Background: People who have painful neck have reduced proprioception in the neck. Reduced proprioception in the neck is related to the muscle spindle activity in the deep neck flexors. The aim of this study was to know that proprioception in the neck was increased by strengthening exercise of the deep neck flexors. Methods: Thirty subjects with chronic neck pain were randomly assigned to the experimental group (n=15) and the control group (n=15). Deep neck flexor muscle exercise with stabilizer were conducted for the study group three times a week for six weeks. Relocation errors in 30 degree rotation to the left were measured three times before and after intervention each. Results: Neck disability index were decreased in the experimental group (p<.05) but not in the control group (p>.05). Relocation errors were decreased in the experimental group after intervention (p<.05), but not in the control group (p>.05). Conclusions: Proprioception in the neck can be increased by the strengthening exercise of deep neck flexors in the subject with chronic neck pain.

53 세 남자가 하부 식도에서 발생한 거대한 용종성 종괴에 의한 연하곤란을 주소로 내원하여 식도 절제술과 식도-위 문합술을 시행하였다. 현미경소견상 기저세포양 편평세포암과 연골 분화가 여러 곳에서 관찰되는 연골육종이 혼재하는 진성암육종이었다. 위장주위 임파절과 기도분기부하 임파절에 편평세포암이 전이되어 있었다. 면역조직화학적 검사상 편평세포 지역은 cytokeratin 에 강한 양성이었고 기저세포양 지역은 고분자량 cytokeratin 에 양성이었다. 방추세포육종은 vimentin 과 smooth muscle actin 에 양성이었고 연골육종 지역은 vimentin 과 S-100 단백질에 양성이었다. 술후 항암제 치료와 방사선 치료의 보조병용요법을 시행하였으며 술후 11 개월인 현재까지 재발의 소견 없이 정상 식사를 하고 있다.

원발성으로 폐에 발생하는 평활근육종(leiomyosarcoma)은 매우 드물다. 한림대학교 강동성심병원 흉부외과학 교실에서는 46세 남자환자의 좌측 폐상엽기관지에서 발생한 종 양을 흉부전산화 단층촬영에서 발견하였다. 객담검사및 기관지내시경하 조직생검에서 종양세포는 발견되 지 못하였다. 환자는 좌측 수상 폐상엽 절제술을 통해 폐실질에 부분적으로 침범된 기관지내에 발생한 종양을 제거 하였다. 술후 현미경하 조직소견을 관찰한 결과 종양세포는 이형성 방추형세포로 15/10HFP의 유사분열을 하고 있었고 긴다발로 배열되어 있었으며, 면역조직화학적 염색상 smooth muscle actin과 desmin에 양성으 로 염색되었다. 종양은 폐에서 원발성으로 발생한 평활근육종으로 진단되었고, 문헌고찰과 함께 보고하는바이다.

Infantile myofibromatosis is a rare benign neoplasm which is usually found in males. Most cases of the infantile myofibromatosis are discovered before the age of two years and about half of cases are found at muscle, tendon, and soft tissue of head-neck region. However, it is especially uncommon that infantile myofibroma occurs at the pre-auricular area. In current case, we report a twenty two months old male patient with $2.5{\times}1.5cm$ sized pre-auricular mass at right side. It was surgically removed and histologically showed broad bundle of plump myoid spindle cells with eosinophilic cytoplasm and weakly positive smooth muscle actin expression in myoid cells. In three months of follow up, there was no recurrence.

본 연구에서는 요추강화 운동기기를 이용하여 훈련 전후에 대한 유연성 및 근력 특성분석을 고찰하고, 요추 재활운동으로서 가능성을 검증하고자 한다. 요추관련 상해가 없으며 신체 건강한 성인 남녀 20명이 실험에 참여하였으며 각각 10명씩 대조군과 요추강화 운동을 제공받는 훈련군으로 나누었다. 실험에 사용된 요추강화운동기기는 하이스파인(Hi-Spine, 메디컬사이언스, 한국)이며 운동은 하루 40분씩 주 3일, 총 4주간 실시하였다. 또한 본 연구에서 피험자들은 누운 자세, 앉은 자세, 직립 자세, 전신스트레칭자세 등 총 4가지의 운동자세를 각 10분씩 제공받았다. 운동 전 후 요추의 유연성과 근력변화를 알아보기 위해 매주 1회 체간 굴곡 및 신전, 기초체력평가와 요추관절토크를 측정하였다. 실험 결과 훈련군에서 유연성과 근력이 모두 증진되었다. 요추강화운동기기의 플랫폼이 3차원 회전을 함에 따라 피험자들의 근육들은 자극받으며, 회전 방향과 각도에 따라 근긴장성과 근수축 작용을 유도시켜 유연성 및 요추 근력을 증가시키는 것으로 판단된다. 본 연구는 향후 요추관련 상해자 또는 고령자들의 요추 재활운동 프로그램에 적용될 수 있을 것이다.

