A Novel Homozygous CLCNKB Mutation of Classic Bartter Syndrome Presenting with Renal Cysts in 6-year-Old Identical Twin Boys : A Case Report

  • Bartter syndrome is an autosomal recessive hypokalemic salt-losing tubulopathy, and classic Bartter syndrome is associated with mutations in the CLCNKB gene. While chronic hypokalemia is known to induce renal cyst formation in different renal diseases, renal cyst formation in Bartter syndrome is rarely reported. Russian six-year-old identical male twins were referred to our hospital for the evaluation of renal cysts, which were incidentally detected on abdominal sonography due to diarrhea. Both twins had shown symptoms of polydipsia, polyuria, and nocturia since they were one year olds. Vital signs including blood pressure were normal in both twins. Renal sonography revealed nephromegaly, increased echogenicity of renal cortex, and various sized multiple cysts in both kidneys for both twins. Laboratory findings included hyponatremia, hypokalemia, hypochloremia, and metabolic alkalosis. Bartter syndrome with renal cysts were suspected. Genetic analysis for both twins confirmed a homozygous c.1614delC deletion on exon 15 of the CLCNKB gene, which was confirmed as a previously unreported variant to the best of our knowledge. They were managed with potassium chloride, nonsteroidal anti-inflammatory drugs, and angiotensin-converting-enzyme inhibitors. Metabolic alkalosis, hypokalemia, hypochloremia, and polyuria partially improved during the short course of treatment. This is the first report of a homozygous mutation in the CLCNKB gene in an identical twin, presenting with renal cysts.

한국의 중증외상 사망률의 지역적 차이에 대한 비교 연구 (Comparative study of geographic differences of severe trauma mortality in Korea)

  • Purpose: Mortality due to trauma is relevant to both low-income and high-income countries. A diversity of causes leads to mortality such as, socioeconomic status and geographic factors. This study sought to differentiate between cases of mortality in a metropolitan city and a rural area, with data from critical trauma patients. Methods: Community-based severe trauma surveillance data from 2018 was used in this study. Logistic regression was conducted to compare the odds ratios between deaths that occurred in a metropolitan city and a rural area. Multiple logistic regression by controlling variables such as type of medical institution and injury severity score was conducted to estimate the effect on the trauma patients. Results: In total, 28,217 participants were selected as total population. We observed that the odds of death decreased as the level of the trauma center increased. Compared to the metropolitan city, the odds ratio of rural areas was 1.44. The odds ratio increased as the injury severity score increased. Conclusion: This study suggests that the mortality of critical trauma patients is higher in rural areas than in metropolitan cities. More studies are needed to expand on this.

중년 여성의 하부요로증상에 영향을 미치는 요인 (Factors Affecting on Lower Urinary Tract Symptoms in Middle-aged Women)

  • Purpose: This study was performed to investigate demographic and disease-related characteristics that influence lower urinary tract symptoms in middle-aged women. Methods: The participants in this study were 301 middle-aged women and they completed structured questionnaire between May to June, 2008. Lower urinary tract symptoms (LUTS) were measured with Bristol Female Lower Urinary Tract Symptoms-short form (BFLUTS-SF) and categorized as voiding, filling and incontinence symptoms. The data were analyzed by using descriptive statistics, t-test, ANOVA, Pearson Correlation Coefficient, and multiple regression with SPSS PC 15.0 version. Results: The participants who have higher economic status and drink alcohol were more likely to have LUTS score than other woman who have lower economic status and don't drink alcohol at all. Also, frequent childbirth experiences and chronic diseases conditions such as diabetes and heart disease are positively associated with LUTS score. The symptoms of filling and incontinence were affected by number of childbirth, alcohol drinking habit and chronic disease conditions while voiding symptom was influenced only by alcohol drinking habit. In multiple regression analysis, LUTS were significantly predicted by parity, drinking carbonated beverage and alcohol. Conclusion: For proper nursing care related to lower urinary tract symptoms, nursing intervention should focus on improving alcohol drinking habit and carbonated beverage comsumption.

