하천에서의 Oxytetracycline 내성주에 관한 연구 (Study on Oxytetracycline Resistant Bacteria in the Surface Water Environment)

  • Objectives: This study aims to understand the concentration, diversity, and antibiotic characteristics of oxytetracycline resistant bacteria present in a surface water environment. Methods: Water sampling was performed in Cheongmi Stream in Gyeonggi-do, Korea in February and August 2014. Water samples collected from two sites were plated in triplicate on tryptic soy agar plates with 30 mg/L of oxytetracycline. Oxytetracycline resistant bacteria were selected from surface water in Cheongmi Stream and were subjected to 16S rDNA analysis for oxytetracycline resistant species determination. Identified resistant strains were tested for resistance to various antibiotics. Results: Results from this study indicate that the dominant resistant organisms in this aquatic environment are from family Acinetobacter and family Aeromonas. As to culturable heterotrophic bacteria, Oxytetracycline resistant bacteria were present 0.45-0.93% during winter and 0.08-0.38% during summer. Most oxytetracycline resistant bacteria exhibited resistance to more than ten of the antibiotics studied. The diversity of oxytetracycline resistant bacteria in winter was higher than in summer. Conclusion: Most of these resistant bacteria are Gram negative and are closely related to pathogenic species. These results suggest that increasing multi-antibiotic resistant bacteria in the surface water environment has a close relation to the reckless use of antibiotics in livestock.

내성결핵의 보험의학적 위험분석 (Insurance risk analysis of drug-resistant tuberculosis)

  • Background: Recent emergence of drug-resistant tuberculosis such as multidrug-resistant tuberculosis(MDR-TB) or extensively drug-resistant tuberculosis(XDR-TB) has become important health care problems. It has also became grave issues for insurance industries in determining medical risks. We have therefore strived to analyze the comparative mortality rates for drug-resistant tuberculosis through utilization of results from previous articles. Methods: Comparative mortality was calculated from source articles using mortality analysis methods. Results: Mortality ratio of MDR-TB was estimate to 1200%, and excess death rate was 110 per 1,000. Comparative mortality between MDR-TB and XDR-TB by Korean $study^{(1)}$ were 1750, 382, 405, 443, 1025, and 357%, for each 10 months study intervals, respectively. Total mortality ratio was 594% and total excess death rate was 60 per 1,000person. It was determined that the risk of XDR-TB was much greater than MDR-TB. Discussion; Pending the development of a novel anti-tuberculosis drug, it would be prudent to steer clear insuring XDR-TB during underwriting phase due to high medical cost that it creates.

국내 다제내성 및 광범위내성결핵의 최근 현황 (The Recent Status of Multidrug- and Extensively Drug-Resistant Tuberculosis in Korea)

  • Background: The increasing incidence of multidrug-resistant tuberculosis (MDR-TB) and extensively drug-resistant tuberculosis (XDR-TB) has become a serious worldwide problem. However, there is insufficient data regarding the current status of MDR-TB and XDR-TB in Korea. This study examined the recent status of MDR- and XDR-TB using the data from 7 laboratories, in which almost all drug susceptibility tests (DST) for Mycobacterium tuberculosis were performed. Methods: The patients' identification data and DST results were collected from all 7 laboratories from 2001 to 2006 and the number of patients with MDR-TB and XDR-TB were calculated. Results: The number of DSTs was 140,638 for 6 years with an increasing incidence each year (p<0.001). The number of DST with MDR results was 18,510 and personal identifying information was obtained in 16,640 (89.9%) tests. The number of MDR-TB patients from 2001 to 2006 was 2,329, 2,496, 2,374, 2,300, 2,354, and 2,178, respectively, when counting the duplications in a year as one patient. The number of MDR-TB patients when counting the duplications in 6 years as one patient was 2,281, 1,977, 1,620, 1,446, 1,512, and 1,373, respectively. When the same method was adopted, the number of XDR-TB patients was 191, 238, 282, 260, 272, and 264, respectively, and 189, 150, 130, 90, 122, and 110 patients, respectively. Conclusion: Despite the national efforts to control TB, there are still a large number of MDR- and XDR-TB patients in Korea.

