Dieticians' Perception of Safety Supervision in Institutional Foodservices (II) - Status of Safety System and Safety Education - (단체급식 안전관리에 대한 영양사 인식 조사(II) - 안전시스템 및 안전교육 현황 -)

  • The purpose of this study was to investigate the status of the safety system and safety education in institutional foodservices in the Changwon and Masan areas. The survey was conducted from February 1 to March 31, 2009 via questionnaires that were sent to 300 dietitians, and 142 dietitians responded. It was determined that most of the safety managers were 'dietitians (87.1%)', whereas facilities/equipment managers consisted of 'dietitians (45.7%)', 'department of facilities management in the organization (36.4%)' and 'outsourced company of facilities management (17.9%)'. Out of the 11 safety practices, seven safety practices showed less than 50% of total implementation ratio, which meant that the safety systems were not functioning properly. Except for 'non-skid shoes (85.9%)', other safety equipment was seldom used. The survey respondents recognized that safety education was very necessary (4.47 points); however, they responded so-so (3.46 points) to the question of whether or not the actual frequency and time spent on safety education were enough. The average time spent on safety education was 28 minutes 11 seconds. Regarding the difficulties in performing safety education, 'not many safety education materials and media (3.44 points)', 'not many varieties in the subjects and contents for safety education (3.40 points)', and 'not much organizational support on safety education (3.33 points)' showed higher scores than 'lack of education time due to workload (3.20 points)'. The following were cited as education materials that should be developed as it is currently difficult to obtain relevant information and data: 'root causes of musculoskeletal injury and preventive measures (15.8%)', 'healthcare and disease preventive exercises for employees who do simple and repetitive works (14.9%)', and 'instruction on safe handling of chemicals (12.7%)'

Effect of Proprioceptive Neuromuscular Facilitation Applied to the Unilateral Upper Extremity on the Muscle Activation of Contralateral Lower Extremity (펀측 상지에 적용된 고유수용성 신경근 촉진법이 반대측 하지의 근 활성도에 미치는 영향)

  • Purpose: The purpose of this study was to investigate the effect of Proprioceptive Neuromuscular Facilitation (PNF) applied to the unilateral upper extremity on the muscle activation of contralateral lower extremity. Twenty-two healthy subjects (mean age of 23.7 years) participated in this study. Method : PNF patterns applied on the unilateral upper extremity in all subjects were the flexion/abduction/external rotation and lifting pattern. The hold and appoximation techniques for the irradiation were applied to end range of both patterns. Muscle activations in four patterns were measured in vastus medialis, tibialis anterior, rectus femoris, and gastrocnemius medial muscles of contralateral lower extremity using surface EMG system. Each EMG value in individual muscle was normalized for maximal voluntary contraction. The data were analyzed by one factor analysis of variance with repeated measure test. Result : There were significant differences in the between-subject effect (muscles) and within-subject effect (patterns) in comparison of muscle activation by application of PNF patterns (p<.05). The irradiation led to higher activation in the flexion/abduction/external rotation pattern than that of lifting pattern in all muscles (p<.05). The approximation techniques revealed more activations than these of hold technique in all muscles (p<.05). Conclusion : These results suggest that the application of PNF patterns to the unilateral upper extremity affect on the muscle activation of contralateral lower extremity and increase according to the intensity of resistance. This mechanism of contralateral effect might provide a help to the development of treatment method for the affected side and functional improvement for the patients who have damages of central nervous system or musculoskeletal problems by orthopedic injury.

Use of thermography in lameness caused by musculoskeletal disease in Thoroughbred mare (적외선 체열촬영술을 이용한 thoroughbred 말의 근골격계 파행진단 증례)

  • The mare had lameness and asymmetry edematous on its tarsal and metatarsal joints at the initial physical examination. The pain was elicited with a palpation along the metatarsal articulations. No significant abnormalities were detected in the screening test. However, thermographic images revealed a significant increase in the surface temperature at the joint of the hindlimb when compared to the reference range. At necropsy, an irregularity of the surface and excessive synovial fluid were observed on the right tarsal joint. No bacterial growth was shown in the cultures of synovial fluid. Staphylococcus aureus was detected in the subcutaneous discharge. Taken together, the thermography images were very useful in localizing the area of injury and were an effective diagnostic methodology for assessing lameness.

