119 구급대원의 근골격계 질환 위험성 평가 - 소방기술경연대회 구급종목을 대상으로 - (Evaluation of musculoskeletal disorders risk of 119 emergency medical technicians during emergency medical services procedures in firefighter combat challenge)

  • Purpose: The purpose of the study was to evaluate the ergonomic risk factors of 119 emergency medical technicians (EMT) with musculoskeletal disorders, performing emergency medical services (EMS) procedures in a firefighter combat challenge. Methods: The evaluated EMT procedures were cardiopulmonary resuscitation (CPR) & intubation, trauma patient assessment, and intravenous (IV) injection. Measurement of working posture was done during training. Results: In CPR & intubation, OWAS-score was 2 (mean 1.9, maximum 4), requiring correction action, while REBA-score was 11 (mean 7.28, maximum 11), requiring immediate improvement. In trauma patient assessment & IV injection, OWAS-score was 4 (mean 2.9, maximum 4), requiring immediate correction action, while the REBA score was 7 (mean 7.5, maximum 11), requiring improvement. Conclusion: Both OWAS score and REBA-score showed improvement of posture and high-risk of musculoskeletal disorders. Occupational health management in EMS procedures during combat challenge and effective injury prevention program in fire stations are warranted.

근골격계 질환에 대한 민감성, 사회적 지지 그리고 환경적 지원이 대학생의 운동지속의도에 미치는 영향 (Effects of Susceptibility to Musculoskeletal Disorder, Social Support, and Environmental Aids on Exercise Adherence Intention among University Students)

  • Purpose: The study was undertaken to investigate effects of susceptibility to musculoskeletal disorder, social support, and environmental aids on exercise adherence intention and to identify factors contributing to exercise adherence intention among university students. Methods: The study was a descriptive study with 277 students from 3 universities. Data were collected from March 5 to May 30, 2014 using a structured self-report questionnaire. Data were analyzed using descriptive statistics, t-test, one-way ANOVA, Pearson correlation coefficients, and Hierarchical multiple regression. Results: The explanatory power of the predictive model involving the demographic factors, susceptibility to musculoskeletal disorder, social support, and environmental aids was 29%, and the subjects' gender, health concern, experience of musculoskeletal injury, regular exercise, and social support were identified as main factors having influence. Conclusion: The results of this study will be helpful in understanding the importance of environmental factors for increasing physical activities and will be used as basic data for development of exercise programs to increase exercise adherence intention for their continuous exercise.

Vitamin D in athletes: focus on physical performance and musculoskeletal injuries

  • [Purpose] The aim of this review was to discuss the effects of vitamin D on physical performance and musculoskeletal injuries in athletes and provide information on the field applications of vitamin D. [Methods] A systematic review was conducted to identify studies on vitamin D in athletes that assessed serum vitamin D levels, vitamin D and physical performance, vitamin D and musculoskeletal injuries, and practical guidelines for supplementation of vitamin D. [Results] Several studies reported that a high proportion of athletes had vitamin D insufficiency or deficiency. Low serum levels of vitamin D in athletes were more pronounced in winter than in other seasons, and indoor athletes had lower serum vitamin D levels than outdoor athletes. Low vitamin D levels have been demonstrated to have negative effects on muscle strength, power, and endurance; increase stress fractures and other musculoskeletal injuries; and affect acute muscle injuries and inflammation following high-intensity exercises. Therefore, periodic assessment and monitoring of vitamin D levels are necessary in athletes; the recommended serum level of 25(OH)D is > 32 ng/mL and the preferred level is > 40 ng/mL (-1). In those with low levels of vitamin D, exposure to sunlight and an improved diet or supplements may be helpful. Particularly, 2000-6000 IU of supplemental vitamin D3 can be consumed daily. [Conclusion] Vitamin D is a potential nutritional factor that can significantly affect physical performance and musculoskeletal injuries in athletes. The importance and role of vitamin D in athletes should be emphasized, and the current levels of vitamin D should be assessed. Therefore, it is essential to periodically evaluate and monitor serum vitamin D levels in athletes.

