Purpose: The purpose of this study was to describe performance and education needs related to neurologic assessment by neurologic ward nurses. Methods: The study was conducted with a cross-sectional descriptive design. Participants, 297 neurologic ward nurses working in ten major hospitals in Seoul and Gyeonggi were surveyed from September 7 to October 1, 2015. Results: The mean score for performance of neurologic assessments was 3.04 points. Performances performed with high quality were orientation exam, mental status documentation, and muscle strength of extremities in that order. Performances performed with low quality were assessment of accessory nerves, hypoglossal nerves, and cerebellum function in that order. There were significant differences according to whether there was ward education programs on neurologic assessment and teaching experiences within a year. These variables significantly affected the quality of neurologic assessment performance. The need for neurologic assessment training was 3.14 points. Conclusion: Amount of performance and demand for education on neurologic assessment, which are required in the clinical setting, were significantly high. To perform neurologic assessments with high quality, development of education program for neurologic assessment and continuous education courses are required at either the department or neurological nursing organization level.
The twenty one individuals with total knee replacement who were admitted to Kang-Dong Hospital for comprehensive physical therapy were studied in order to demonstrate the effectiveness of an isokinetic test program using the Cybex 6000 machine. The subjects were divided into three groups with the isometric exercise group receiving isometric exercise. the isokinetic eccentric exercise group, and isokinetic eccentric exorcise group receiving isokinetic exercise(eccentric. concentric) to knee flexors and extensors muscles for a six weeks period using the Cybex 6000. The results were follow :1 . The peak torque was increased significantly in the flexors and extensors of the isokinetic conccentric exorcise among three groups(p<.05). 2. The progress of peak torque was increased significantly in the flexors and extensors of the isokinetic conccentric exercise among three groups(p<.05). 3. The total work was more increased significantly in the flexors and extensors of the isokinetic conccentric exercise among three groups(p<.05). 4. The progress of total work was more increased significantly in the flexors and extensors of the isokinetic conccentric exercise among three groups(p<;.05). 5. The ratio of endurance was not increased significantly in all three groups. 6. The extensors in the peak torque was likely to recover rapidly than the flexors. 7.The average R.O.M in the pre-exercise and post-exercise was not different significantly in all three groups. According to the above results, In the muscle strength recovery for total knee replacement patients, isokinetic concentric exercise group was significantly greater than the isokinetic eccentric and isometric exercise groups after a six weeks training.
Purpose: This study was conducted to evaluate the effectiveness of non-pharmacologic interventions in chemotherapy-induced peripheral neuropathy (CIPN). Methods: PubMed, Cochrane Library CENTRAL, EMBASE, CINAHL, and several Korean databases (Until August 2017) were searched. The main search strategy combined terms for peripheral neuropathy and presence of neoplasms. The risk of bias was assessed using the Cochrane's Risk of Bias tool for randomized studies and the Risk of Bias Assessment tool for non-randomized studies. To estimate the effect size, a meta-analysis of the studies was performed using the Rev Man 5.3 program of the Cochrane Library random-effects models were used in the analyses. Results: Twenty-two studies with a total of 954 participants met the inclusion criteria. Of the 22 studies, 12 were used to estimate the effect size of the non-pharmacologic interventions. The non-pharmacologic interventions used in patients with CIPN were exercise, acupuncture, massage, and foot bath. The acupuncture significantly reduced CIPN symptoms and signs (d=-0.71) and CIPN pain (d=-0.73) (p<.001). Massage and foot bath were also effective in reducing CIPN symptoms (d=-0.68; 95% CI=-1.05, -0.30; p<.001; $I^2=19%$).Exercises were effective in improving muscle strength and endurance(d=-0.55) and quality of life (d=-2.96), but they were not significantly effective in improving CIPN. Conclusion: Although these results provide little evidence of the effectiveness of acupuncture, massage, and foot bath in the treatment of CIPN, they suggest that these interventions can reduce CIPN symptoms in patients with cancer. However, the findings of this study should be interpreted with caution as there is a relative lack of data in this field, and additional well-designed studies are needed. Purpose : This study was conducted to identify the common characteristics of older persons with sarcopenia and to explore the relationship between gait, balance, and stress using an integrated assessment tool. Methods : In this study, 95 people aged 65 years or older were screened using the sarcopenia diagnostic evaluation algorithm presented by the Asian Working Group for Sarcopenia in 2019. Skeletal muscle index, grip strength, and short physical performance battery were used as sarcopenia evaluation measurements. Based on the results of this evaluation, participants were grouped into the 'non-sarcopenia group' (41 participants) or the 'sarcopenia group' (54 participants). Participants underwent further assessment using an integrated evaluation tool capable of measuring gait, balance, and stress. Gait ability was evaluated using the timed up and go test, and balance ability was evaluated using the berg balance scale. And the stress of the last month was measured by modifying the stress index developed by a Korean researcher. Collected data were statistically analyzed using the independent t-test and Mann Whitney-U test. Results : The sarcopenia group and the non-sarcopenia group showed significant differences in all elements of the sarcopenia diagnostic evaluation. There were significant differences in all three integrated evaluation tools. For the evaluation of walking ability, the time measured in the timed up and go test was longer in the sarcopenia group, the berg balance scale score for the evaluation of balance ability was lower in the sarcopenia group, and the stress index was higher in the sarcopenia group. Conclusion : Through sarcopenia analysis using an integrated evaluation tool, it was confirmed that sarcopenia is closely related to decreased walking ability, poor balance, and increased stress. We recommend using this tool to reduce the risk of sarcopenia progression and stress exposure through the planning and implementation of an exercise program for sarcopenia prevention.

