  • The pathogenesis and mechanism of obstructive sleep apnea (OSA) has been under investigation for over 25 years, but its etiology and mechanism remains elusive. Skeletal (maxillary and/or mandibular hypoplasia or retrodisplacement, inferior displacement of hyoid) and soft tissue (increased volume of soft tissue, adenotonsillar hypertrophy, macroglossia, thickened lateral pharyngeal walls) factors, pharyngeal compliance (increased), pharyngeal muscle factors (impaired strength and endurance of pharyngeal dilators and fixators), sensory factors (impaired mechanoreceptor sensitivity, impaired pharyngeal dilator reflexes), respiratory control system factors (unstable respiratory control) and so on facilitate collapse upper airway. Therefore, OSA may be a heterogeneous disorder, rather than a single disease entity and various pathogenic factors contribute to the OSA varies person to person. As a result, patients may respond to different therapeutic approaches based on the predominant abnormality leading to the sleep-disordered breathing.

  • Purpose: The purpose of this study was to investigate whether changes in electromyography (EMG) activations of spinal stability muscles with respiratory demand change were due to changes in respiratory demand or in postural demand. Methods: Forty healthy subjects (19male, 21female, $20.8{\pm}1.9$years old) performed quiet breathing and four different forced respiratory maneuvers (FRM) (Pulsed Lip Breathing, Diaphragmatic Breathing, Combination breathing, and respiratory muscle endurance training breathing) while in sitting and standing positions. EMG data for four muscles (TrA/IO, EO, RA, and ES) were collected and filtered using a band pass filter (20~200Hz) and a notch filter (60, 120, 180Hz). Results: There were no significant differences on percentage of change on %MVIC between QB and FRM (PLB, CB, DB, and RMET) between positions (all p>0.05).

  • 5-Aminoimidazole-4-carboxamide ribonucleotide (AICAR), a structural analog of adenosine monophosphate (AMP), promotes oxidative remodeling in muscle cells. AICAR activates AMP-dependent protein kinase (AMPK), thus increasing lipid metabolism, respiration, and mitochondrial counts. This process is called oxidative remodeling, which enhances the physical endurance of mice. To test this drug on an invertebrate that is genetically similar to humans, we used the small water crustacean Daphnia magna, which is sensitive to changes in water conditions. We tested the effects of pH on the efficacy of AICAR using two methods. One method measured oxygen consumption of Daphnia in oxygen chambers. The other method determined lipid levels of Daphnia through fluorescent tagging of lipids. The results showed that when exposed to AICAR at pH 6.58, D. magna consumed more oxygen and had lower overall levels of lipids, which is consistent with the expected effects of AICAR, such as increased respiration and lipid metabolism.

  • Sarcopenia, a condition characterized by the insidious loss of skeletal muscle mass and strength, represents a significant and growing healthcare challenge, impacting the mobility and quality of life of aging populations worldwide. This study investigated the therapeutic potential of soybean leaf extract (SL) for dexamethasone (Dexa)-induced muscle atrophy in vitro and in an in vivo model. In vitro experiments showed that SL significantly alleviated Dexa-induced atrophy in C2C12 myotube cells, as evidenced by preserved myotube morphology, density, and size. Moreover, SL treatment significantly reduced the mRNA and protein levels of muscle RING-finger protein-1 (MuRF1) and muscle atrophy F-box (MAFbx), key factors regulating muscle atrophy. In a Dexa-induced atrophy mouse model, SL administration significantly inhibited Dexa-induced weight loss and muscle wasting, preserving the mass of the gastrocnemius and tibialis anterior muscles. Furthermore, mice treated with SL exhibited significant improvements in muscle function compared to their counterparts suffering from Dexa-induced muscle atrophy, as evidenced by a notable increase in grip strength and extended endurance on treadmill tests. Moreover, SL suppressed the expression of muscle atrophy-related proteins in skeletal muscle, highlighting its protective role against Dexa-induced muscle atrophy. These results suggest that SL has potential as a natural treatment for muscle-wasting conditions, such as sarcopenia.

  • BACKGROUND/OBJECTIVES: Irisin, a newly identified hormone, is associated with energy homeostasis. We investigated whether aged garlic extract (AGE) and exercise training intervention could improve body weight, insulin sensitivity, skeletal muscle fibronectin domain containing protein 5 (FNDC-5) levels, and plasma irisin in high-fat diet (HFD). MATERIALS/METHODS: Male Sprague Dawley rats were fed a ND (normal diet, n=5) or HFD (n=28) for 6 weeks. After 6 weeks, all rats were divided into 5 groups for the next 4 weeks: ND, (normal diet, n=5), HFD (high-fat diet, n=7), HFDA (high-fat diet + aged garlic extract, n=7), HFDE (high-fat diet + exercise, n=7), and HFDEA (high-fat diet + exercise + aged garlic extract, n=7). Exercise groups performed treadmill exercises for 15-60 min, 5 days/week, and AGE groups received AGE (2.86 g/kg, orally injected) for 4 weeks. RESULTS: Significant decreases in body weight were observed in the ND, HFDE, and HFDEA groups, as compared with the HFD group. Neither intervention affected the masses of the gastrocnemius muscle or liver. There were no significant differences in glucose levels across the groups. The homeostatic model assessments of insulin resistance were significantly higher in the HFD group, as compared with the ND, HFDA, HFDE, and HFDEA groups. However, skeletal muscle FNDC-5 levels and plasma irisin concentrations were unaffected by AGE or exercise in obese rats. AGE supplementation and exercise training did not affect skeletal muscle FNDC-5 or plasma irisin, which are associated with insulin sensitivity in obese rats. CONCLUSION: Our results suggest that the protection against HFD-induced increases in body fat/weight and insulin resistance that are provided by AGE supplementation and exercise training may not be mediated by the regulation of FNDC-5 or irisin.

  • This report describes a rare, congenital hypertrophy of the left upper extremity that appeared after compressive bandage of right arm at the age of two. He is eighteen years old, and hypertrophy was aggravated for about 2 years since he started weight training exercise. Recently, skin turgor changed and he visited the Dermatology department. Skin biopsy revealed increased thickness of the dermis. On Orthopaedic examination, the left arm showed nonspecific neuro-muscular changes other than easy fatigability a.nd increased skin consistency after exercise, compared to the right arm. The differences of circumference were 2.5 to 4cm according to the level of the upper limb. But the relative proportion of hypertrophy of the limb was balanced., On X-ray examination, bony changes were not shown. Through the MRI, we could find edematous changes of subcutaneous fatty tissue. Muscular structures showed unremarkable changes. Through the endurance test of both arms, we could find a decrease in endurance of the left upper arm musculatures. On histologic examination, infrequent focal necrosis and peri fascicular degeneration of the muscle fiber were present.