우즈베키스탄 국립응급병원에 내원한 낙상환자들의 임상적인 특성 (Features of Patients Associated with Falls from Heights Admitted to Republican Research Center of Emergency Medicine)

  • Purpose: study of the mutual influence of the individual anatomical regions damage in patients associated with falls from heights. Methods: 561 medical reports of patients associated with falls from heights admitted to Republican Research Center of Emergency Medicine (RRCEM), in period of 2010-2013 yy, were analyzed retrospectively. Patient's age range was from 15 to 89. Treatment of these patients held in three stages: period of acute disorders of vital functions; relative stabilization period; stabilization of vital functions. The scope and content of medical diagnostic procedures performed on pre-hospital and intensive care stages. The severity of each injury was scored according to the AIS scale, the total severity of lesions was scored by points due to ISS. Digital material is treated by methods of mathematical statistics. Results: Falls from heights leads to associated injuries for 4 times more than isolated trauma, and increases according to height of fall and falling surface. Patients with TBI+chest trauma, are most serious contingent that has highest mortality and complications (36.8%). TBI+musculoskeletal system trauma are characterized by high blood loss, traumatic shock and fat embolism, forming a vicious circle. Conclusion: in patients associated with falls from heights clinical manifestations of injuries mutual aggravation syndrome will be seen. TBI leads to complication of chest trauma, delayed diagnostics of abdominal and chest trauma, aggravation of coma condition. Developing of high blood loss and fat embolism in musculoskeletal system trauma leads to shock and pulmonary embolism, which increases probability of death in the presence of abdominal or chest trauma, but abdominal trauma or chest trauma has no significant effect on the severity of the musculoskeletal system trauma.

낙하 충격을 받는 다점 구속 장치를 착용한 헬기 승무원의 상해도 평가에 관한 연구 (A Study on the Injury Assessment of Helicopter's Crew with Multi Point Restraint System under Drop Impact)

  • In this paper, a method of modeling seat belts on crew seat during dynamic seat testing was studied. The body segments of the occupant were modeled with joints. The joints consisted with various stiffnesses, dampings, and frictions. Three types of seat belt restraint systems were investigated. The analysis for on the injury assessment of helicopter's crew under drop impact was conducted. The effectiveness of the seat belt system for crashworthiness and safety was evaluated. As the results of impact analysis, head, neck and spine of the crew can be easily damaged in the vertical direction more than the longitudinal direction. Based on the verified model, behavior of human body was studied with three-point restraint systems. The displacement and injury level of the 12-point restraint system was the smallest.

개방성 견갑-흉부 해리 증례보고 (Open Scapulothoracic Dissociation - Case report -)

  • Traumatic scapulothoracic dissocation is a rare upper extremity injury caused by massive traction or blunt trauma. Most cases are associated with a large spectrum of concomitant injuries, including severe musculoskeletal injuries and neurovascular injuries around the shoulder. But, it occurs without damaging overlying skin as a closed injury rather than an open injury. We present a case of open scapulothoracic dissociation and describe clinical features with literature review.

총기 및 폭발물에 의한 군인의 근골격계 손상: 최근 4년간 분석 (Musculoskeletal Injuries by Weapons in Korean Soldiers: Four-Year Follow-Up)

  • 목적: 현재까지 군에서 발생하는 총기 및 폭발물 손상은 그 중요성에 비해 연구된 것이 제한적이었다. 이를 극복하기 위해 최근 4년간 총기 및 폭발물에 의해 수상한 군인 환자의 근골격계 손상에 대한 특징을 확인하고자 하였다. 대상 및 방법: 2015년 1월부터 2019년 7월까지 총기 및 폭발물에 의해 근골격계 손상을 당한 군인 환자를 대상으로 하였다. 의무 기록과 의료영상저장전송시스템 영상자료를 분석하고, 전화 면담을 통해 short musculoskeletal function assessment (SMFS)를 이용한 기능평가를 후향적으로 시행하였다. 기능평가에 대한 통계 분석으로 무기종류에 따른 분석은 t-test를, 다른 요인에 따른 분석은 ANOVA 분석을 실시하였다. 결과: 총기 및 폭발물에 의한 손상으로 치료받은 총 61명의 환자 중 30명(49.2%)이 근골격계 손상으로 정형외과적 치료를 시행받았다. 평균 수상 나이는 26.4세(21-52세)였다. 간부와 병사의 수는 비슷하였다. 총기 손상이 11명, 폭발물 손상이 19명이었다. 30명 중 수도병원에서 16명(53.3%)이, 민간병원에서 10명(33.3%)이 치료받았다. 12명에게 복수부위 손상이 있었고, 골절이 동반된 16명(53.3%)의 환자 중 절반 이상인 7명의 환자가 다발성 골절이었다. 수부와 하퇴부가 가장 흔한 수상 부위였다. 2명을 제외하고는 모두 개방창이 있었으며, 연부조직 재건술이 필요한 환자는 14명(46.7%)이었다. SMFS 기능평가에는 15명이 응답하였다. 계급에 따른 분석 결과, Borther Index에서 간부의 결과가 통계적으로 유의하게 낮은 것으로 분석되었다(p=0.05). 치료 병원에 따른 분석 결과, Dysfunction Index와 Bother Index 모두 수도병원에서 치료 받은 인원의 점수가 통계적으로 유의하게 낮은 것으로 분석되었다. (p=0.0008, p=0.0149) 결론: 국내 무기에 의해 수상한 군인 환자에 대해 분석한 첫 연구이다. 무기 손상 환자의 치료에 있어 orthopedic burden이 높음을 확인하였으며, 장기적으로 이런 환자들에 대한 치료평가 및 군 외상 시스템 발전을 위해 총기 및 폭발물에 의한 손상 환자를 포함한 군 외상환자 레지스트리 구축이 필요하겠다.

