The Effects of a Bridging Exercise With Hip Adductor Contraction on the EMG Activities of Abdominal Muscles in Patients With Sub-Acute Stroke

  • Background: Muscle weakness and impaired trunk muscle control are common in stroke patients. The bridging exercise (BE) is generally used for trunk stabilization and improving the overall function of stroke patients. The effectiveness of the BE with hip adductor contraction (BEHA) in facilitating trunk muscle activation has been well studied in healthy adults. However, the impact of BEHA in sub-acute stroke patients has not yet been investigated. Objects: The purpose of this study was to determine the effects of BEHA on the electromyography (EMG) activities and the asymmetry of the rectus abdominis (RA), external oblique (EO) and internal oblique (IO) abdominal muscles. Methods: Twenty participants with sub-acute stroke (11 males and 9 females) were recruited. Each participant was asked to perform bridging exercises for five seconds under three different conditions: BE in a neutral position (BEN), BEHA with a large ball (BEHAL) and BEHA with a small ball (BEHAS). The EMG amplitudes of the bilateral RA, EO and IO and the asymmetry of the EMG activity between the sound and affected sides were compared among the conditions. The significance level was set at ${\alpha}=.05$. Results: The EMG activities of RA, EO and IO were significantly greater during BEHAL and BEHAS than during BEN (p<.05); the asymmetry of the RA, EO and IO decreased significantly during BEHAL and BEHAS compared to BEN (p<.05). However, no measured variables showed any significant differences between BEHAL and BEHAS (p>.05). Conclusion: This study compared the EMG activities of the RA, EO and IO on both sides and the asymmetry of the RA, EO and IO during BEN, BEHAL and BEHAS. Our findings suggest that BEHA was more effective for individuals with hemiplegic stroke at facilitating and normalizing abdominal muscle control than BEN.

  • A Literatural study on the etiological factors, classification, prescription of hemorrhoids and hemorrhoids complicated by anal fistula following results were obtained. 1. The cause of hemorrhoids are long time sit, long time gate, overfatigue, overeating, imbalance of stool( constipation or diarrhea), pregnant fertility(overfatigue after childbirth, insufficiency of middle warmer energy), uncontrol sexual excess, pathgenic factors of wetness, heat, wind, dry, genetic cause, excess of anxiety, pile up of heat poison, weakness of entrails and viscera. The cause of hemorrhoid complicated by anal fistula are attack of external wind, heatness, dry, fire, wetness(pathgenic factors), inapporiate treatment and chronic disease, greasy diet, excess of anxiety, constipation, uncontrol sexual excess, obstacle of circulation of vital energy and blood on anal site. 2. Classification of hemorrhoids are female hemorrhoids, male hemorrhoids, pulse hemorrhoids, intestines hemorrhoids, vital energy hemorrhoids, wine hemorrhoids, blood hemonhoids, flowing hemorrhoids. Classification with other method are external hemorrhoids, internal hemorrhoids, mixed hemorrhoids, excrescence hemorrhoids, nipple homorrhoids. External hemorrhoids is classified of varicosis of hemorrhoidal vein, connective tissue form, thrombus form. Classification of hemorrhoid complicated by anal fistula are simple lower hemorrhoid, lower mixed hemorrhoid, deep hemorrhoid, outer of one hole hemorrhoid, a horseshoe hemorrhoids. Once more classificated of four are space of sphincter muscle form, penetration sphincter muscle form, upper of sphincter muscle form, outer of sphincter muscle form. 3. Therapy method of hermorrhoid and hemorrhoid complicated by anal fistula are internal method, fumigation method method, ointment, method of close with medicine, necrotizing method, hot medicated compress( gxternal method), injection, insertion, bind, (operation) and acupuncture therapy (the others method) 4. Herb medicine for many used of internal method are Scutellaria baikalensis George(黃芩), Coptis japonia Makino(黃連), Rehmania giutinosa Liboschitz ex Fischer & Meyer(生地黃), Poncirus trifoliata Refinesque(枳殼), Sanguisorba officinalis Linne(地楡), Sophora japonica L.(槐花), Cnidium officinale Makino (川芎), Astragalus membranaceus Bunge(황기), Angelica gigas Nakai (當歸). 5. Herb medicine for many used of fumigation are Schlechtendalia Chinesis J. Bell (五倍子), Artemisia Vulgaris L. var indica Maxim(艾葉), Poncirus trifoliata Refinesque (枳殼), Nepeta japonica Maximowicy(荊芥), And herb medicine for many used of ointment are Calomelas(輕粉), Alum(白礬), Boswellia carterii Birdwood(乳香), Os Draconis Fossilia Ossis Mastodi(龍骨).