  • 최근 산업안전/보건분야에서 누적외상성질환(Cumulative Trauma Disorders: CTDs)과 직업성요통(Low Back Injury)과 같은 근골격계질환은 산업현장에서 어렵지 않게 발견할 수 있는 신종 산업재해의 일종이다. 특히 미국의 경우, 발생건수와 그로 인한 인적/물적 손실이 급격하게 증가되고 있는 것으로 나타나고 있다. 그러나 미국과 같이 근골격계질환의 폐해를 일찌감치 인식하고 그 해결방안을 연구해온 나라들에서는 현재 환자관리 및 인간공학적인 기준 등을 포함하는 예방지침들이 잘 정착되어 있는 실정이다. 우리나라의 경우에는 작업환경 등의 조건과 상황이 미국과 같은 구미 선전국과 비숫하고, 특히 제조업의 생산체계가 그런 나라들과 매우 유사하여 근골격계질환의 발생과 그로 인한 인적/물적 손실이 적지 않을 것이라고 예상할 수 있지만 아직까지 CTDs에 대한 전반적인 이해가 부족하고, 정확한 실태가 파악되지 못하였으며, 따라서 체계적인 관리 대책 또한 마련되어있지 않은 실정이다. 본 조사에서는 서울 근교의 수송기계관련 사업장의 작업들에 대한 작업자세에 중심을 둔 인간공학평가를 통하여 조사 작업의 근골격계질환에 대한 상대적인 위험도, 위험요인 등을 파악하고, 그에 따른 개선 대책의 방향을 제시하고자 하였다. 특히, 각 부위별 위험점수와 부위별 증상호소율의 상환분석결과, 작업자 개인적 수준에서는 통계적으로 유의한 차는 없는 것으로 분석되었으나, 부서별로 분석한 결과, 작업시 주로 사용하는 오른쪽 상지의 경우, 오른쪽 어깨, 팔꿈치, 손목 순으로 통계적으로 유의하게 관련이 있는 것으로 나타났다. 즉 증상호소율과 해당 신체부위의 위험도 점수는 서로 정적(positive)한 관계에 있는 것으로 나타났고, 상지 중 주로 사용하는 오른쪽 부담의 위험도 자세 점수와 증상호소는 비교적 높은 상관관계에 있는 것으로 나타났다. 따라서 이와 같은 인간공학적 분석결과는 향후 인간공학적인 개선대책의 개발 뿐만 아니라 의학쪽에서 근골격계질환을 접근하는 초기단계에서 매우 유용한 역할을 할 수 있으리라 생각된다.

Effect of Prior Muscle Contraction or Passive Stretching on Eccentric-Induced Muscle Damage

  • Purpose: This tutorial review investigated the effect of prior fatigue and passive stretches on eccentric contraction-induced muscle injuries, as well as the underlying mechanisms of eccentric contraction-related injuries. Methods: Contraction-induced muscle damage is the most common disabling problem in sports and routines. The mechanisms underlying the pathology and prevention of muscle damage lessened by prior fatigue or stretches are critical in assessing musculoskeletal injuries. Even though there are treatments to reduce eccentric contraction-induced muscle injuries, fatigue negatively influences them. Therefore, we reviewed previous studies on eccentric contraction-induced muscle injuries with prior treatments using the MEDLINE and PubMed databases. Results: Prior passive stretching had a preventative and therapeutic effect, but prior lengthening contractions did not. On the other hand, prior isometric contractions involving relatively small forces may not provide a sufficient stimulus to induce protection. As a result, high force isometric contractions may be necessary. The studies supported the positive effects of prior fatigue, concluding that it was a factor in determining the amount of damage caused by eccentric exercise. This was due to a reduction in force and increased temperature. Studies that did not support the positive effects of prior fatigue concluded that a shift in optimal length to a longer length and reduced energy absorption during lengthening are evidence that fatigue is not related to muscle injuries induced by lengthening. Conclusion: The variability of the experiment models, conditions, muscles, and treatment methods make it necessary to interpret the conditions of previous studies carefully and draw conclusions without making direct comparisons. Thus, additional studies should be carefully conducted to investigate the positive effect of fatigue on lengthening.

