Overviews on the Clinical Use of Stem Cells in Orthopaedics (근골격계에서 줄기세포를 이용한 재생치료의 개관)

  • Stem cell research arose from the need to explore new therapeutic possibilities for intractable and lethal diseases. Although musculoskeletal disorders are basically nonlethal, their high prevalence and the relative ease of performing clinical trials have facilitated the clinical application of stem cells in this field. On the other hand, despite the plethora of in vitro and preclinical studies in stem cell research for regenerative medicine in the musculoskeletal system, few reliable clinical studies have been published. Stem cell therapy can be applied locally for bone, cartilage, and tendon regeneration. The candidate disease modalities in bone regeneration include large bone defects, nonunion of fractures, and osteonecrosis. Focal osteochondral defect and osteoarthritis are the current targets for cartilage regeneration. For tendon regeneration, bone-tendon junction problems, such as rotator cuff tears are hot topics in clinical research. To date, the literature supporting stem cell-based therapies comprises mostly case reports or case series.

  • The giant cell tumor comprises approximately 5% of all bone tumors especially in the long tubular bones, particularly in proximity to the epiphysis. A rare case of giant cell tumor involving the patella was recently experienced by authors. Case summary with brief review of reference is presented.

  • Giant cell tumor of the talus is an extremely rare condition and therapeutic options are ill defined. Recently we experienced a case of giant cell tumor of the talar body in a 20-year-old male treated by curettage and bone graft. There was no recurrence at 1year follow-up. A case report and review of the literature are presented.

  • Giant cell tumor of bone is relatively common neoplasm usually involving epiphysis of long bone. And rarely it involves the diaphysis or metaphysis without epiphyseal extension. We report on an 18-year-old girl with giant cell tumor of ulnar diaphysis. She was treated with wide excision and reconstuction with nonvascularized autogenous fibular graft. We harvested fibular fragment preserving fibular continuity to reduce donor site morbidity. Surgical outcome and functional result was excellent.

  • Giant cell tumor is slow-growing, unilateral and solitary lesion that is most commonly seen in the digit of the hand, but occasionally occurs in the hips, ankles, toes and wrists and rarely in knee. We experienced 1 case of giant cell tumor in knee joint. That was excised arthroscopically and pathologically confirmed. So we report this case with a review of the literatures.

  • Giant cell-rich osteosarcoma is a rare tumor that is hard to distinguish from giant cell tumors of bone. The location and radiographic aspect of these tumors can be identical. The histologic differentiation between a giant cell-rich osteosarcoma and a giant cell tumor can be difficult. Due to the different prognosis and treatment strategies of these tumors it is essential to make the right diagnosis at presentation. This article reports a case of a giant cell-rich osteosarcoma of the proximal tibia that initially was diagnosed histologically as a giant cell tumor by needle biopsy.

  • Stem cells are attracting attention as a key element in future medicine, satisfying the desire to live a healthier life with the possibility that they can regenerate tissue damaged or degenerated by disease or aging. Stem cells are defined as undifferentiated cells that have the ability to replicate and differentiate themselves into various tissues cells. Stem cells, commonly encountered in clinical or preclinical stages, are largely classified into embryonic, adult, and induced pluripotent stem cells. Recently, stem cell transplantation has been frequently applied to the treatment of pain as an alternative or promising approach for the treatment of severe osteoarthritis, neuropathic pain, and intractable musculoskeletal pain which do not respond to conventional medicine. The main idea of applying stem cells to neuropathic pain is based on the ability of stem cells to release neurotrophic factors, along with providing a cellular source for replacing the injured neural cells, making them ideal candidates for modulating and possibly reversing intractable neuropathic pain. Even though various differentiation capacities of stem cells are reported, there is not enough knowledge and technique to control the differentiation into desired tissues in vivo. Even though the use of stem cells is still in the very early stages of clinical use and raises complicated ethical problems, the future of stem cells therapies is very bright with the help of accumulating evidence and technology.

  • It has been reported that mechanical stimulation takes a role in improving eel/ growth in skeletal system. Various research groups have been showed their own bioreactors which stimulate cell-seed three-dimensional scaffold. In this study, we hypothesized that the various conditions of mechanical stimulation would affect cell growth and proliferation. To prove our hypothesis, we designed a custom-made bioreactor capable of applying controlled compression to cell-encapsulated scaffolds. This device consisted of a circulation system and a compression system. Each parts of the bioreactor was fabricated using the rapid prototyping technology By using the rapid prototyping technology, we can modify and improve the bioreactor very rapidly For dynamic cell-culture, cell-encapsulated agarose gel was fabricated in 2% concentration. We performed dynamic cell-culture using this agarose gel and developed bioreactor in 3 days.

  • Diffuse-type giant cell tumor is relatively rare than localized giant cell tumor. Moreover, diffuse type giant cell tumor is common in intraarticular area, rarely occurs at intramuscular or subcutaneous layer. We experienced 1 case of giant cell tumor within the deltoid muscle. So we report this case with review of the literatures.

  • It is not uncommon for sarcomatous transformation of giant cell tumor of bone to occur after radiation, but osteosarcoma arising from giant cell tumor after surgical treatment is very rare and remains an aggressive form of sarcoma of bone with high mortality rate. We experienced 2 cases in whom a osteosarcoma developed long after benign giant cell tumor of bone was removed surgically from the same site. Malignant transformation was presented at 2 years 1 month and 9 years 8 months each after initial surgery. We describe our experience concerning clinical features, methods of treatment and outcomes of osteosarcoma arising from giant cell tumor.