Establishment of Paclitaxel-resistant Breast Cancer Cell Line and Nude Mice Models, and Underlying Multidrug Resistance Mechanisms in Vitro and in Vivo

  • Background: Breast cancer is a common malignant tumor which affects health of women and multidrug resistance (MDR) is one of the main factors leading to failure of chemotherapy. This study was conducted to establish paclitaxel-resistant breast cancer cell line and nude mice models to explore underlying mechanisms of MDR. Methods: The breast cancer drug-sensitive cell line MCF-7 (MCF-7/S) was exposed in stepwise escalating paclitaxel (TAX) to induce a resistant cell line MCF-7/TAX. Cell sensitivity to drugs and growth curves were measured by MTT assay. Changes of cell morphology and ultrastructure were examined by optical and electron microscopy. The cell cycle distribution was determined by flow cytometry. Furthermore, expression of proteins related to breast cancer occurrence and MDR was tested by immunocytochemistry. In Vivo, nude mice were injected with MCF-7/S and MCF-7/TAX cells and weights and tumor sizes were observed after paclitaxel treatment. In addition, proteins involved breast cancer and MDR were detected by immunohistochemistry. Results: Compared to MCF-7/S, MCF-7/TAX cells had a higher resistance to paclitaxel, cross-resistance and prolonged doubling time. Moreover, MCF-7/TAX showed obvious alterations of ultrastructure. Estrogen receptor (ER) expression was low in drug resistant cells and tumors while expression of human epidermal growth factor receptor 2 (HER2) and Ki-67 was up-regulated. P-glycoprotein (P-gp), lung resistance-related protein (LRP) and glutathione-S-transferase-${\pi}$ (GST-${\pi}$) involved in the MDR phenotype of resistant cells and tumors were all overexpressed. Conclusion: The underlying MDR mechanism of breast cancer may involve increased expression of P-gp, LRP and GST-${\pi}$.

대전지역 소재 대학병원에 blaOXA-23 유전자를 가지고 있는 다제내성 Acinetobacter baumannii의 확산 (Clonal Dissemination of Multidrug Resistant Acinetobacter baumannii Isolates Harboring blaOXA-23 at One University Hospital in Daejeon, Korea)

  • Acinetobacter species는 중요한 기회감염균으로 빈번하게 원내감염을 일으킨다. 뿐만 아니라 다제내성을 보이는 경우가 많아 치료를 위한 항균제 선택이 매우 제한적이다. 본 연구에서는 Acinetobacter species 68균주를 대상으로 하여 다양한 carbapenemase 유전자를 조사했다. 디스크확산법으로 항균제 감수성 양상을 조사했으며 다중 중합효소연쇄반응을 통해 carbapenemase 유전자를 포함하고 있는 균주를 선별하였고 PCR과 염기서열분석을 통해 최종적으로 carbapenemase 유전형을 확인했다. 한편 REP-PCR 방법으로 균주간의 clonality를 분석했다. 본 연구에서 A. baumannii 균주는 분석된 모든 항균제에 대해 높은 내성을 나타냈으나 non-A. baumannii 균주는 분석된 항균제 중 aztreonam과 cefotaxime을 제외한 항균제에 모두 감수성을 보였다. A. baumannii 51 균주는 $bla_{OXA-51}$ 유전자를 가지고 있었으며 그 중 37(72.5%)균주는 $bla_{OXA-23}$ 유전자도 동시에 가지고 있었다. 본 연구에서 39 균주의 다제내성 A. baumannii 가 분리되었는데 그 중 37균주가 $bla_{OXA-23}$ 유전자를 가지고 있었다. $bla_{OXA-23}$ 유전자를 포함하고 있는 균주들은 I (n=22) 형 또는 II (n=15) 형의 REP-PCR band 패턴을 보였는데 이는 대전에 위치한 일개의 대학병원에 $bla_{OXA-23}$ 유전자를 포함하고 있는 다제내성 균주들이 수평 확산 되어 있음을 의미한다. 다제내성 A. baumannii 균주에 의한 감염 및 집락화를 막기 위해서는 지속적으로 내성을 유발하는 인자를 조사하고 MDR균주의 출현 및 확산을 감시할 필요가 있을 것으로 사료된다.

