mineralized in C. japonica leaf litter during experimental period.
