Treatment of post-traumatic chin deformities using bilateral botulinum toxin injections

  • Post-traumatic hematoma formation is a common complication of contusion. If the hematoma is large enough to aspirate or drain, it can be treated quickly and appropriately. However, if the hematoma is small or concealed by local swelling, it may be overlooked and left untreated. In most cases, a hematoma will resolve following conservative treatment; however, associated infection or muscle fibrosis can occur. Herein, we present the case of a patient with a chin deformity caused by a post-traumatic hematoma. The deformity was treated using botulinum toxin and triamcinolone acetonide injections as minimally invasive treatments. The course of treatment was good.

요통환자의 천장관절에 대한 도수교정 전과 후의 골반경사 비교 (A Comparison of Pelvic Tilt Before and After Manipulation of Sacroiliac Joint in the Patients with Low Back Pain)

  • The purposes of this study were to compare pelvic tilt before and after manipulation of sacroiliac joint in 31 low back pain patients (11 males, 20 females) with sacroiliac joint dysfunction. The sacroiliac joint of patients was manipulated on the side of anterior pelvic tilt, using the technique described by Stoddard (1962) and Greenmann (1996). I used this technique because it usually eliminated sacroiliac joint dysfunction in one treatment session. SPSS for window computer system was used to analyze the data. Also t-test was performed for comparison of pelvic tilt angle before and after manipulation, and Pearson product-moment correlation analysis was performed for intratester reliability for measurements of pelvic tilt angle before and after manipulation. The result were as follows: 1. Intratester reliability was good for measures of pelvic tilt (r=.98). 2. The pelvic tilt after manipulation was significantly decreased (mean=$3.40^{\circ}$) compared with the pelvic tilt before manipulation (p=.001). All subjects showed asymmetrical right versus left pelvic tilt before manipulation. 40% of subjects showed decreased asymmetrical right versus left pelvic tilt after manipulation, and 60% of subjects showed symmetrical right versus left pelvic tilt after manipulation. I think that pelvic tilt asymmetry with hypomobility due to loss of joint play could be symmetrized by manipulation or mobilization, but pelvic tilt asymmetry with unilateral pelvic muscle shortening could not be symmetrized by manipulation or mobilization without relaxation and stretching of shortened muscles.

Poland 증후군 환자의 흉벽 및 유방 재건술 (Chest Wall and Breast Reconstruction in Poland's Syndrome)

  • Purpose: As a rare congenital anomaly, Poland's syndrome has been known to show hypoplasia in breast and nipple, absence of pectoralis major muscle, and aplasia or deformity of rib or costal cartilage which has been reported to be more common in male. However, most patients who are seeking operation are female patients having one-side deformity. In the field of plastic surgery, the major surgical indications could be asymmetric chest wall depression in man or breast hypoplasia in woman. There are many reconstruction options according to the degree of patient's deformity: a prosthetic implant, breast implant with or without tissue expander, latissimus dorsi musculocutaneous pedicled flap with or without implant and/or tissue expander, and free tissue transfer with or without tissue expander. Methods: The authors have treated 4 patients(2 male, 2 female) who had a diagnosis of Poland's syndrome. According to the degree of patient's deformity, all patients underwent correction of breast asymmetry and unilateral anterior thoracic hypoplasia with one-staged or two-staged reconstruction. Results: All patents were satisfied with the results and there occurred no specific complications. Conclusion: The authors propose the treatment plan for patient with Poland's syndrome, according to the degree of patient's deformity. In case of male patient with mild deformity, the prosthetic implant or latissimus dorsi musculocutaneous pedicled flap will simulate the missing pectoralis and improve the contour deformity. In case of female patient with moderate to severe breast asymmetry and upward displaced nipple areolar complex (NAC), NAC can be lowered with tissue expander, breast can be enlarged with autologous free flaps or latissimus dorsi musculocutaneous pedicled flap with implant.

Botulinum Toxin Therapy versus Anterior Belly of Digastric Transfer in the Management of Marginal Mandibular Branch of the Facial Nerve Palsy: A Patient Satisfaction Survey

  • Background Botulinum toxin (BT) chemodenervation and anterior belly of digastric muscle (ABD) transfer are both treatment options in the management of an isolated marginal mandibular branch of the facial nerve (MMB) palsy. We compare the patient satisfaction following either BT injections or ABD transfer in the management of their isolated MMB palsy. Methods Patients in the ABD-arm of the study were identified retrospectively from September 2007 to July 2014. The patients in the BT-arm of the study were identified prospectively from those attending the clinic. Both groups of patients completed a validated patient satisfaction survey. Statistical analysis was performed and a P-value <0.05 was considered statistically significant. Results Seven patients were in the ABD-arm and 11 patients in the BT-arm of the study. The patient satisfaction in both groups was high with 45% of ABD-arm patients and 40% of BT-arm patients rating their overall outcome as 'better' or 'much better', which was significantly more than the proportion rating their outcome as 'worse' or 'much worse' (P<0.001), although there was a significant trend towards those in the ABD-arm being more likely to be dissatisfied with their outcome (P=0.01). Conclusions BT therapy is a good first-line intervention in the management of isolated MMB palsy. We have, however, shown that the overall satisfaction in both groups is high. Therefore, in patients who would prefer a more permanent solution to manage their facial asymmetry, ABD transfer remains a satisfactory treatment option with a good level of patient satisfaction.

