Usefulness of the BACTEC MGIT 960 System for Mycobacterial Culture and TB Ag MPT64 Immunochromatographic Assay to Identify Mycobacterium tuberculosis (항산균 배양에서 BACTEC MGIT 960 System의 유용성 및 MGIT 양성배지에서 결핵균 진단을 위한 TB Ag MPT64 면역발색법의 유용성)

  • Background: This study was conducted to evaluate the usefulness of the BACTEC MGIT (Mycobacterium Growth Indicator Tube) 960 system for mycobacteria culture and immunochromatographic assay to identify Mycobacterium tuberculosis (MTB) in positive MGIT culture. Methods: Mycobacteria-culture-positive cases were retrospectively analyzed from December 2010 to July 2011. The detection rates and the recovery times of the mycobacteria between the Ogawa media and the MGIT were compared. An immunochromatographic assay (ICA) (SD BIO-LINE) was also performed in the positive MGIT culture for identification, and the results were compared with those of the Ogawa media in the Korea National Tuberculosis Association. Results: Among the 261 patients (M:F, 168:93; mean age, $61.6{\pm}17.16$ yrs), 450 specimens (sputa, 365; bronchial washing, 61; and pleural effusion, 24) were found positive with mycobacteria. Mycobacteria were grown both on the MGIT and Ogawa media in 310 cases (68.9%); only on the MGIT in 115 cases (22.6%); and only on the Ogawa media in 25 cases (5.5%) (p<0.05). The recovery time was $28.2{\pm}8.9$ days in the Ogawa media and $11.1{\pm}5.8$ days in the MGIT (p<0.05). Among the 127 cases from the positive MGIT culture, all 92 cases that were confirmed as MTB cases bythe Korea National Tuberculosis Association were identified as MTB by ICA, with 100% sensitivity. Conclusion: MGIT increases the detection rate and shortens the recovery time of mycobacteria in clinical respiratory specimens, and the TB Ag MPT64 kit using ICA is useful in identifying MTB in a positive MGIT culture.

  • Nontuberculous mycobacteria (NTM) contains M. avium-intracellular complex (MAC), M. fortuitum, M. chelonae, M. abscessus, M. kansasii, etc., which causes infections in swine, poultry, and other animals. These bacteria are opportunistic pathogens that can contaminate people, and in doing so, cause various social economic problems. This study proved that NTM are also present in air conditioners of multiuse facilities in frequently visited populated areas. The results of this study are meaningful because they showed that pathogenic microbes can live in the surroundings and cause diseases to people with impaired immunity. This study used the AFB stain, L-J medium culture, and PCR method for the detection of MTB and NTM in air conditioner dust. MTB was not detected in any of the collected samples, while NTM was detected in 2 out of 40 samples (5%). Most people living in the modern environment cannot avoid being exposed to air conditioners so special attention is needed for sanitary inspections and the management of air conditioners. Moreover, it is important to study NTM through various individuals, accumulate the related data and establish the methods and standards of hygiene management system to reduce NTM infections in public areas. Moreover, it would be necessary to identify the precise species of NTM through DNA sequencing and evaluate the effects of NTM on the pulmonary tuberculosis in immunodeficiency patients.

Study on the Growth Factors for Rapidly Cultivating Mycobacterium spp. (마이코박테리움을 신속하게 배양할 수 있는 성장 인자에 관한 연구)

  • Mycobacteria grow slowly. Therefore, a solid medium should be used for eight weeks and a liquid medium for six weeks. The purpose of this study was to find the growth factors that can grow Mycobacterium rapidly and to help develop a solid medium for rapid identification. Three types of Mycobacterium growth factors were evaluated with 10 Mycobacteria by adding activated charcoal, defibrinated sheep blood, and L-ascorbic acid to $Difco^{TM}$ Mycobacteria 7H11 agar (Becton, Dickinson and Company, Sparks, MD, USA). The time to detection and the distinguishability of a colony were compared with that of the current method. In the rapidly growing Mycobacterium, the difference in detection time between the new media and conventional media confirmed that the new media was faster. M. kansasii and M. intracelluare grew faster in 7H11 C than in 7H11 medium. MTB grew faster than the other media in 7H11 C. This study confirmed that the two growth factors affect fast-growing Mycobacteria and slow-growing Mycobacteria. 7H11 C showed better distinguishability than the conventional media in all 10 Mycobacterium due to the color contrast. In particular, when the MTB was grown, the size of the colonies was larger than with other media, so visualization was easy.

RT-PCR Targeting rpoB mRNA for Drug Susceptibility Test of Mycobacterium tuberculosis in Liquid Culture

  • The problems of tuberculosis and its drug resistance are very severe. Therefore, rapid and accurate drug susceptibility assay is required. Recently, there has been an increased understanding of the genetic mechanism of Mycobacterium tuberculosis (MTB) drug resistance as well as advancement of molecular technologies. While many gene mutations correlate well with drug resistance, many genes do not show a strong correlation with drug resistance. For this reason, the current study assessed the utility of rpoB mRNA as a target to detect live mycobacteria. In this study, RT-PCR targeting of rpoB mRNA in BCG treated with rifampin was performed. Conventional RT-PCR and real-time PCR targeting rpoB mRNA as well as 85B mRNA was performed to determine whether these two methods could distinguish between viable and non-viable MTB. The levels of rpoB and 85B mRNA detected by RT- PCR were compared in parallel with colony forming unit counts of BCG that were treated with rifampin for different periods of time. The data suggests that that even though both mRNA levels of rpoB and 85B decreased gradually when rifampin-treatment increased, the rpoB mRNA seemed to represent live bacteria better than 85B mRNA. This study clearly indicates that RT-PCR is a good method to monitor viable cell counts in the liquid culture treated with the anti-tuberculosis drug.

