목적 : 본 연구는 음허로 인한 야간발열 환자 2명에 대한 동씨침의 영골 및 대백혈의 치료효과를 알아보고, 이를 경락학적으로 고찰하는데 목적이 있다. 방법 : 음허로 진단된 2명의 환자를 대상으로 영골 및 대백혈에 침 치료를 시행하고, 15분 간 유침하였으며, 치료 전 후에 체온과 맥박을 측정하고, 구갈에 관한 NRS를 치료전에 측정하였다. 결과 : 영골 및 대백혈 자침 1시간 후 체온이 두명의 환자 모두 정상범위로 하강하였으며, Case 1의 경우는 영골 및 대백혈 자침치료를 시작한지 13일후, Case 2의 경우에는 14일 후에 야간발열 증상이 소실되었다. 11 pm에 측정한 맥박수는 Case 1에서 침 치료 시작 전 평균 82.6에서 치료 기간 중에는 75.2, 치료 후 74.8로 변화하였고, Case 2에서는 침 치료 시작전 평균 83.5에서 치료 기간 중에는 78.5, 치료 후 74.3으로 변화하였다. NRS는 Case 1에서 치료 전 평균 4.2에서 치료 기간 중에는 3.2, 치료 후 1.7로 감소하였으나 Case 2에서는 치료 전 평균 3.3에서 치료 기간 중에는 2.3, 치료 후 2.4로 변화하였다. 결론 : 두 가지 Case에서 영골 및 대백혈 자침은 발열증상을 완화하는데 효과적이었다. 이는 영골 및 대백혈이 수양명대장경의 경락 및 경근이 분포하는 영역에 위치하고 있기 때문에 수양명대장경의 열성병 치료효과와 대장주진의 생리학적 관점에서 고찰해 볼 수 있으며, 앞으로 이와 관련한 체계적이고 다양한 연구가 필요하리라 사료된다.
본 연구는 치위생학과 학생들을 대상으로 본인자각구취의 관련요인과 스트레스 및 강박증과의 관계를 파악하여 구취예방을 위한 기초자료를 제공하고자 실시되었다. 연구결과 전체 대상자의 스트레스 평균은 1.76점, 강박증의 평균은 1.62점, 본인자각구취도의 평균은 1.84점으로 나타났다. 혀에 흰 막이 가끔 끼는 군에서 스트레스 1.79점(p=0.003), 강박증 1.64점(p=0.042), 본인자각구취 1.89점(p=0.001), 본인의 구강 내 증상에서는 혀가 하얗고, 입안이 끈적인다고 느끼는 군에서 스트레스 2.02점(p=0.000), 본인자각구취 2.00점(p=0.000)으로 나타났다. 구강건조감에서는 자주 느끼는 군이 스트레스 2.23점(p=0.000), 강박증 1.95점(p=0.000), 본인자각구취 1.89점으로 나타났다(p=0.046). 본인자각구취는 강박증과 r=0.133으로 약한 양의 상관관계를 나타냈고, 스트레스와 강박증은 r=0.425로 높은 양의 상관관계를 나타냈다. 이에 구취의 다양한 원인을 파악함과 더불어 정서적 요인에 대해 관심을 가지며, 미래의 환자 구취 관리를 담당할 치과위생사로서 꾸준한 교육을 통한 인식의 고취가 필요하다.

In this Chapter, Terror and Palpitation due to Fright(驚悸) treat with the Kyeji-ke-jakyak-ka-chokchil-moryu-yongkol-kuyuk-tang(桂枝去芍藥加蜀漆牡蠣龍骨救逆湯) and Banha-mahwang-hwan(半夏麻黃丸). There are two type in Hemorrhagic disease(血證). One is bleeding(出血) and another is blood stasis(瘀血). The contents which relate with the Hemorrhagic disease(血證) are Hematemisis(吐血), Rhinorrhagia, Hemafecia(下血). In hemorrhage pathological mechanisms, there are two mechanisms. One is that Fire and Heat(火熱) pressure blood. Another is that cold and deficiency(虛寒) disable Ki(氣) from keeping blood flowing within vessels. Blood stasis(瘀血) can be called Extravasated Blood(惡血), Coagulated Blood, Blood retention(蓄血,積血), Dead Blood(死血) and Emaciation due to Blood disorder(乾血). It refer to a morbid state of unsmooth circulation and blood stagnancy often resulting from Ki(氣) stagnation, Ki(氣) deficiency and accumulation of pathogenic coldness. The symptom of Blood stasis are 'Fullness sensation in the chest, Lip Flaccidity, Cyonotic Tongue and Dryness of Mouth'. And the man who have Blood stasis, want to rinse his mouth with the water, but he can't drink the water because there isn't interior Heat of Excess Type. The symptom of Cyonotic Tongue(舌靑) had influence on diagnosing Blood stasis(瘀血) in offspring.