보건의료계열 대학생의 코로나바이러스감염증-19 지식, 불안, 사회심리적 건강이 감염예방행위에 미치는 영향 (The effect of coronavirus disease-19 knowledge, anxiety, and psychosocial well-being on the infection prevention behavior in college students in health care)

  • Purpose: The purpose of this study was to investigate the effect of coronavirus disease-19 (COVID-19) knowledge, anxiety, and psychosocial well-being on the infection prevention behavior of college students in health care. Methods: This study is a descriptive survey research. A total of 301 college students at four health care departments in three regions were surveyed using a structured questionnaire from February 10 to February 16, 2022. The collected data were analyzed using descriptive statistics, independent t-test, and one-way ANOVA. Pearson correlation coefficients and multiple regression analyses were done using SPSS ver. 18.0. Results: Knowledge on COVID-19 was significantly dependent on a family history of chronic disease (p=.049). Anxiety was significantly dependent on test of COVID-19 (p=.040). Gender (p=.049), perceived physical and mental health status (p=.000), and chronic disease (p=.000) had significant effects on infection prevention behavior. When the level of anxiety was higher, the infection prevention behavior was also higher (p=.000) and the psychosocial well-being was improved (p=.017). Stepwise multiple regression analysis revealed that anxiety significantly improved the infection prevention behavior (𝛽=.396, p<.001) and psychosocial well-being (𝛽=-.139, p=.008), which accounts for 18.5% (F=22.444, p<.001) of the variance. Conclusion: In order to prevent infectious diseases such as COVID-19 and its spread, measures to prevent infection and improve the psychosocial well-being should also be sought.

Do age, gender, and subjective health-related factors influence health-related life satisfaction in people with disabilities who are physically active?: a secondary analysis

  • Purpose: This study explored the factors influencing the health-related life satisfaction of people with disabilities who engaged in physical activity, by age and gender. Methods: A secondary analysis was conducted of the 2020 Third Disability and Life Dynamics Panel (2021). The participants were 2,796 people who performed regular physical activity at least once a week. The variables selected were disability-related factors (degree of disability, multiple disabilities, and type of disability), sociodemographic factors (age, gender, living alone, and mean monthly family income), and health-related factors (amount of physical activity, self-esteem, depression, chronic disease, subjective health, and health-related life satisfaction). Descriptive statistics, the chi-square test, the t-test, two-way analysis of variance, and multiple regression analysis were conducted. Results: In total, 58.0% of participants were male, and 42.0% were female. For age groups, 14.4% were children/adolescents (0-19 years), 42.6% were adults (20-59 years), and 43.0% were seniors (≥60 years). The mean score for health-related life satisfaction was 5.0±2.15 out of 10. Adults and seniors whose level of physical activity met or exceeded recommendations had higher subjective health. Moreover, men had better subjective health than women in seniors. Health-related life satisfaction was higher among those who had higher self-esteem, were not depressed, did not have chronic diseases, and had better subjective health. Conclusion: Gender significantly influenced health-related life satisfaction in children/ adolescents and seniors. Disability-related factors were significant in adults, and health-related factors were significant in all age groups. Therefore, these factors should be considered when designing interventions to promote subjective health and health-related life satisfaction of people with disabilities.

  • A wide range of studies have steadily pointed out the relation of oxidative stress to the primary and secondary causes of human disease and aging. As such, there have been multiple misconceptions about oxidative stress. Most of reactive oxygen species (ROS) generated from chronic diseases cause oxidative damage to cell membrane lipids and proteins. ROS production is increased by abnormal stimulation inside and outside in the body, and even though ROS are generated in cells in response to abnormal metabolic processes such as disease, it does not mean that they directly contribute to the pathogenesis of a disease. Therefore, the focus of treatment should not be on ROS production itself but on the prevention and treatment of diseases linked to ROS production, including types 1 and 2 diabetes, cancer, heart disease, schizophrenia, Parkinson's disease, and Alzheimer's disease. In this regard, Korean Red Ginseng (KRG) has been traditionally utilized to help prevent and treat diseases such as diabetes, cancer, inflammation, nervous system diseases, cardiovascular disease, and hyperlipidemia. Therefore, this review was intended to summarize in vivo animal and human clinical studies on the antioxidant activities of KRG and its components, ginsenosides.

