  • Tarsal tunnel syndrome caused by space occupying lesion is unusual but it rarely occur by ganglion which is presented below the flexor retinaculum due to compression neuropathy of posterior tibial nerve and its branches. The object of the current study is to report our experience of surgical treatment about tarsal tunnel syndrome caused by the ganglion of flexor digitorum longus tendon sheath and multifocal myxoid degeneration of medial plantar nerve with a review of the literatures.

Electrophysiological and radiological evidence for the multifocal nature of a case of multifocal acquired demyelinating sensory and motor neuropathy

  • Multifocal acquired demyelinating sensory and motor (MADSAM) neuropathy is a variant of chronic acquired demyelinating polyneuropathy. A 65-year-old women presented with upper arm weakness. A nerve conduction study showed conduction blocks over intermediate segments with sparing of distal compound action potentials. Magnetic resonance imaging revealed asymmetric hypertrophy of the brachial plexus on the affected side. These findings represent important electrophysiological and radiological evidence of MADSAM neuropathy. The condition of the patient began to improve after starting intravenous immunoglobulin administration.

  • We report a 29-year old female who developed bilateral femoral neuropathy combined with multiple abscesses in both thigh muscles. She was present with weakness in both lower extremities for 15 days and intermittent chilling sense for 3 months. Nerve conduction study showed complete absence of compound muscle action potentials in bilateral femoral nerves when stimulated at inguinal area. Electromyographic examination revealed no motor unit action potentials in both rectus femoris and vastus medialis muscles. CT revealed multifocal abscesses in bilateral thigh muscles. After antibiotic treatment, the patient's neurologic symptoms were improved.

  • A 44-year-old man with a past medical history of splenectomy came to hospital because of epigastric pain. Abdominopelvic computed tomography(CT) showed a soft tissue mass and multifocal variable-sized nodules as well as findings suggestive of cholecystitis. Subsequently, he underwent Tc-99m-labeled heat-denatured red blood cell(RBC) scintigraphy to evaluate the mass and nodules. The scintigraphy confirmed multifocal peritoneal splenosis in the abdominopelvic cavity.

Double-Pass System (Optical Quality Analysis System) for Analysis of the Multifocal Function of a Diffractive Multifocal Intraocular Lens (Acrysof ReSTOR®) Compared to a Monofocal Intraocular Lens (Acrysof IQ®)

  • In this study, we objectively determined whether the ReSTOR as a multifocal IOL (intraocular lens) has a multifocal function compared to the IQ as a monofocal IOL in vivo by OQAS (Optical Quality Analysis System). Eighteen patients who had cataract surgery with implantation of ReSTOR (27 eyes) and 15 patients with IQ (21 eyes), were included inthis study. Uncorrected distance visual acuity (UCDVA), corrected distance visual acuity (CDVA), uncorrected near visual acuity (UCNVA)and distance corrected near visual acuity (DCNVA) were measured. After setting the artificial pupil size to 3 mm, we performed 'Optical quality'. We inputted defocus diopters of (objective spherical refraction)(far), (objective spherical refraction-1.25 D)(intermediate),(objective spherical refraction-2.5 D)(near), and (objective spherical refraction-3.5 D)(very near) into 'selected spherical refraction' simulating the optical quality at far, intermediate, and near distance. We changed the pupil size to 5 mm and repeated the same measurements. The UCDVA and CDVA did not show significant differences between the 2 groups. But, the UCNVA and DCNVA of the ReSTOR group were better than those of the IQ group (p=0.000, p=0.000). For 3 mm pupil, at far distance, modulation transfer function (MTF) cut off and point spread function (PSF) width at 50% of ReSTOR were worse than those of IQ (p=0.039, p=0.020). At intermediate distance, MTF cut off, Strehl ratio and PSF width at 50% of ReSTOR were worse than those of IQ (p=0.001, p=0.001, p=0.000). At near distance, MTF cut off of ReSTOR was worse than that of IQ (p=0.033). At very near distance, MTF cut off and Strehl ratio of ReSTOR were worse than those of IQ (p=0.002, p=0.002), but PSF width at 50% of ReSTOR was better than that of IQ. For 5 mm pupil, most parameters at each distance, there was no significant difference between the 2 groups. Only PSF width at 50% of ReSTOR were worse than those of IQ at intermediate distance (p=0.013). It was impossible to show the multifocal function of ReSTOR compared to the IQ byOQAS.