  • 기업의 소유구조와 기업가치에 미치는 영향에 관하여는 기존의 많은 선행 연구에서 분석되고 논의되어 왔으나, 일반 제조업과는 상이한 업종상 회계상 특성을 가진 건설기업의 경영성과를 소유구조에 따라 구분하고, 그 성과를 일반 제조기업과 비교한 연구는 미진하였다. 이에 본 연구에서는 한국기업의 주요 지분율의 증감과 경영성과와의 동질성여부, 소유구조에 따른 제조기업과 건설기업과의 성과 차이, 건설기업중 소유경영자와 전문경영자간 성과 관계에 대하여 검증하고자 하였다. 본 연구의 연구결과를 요약하면 다음과 같다. 첫째, 외국인 지분율의 증가는 기업의 미래 성장성(MTB)에 긍정적 영향을 미치는 반면 내부 및 외국인의 지분율의 증가는 기업의 효율적 운영과 동질적 상호 작용을 하는 것으로 나타났다. 또한 건설 및 제조기업에 있어서 주요 지분율의 증감은(내부 지분율, 외국인 지분율, 기관투자자 지분율) 기업 경영성과 측정 지표인 MTB, ROA에 대하여 업종간 차이를 보여주지 못하는 것으로 나타났다. 마지막으로 건설기업에 있어서, 기업의 미래 성장 가능성은 전문경영자가 경영하였을 경우가 소유경영자가 경영하였을 때보다 높은 반면 얼마나 기업을 효율적으로 운영하였는가를 측정하는 ROA지표는 상이한 결과를 산출하지 못하였다.

Replicated Association Study between Tuberculosis and CLCN6, DOK7, HLA-DRA in Korean

  • Tuberculosis is a global public health problem and manifests itself as a difference in the genetic susceptibility of the host, along with the properties of Mycobacterium tuberculosis (MTB). The single nucleotide polymorphisms (SNPs) and candidate genes proposed in the Genome-wide association study (GWAS) on tuberculosis in a recently published Chinese population were reported. In this study, we investigated whether the genetic polymorphism of candidate genes related to tuberculosis is reproduced when targeting Koreans. The CLCN6 (rs12404124, rs198391, rs535107), DOK7 (rs1203104, rs1203103) and HLA-DRA (rs1051336) gene polymorphisms showed statistically significant results. In addition, it was also found whether it acts as an expression quantitative trait loci (eQTL) that can influence gene expression. This study confirmed that the genetic polymorphism of the three genes (CLCN6, DOK7, HLA-DRA) affects the development of tuberculosis and will help to understand the genetic specificity of tuberculosis and the interaction between pathogens and hosts.

Mixed Infection of Mycobacterium abscessus subsp. abscessus and Mycobacterium tuberculosis in the Lung

  • A mixed infection of Mycobacterium abscessus subsp. abscessus (Mab) and Mycobacterium tuberculosis (MTB) in the lung is an unusual clinical manifestation and has not yet been reported. A 61-year-old woman had been treated for Mab lung disease and concomitant pneumonia, and was diagnosed with pulmonary tuberculosis (PTB). Despite both anti-PTB and anti-Mab therapy, her entire left lung was destroyed and collapsed. She underwent left pneumonectomy and received medical therapy. We were able to successfully treat her mixed infection by pneumonectomy followed by inhaled amikacin therapy. To the best of our knowledge, thus far, this is the first description of a mixed Mab and MTB lung infection.

Effects of Imeup($GB_{41}$) Supplementation and Tonggok($BL_{66}$) Draining on Changes in Cerebral Hemodynamics in Normal Rats (수소양삼초정격(手少陽三焦正格) 타경보사(他經補瀉)(임읍 보(臨泣 補), 통곡 사(通谷 瀉)) 자침(刺鍼)이 정상 흰쥐의 뇌혈류역학(腦血流力學) 변화(變化)에 미치는 영향(影響))

  • Objectives : The purpose of this study is to investigate the effects of $GB_{41}$ supplementation and $BL_{66}$ draining on changes in cerebral blood flow in normal rats. Methods : Regional cerebral blood flow(rCBF) and mean arterial blood pressure(MABP) in normal rats are observed, and those mechanisms were also investigated with pre-treatment of indomethacin(IDM) and methylene blue(MTB) each. Results : In this study, $GB_{41}$ supplementation and $BL_{66}$ draining elevated rCBF in time-dependent manner. Pre-treatment with indomethacin(IDM), an inhibitor of cyclooxygenase, inhibited increase of rCBF effectively. But pre-treatment with methylene blue(MTB), an inhibitor of guanylate cyclase, didn't affect rCBF levels. In addition, pre-treatment with IDM also decreased MABP levels. Conclusions : In conclusion, these results suggest that $GB_{41}$ supplementation and $BL_{66}$ draining is effective to increase rCBF, and the mechanisms are thought to be related to cyclooxygenase pathways.