  • 최근 P. aeruginosa의 carbapenem에 대한 내성은 전 세계적으로 증가하고 있는 실정이다. 특히 $metallo-{\beta}-lactamases$ (MBLs)는 carbapenem의 고도 내성에 관여하고 있는 것으로 보고되고 있다. 한편, Sequence type 235 (ST235)는 다제내성 클론으로써 국제적으로 보고되고 있으며, IMP-6와 VIM-2 유전자의 확산에도 관여하는 것으로 알려져 있다. 본 연구에서는 2008년 3월부터 2014년 6월까지, 대전지역의 3차 병원에서 분리된 carbapenem 내성 P. aeruginosa에서 MBL 유전자를 분석하고 이에 대한 역학관계를 조사하고자 하였다. 항균제 감수성 양상은 디스크 확산법으로 확인하였고, MBL 유전자의 분석을 위해 PCR과 염기서열분석을 수행하였다. 더불어, 역학 관계를 조사하기 위해 multilocus sequence typing (MLST)를 실시하였다. 110 균주의 carbapenem 내성 P. aeruginosa 중, 32균주(29.1%)가 MBL를 생성하였고, IMP-6 (29균주, 90.6%)가 주요하게 확인되었다. VIM-2는 3균주(9.4%)에서 확인되었으며, 모두 ST357로 확인되었다. IMP-6를 생성하는 P. aeruginosa는 모두 다제내성을 보였고, ST235로 확인되었다. ST235 (55균주, 50.0%)는 가장 높은 비율로 확인된 클론이며 7년 동안 지속적으로 확인되었다. 이러한 다제내성 ST235의 확산을 방지하기 위해, carbapenem의 과도한 사용을 제한하고, 지속적으로 모니터링하는 전략이 개발되어야 할 것으로 사료된다.

일개 부산지역 3차 병원에서 관찰한 다제내성 결핵의 실태, 2005~2009 (The Current Status of Multidrug-Resistant Tuberculosis in One Tertiary Hospital in Busan, 2005~2009)

  • Background: Although the prevalence of pulmonary tuberculosis has progressively decreased all over the world, drug-resistant tuberculosis is major obstacle in treating tuberculosis. This study was performed to examine the current prevalence and risk factors of drug resistant tuberculosis in a single tertiary hospital in Busan, Korea. Methods: We enrolled 367 patients with active pulmonary tuberculosis on a retrospective basis who had undergone mycobacterium culture and drug sensitivity tests between January 2005 and December 2009. We analyzed all clinical and radiographic parameters to find predictors related to drug resistant tuberculosis. Results: At least one incident of drug resistance was found in 75 (20.4%) patients. Isoniazid (18.8%) was the most frequent resistant drug, followed by rifampin (10.9%), ethambutol (7.1%), streptomycin (4.9%), and fluoroquinolone (2.7%). Resistance to second-line drugs was found in 37 (10.1%) patients. Multidrug resistance and extensively drug resistance was evident in 39 (10.6%) and 4 (1.1%) patients, respectively. Using multiple logistic regression analysis, history of previous treatment including relapse (odd ratio [OR], 11.3; 95% confidence interval [CI], 4.92~26.08; p<0.01), treatment failure (OR, 24.1; 95% CI, 5.65~102.79; p<0.01) and an age of below 46 years-old (OR, 3.8; 95% CI, 1.62~8.65; p<0.01) were found to be independent predictors of multidrug resistant tuberculosis. Conclusion: We found that the prevalence of drug resistant tuberculosis was considerably high. A careful consideration for possible drug resistant tuberculosis is warranted in patients with a history of previous treatment or for younger patients.