Effects of Aquatic Exercise on Skinfold Thickness and Circumference of Upper and Lower Extremities in Patients with Rheumatoid Arthritis (수중운동 프로그램이 류마티스 관절염 환자의 사지 피부두겹 두께와 둘레에 미치는 영향)

  • Many arthritis patients experience weakness of muscles in extremities mainly due to disuse atrophy and weight gain because of the limited activities and exercises. This study examines the effects of the 6-week aquatic-exercise program on the body fat and the muscle of the rheumatoid arthritis patients. Seventeen patients in the experimental group and 18 in the control group were assigned depending on their preference and physical condition. These patients had more than 3 points of pain out of 10, deformities in knee, wrist and ankle joints. The amount of aquatic exercise increases from 35 minutes in the first week to 60 minutes in the 6th week. In the resting period they discussed their own experiences about exercise, personal and family affairs, and performed some recreation programs in order to increase the self-efficacy and promote the relationship with other patients by the group activities. Skinfold thickness and circumferences of both extremities were measured before and after experiment to compare the difference. For testing the body fat Saham Model was used. Prior to the experiment two group's body weight, skinfold thickness and skin circumferences were not significantly different which indicates the homogeneity of two groups. Body weight and most parts of skinfold thickness of the experimental group were significantly lower than the control group after 6-week aquatic exercise program. Circumference was not significantly lower than the control after the program. These findings indicate the in-crease of muscle sizes and the reduction of the body fat. Therefore a more active application of aquatic exercise into a variety of clients is strongly suggested.

Idiopathic Polymyositis in a Young Mature Alaskan malamute (젊은 성숙 알라스칸 말라뮤트에서 특발성 다발성근염 증례)

  • Clinical and histopathologic features of idopathic polymyositis in twenty-month-old Alaskan malamute dog are described. The clinical signs were progressive exercise intolerance with acute exacerbation of weakness, muscle atrophy, synchronous pelvic limb gait, short stiff steps and tip-toeing as like walking on eggshells. Physical and clinical examination revealed no evidence of neurologic, skeletal and secondary muscular disorders associated with other diseases. Therefore muscle biopsy was performed at the most severe muscle atrophy lesions to confirm by histopathology. Histopathologic findings documented mononuclear cell infiltration and necrosis of muscle fiber and it was diagnosed as idiopathic polymyositis. Initial treatment was focused on pain relief. Prednisone at immunosuppressive dose (2 mg/kg) was administered orally twice daily. After 3 weeks of starting treatment, the patient showed improvement of gait, appetite, exercise as well as gradually return to normal state of hematologic and serum chemistry profiles.

The Effects of Bridge Exercise with Contraction of Hip Adductor Muscles on Thickness of Abdominal Muscles (고관절 내전근 수축을 이용한 교각운동이 복부근육의 두께에 미치는 영향)