Comparison of Both Legs EMG Symmetry during Over-Ground Walking and Stair Walking in Stroke Patients

  • Purpose: Gait is the most basic element when evaluating the quality of life with activities of daily living under ordinary life circumstances. Symmetrical use of the lower extremities requires complicated coordination of all limbs. Thus, this study examined asymmetry of muscle activity quadriceps femoris and tibialis anterior as a baseline for training during over-ground walking and stair walking of stroke patients. Methods: Subjects were 14 stroke patients included as one experimental group. Gait speed used in this study was determined by the subject. Low extremity paretic and non-paretic EMG was compared using the surface EMG system. Results: The low extremity EMG difference was statistically significant during over-ground walking and stair walking (p<0.05). The result of low extremity EMG substituted symmetry ratio formula was compared to EMG symmetry ratio in both legs during over-ground walking and stair walking. The average symmetry ratio of quadriceps femoris during over-ground walking was 0.65, and average symmetry ratio of quadriceps femoris during stair walking was 0.47, with significant difference (p<0.05). Conclusion: EMG data was higher in stair walking than over-ground walking. However, in the comparison of symmetry ratio, asymmetric EMG of quadriceps femoris was significantly increased during stair walking. These findings suggested that application of stair walking for strengthening of both legs can be positive, but the key factor is maintaining asymmetrical posture of both legs. Therefore, physical therapists should make an effort to reduce asymmetry of quadriceps femoris power during stair walking by stroke patients.

비대칭적 입식자세에서 상지 조립 작업 시 체간 및 둔부 근육의 좌우 근활성도 비교 (Comparison Between Right and Left Muscle Activities of Hip and Trunk During Manual Task in Asymmetric Weight-Bearing Posture)

  • The purpose of this study was to compare the electromyographic(EMG) activities of trunk and hip muscles between right and left sides while subjects performed prolonged manual task in asymmetric and symmetric weight-bearing posture. Fifteen healthy male college students were recruited for this study. The subjects were asked to perform bimanual upper extremity task for 6 minutes in two different standing postures. In the symmetric weight-bearing posture, the subjects were standing with evenly distributed body weights to both legs. In the asymmetric weight-bearing posture, the subjects distributed about 90% of their body weight onto their preferred(supporting) leg and 10% of their body weight onto the opposite leg while they were standing. EMG activities of the right and left internal oblique, erector spinae, gluteus maximus, and gluteus medius were measured and normalized as % MVIC. Then the EMG data were statistically analyzed using paired t-tests. The EMG activities of all measured muscles were not significantly different between the right and left side in the symmetrical weight-bearing posture(p>0.05). However, the EMG of the supporting side internal oblique was significantly lower than the opposite side(p<0.05), and the EMG of the erector spinae, gluteus maximus, and gluteus medius were significantly greater on the supporting side(p<0.05). The results of this study support that unbalanced use of right and left muscle possibly causes the changes in muscle length which results in asymmetry of trunk and hip muscles. Furthermore, the uneven weight support onto right and left legs will cause a distortion of viscoelastic ligaments around hip and sacroiliac joints in the long run. Further studies to determine the effect of various manual tasks on the trunk and hip muscles as well as the effect of asymmetrical weight-bearing standing posture on hip and back muscle fatigue may be required.

편마비 환자의 반 쪼그려 앉기(semi-squat)동작 시 양하지 지지면의 형태가 하지 근활성도와 체중분포에 미치는 영향 (The Effect of Types of Weight-Bearing Surfaces on Muscle Activities of Lower Limbs and Weight Distribution During Semi-Squat Movement of Patients With Hemiplegia)

  • This study used an unstable platform to change the support surface type and position of both lower limbs in order to determine changes in weight distribution and muscle including the vastus medialis, tibialis anterior, lateral hamstring, and lateral gastrocnemius of both lower limbs were evaluated during knee joint flexing and extending in a semi-squat movement in 32 hemiplegic patients. The support surface conditions applied to the lower limbs were divided into four categories: condition 1 had a stable platform for both lower limbs; condition 2 had an unstable platform for the non-hemiplegic side and a stable platform for the hemiplegic side; condition 3 had a stable platform for the non-hemiplegic side and an unstable platform for the hemiplegic side; and condition 4 had an unstable platform for both sides. The normalized EMG activity levels of muscles and weight bearing ratio of both sides in the four surface conditions were compared using repeated measures ANOVA. A significant increase was found in the weight support distribution for the hemiplegic side in flexing and extending sessions in condition 2 compared to the other conditions (p<.05). A statistically significant decrease in significant decrease in asymmetrical weight bearing in flexing and extending sessions was observed for condition 2 compared to the other conditions (p<.05). A similar significant decrease was found in differences in muscular activity for both lower limbs in condition 2 (p<.05). The muscular activity of the hemiplegic side, based on the support surface for each muscle showed a significantly greater increase in condition 2 (p<.05). An unstable platform for the non-hemiplegic side and a stable platform for the hemiplegic side therefore increased symmetry in terms of the weight support distribution rate and muscle activity of lower limbs in hemiplegic patients. The problem of postural control due to asymmetry in hemiplegic patients should be further studied with the aim of developing continuous effects of functional training based on the type and position of the support surfaces and functional improvement.