소방공무원의 근골격계 질환: 2011년~2013년 공상신청 분석 (Muscuoloskeletal Disorders of Korean Fire Fighters: Applicants for Public Worker's Compensation from 2011 to 2013)

  • 본 연구의 목적은 전국 소방서에 근무하는 소방공무원 중 2011년부터 2013년까지 근골격계 질환으로 공상을 신청 한 510명을 대상으로 자료를 분석하여 그 특성에 대해 알아보고, 공상 예방을 위한 대책을 제시하는 것이다. 2011년 1월부터 2013년 12월까지 공상을 신청한 1596명의 공상신청자의 정보를 국민안전처로부터 제공받아 주병명을 기준으로 근골격계 질환을 판별하여 이 중 510명을 최종 연구대상으로 하였다. 공상신청자는 남성이 92%로 여성보다 월등히 많았고, 공상발생 연령은 40대 초반이 가장 많았다. 공상이 발생한 시기는 5월과 6월이 가장 높았다. 지역별로 공상신청이 가장 많은 지역은 서울이었고 공상 발생만인율이 가장 높은 곳은 전북이었다. 공상신청이 가장 잦은 부위는 허리가 제일 많았고 무릎이 그다음 이었다. 신체구조는 인대가 가장 많고 추간판과 뼈 순이었다. 공상발생 당시 직무는 화재진압 38.3%, 구급 37.7%, 구조 12.4%, 기타 순이었다. 앞으로 지역 간 공상신청 빈도차이를 좀 더 세밀히 조사할 필요가 있고, 소방 직부별 부상기전과 예방방안에 대한 연구가 필요하다.

상지 반복 작업 후 발생한 손목 및 손 부위의 기타 신경손상 치험 1례 (A Case Report of Injury of Other Nerves at the Wrist and Hand Level After Repetitive Work)

  • Background and Objective: The Korean agricultural population is now rapidly aging. Older people in rural areas are weak due to work-related musculoskeletal disorders. The aim of this case is to report treatment of injuries to the nerves at the wrist and hand level by Korean medicine. Methods: The patient in this case was a 74-year-old male. He had injured nerves at the right wrist and hand level due to agriculture work. We treated him with acupuncture, herbal medicine, etc. Results: The patient showed the first sign of recovery of his wrist after 13 days treatment. After 37 days of treatment, his wrist movement was restored to a nearly normal range. He showed the first sign of recovery in a finger after 19 days of treatment and his grasping power increased from 0% to 60%. The cold sensation in his hand and arm also disappeared. Conclusions: We cannot generalize based only on this one case study. Nevertheless, this report suggests that Korean medicine may be an effective treatment for injury of nerves at the wrist and hand level.

치위생학과 스케일링 실습수업이 상지 근골격계에 미치는 영향 (Repercussions to the musculoskeletal system of the Upper Limb caused by scaling training exercise)

  • Purpose: We evaluated the physical stress and pain to the musculoskeletal system of a dental practitioner when engaging in a dental scaling training exercise to prevent the development of musculoskeletal injuries. Methods: The 18 female (average age: 21$\pm$1 years) subjects were voluntarily picked from a group of juniors who have completed a one-and-a-half year training course that includes training exercises on the dentiform and on live subjects (other trainees). The test is done by measuring pain, activity, grip strength, and finger dexterity for each subject's hand and wrist. Before the test all subjects were confirmed to be right-handed and were informed of the study and its objective. Measuring was done before and after each subject performed dental scaling for one hour using the scaler and the curet. Results: Pain levels increased for both hand and shoulders, but hand pain was often greater than shoulder pain. Grip strength significantly declined in the right hand but not the left. For joint mobility, the flexion and the extension for the shoulder joint did not change; but the range of motion for both wrist joints significantly increased. For the dexterity test, both hands showed increased dexterity after the exercise. Conclusion: Dental scaling can affect the shoulders and wrists/hands. Therefore, a musculoskeletal injury prevention program for dental practitioners, which may include encouraging them to assume correct body posture when at work, must be sought. This study evaluated only the shoulders, wrists, and hands; but future studies should include areas such as the cervical area, the back, and the lower limbs.