  • PURPOSE: The purpose of this study was to determine the correlation between the hip adductor muscles and abdominal muscles during bridge exercise. METHOD: Participants who met the criteria for this study(n=36) were divided into the three groups. The first experimental group performed normal bridge exercises and the second group performed bridge exercises with the contraction of the hip adductor muscles and the control group didn't perform any exercise. Transversus abdominis muscle thickness was measured by ultrasound imaging with a special transducer head device, at pre exercise, after 2 weeks, 4 weeks, and 6 weeks. RESULT: Data were analyzed using repeated ANOVA with the level of significance set at ${\alpha}=.05$. Transversus abdominis muscle thickness was influenced by contraction of the hip adductor muscles during bridge exercise in people without lower back pain. Compared with normal bridge exercise, transversus abdominis muscle thickness significantly increased in thickness during bridge exercise with contraction of the hip adductor muscles(p<.05). CONCLUSION: The results from this study showed that contraction of the hip adductor muscles during bridge exercise increased change in the transversus abdominis muscle thickness. These results can be a good source to prevent low back pain due to hip adductor weakness. Therefore, inducing activation of hip adductor with abdominal stabilizing exercise is more effective in patients with low back pain.

Effects of Digital Exercise Intervention Using Artificial Intelligence (AI) on the Physical Abilities of Adults (인공지능(AI)을 이용한 디지털 운동중재가 성인의 신체능력에 미치는 영향)

  • Purpose : Along with the rapid development of digital technology, the application of digital healthcare in the medical field is also increasing. According to many experts, increasing the amount of exercise and physical activity is a helpful way to prevent and manage physical problems in modern society. However, a lack of exercise, which is of the lifestyle of modern people, leads to the development of various diseases. This study aimed to examine the effects of digital exercise intervention using artificial intelligence (AI) on the physical abilities of adults whether digital exercise intervention can be a reliable and effective therapeutic option for musculoskeletal disorders in real-world clinical settings. Methods : In this study, exercise was conducted using a digital application to investigate the effects of an AI-based digital exercise intervention on the physical abilities of adults. A total of 13 adults were evaluated, and their physical abilities before and after the exercise intervention were compared. Hand-grip strength, functional leg muscle strength, dynamic balance, and quadriceps muscle strength were assessed. Exercise was performed using a digital application and in a non-face-to-face manner. AI identified the exercise status of each participant and adjusted the exercise difficulty level accordingly. The exercised daily for 4 weeks. Results : A total of 12 participants were analyzed for the final results. Significant improvements were observed in hand-grip strength, functional leg muscle strength (evaluated using the stand-up test), dynamic balance, and straight-gait ability (p<.05), indicating an increase in the overall muscular strength and physical function of the participants. Conclusions : Digital exercise intervention using AI is effective in improving physical abilities related to musculoskeletal function. It can be useful in clinical practice as an effective treatment option for patients with musculoskeletal disorders or muscle weakness.

A Patient with Spinal Accessory Neuropathy after Self Neck Massage Treated with TMJ Balancing Therapy: A Case Report (경부 자가 마사지 후 척수부신경병증을 호소하는 환자의 턱관절균형요법 치험례)

  • The purpose of this study is to describe the effects of temporomandibular joint balancing therapy on a patient with a spinal accessory neuropathy. A patient reported sternocleidomastoid muscle and trapezius muscle weakness after self-massage. The patient was instructed to wear the Accurate Balancing Appliance (ABA) for at least 8 hours per day and maintain an upright posture. The patient received cervical spine manipulation on day 2, 4, and 6. The patient's progress was tracked based on the angle at which the head turned to the right from the midline when she lifted her head while lying down, as well as the muscle strength and pain in the right trapezius muscle. The patient recovered spontaneously after 7 days. The ABA and cervical spine manipulation assisted the patient in reducing fatigue and discomfort during daily activities.

Comparison of Rotator Cuff Muscle Strength With and Without Scapular Dyskinesis in Adolescent Baseball Players