신경보철 의수를 제어하기 위해서는 말초 신경신호를 측정하고 분석함으로써 사용자의 동작의도를 인식해야 한다. 본 논문에서는 다채널 신경전극 어레이, 전극 삽입 장치, 신경신호 증폭기로 구성된 말초 신경신호 측정 시스템을 제안한다. 마취상태에서 전극 삽입 장치를 이용하여 16 채널의 신경전극 어레이를 백서의 좌골신경에 안정적으로 삽입하였다. 수동적 관절 운동을 유도하거나 기계적 피부 자극을 인가하였을 때, 신경신호 증폭기를 통해 근육 또는 피부 감각 신경신호가 측정됨을 확인하였다. 또한, 개별 신경 단위를 분리하기 위해 활동전위 분류방법을 제안한다. 근육의 내부 수용기들은 근방추와 골지건 기관 수용기로 구분되었으며, 피부의 외부 수용기들은 완만적응과 급속적응 수용기로 분리되었다. 이와 같은 실험결과는 제안한 방법이 신경보철 시스템에서 말초 신경신호의 측정과 분석에 적용될 수 있음을 보였다.

A 7-year-old intact female Golden Retriever was presented for examination. The dog had large irregular subcutaneous masses in the abdomen which were ruptured or encapsulated. Those were removed surgically. Histopathologically, the masses consisted of spindle cells that often formed distinct whorls around a central capillary. Immuno-histochemical analysis revealed that the neoplastic cells were strong diffuse cyto-plasmic immunolabelling for vimentin and focal immunoreactivity for smooth muscle actin, whereas not immunoreactive for cytokeratin, desmin, von Willebrand factor, glial fibrillary acidic protein, or S-100. The neoplastic cells ultrastructurally had processes attached by desmosome-like structures, swollen mitochondria and dilated rough endoplasmic reticulum. Based on the above results, this case was diagnosed as a canine hemangiopericytoma in the abdominal subcutis of a Golden Retriever.

Angiomyolipoma(AML) of the liver is a rare benign tumor; about 50 cases, almost solitary, have been reported. We present here a extremely rare case of multiple AML in the liver diagnosed by fine needle aspiration cytology(FNAC). Two large masses were found in a 51 year-old woman complaining of abdominal discomfort by computed tomography and several smaller masses are scattered in the liver. FNAC was performed, showing bundles of spindle shaped smooth muscle cells intermingled with mature fat cells. FNAC may be a valuable method for definitive diagnosis of hepatic AML. The diagnosis was further confirmed by histologic examination with immunohistochemical studies.

A case of cutaneous fibroma was diagnosed in the skin of the right hindlimb of a Korean indigenous cattle in Kyungpook province, Korea. Grossly, the protruding skin nodules consisted of a solitary mass $(1.5{\times}1.5{\sim}3.0{\times}3.0cm)$ above large grapelike warty nodules $(11.0{\times}11.0cm)$ on the leg. These masses were firm and rubbery, and the cut surface was gray to white. Histopathologically, the benign neoplastic nodules consisted of spindle-type fibroblasts with collagen. Melanoma, sarcoma, and fibropapilloma were excluded because there was no sign of melanin, muscle type cells, or epidermal proliferation. To our knowledge, this is the first report of a cutaneous fibroma in a Korean indigenous cattle in Korea.

Cytologic findings of 2 cases of metastatic leiomyosarcoma diagnosed by fine needle aspirtion cytology are reported. Case 1 is pleomorphic leiomyosarcoma which had metastsized to the liver from the stomach of a 54-year-old male patient. The cytologic features showed highly cellular aspirates with nuclear pleomorphism and interlacing pattern. Case 2 is low grade leiomyosarcoma that occurred in the uterus of a 43-year female patient and metastsized to both lungs. The aspirates were less cellular than that of case 1, and showed spindle cells with minimal pleomorphism, but ceil block revealed interlacing patterns of smooth muscle cells with occasional mitosis.