Endogenous lipid pneumonia in a ringed seal (Pusa hispida subsp. ochotensis)

  • An adult female ringed seal died suddenly and was subsequently examined for diagnostic purposes. The animal's lungs demonstrated mild non-collapsibility and multifocal white to yellow patches. Histopathological examination revealed multifocal pulmonary histiocytosis. Alveoli were filled with numerous foamy macrophages cytoplasm and scattered multinucleated giant cells containing cholesterol clefts. The foamy cytoplasm of the macrophages stained with oil red O stain. Further, lipid droplets within the cytoplasm were detected by electron microscopy. To the author's knowledge, this is the first case report describing the histochemical staining and electron microscopic findings associated with endogenous lipid pneumonia in ringed seal.

Multifocal interstitial nephritis of pigs slaughtered in Jeju (제주지역 도축돈의 간질성 신염)

  • Total 160 head of porcine kidneys were examined for gross and histopathological lesions and polymerase chain reaction (PCR) for porcine circovirus type 2 (PCV-2), porcine parvovirus (PPV), Leptospira species and porcine reproductive and respiratory syndrome virus (PRRSV). Grossly, 137 kidneys (85.6%) had lesions characterized by the presence of the scattered white foci. Microscopically, multifocal interstitial nephritis, which classified into 4 grades such as, no lesion (Score 0), mild lesion (Score 1), moderate lesion (Score 2) and severe chronic lesion (Score 3) with fibrosis, was observed in 159 cases (99.4%). The histopathologic mean score for multifocal interstitial nephritis was significantly different (P<0.05) between the cases of PCV-2 single infection and the cases of co-infection with PCV-2 and PPV. According to PCR evaluation, PCV-2 were detected in 73.8% (118 cases), PPV were in 66.9% (107 cases), however Leptospira spp. and PRRSV were negative in all kidneys. Both PCV-2 and PPV were detected in 52.5% (84 cases). In 84 cases co-infected with PCV-2 and PPV, the occurrence of lymphoid follicle and vasculitis were observed as 65.5% (55 cases) and 26.2% (22 cases), respectively. These results revealed that PCV-2 and PPV were major infectious agents for interstitial nephritis in slaughtered pigs, Jeju. And the histopathologic lesions of multifocal interstitial nephritis were more severe in the case co-infected with PCV-2 and PPV.

Clinical Study on Tendency of Addition Power with Age (연령별 노안 가입도 경향에 관한 임상적 연구)

  • The purposes of this study is examining a wearing tendency of a progressive multifocal lens. We measured refractive error values using subjective and objective refraction method with subjects in 40-65 ages. Also, we investigated and analyzed distribution of addition power and wearing tendency of progressive multifocal lens. Detected addition powers were ranged of +0.75D - +3.00D. In case of average addition, they were detected with +1.18D in 40's, +1.55D - +1.97D in 50's and +2.38D in above 61's age. The distributions of visual ability at a distant were measured that the hyperopia and hyperopic astigmatism 43%, the myopia and myopic astigmatism characteristic astigmatism 7% appeared highly with 36%. The male who uses the interior aspect multi focal lens with the short-range work were appeared the male 20%, the woman 19%.

Neural Reorganization in Retinal Outer Plexiform Layer Induced by Eccentric Viewing Training (중심외주시 훈련 후 망막 외망상층에서의 신경 재조직화)

  • Purpose: This study was to investigate the properties and the location of neural reorganization following eccentric viewing training. Methods: 14 subjects with normal vision took part in eccentric viewing training. The measurements of the light sensitivity and multifocal electroretinogram were performed before and after the training. Results: The measurements of the light sensitivity and multifocal electroretinogram for pre-eccentric viewing training and post-eccentric viewing training showed the significant difference (p<0.047 and p<0.028, respectively). Conclusions: The retinal outer plexiform layer is unable to regenerate. However, the neural reorganization in the retinal outer plexiform layer is able to take place following eccentric viewing training.

Multifocal nodular lymphoid hyperplasia of the lung

  • Nodular lymphoid hyperplasia (NLH) is a benign lymphoproliferative disease that can affect the lung. Because of its rarity, little is known about the etiology and natural history of NLH. Most cases are usually asymptomatic and found incidentally on imaging studies. Imaging finding of NLH has shown most commonly as a solitary lesion, although multifocal pulmonary nodules may be seen. Surgical resection has proved curative in the cases previously described. We report a rare case of NLH in a 55 year-old man who presented with bilateral multiple pulmonary nodules on chest radiography. Open biopsy was performed from the upper and lower lobe of the left lung. The lesions were pathologically diagnosed as pulmonary NLH. Multifocal residual nodules in both lungs remain stable without spontaneous regression during the 3 years of follow-up.