  • Background: Scapular dyskinesis (SD) is closely related to imbalance of the rotator cuff (RC) muscles. However, previous studies have only focused on isometric strength. To our knowledge, there has been no study examining potential differences in concentric and eccentric strength and functional strength ratio (FSR) of the RC muscles based on functional parameters related to throwing in with or without SD. Objects: This study aimed to determine whether there was a difference in the RC muscle strength and FSR between the dominant shoulder with SD and the non-dominant shoulder without SD in adolescent baseball players. Methods: Forty adolescent baseball players participated and classified types of SD based on movement patterns using the SD test by two examiners. The isokinetic concentric and eccentric peak torque of the internal rotation (IR) and external rotation (ER) were measured and quantified as peak torque to body weight (PT/BW). Also, the FSR was obtained by calculating the strength ratios of eccentric IR to concentric ER (IRecc/ERcon) and eccentric ER to concentric IR (ERecc/IRcon), respectively. Results: There was a significant decrease in the IR and ER PT/BW in the dominant shoulder with SD compared to the non-dominant shoulder without SD (p < 0.05), regardless of contraction types. However, no significant difference was observed in the FSR in both IRecc/ERcon and ERecc/IRcon ratios. Conclusion: The findings indicate that the isokinetic concentric and eccentric PT/BW of the IR and ER muscles were significantly lower in the dominant shoulder with SD than in the nonthrowing shoulder without SD. Therefore, when establishing a strategy for addressing RC muscle weakness in adolescent baseball players with SD, it is essential to consider an approach that accounts for scapular kinematic recovery.

A familial case of limb-girdle muscular dystrophy with CAV3 mutation

  • Limb-girdle muscular dystrophy (LGMD) is a group of muscular dystrophies that has extremely heterogeneous clinical features and genetic background. The caveolin-3 gene (CAV3) is one of the causative genes. LGMD appears as a clinical continuum, from isolated skeletal muscle involvement to long QT syndrome. Here we report two patients without apparent muscle weakness in a family with CAV3 mutation. A 7-month-old Korean boy visited our muscle clinic because of an incidental finding of elevated serum creatine kinase (CK) concentration (680 IU/L, reference range, 20-270 IU/L) without clinical symptoms. The patient was born after an uneventful pregnancy and showed normal developmental milestones. He developed pseudohypertrophy of his calf muscle during the follow-up. We obtained a muscle biopsy at age 14 months, which showed size variations and degenerating/regenerating myofibers with endomysial fibrosis and immunohistochemical evidence of normal dystrophin. Under the impression of LGMD, we performed target panel sequencing and identified a heterozygous in-frame mutation of CAV3, c.307_312delGTGGTG (p.Val103_Val104del). Immunohistochemical staining of muscle indicated complete loss of caveolin-3 compared with normal control muscle, which supported the variant's pathogenicity. We performed segregation analysis and found that the patient's mother had the same variant with elevated serum CK level (972 IU/L). We report on autosomal dominant familial caveolinopathy caused by a pathogenic variant in CAV3, which was asymptomatic until the fourth decade. This case highlights the utility of next generation sequencing in the diagnosis of muscular dystrophies and the additive role of muscle biopsy to confirm the variants.

Extraction of Muscle Areas from Ultrasonographic Images using Information of Fascia (근막 정보를 이용한 초음파 영상에서의 근육 영역 추출)

  • Ultrasonography constructs pictures of areas inside the body needs in diagnosis by bouncing high-enorgy sound waves(ultrasound) off internal tissues or organs. In constructing an ultrasonographic image, the weakness of bounding signals induces noises and detailed differences of brightness, so that having a difficulty in detecting and diagnosing with the naked eyes in the analysis of ultrasonogram. Especially, the difficulty is extended when diagnosing muscle areas by using ultrasonographic images in the musculoskeletal test. In this paper, we propose a novel image processing method that computationally extracts a muscle area from an ultrasonographic image to assist in diagnosis. An ultrasonographic image consists of areas corresponding to various tissues and internal organs. The proposed method, based on features of intensity distribution, morphology and size of each area, extracts areas of the fascia, the subcutaneous fat and other internal organs, and then extracts a muscle area enclosed by areas of the fascia. In the extraction of areas of the fascia, a series of image processing methods such as histogram stretching, multiple operation, binarization and area connection by labeling is applied. A muscle area is extracted by using features on relative position and morphology of areas for the fascia and muscle areas. The performance evaluation using real ultrasonographic images and specialists' analysis show that the proposed method is able to extract target areas being approximate to real muscle areas.