Biomechanical Analysis of Wearing Carbon Nanotube-Based Insole during Drop Landing (탄소나노튜브 인솔 착용에 따른 드롭 착지 동작의 생체역학적 분석)

  • The purpose of this study was to determine the biomechanical effect of wearing carbon nanotube-based insole on cushioning and muscle tuning during drop landing. Twenty male university students(age: $21.2{\pm}1.5yrs$, height: $175.4{\pm}4.7cm$, weight: $70.2{\pm}5.8kg$) who have no musculoskeletal disorder were recruited as the subjects. Average axial strain, average shear strain, inversion angle, linear velocity, angular velocity, vertical GRF and loading rate were determined for each trial. For each dependent variable, a one-way analysis of variance(ANOVA) with repeated measures was performed to test if significant difference existed among different three conditions(p<.05). The results showed that Average axial strain of line 4 was significantly less in CNT compared with EVA and PU during IP phase. The average shear strain was less in CNT compared with EVA and PU during other phases. The inversion angle was increased in CNT compared with EVA and PU during all phase. In linear velocity, angular velocity, vertical GRF and loading rate, there were no significant difference between the three groups. This result seems that fine particle of carbon nanotube couldn't make geometric form which can absolve impact force by increasing density through eliminating voids of forms. Thus, searching for methods that keep voids of forms may play a pivotal role in developing of insole. This has led to suggestions of the need for further biomechanical analysis to these factors.

Internal Oblique and Transversus Abdominis Muscle Fatigue Induced by Slumped Sitting Posture after 1 Hour of Sitting in Office Workers

  • Background: Prolonged sitting leads to low back discomfort and lumbopelvic muscle fatigue. This study examined the characteristics of body perceived discomfort and trunk muscle fatigue during 1 hour of sitting in three postures in office workers. Methods: Thirty workers sat for 1 hour in one of three sitting postures (i.e., upright, slumped, and forward leaning postures). Body discomfort was assessed using the Body Perceived Discomfort scale at the beginning and after 1 hour of sitting. Electromyographic (EMG) signals were recorded from superficial lumbar multifidus, iliocostalis lumborum pars thoracis, internal oblique (IO)/transversus abdominis (TrA), and rectus abdominis muscles during 1 hour of sitting. The median frequency (MDF) of the EMG power spectrum was calculated. Results: Regardless of the sitting posture, the Body Perceived Discomfort scores in the neck, shoulder, upper back, low back, and buttock significantly increased after 1 hour of sitting compared with baseline values ($t_{(9)}=-11.97$ to -2.69, p < 0.05). The MDF value of the EMG signal of rectus abdominis, iliocostalis lumborum pars thoracis, and multifidus muscles was unchanged over time in all three sitting postures. Only the right and left IO/TrA in the slumped sitting posture was significantly associated with decreased MDF over time (p = 0.019 to 0.041). Conclusion: Prolonged sitting led to increased body discomfort in the neck, shoulder, upper back, low back, and buttock. No sign of trunk muscle fatigue was detected over 1 hour of sitting in the upright and forward leaning postures. Prolonged slumped sitting may relate to IO/TrA muscle fatigue, which may compromise the stability of the spine, making it susceptible to injury.

The Pilot Study of Robot-Assisted Training for the Lower Extremity Rehabilitation of Burn Patients (하지 화상 환자의 재활치료를 위한 보행보조 로봇훈련의 선행연구)

  • Purpose: Scar contracture influence the outcome of burn patients significantly. This study aims to investigate the feasibility of robot-assisted training for the lower extremity rehabilitation of burn patients. Methods: This pilot study was conducted on 7 burn patients for 8 weeks between January 2019 and November 2019. Two of 7 patients withdrew from this study because one had skin abrasion on the legs which thigh fastening devices were applied on and the other was not participate in the assessment at 4 weeks after training. Final 5 patients received gait training with SUBAR® and numeric rating scale (NRS), 6-minutes walking test, and range of motion in flexion and extension of knee and ankle joint were evaluated before training, 4 weeks and 12 weeks after training. Results: The subjects had a mean age of 51.8±98 years, mean total burn surface area of 30.8±13.7%, mean duration from injury to 1st assessment of 102.8±39.3 days. Anyone of 5 patients did not have musculoskeletal or cardiovascular side effects such as increased or decreased blood pressure or dizziness. The significant improvement in NRS, gait speed, and range of motion in knee extension and ankle plantarflexion after robotic training (all P<0.05). Conclusion: Robot-assisted training could be feasible for the rehabilitation of burn patients and it could improve muscle strength and range of motion in lower extremities, and gait function.

