A Review of Discharge Education Needs of Children after Musculoskeletal Surgery (근골격계 수술 아동의 퇴원교육 요구에 대한 고찰)

  • Purpose: The purpose of this study is to evaluate the current status of discharge education and nursing needs of children after musculoskeletal surgery. Methods: The literature from research databases was reviewed and analyzed (using PubMed, RISS review, KISS, and Dbpia). Results: Children who have undergone musculoskeletal surgery face physical challenges due to the effects of splints and immobilization, and experience pain for a long time. Therefore, home care after discharge is of paramount importance in these cases. The discharge education needs of each patient vary with regard to the topic and period. Discharge education should be systematic and consistent, with an emphasis on understanding the needs of the patients and parents. Conclusion: While there are studies on the nursing needs of adult musculoskeletal surgery patients at discharge, studies on discharge nursing needs in children who have undergone musculoskeletal surgery are still insufficient. It is necessary to specifically identify the educational needs of parents of children undergoing musculoskeletal surgery. Interventions that improve the voluntary participation of patients and parents in active self-care need to be developed.

  • To evaluate the clinical result of surgical treatment of malignant tumors in shoulder girdle, nine patients who were treated in Department of Orthopedics, Catholic University Medical College between January 1991 and December 1993, were evaluated. There were 5 men, 4 women. The mean age at operation was 47 years(range from 22 to 64 years). Of 9 patiens, 2 were soft tissue tumors(1 MFH, 1 dermatofibrosarcoma protuberance); one was treated with forequarter amputation, and the other with wide excision, Seven were bone tumor(2 chondrosarcoma, 1 osteosarcoma, 1 MFH, 1 plasmacytoma, 1 thyroid carcinoma metastasis, 1 malignant schwannoma); one patient was treated with segmental excision of proximal humerus, 4 with Malawer type I-A resection and arthroplasty or arthrodesis, 1 with Malawer type V-B resection and arthrodesis. Five patients received adjuvant chemotherapy, with or without local radiation therapy, and one patient received radiation therapy alone. All patients have survived now, but I had local recurrence. Functional results of arthrodesis and arthroplasty were similar.

Trends of Korean Medicine Treatment after Musculoskeletal Disorder Surgery: A Literatural Review (근골격계 질환 수술 후 한방치료 동향(국내 학술지를 중심으로))

  • Objectives The purpose of this review is to analyse the trend in papers related with Korean Medicine Treatment after musculoskeletal disorder surgery. Methods We reviewed Korean Medicine papers by searching Korean web databases 'Korea Traditional Knowledge Portal', 'Scientific and Technological Information Integration Service (NDSL)', 'Academic Research Information Service (RISS)', 'Korea Medical Informati on Portal (OASIS)'. We classified the papers by the year of publishment, the title of journals, the type of study, surgery region, chief complain after surgery, main treatment, periods after surgery, assessment for outcomes. Results 1. Korean Medicine treatment after musculoskeletal disorder surgery has received more attention than in the past and there are attempts to do various studies besides the case reports. 2. 41 research papers were divided in to 3 original articles, 3 review articles, 35 case reports. But almost presented a low level of evidence. 3. Pain was the most common symptom after the musculoskeletal disorder surgery. Pain should be the primary goal of Korean rehabilitation treatment after musculoskeletal disorder surgery. 4. Assessment tools for outcome were concentrated in questionnaries, VAS and NRS. In order to evaluate better, it is necessary to evaluate the overall condition of the patient such as the quality of life evaluation and patient satisfaction. Conclusions In this study, we expect that the development and clinical application of Korean rehabilitation treatment program after musculoskeletal disorder surgery will be actively pursued.

The Analysis of the Cytokine Expression in Musculoskeletal Tumors (근골격계 종양에서 사이토카인 발현의 분석)

  • The cytokines are the hormone-like proteins, which are produced in the mononuclear cells. They have many roles, such as immune mediators, cell differentiations, angiogenesis. The chemokines have chemotactic effects which control the host immune response. There were few reports about the cytokines associated with musculoskeletal tumors. From late 1980s, the cytokine studies of bone tumors such as osteosarcoma were started, but most studies for benign and malignant musculoskeletal tumors were left to be explored. To evaluate the characteristics of the cytokines in variable musculoskeletal tumors, tissues were obtained from the seven patients who visited the Yeungnam University hospital from February to July 2000. They were lipoma (1 case), parosteal osteoma (1 case), enchondroma (2 cases), pigmented villonodular synovitis (1 case), ganglion (1 case), and metastaic squamous cell carcinoma (1 case). The gene experession of the cytokines were analyzed by RNase protection assay (RPA) and reverse transcription-polymerase chain reaction (RT-PCR). The lipoma and parosteal osteoma expressed MIP-$1{\beta}$, and IP-10 genes. The two enchondromas showed different results, one expressed all of MIP-$1{\alpha}$, MIP-$1{\beta}$ and IP-10 genes but the other expressed none of above. The pigmented villonodular synovitis strongly expressed MIP-$1{\alpha}$ and IP-10 when compared with the other cases. The ganglion did not express all of the chemokines mentioned above. And the metastatic squamous cell carcinoma expressed all of the chemokines and especially IP-10 was highly expressed. Even though this study has only a few cases, these results provide a basis for the cytokine mediating network study in musculoskeletal tumors.