This study was understood the risk mechanism of falls in the elderly and reviewed previous research data to see if the principle of Pilates rehabilitation exercise could have a positive effect on balance ability and postural stabilization in the elderly, and the purpose of this study is to present literature-based data on the fall prevention effect of rehabilitation Pilates exercise. First, the rehabilitation Pilates exercise makes it available for strengthening the center of the body to stabilize the spine and pelvis and stimulates the nerve root, thereby having an effect on the balance and the joint stabilization. Second, a proprioceptive sensory impairment and a musculoskeletal degenerative disease due to aging increase the loss of balance ability and the instability in posture maintenance, thereby coming to raise the risk of a fall damage caused by the difficulty in performing motor functions and by the gait disturbance. Third, the rehabilitation Pilates exercise leads to improving the core muscle strength in older adults, resulting in being capable of expecting the betterment in the balance and the reaction time motor function. And there is a positive impact on the improvement in body imbalance and on the stability in movement involved in the fall prevention, resulting in being able to suggest the possibility of likely contributing to a reduction in a fall risk rate. In conclusion, the rehabilitation Pilates as the elderly exercise program showed effects on the improvement in the body's muscular strength, on the upgrade in a sense of balance, and on the stabilization of core. Thus, it was considered to diminish the risk factors for a fall damage caused by musculoskeletal degeneration and to be capable of preventing a serious disorder of activity due to a chronic senile disease.
The purpose of this study was to investigate the effects of the regular Upright Body Type Excercise on posture, muscle strength of leg, lung capacity and depression of people with mental illness. The subjects of this study were recruited from B mental health center (5 years and more mental illness patients, n=19) located in P city. For the exercise group, They were carried out the Upright Body Type Excercise Program during 8 weeks (60 min/time, 3 times/week). They were randomly divided into two groups. Exercise group (n=10) and Control group (n=9). And also, they were agreed with consent forms before the experiments. Research results were as follows. Through the upper body type exercise, there was significantly difference in PEF and FEV1/FVC in the trained group. And also, there was much improved in depression level in the trained group. The change of melancholy feeling before and after the program was not statistically significant. However, because of limitations of sampling size due to the peculiarities of the program participants, in consideration of the limit of statistical validation exists clearly, intended to better understand the subjective experience of attendees, qualitative analyzing(qualitative research) was carried out in parallel. It was conducted a deep interview only person accepted among program participants, thematic analysis, subject analysis tasks to be subdivided by classifying by considering the semantic units of what participants expressed, was thus carried out. It found that the degree of melancholy feeling of mental disorders who participated in the Upright Body Typed Exercise Program was reduced. The course of the experience of change in depression appered the three subjects and six sub-themes such as "the start of the change", "interest of the program", "recognition of the need of the body’s health", "physical health promotion", "recovery of physical function", "negative change of habits (attitude)", "reduction of sense of depression", "confidence that it is possible to", "hope for the future". Therefore, upper body type exercise is much helpful in lung capacity and mental health of people with mental illness. So, this type of exercise mostly needed in the people with mental illness group than the other group for the quality of life.

The purpose of this study was to investigate the effect of 6 weeks' accelerated rehabilitation with anti-gravity treadmill exercise on VAS, ROM, isokinetic myofunction, and dynamic stability after surgery of modified brostrom operation in chronic ankle instability patients. The subjects of this study were 12 chronic ankle instability patients who underwent modified Brostrom operation(MBO) by the same doctor. 6 weeks' accelerated rehabilitation program is scheduled to perform for 60min, everyday, and also anti-gravity program performed for 15~30min, everyday. The visual analog scale(VSA) and significantly decreased(p<.001) and ROM in all of dorsal flexion, plantar flexion, inversion and eversion significantly increased(p<.05) after 6 weeks' accelerated rehabilitation with anti-gravity treadmill exercise. Both inversion and eversion peak torque at $60^{\circ}/sec$(p<.001, p<.01) and at $180^{\circ}/sec$(p<.001) significantly increased after 6 weeks' accelerated rehabilitation with anti-gravity treadmill exercise respectively. In muscle defect, although inversion(p<.01) and eversion(p<.001) at $60^{\circ}/sec$ and inversion(p<.01) at $180^{\circ}/sec$ significantly decreased, eversion at $180^{\circ}/sec$ tended to decrease but did not change significantly after 6 weeks' accelerated rehabilitation with anti-gravity treadmill exercise. The dynamic stability significantly increased after 6 weeks' accelerated rehabilitation with anti-gravity treadmill exercise(p<.001). These results suggest that 6 weeks' accelerated rehabilitation with anti-gravity treadmill exercise has positive effect of VAS, ROM, isokinetic myofunction, and dynamic stability after surgery of modified brostrom operation in chronic ankle instability patients. Therefore, we consider that the accelerated rehabilitation with anti-gravity treadmill exercise, which is safely and fast method, has effect on more faster recovery of ankle stability, play ground and normal daily activities.

