FEM Analysis of the Effects of Mouth guard material properties on the Head and Brain under Mandibular Impact (구강보호장치의 재료적인 특성이 하악골 충격 시악골 및 두부에 미치는 영향에 관한 유한요소분석)

  • Statement of problem & Purpose: The purpose of this study was to investigate the effect of a mouth guard material properties on the skull and brain when they were under impact loads on mandible. Material and methods: Two customized mouth protectors having different material propeerst ieach other were made for a female Korean who had no history of brain trauma, no cerebral diseases, nomal occlusion and natural dentition. The 3D finite element model of human skull and brain scanned by means of computed tomography was constructed. The FEM model of head was composed of 407,825 elements and 82,138 nodes, including skull, brain, maxilla, mandible, articular disc, teeth and mouth guard. The stress concentrations on maxillary teeth, maxilla and skull with two mouth guards were evaluated under oblique impact load of 800N onto mandibular 3 loading points for 0.1sec. And the brain relative displacement was compared in two different mouth guard materials under same condition. Result and Conclusion: The results were as follows; 1. In comparison of von Mises stress on maxillary teeth, a soft mouth guard material had significantly lower stress values on measuring point than a hard mouth protector materials (P < .05). 2. In comparison of von Mises stress on maxilla and skull, A soft mouth protector material had significantly lower stress values on measuring point than a hard mouth protector materials (P < .05). 3. For impact loads on mandible, there were more stress concentrated area on maxilla and skull with hard mouth guard than soft with mouth protector. 4. For impact loads on mandible, brain relative displacement had little relation with mouth guard material properties. In results of this study, soft mouth guard materials were superior to hard mouth guard materials for mandible impact loads for prevention of sports injuries. Although the results of this study were not enough to figure out the roles of needed mouth guard material properties for a human head, we got some knowledge of the pattern about stress concentration and distribution on maxilla and skull for impact loads with soft or hard mouth protector. More studies are needed to substantiate the relationship between the mouth guard materials and sports injuries.

Usefulness Evaluation of Open Mouth View when PET/CT scan In Tongue Cancer Patients (Tongue Cancer 환자에서 PET/CT 검사 시 Open Mouth 촬영법의 유용성 평가)

  • Purpose Tongue cancer is 1.8% of all cancer tumors occur in the tongue, it is known that the high incidence enough to account for 75% of oral cancer conducted a PET / CT examination for early diagnosis, metastasis, staging, etc. and. Tongue when PET / CT scan of a cancer patient and a Torso taken to close mouth lesions if the condition was caused due to the overlapping or corresponding artifacts are not clearly observed. The purpose of this study is to evaluate the changes that occur during PET / CT scan with open mouth and its usefulness under. Materials and Methods From June 2015 to March 2016 complained of herein by May 21 had received a diagnosis of tongue cancer underwent PET / CT scan patients were treated with a target (16 males, 5 female). The first was taken to close mouth Torso state, it was taken to add 1 bed open mouth condition. Tumor (T), measuring the Normal Tongue (NT), Lymph Node (LN) standard intake coefficient by setting a region of interest in the (standardized uptake value, SUV) SUVmean, the average value was measured SUVmax, drawn to each region of interest 3 times and Background (Carotid artery) was out of the SUV. In Chapter 3 of the slice to the tumor clearly visible by setting the region of interest to measure the change Tumor size was calculated average value. Gross Image resolution assessment were analyzed statistically through were divided into 1-5 points by the Radiation 7 people in 2, more than five years worked in specialized nuclear medicine compare to proceed with the blind test nonparametric test (wilcoxon signed rank test). (SPSS ver.18) Results $SUV_{mean}$ T's were in close mouth $5.01{\pm}2.70$ with open mouth $5.48{\pm}2.88$ (P<0.05), $SUV_{max}$ were respectively $8.78{\pm}5.55$ and $9.70{\pm}5.99$ (P<0.05). $SUV_{mean}$ in the NT were respectively $0.43{\pm}0.30$ and $0.34{\pm}0.24$ (P=0.20), $SUV_{max}$ was $0.56{\pm}0.34$ and $0.45{\pm}0.25$ (P=0.204). LN $SUV_{mean}$ were respectively $1.62{\pm}1.43$ and $1.69{\pm}1.49$ (P=0.161), $SUV_{mean}$ was $2.09{\pm}1.88$ and $1.99{\pm}1.74$ (P=0.131). Tumor size change is close mouth $4.96{\pm}4.66cm^2$ $5.33{\pm}4.64cm^2$ with 7.45% increase was (P<0.05), gross image resolution evaluation is $2.87{\pm}0.73$, $3.77{\pm}0.68$ with open mouth examinations 30.5% increase was (P<0.05). Conclusion Tumor SUV on the changes that had an increase in open mouth during inspection, the normal tongue and lymph node, but there was no significant difference in the change slightly. It is also one open mouth PET / CT scan will provide improved image to all patients with tongue cancer, but it could be confirmed that similar overall through the blind test, or tumor size changes and showing a high resolution image. It can be the perfect alternative method for problems that occur when the close mouth Open mouth PET / CT scan, but is believed to be through the open mouth to observe the boundary of overlapping or tumor of the oral cavity other structures a little more clearly. Tongue cancer patients how to recommend that the shooting further open mouth PET / CT.