고령자의 감성 만족과 정신건강을 위한 스마트폰 응용 기술 (Application Technologies of Smart Phone for Emotional Satisfaction and Mental Health-care of the Elderly)

  • Considerations of how to facilitate aging-in-place(AIP) are becoming increasingly pertinent as care-givers are overwhelmed by an aging population. Aging friendly home-care services combined with IT can satisfy the needs of the elderly suffer from chronic diseases such as depression and dementia. Therefore, we propose future smart phone services and application technologies which can estimate emotional states of the aged and respond to the desire to be happy with mental health, connectedness and consolation from peoples. Firstly, we introduce depression measurement techniques to estimate the severity of depression using multiple sensors. At second, the emotional responding services are categorized to four parts and the details are described. Lastly, we propose the process to implement emotional communication and the application techniques(services) to fulfill the emotional satisfaction and mental healthcare for AIP using smart phone as a mediator.

Autosomal Dominant Polycystic Kidney Desease Coexisting with Renal Dysplasia. First Case Described and Followed Since Prenatal Period

  • Autosomal dominant polycystic kidney disease (ADPKD) is the most frequent hereditary renal disease and causes terminal chronic renal failure. ADPKD is characterized by bilateral multiple renal cysts, which are produced by mutations of the PKD1 and PKD2 genes. PKD1 is located on chromosome 16 and encodes a protein that is involved in cell cycle regulation and intracellular calcium transport in epithelial cells and is responsible for 85% of ADPKD cases. Although nine cases of unilateral ADPKD with contralateral kidney agenesis have been reported, there have been no reports of early childhood ADPKD. Here, we report the only case of unilateral ADPKD with contralateral kidney dysplasia in the world in a four year-old girl who was intrauterinely diagnosed since she was 20 weeks old and followed for four years until present.

농촌 지역 노인의 기능적 의료정보 이해능력 실태와 영향요인 (Influencing Factors on Functional Health Literacy among the Rural Elderly)

  • Purpose: The purpose of this study was to describe the status of the functional health literacy of the elderly living in a rural community and to identify the influencing factors on the functional health literacy. Methods: Subjects(n=238) aged over 60 years were selected for data collection. Functional health literacy was measured by Korean Health Literacy, which consisted of 15 items including eight numeracy and seven comprehension items. Descriptive statistics, t-test, ANOVA with Scheffe test, and stepwise multiple regression were used to analyze the data. Results: The mean score of total functional health literacy was $6.66{\pm}3.31$, numeracy was $3.52{\pm}1.76$, and comprehension was $3.41{\pm}2.10$. Of the subjects, 39.1% showed a low score below the median split. In the results of stepwise multiple regression, the factors affecting functional health literacy were basic literacy, age, number of chronic diseases, self-reported health status, and gender. These factors accounted for 33.0% of functional health literacy. Conclusion: These results suggest that interventions for improving functional health literacy are necessary to enhance elders' ability of self-care management. In addition, it is needed to take into account the individual characteristics of the subjects depending on their gender, age, and health status.

홧병환자에서 DITI의 진단활용

  • Objectives : This study was performed to apply thermography as an method in diagnosis of hwabyung patients. We studied 11 Hwabyung patients who visited to chronic diseases center and circulatory oriental internal medicine of Kangnam oriental medicine hospital and 11 patients control group. Methods : Diagnosis of Hwabyung was based on the dignostic criteria of Hwabyung. The temperature was measured on Chonjung(CV17) Shimsu(B15), Kansu(B18), Kyonjong(G21) in each group. The ${\Delta}T$ was measured between Chonjung(CV17) and Chungjong(CV16), left and right Chungjong(CV16), Shimsu(B15), Kansu(B18), Kyonjong(G21) in each group. We compared the ${\Delta}T$ and DITI types between patients and control group. Results : The ${\Delta}T$ between left and right Chungjong(CV16), Shimsu(B15), Kansu(B18), Kyonjong(G21) were not statistically significant. But the ${\Delta}T$ between Chonjung(CV17) and Chungjong(CV16) was statistically significant(P<0.05) in each group. In control group, DITI type was straight 36%, diamond 27%, multiple small spot 18%, others 18%. In Hwabyung patients group, DITI type was inverse triangle 64%, multiple small spot 9.1%, round 9.1%. Conclusions : The ${\Delta}T$ between Chonjung(CV17) and Chungjong(CV16) and DITI type is considered useful diagnostic methods on Hwabyung patients.