Vertigo is one of the subjective symptoms which appears commonly in so many diseases that it is often neglected. But it needs active medical care because it can be a forewarning of serious illness like cerebral apoplexy. This thesis makes a study of vertigo in view of Hyungsang medicine. The followings are the conclusion drawn in diagnosis and treatment of Vertigo: Excess in the upper and deficiency in the lower part(上實下虛) is a main cause of male vertigo. Deficiency in the upper and excess in the lower part(下虛上實) is a general cause of female vertigo. Vertigo is also caused by the insufficiency of the kidney water(腎水) or reservoir of marrow(髓海). persons of Jung pe(精科) with large check bones , those of Fish type(魚類) with Dig mouth and thick lips ; and those with large heads are mainly attacked by this cause. A fleshy person's vertigo comes from the deficiency of Ki and damp phlegm(氣虛濕症) so it is cured by reinforcing Ki, removing dampness and dispelling phlegm. A slim person's vertigo comes from the deficiency of blood and dryness heat(血虛有火) so it is treated by tonifying the blood, cleaning heat away the heat. In case of womanly shaped male and manly shaped female, the vertigo is due to the phlegm fire. Wind-heat(風熱) can cause vertigo generally 施 the persons of following types : wind type(風人), running animal type(走類),bird type(烏類), and Yangmyung meridian type (陽明形). Vertigo also comes from consumption. The deficiency of Ki can be a cause of vertigo in case of male; persons with pale complexion; and those with pronounced noses. The deficiency of blood can be a reason of vertigo in the persons of Hyul type(血科) and those with pale eyelids and lips. The hypofunction of the splean and stomach brings on the deficiency of Ki and blood, which can give rise to the vertigo for those with large nose and mouth. The old people's vertigo comes from the hypofunction of Ki, blood, Yin and Yang.
Objectives: We intended to identify factors related to the severity of hot flush of climacteric women from an East-West medical point of view. Methods: We surveyed 446 climacteric women who had received Korean oriental medical questionnaires about clinical symptom patterns and health medical examinations at Kyung-Hee University Medical Center over 1 year, from June 2007 to May 2008. Then, we compared hot flush with clinical symptom pattern and health medical examination result. Results: As the severity of hot flush increased, hypnagogic disorder in sleep pattern, abdominal gaseous distention in digestion pattern, tenesmus in evacuation pattern, yellow or reddish urine in voiding pattern, spontaneous sweating in sweating pattern, chest oppression in psychologic pattern, not-pulling-bedclothes in cold-heat pattern and mouth dryness in craniocervical symptom increased (p<0.05). In relation to digestion pattern, the severity of hot flush showed statistical significance according to prevalence of gastritis diagnosed by gastroscopy and upper GI series(p<0.01). In relation to voiding pattern, the severity of hot flush showed statistical significance according to prevalence of urine protein diagnosed by urinalysis (p<0.05). In relation to sweating, psychologic & cold-heat pattern, triiodothyronine (T3) increase and thyroid stimulating hormone (TSH) decrease were significantly correlated as the severity of hot flush increased (p<0.01). Conclusions: The result showed that hot flush of climacteric women had to be considered in respect of digestion disorder related to gastritis and sweating psychologic cold-heat disorder related to thyroid hormone.
Purpose: This study compared the effects of three oral care methods on thirst and oral status in patients after general anesthesia surgery. Methods: Sixty five surgical patients were prospectively randomized into three groups. Each group received one of three oral care methods: wet water gauze, frozen water gauze, or frozen normal saline gauze. The outcomes of thirst and oral status were assessed three times by trained investigators blinded to the oral care methods. Assessment times were right after a participant's arrival to his/her room, and at 30 and 60 minutes after the pre-test. Results: The levels of thirst and oral status were significantly improved as time passed in all three oral care methods. While the level of thirst was not significantly different between the three groups, the level of oral status was significantly better with wet water gauze than frozen gauze groups, soaked in either water or normal saline. Conclusion: We recommend nurses choose one of three oral care methods based on patients' preferences since all three oral care methods showed similar effects in improving postoperative patients' levels of thirst and oral status.