The purpose of this study was to investigate the effects of combined exercise program on short physical performance battery, bone mineral density and parathyroid hormone levels in elderly women. This study subjects were 22 elderly female volunteers, aged 65-75 years and they divided into the combined exercise group(n=11) and the "no exercise" control group(n=11). The combined exercise included the line dance program performed twice per week for 60 minutes and the resistance exercise program performed once per week during 12 weeks. Exercise intensity was progressively increased from RPE "fairly light" to "slightly hard" (from 11 to 14). The findings of this study were as follows; Short physical performance battery(Balance test, gait speed, and rising from a chair five times) and bone mineral density (lumbar spine L2~L4 bone densities, and the T-score) were significantly increased after 12 weeks in the exercise group compared to the baseline. The PTH level was significantly decreased in the exercise group compared to the baseline. In conclusion, regular and continuous physical activity was effective for improving short physical performance and had a positive effect on bone mineral density and parathyroid hormone. With aging, women have decreased muscle strength and bone density and therefore, it is strongly recommended that women need to carry out continuous physical activity to prevent disease and ensure a healthy retirement.
The purpose of this study was to identify factors associated with falls among older adults. This cross-sectional study used the data of 10,272 older adults who participating in the Korean Elderly survey, including demographic and health related characteristics. The data were analyzed with complex samples in SPSS ver. 23.0. The results of multiple logistic regression analysis showed that age (OR=1.262, 95%CI 1.111-1.433), education (OR=0.846, 95%CI 0.764-0.937), subjective health status (OR=1.172, 95% CI 1.041-1.320; OR=1.422, 95%CI 1.289-1.570), number of chronic illness (OR=1.438, 95%CI 1.050-1.968), number of medications (OR=0.798, 95%CI 0.659-0.967), muscle strength of low extremities (OR=1.260, 95%CI 1.101-1.441), and fear of falling (OR=3.576, 95%CI 3.166-4.039; OR=14.236, 95%CI 12.637-16.038)were significantly associated with fall accidents in the elderly. Thus, it is necessary to develop fall prevention program to reduce fear of falling and improve health status in the elderly.
Objective: The purpose of this study was to develop and evaluate high technology adaptive driving controls, such as mini steering wheel-lever system and joystick system, for the people with physical disabilities in the driving simulator. Background: The drivers with severe physical disabilities have problems in operation of the motor vehicle because of reduced muscle strength and limited range of motion. Therefore, if the remote control system with driver-by-wire technology is used for adaptive driving controls for people with physical limitations, the disabled people can improve their quality of life by driving a motor vehicle. Method: We developed the remotely controlled driving simulator with drive-by-wire technology, e.g., mini steering wheel-lever system and joystick system, in order to evaluate driving performance in a safe environment for people with severe physical disabilities. STISim Drive 3 software was used for driving test and the customized Labview program was used in order to control the servomotors and the adaptive driving devices. Thirty subjects participated in the study to evaluate driving performance associated with three different driving controls: conventional driving control, mini steering wheel-lever controls and joystick controls. We analyzed the driving performance in three different courses: straight lane course for acceleration and braking performance, a curved course for steering performance, and intersections for coupled performance. Results: The mini steering wheel-lever system and joystick system developed in this study showed no significant statistical difference (p>0.05) compared to the conventional driving system in the acceleration performance (specified speed travel time, average speed when passing on the right), steering performance (lane departure at the slow curved road, high-speed curved road and the intersection), and braking performance (brake reaction time). However, conventional driving system showed significant statistical difference (p<0.05) compared to the mini steering wheel-lever system or joystick system in the heading angle of the vehicle at the completion point of intersection and the passing speed of the vehicle at left turning. Characteristics of the subjects were found to give a significant effect (p<0.05) on the driving performance, except for the braking reaction time (p>0.05). The subjects with physical disabilities showed a tendency of relatively slow acceleration (p<0.05) at the straight lane course and intersection. The steering performance and braking performance were confirmed that there was no statistically significant difference (p>0.05) according to the characteristics of the subjects. Conclusion: The driving performance with mini steering wheel-lever system and joystick control system showed no significant statistical difference compared to conventional system in the driving simulator. Application: This study can be used to design primary controls with driver-by-wire technology for adaptive vehicle and to improve their community mobility for people with severe physical disabilities.