Individualized Therapy to Manage Quadriplegic Spastic Cerebral Palsy with Unani Treatment Modalities

  • Objective: Cerebral palsy (CP) is a neurodevelopment disorder attributed to an insult or injury to the developing brain with abnormalities in muscular tone, movement and motor skill. Improvement in quality of life and ameliorating symptoms can be achieved. Therefore, this case report details a distinctive approach to treating a 5-year-old male child with quadriplegic spastic cerebral palsy utilizing Unani treatment modalities. Methods: The treatment regimen commenced with 'Habb Ayarij for constipation followed by Sharbat Ustukhuddus administered orally. Notably, Sharbat Ustukhuddus was combined with Melia Azedarach L. leaves vapour bath. Subsequently, Roghan Babunna douche was performed followed by Dalk Layyin andcontinued until symptomatic improvement was observed. Majun Falasfa, Khamira Marwareed and Khameera Gauzaban were administered for 30 days. The therapeutic outcome included anthropometrical measurements, developmental milestones, spasm/reflex scale, and muscle power grading. Results and conclusion: Over the course of a 2-year follow-up, several clinical findings emerged. These included notable improvements in anthropometric measurements, developmental milestones such as improved head control and sitting ability, and a reduction in spasticity of the upper limbs, along with decreased muscle spasms. The therapeutic outcome of this individualized and holistic approach is potentially due to the multifaceted properties of medicinal plants (Musakkin wa Muharrik wa Muqawwi-i- A'sab wa Dimāgh, Munawwim, Dafi-i-Tashannuj, Muqawwi-i-Qalb-i-Ruh). Furthermore, the use of Dalk and Naṭūl was instrumental in providing nourishment to musculoskeletal cells and initiating intracellular signaling cascades. While these findings are encouraging, further research in the form of case series andrandomized controlled trials is warranted to validate the efficacy of this unique holistic approach.

Analysis of Frequent Disease and Medical Expenses Structure of Patients Admitted in a Vaterans Hospital (일개 보훈병원 입원환자의 상병 및 진료비 구조분석)

  • Objectives: This study attempts to analyze the length of hospital stay and expenses of frequent disease admitted in a Vaterans Hospital. Methods: Data was collected from January 1, 2001 to December 31, 2003 from the Claim records of 9,640 patients in a Vaterans Hospital. Results: The results were as follows: 1. In age & sex distribution, there was male 70.9%, female 29.1%, and 35.8% of them is 70 age group. Frequency by insurance program was Health insurance 78.1%, Medical aid 14.2%, no insurance 4.1%, others 3.6%. Distribution of each department was internal medicine 28.3%, orthopedic surgery 21.3%, surgery 16.6%, neurosurgey 7.1%, pediatrics 5.9%. Also, in the veterans group, male to female patient ratio was 99.3% male to 0.7% female, them over 70 years old was 51.6%, and them which live in daejeon was 43.5%. 2. In frequency of disease, there was gastroenteritis 4.8%, pneumonia 3.8%, cartaract 3.7%, cerebral infarct 3.2%, hyperplasia of prostate 3.0%. In frequency of korean standard classification of diseases, there was injury and poisoning and certain other consequences of external causes 17.1%, diseases of digestive system 16.1%, diseases of musculoskeletal system and connective tissue 13.9%, diseases of respiratory system 9.4%, diseases of genitourinary system 8.6%. Also, in veterans group, frequency of them was diseases of musculoskeletal system and connective tissue 19.4%, diseases of digestive system 16.8%, injury and poisoning and certain other consequences of external causes 15.7%, diseases of genitourinary system 9.7%, diseases of circuatory system 8.2%. 3. Average length of hospital stay was 29.0 days for total patients, 51.8 days for the veterans group, 15.7 days for the non-veterans one. Average total expenses was 3,669,579 won, the veterans group 7,263,877 won, the non-veterans one 1,560,333 won. The ratio of insurer to insuree was 55.2 : 44.8, the ratio of amount paid by patient in the veterans group 61.7%, in the non-veterans one 33.0%. 4. In items of medical expenses, fee for hospital accommodation was 34.7%, fee for medication 13.2%(injection 7.8%, drug 5.4%), fee for service 48.6%(physical therapy 26.3%, operation 9.7%, laboratory examination 5.2%, radiological examination 3.1%, etc), others 3.4%. In them for the veterans group, fee for physical therapy was 35.3%, fee for hospital accommodation 35.2%, fee for injection 6.2%, fee for operation 5.9%, for the non-veterans one, fee for hospital accommodation 35.7%, fee for operation 16.4%, fee for injection 11.4%, fee for laboratory examination 8.3%. 5. In the comparison of the frequency by Korean standard classification of diseases and distance between the hospital and home, the region under 21.5Km was more frequent in symptoms, signs an abnormal clinical and laboratory findings 56.0%, injury and poisoning and certain other consequences of external causes 55.6%, diseases of the eye and adnexa 52.9%, the one over 21.5Km was more frequent in neoplasms 57.4%, diseases of musculoskeletal system and connective tissue 55.9%, diseases of genitourinary system 53.5%.