1. Objectives: We report the relation between the questionnaire and the health state on the Sasang Constitution. 2. Methods: We carried out a clinical test, in which 2 Oriental medical doctors participated to diagnose health state in each Sasang Constitution with 239 subjects. The subjects were divided into 3 subgroups, which are allocated to healthy group, poor health group and disease group. 3. Results: We extracted 4 questions for Taeeumin, 5 questions for Soyangin and 3 questions for Soeumin from the whole 98 questions respectively. Soyangin and Taeeumin share a same question that shows significant difference by health status. For the statistical analysis, we used a Chi-square test. As a result, we found out that Taeeumin showed different distributions of answers in direct/indirect characteristic, perspiration, frequent urination, regular defecation. According to the health status Soyangin showed the different distribution of the answers in color of urination, frequent urination, mouth dryness, abnormal perspiration, discomfort of head. Soeumin showed the different distribution of the answers in cold and chilly feel of abdomen, digestion status and cause of digestion problems when body condition gets worse. 4. Conclusions: This is the first work which reports the relatedness between Questionnaire and Health State in each Sasang Constitutions by an objective clinical test. We found out that several questions have different distributions by health status in each Sasang Constitution. These extracted questions are categorized not only by Wansilmoobyung(완실무병), but also by other symptoms and body status.
Purpose: This study was aimed to compare the effects of lemon ice and water ice on decreasing thirst of the patients with nasal surgery. Methods: A nonequivalent control group non-synchronized design was used for the study. The level of thirst and oral assessment were measured. A total of 60 subjects was recruited; 30 subjects for the experimental group in which lemon ice was provided and the other 30 subjects were in the control group in which on water ice was administered. Results: The scores of thirst were decreased in both lemon ice and water ice group. But it appeared that the score of thirst in the experimental group is significantly lower than that of the control group. Moreover, the score of the assessment of the patient's oral cavity was also significantly improved in the experimental group than that of the control group. Conclusion: The lemon ice seems an effective and easy-to-apply intervention in reducing thirst and mouth dryness over water ice in nursing practice.
Alcoholic hepatitis is an acute and severe liver disease associated with high mortality rate. This study was conducted to develop the instrument of criterion for symptoms improvement after temperance. We made the symptoms improvement questionnaire for alcoholic hepatitis patients through reviewing traditional oriental medical literatures and got advices from the advisor committee with delphi technique. The advisor committee on this study was organized by 10 professors of internal medicine of oriental medical colleges nationwide. The questionnaire was composed of questions about 10 symptoms - fatigue, pain, anorexia, abdominal bloat, heaviness of the body, itch, nausea, sleep, dryness in the mouth and stools. We named it Alcoholic Hepatitis Symptoms Improvement Questionnaire(AHSIQ). We surveyed 65 male alcoholic hepatitis patients and checked liver function profile and AHSIQ before and after temperance four times for six weeks. As a result of factor analysis the scales of AHSIQ had content validity and construct validity. And internal consistency reliability was good(Cronbach's alpha=0.768-0.871). The total scale scores were statistically significant in gamma-GTP related validity. We suggest that AHSIQ would be effective for measuring symptoms improvement degree in alcoholic hepatitis patients through further investigations with larger clinical trials.
Objectives: The purpose of the study was to compare the oral health related characteristics between a long-term patients and general population. Methods: A direct interview questionnaire was completed by 160 patients and 165 general people from January 16 to April 31, 2014. The questionnaire consisted of general characteristics of the subjects, subjective health status, oral health behaviors, and needs of oral health. The data were analyzed using SPSS 18.0 program. Results: The long-term patients reported that most of them were denture wearers(38.1%), and had oral disease symptoms(62.5%) and xerostomia(65.6%). Most of the long-term patients were ex-smoker(31.3%) and did not receive regular dental check-up(92.5%). They did not know tooth brushing method(31.3%) and brushed their teeth less than twice a day(47.5%). Those who used tooth brush for more than 6 months accounted for 47.5%. Most of them did not use auxiliary oral hygiene devices(85.6%). The patients answered that oral health is not important(6.9%), oral health education is not necessary(7.5%), and oral cleaning(26.3%) should be included in oral health education. Conclusions: The self-reported oral health status of the long-term patients much more serious than the general population. It is necessary to educate the continuing oral health management program for the long-term patients.