Clinical Application Status Analysis of Chuna Manual Therapy after Musculoskeletal Disorder Surgery (근골격계질환 수술후 추나 기법 임상적용 현황분석)

  • Objectives : The purpose of this study was to analyze the clinical application status in papers related to Chuna Manual Therapy after musculoskeletal disorder surgery. Methods : We searched 6 electronic databases(RISS, DBpia, KISS, NDSL, OASIS, Pubmed) and 2 associated journals (Korea CHUNA Manual Medicine for Spine & Nerves, Korean Medicine Rehabilitation). We classified the papers by related musculoskeletal region and analyzed them in detail. Results : 1. Of the 24 papers included in the final analysis list, 13 were carried out for fascia and 18 for joints. 2. There were positive effects on the improvement of symptoms, including the relief of pain in patients, and there were no cases of severe side effects and adverse events. 3. Thus, CMT can be used as a safe and effective treatment if a skilled practitioner makes proper use of the follow-up procedure based on accurate judgment of the patient's postoperative condition. Conclusions : Through this paper, we expect that there will be active research on the clinical applications of Chuna Manual Therapy in Korean rehabilitation treatment after musculoskeletal disorder surgery.

  • The ultrasound can be used primarily to guide exact needle placement for aspirations of fluids, injections of steroid, and biopsies in musculoskeletal field. Recently, ultrasound-guided intervention is widely used because of several advantages such as real-time performance, relatively inexpensiveness, and getting multiple images without additional radiations. However, the modality is operator dependent and requires detailed knowledge of the relevant anatomy and there have been also reported serious complications related to the procedure. So, authors will discuss about the basic techniques, principles and cautions for the use of ultrasound-guided intervention in musculoskeletal field.

The Effectiveness of Korean Medical Rehabilitation Treatment after Musculoskeletal Surgery: A Retrospective Observational Study (근골격계 수술 후 한방재활치료의 효과: 후향적 관찰연구)

  • Objectives This study aimed to evaluate the effectiveness of postoperative Korean rehabilitation treatment for patients with musculoskeletal disorders. Methods The medical records of patients undergoing Korean medical rehabilitation after a musculoskeletal surgery, from January 1, 2016, to December 31, 2019, were analyzed retrospectively. The effect of treatment was evaluated by using a numeric rating scale (NRS) and questionnaire for each surgical site. When the same patient was re-admitted after discharge, it was treated as the same case if it was within 7 days from the discharge date; otherwise, it was treated separately. If the period of hospitalization was within 7 days or the chief complaint was not related to musculoskeletal surgeries, the case was excluded. Results In total, 480 cases were included in this study. Acupuncture (99.8%), moxibustion (95.6%), cupping (97.5%), Chuna manual therapy (81.5%), Korean medical physiotherapy (61.0%), Daoyin exercise therapy (26.3%), and herbal medicine (87.7%) were used as the Korean medical rehabilitation treatments. A significant improvement was noted when comparing the NRS and questionnaire results for each surgical site at the time of admission and discharge. Conclusions Korean medical rehabilitation can be effectively used for patients who have undergone musculoskeletal surgeries, to relieve pain and enable returning to daily activities. However, further research with a high level of evidence is necessary to support this finding.

  • Prosthetic reconstruction of musculoskeletal defects about the knee for tumor has many advantages, particularly the maintenance of motion and immediate functional restoration. But, prosthetic reconstruction has inherent limitations in terms of long-term durability. The authors have reported here a patient who had mechanical failure at 61 months later following use of a modular resection system to reconstruct the segmental defect of proximal tibia in osteosarcoma. In this case, another technique of extensor mechanism reconstruction using Achilles tendon allograft was attempted. Because of the concerns involving durability of tumor prosthesis, increased emphasis has to be placed on innovation in prosthetic design.

Peroneal Nerve Palsy Associated with Musculoskeletal Tumor Resection (종양 절제 후 동반된 비골신경 손상)

  • We retrospectively reviewed the cases of peroneal nerve palsy in seven patients after musculoskeletal tumor resection. Their mean age was 38 years. Three had osteosarcoma in proximal fibula, 2 had chondrosarcoma in proximal fibula and calf and 2 had malignant fibrous histio cytoma in calf. Four of 7 patients had been managed using active dorsiflexion brace. Three patients who underwent tibialis posterior transfer and could walk without brace were able to discontinue the use of the orthosis. Peroneal nerve palsy after wide excision of tumor including peroneal nerve can not resolve spontaneously and results in severe functional disability. To improve the gait function, active surgical treatment should be considered.

Subjective symptoms and risk assessment of musculoskeletal disorders of the dental hygienists (치과위생사의 근골격계질환 자각증상 및 위험도 평가)

  • Objectives : The study was carried out to find out the experience of musculoskeletal disorder symptoms of dental hygienists and evaluate the risk with ergonomic evaluation method(RULA). Methods : The subjects in this study were 344 dental hygienists working at dental clinics and general hospitals in Seoul, Gyeonggi, Daejeon and Chungnam/Chungbuk. The data were collected through the self-questionnaire survey from July 26th 2010 to September 30th 2010. Results : The results showed that those who working at dental hospital, network dental clinics, with 1-5 years of clinical experience, long working hour and sitting hour, no rest hour, more physical and mental burden had higher rate of the experience of musculoskeletal disorder symptoms compared with other groups. The final score of RULA evaluation results was 6 on average requiring continuous observation and urgent demand for work improvements. According to the result of RULA evaluation by work, the orthodontic clinics were 4.5, prosthodontic clinics were 5.0, and dental surgery clinics were 6.8 being the most risky. Conclusions : The study showed high complaints rate on musculoskeletal disorders in dental hygienists. Thus, various measures including provision of proper working and rest hours, use of ergonomic working equipments, strengthening the health education for desirable working posture and the development of musculoskeletal disorder prevention program should be needed.