Sternocleidomastoid and Posterior Cervical Muscle Coordination in Response to Symmetrical and Asymmetrical Jaw Functions in Normal Adults

  • Purpose: The aim of this study was to elucidate the coordination patterns of the sternocleidomastoid and posterior cervical muscles in response to symmetrical and asymmetrical jaw functions in normal adults. Methods: Twenty-seven healthy volunteers (8 females, 19 males; mean age, $30.4{\pm}2.5$ years) participated in this study. Surface electromyography (EMG) was used to record activities in the masseter, suprahyoid, sternocleidomastoid, and posterior cervical muscles at rest and during maximum tooth clenching, biting of a cotton roll with the anterior teeth, unilateral biting of a cotton roll with the posterior teeth, bilateral biting of cotton rolls with the posterior teeth, and jaw opening while seated. Normalized amplitude, activity indices, and asymmetry indices were compared between the muscles and the jaw tasks. Results: During symmetrical jaw functions (e.g., tooth clenching, biting with the anterior teeth, bilateral biting with the posterior teeth, jaw opening), the sternocleidomastoid and posterior cervical muscles showed elevated EMG amplitudes compared with the resting condition. The co-activation pattern of the sternocleidomastoid muscle was more pronounced than those of the posterior cervical muscles during these tasks. During asymmetrical jaw functions (e.g., unilateral biting with the posterior teeth), the ipsilateral sternocleidomastoid and masseter muscles showed higher contraction activity than did the contralateral muscles, but the contralateral posterior cervical muscles were more active than the ipsilateral muscles. Conclusions: The sternocleidomastoid and posterior cervical muscles were shown to be co-activated and coordinated anteroposteriorly or bilaterally according to symmetrical or asymmetrical jaw function. These results suggest an integrated neural control mechanism for the jaw and neck muscles, and provide further evidence supporting the intimate functional coupling between the trigeminal and cervical neuromuscular systems.

가방 끈 길이 차이가 상지 근활성도, 압력중심 및 체중분포에 미치는 영향 (Effect of Back Strap Length Differences on Upper Extremity Muscle Activity, Center of Pressure, and Weight Distribution)

  • The purpose of this study was to investigate the effect on the activity of the upper trapezius and erector spinae according to the length difference of the backpack using electromyogram (EMG). We also conducted a study comparing the center of pressure (COP) and weight distribution before and after using Wii® balance board. Thirty individuals were randomly assigned to the experimental group (n=14) wearing a backpack with a short right strap length and a control group (n=16) wearing a backpack with the same strap length. Data were collected by dividing into groups, carring a 15% weight backpack and walking for 20 minutes on a treadmill. As a result of analyzing the EMG data, there was no significant change in the upper extremity muscle activity of the control group, but it was significantly decreased in the right upper trapezius activity of the experimental group (p<0.05). In addition, there was a significant change of COP in the experimental group (p<0.05), but there was no significant difference of the weight distribution in both groups. Recognizing mechanical changes in the body that may occur due to the asymmetry of the strap length is thought to be helpful for systematic ergonomic intervention according to changes in the external environment in the future.

천장관절에 대한 도수치료가 기능적 다리길이 차이에 미치는 영향 (The Influence of Manual therapy for Sacroiliac Joint on Functional Leg Length Discrepancy)

  • Background: The leg length discrepancy (LLD) have been implicated with degenerative diseases since the early 1970s, which also causes musculoskeletal asymmetry problems. The LLD appears due to factors such as balance ability, response time, and muscle strength between the shorter and longer leg. In this study, the effect of manual therapy to LLD has been studied. Methods: Manual therapy has been applied on shorter leg of 30 patients with LLD. The manual therapy was conducted for 2~3 minutes 3 times a week for 4 weeks. Results: The leg length of shorter leg were measured at initial baseline and after 4 weeks. the average leg length of shorter leg increased from 94.10 cm to 94.33 cm. Conclusion: This paper reports that the manual therapy has positive effects on LLD. It was studied by conducting manual therapy on muscles and joints related to shorter leg. In the future, the muscles and joints of the longer leg should be considered and various age group should be studied to develop the understanding of the effect of manual therapy on LLD.