금속표면이 항생제 내성균주의 생육억제에 미치는 영향 (Inhibitory Effect of Metal Surface on the Antimicrobial Resistance Microorganism)

  • 배경: 본 연구는 국내 병원에서 분리된 methicillin 내성 Staphylococcus aureus (MRSA), vancomycin 내성 Enterococcus faecium (VREFM) 및 다제 내성 multidrug-resistant Pseudomonas aeruginosa (MRPA) 등에 대한 구리, 유기(구리 78%, 주석 22%), stainless steel의 살균력을 비교 실험하여, 국내 병원 환경에 의한 항생제 내성 균주 교차 감염을 예방하는 데 구리와 유기의 활용성을 분석하고자 하였다. 방법: MRSA, VREFM, MRPA 균주는 2017년 의정부성모병원에서 분리 동정된 wild type 3균주씩을 혼합하여 사용하였으며 모두 항생제 다제 내성 균주로 나타났다. MRSA, VREFM, MRPA 균주 각각을 Tryptic soy broth (Oxoid, England)에 접종하여 $35^{\circ}C$에서 24시간 배양한 후, 각각 초기 접종균액의 균수를 $10^5\;log\;CFU/mL$로 조정하였다. 구리, 유기, stainless steel 용기에 준비된 MRSA, VREFM, MRPA 접종균액 100 mL씩을 각각 접종한 후, 습도가 유지되도록 덮개를 덮은 후 $35^{\circ}C$ incubator에서 초기부터 9일까지 생존균수를 측정하였다. 결과: 본 연구결과 국내에서 분리된 항생제 다제 내성 MRSA, VREFM, MRPA 균주에 대한 구리 및 유기의 살균효과를 확인하였다. Stainless steel의 살균력은 구리 및 유기에 비해 매우 미약하였고, MRSA 및 VREFM에 비해 MRPA의 살균효과가 크게 나타났다. 결론: 교차감염을 예방하기 위하여 병원 내 문손잡이, 수도꼭지, 침대레일 등에 구리나 유기를 적용하여, 장기간 실험을 진행하고 있는 국외 사례와 같이, 국내 병원에서도 구리와 유기를 이용한 병원 내 교차 감염 예방 연구가 필요하다고 생각한다.

임상검체에서 분리된 Methicillin Resistant Staphylococcus aureus의 다제내서양상과 내성 관련 유전자의 검색 (Detection of multidrug resistant patterns and associated-genes of Methicillin Rdsistant Staphylococcus aureus(MRSA) isolated from clinical specimens)

  • Methicillin Resistant Staphylococcus aureus (MRSA) was obtained from the clinical specimens at Pusan national university Hospital, Pusan, Korea. The sensitivities against various antibiotics were examined by using disc diffusion test and associated genes such as mecA, mecR1, mecI and femA were detected by polymerase chain reaction. Among Seventy-nine strains of MRSA, 38 strains(48.1%)were sensitive to streptomycin and 32 strains(40.5%) to cefoperazone, while one strain(1.3%) were resistant to vancomycin. In considering the result of this study, 7 strains showed resistance to 9 kinds of different antibiotics, 12 strains were to 8 kinds, 24 strains were to 7,25 strains were to 6, 9 strains were to 5, and 2 strains were to 4 antibiotics. Among 79 strains of MRSA, 67 strains were coagulase positive and 12 were coagulase negative. In the detection of MRSA associated genes by PCR method, mecA, mecR1, mecI, and femA genes were detected in 30 strains(44.8%), 28 strains(41.8%), 23 strains(34.3%) and 15 strains(22.4%), respectively. MecA type that is without femA were found in 21 strains(31.3%), femA type that is without regulator genes were shown in 4 strains(6.0%), while mecA-mecR1-mecI type with regulator genes were shown more to be 17 strains(25.4%). There was little statistical significance between multidrug resistance and MRSA associated genes. Considering these result, it is necessary to include moecular biological studies of related genes to the study drug resistance.