Clinical Characteristics and Evaluation of Yin-deficiency Syndrome in Patients with Burning Mouth Syndrome (구강작열감증후군 환자의 임상적 특징 및 음허증 평가)

  • Objectives : This study was designed to investigate the clinical characteristics and usefulness of comprehensive diagnosis of Yin-deficiency and heart rate variability in patients with burning mouth syndrome (BMS). Methods : We surveyed 30 burning mouth syndrome patients visiting the Oral Diseases Clinic of Kyung Hee University Oriental Medicine Hospital from April to September of 2011. The subjects were evaluated on self-assessed severity of burning mouth syndrome and xerostomia using visual analogue scale (VAS) score and Yin-deficiency condition (based on the 10-item Yin-deficiency questionnaire). Salivary function was measured by the unstimulated salivary flow rate (USFR), and heart rate variability (HRV) parameters were recorded by SA-2000E (Medicore Co., Ltd., Korea). Results : There were substantial significant positive correlations between burning sensation VAS scores in mouth and Yin-deficiency scores. There was significant negative correlation between xerostomia VAS score and USFR. Compared to the normal range of total power (TP) in HRV parameters, the burning mouth syndrome patients showed significant lower values of TP. Conclusions : The results of this study suggest that comprehensive diagnosis of Yin-deficiecny and HRV parameters are useful in diagnosing of burning mouth syndrome patients. Therefore, we assume that improvement of Yin-deficiency condition can be a potentially effective way to treat burning mouth syndrome.

"Word of Mouth" in the Chain Restaurant Industry (체인 레스토랑 산업에서 고객의 '구전 효과' 형성에 관한 연구)

  • The study investigated how 'word of mouth' originates in the chain restaurant industry. It has long been acknowledged that 'word of mouth' is a critical factor for the success of a restaurant business due to its targetability and cost effectiveness. A review of the literature revealed four antecedents of 'word of mouth': service quality, perceived value, satisfaction, and relationship quality. Based on the theoretical/empirical relationships between those constructs, a structural model composed of the hypotheses was proposed. The structural model was tested with data collected from 471 chain restaurant patrons. The structural equation modeling analysis revealed that five constructs in the proposed model are interrelated, and during this process, word of mouth is formed in the chain restaurant industry. A positive relationship between service quality and satisfaction (0.265, p<0.05), service quality and perceived value (0.831, p<0.05), service quality and relationship quality (0.465, p< 0.05), and service quality and WOM (0.263, p< 0.05) were found, indicating that service quality is a key prerequisite for word of mouth and other constructs proposed in the model. It was revealed that perceived value doe not have a direct impact on WOM formation (t=1.275, p=0.202), but a positive relationship between perceived value and satisfaction (0.293, p<0.05) and between satisfaction and WOM (0.627, p< 0.05) were found. Therefore, it was concluded that patrons' perceived value influences word of mouth formation, but that impact is mediated by satisfaction. During this process, relationship quality also plays a mediating role in generating word of mouth. Based on data analysis, theoretical/managerial implications are discussed.

Influencing factors of self-reported dry mouth in the employees in social welfare facilities (일부 지역 사회복지시설 종사자의 주관적 구강건조증에 영향 요인)

  • Objectives: The purpose of the study was to examine the influencing factors of self-reported dry mouth in the employees in social welfare facilities. Methods: A self-reported questionnaire was completed by 260 employees in social welfare facilities from January 5 to 30, 2015 by convenience sampling method. Except 25 incomplete answers, 215 data were analyzed by t test, one way ANOVA, Pearson's correlation coefficient analysis, and multiple linear regression analysis using SPSS 180. program. The questionnaire consisted of general characteristics of the subjects, health-related characteristics, whole body dryness and self-reported dry mouth. The oral health-related quality of life was measured by five point Likert scale, and a higher score indicated a lower quality of life. Results: The self-reported dry mouth in the employees in the social welfare facilities varied by the general health status, stress, oral health status and oral malodor. The self-reported dry mouth was closely related to the quality of life and the four subfactors including dryness of skin, eye, lip and nasal mucosa. The quality of life had the influence on the self-reported dry mouth, nasal mucosa dryness, eye dryness, and oral malodor in order. Conclusions: The self-reported dry mouth was closely related to whole body dryness and the quality of life. It is necessary to develop the quality of life improvement programs that prevent and manage the dry mouth and whole body dryness in the employees in the social welfare facilities.

Influencing factors on oral and maxillofacial trauma prevention education experience of students majoring in physical education (체육전공 대학생의 구강악안면 외상 예방법 교육경험에 영향을 미치는 요인)

  • Objectives: The aim of this study was to investigate the influencing factors on oral and maxillofacial trauma prevention education experience of students majoring in physical education. Methods: A self-reported questionnaire was filled out by 268 students majoring in physical education in Busan. All statistical analyses were performed using SPSS 21.0. The questionnaire consisted of general characteristics(4 questions), TMJ symptoms(9 questions), oral habit(8 questions), and mouth guard awareness(5 questions). The questionnaire was carried out by 5 Likert scale. Higher points of Likert scale showed the negative tendency except the mouth guard awareness. The higher points of mouth guard awareness showed the positive tendency to use the mouth guard. Results: Male students had higher scores of 2.75 points in bad oral habit than the female students(p<0.05). Female students higher score of 1.30 points in mouth guard awareness than male students(p<0.05). TMJ symptoms experience in oral and maxillofacial trauma was 3.15 points which was higher than the prevention education experience without maxillofacial trauma(p<0.001). TMJ symptoms and Mouth guard awareness showed 2.71(p<0.01) and 1.20 points(p<0.001) respectively in students with maxillofacial trauma prevention education experience. These score were higher than those without education experience of trauma prevention education experience. The influencing factors on trauma prevention education experience are mouth guard awareness(p<0.001), oral habits(p<0.01), and temporomandibular joint symptom(p<0.01). Conclusions: The necessity of mouth guards should be emphasized to prevent the serious oral trauma. It is very important to establish the rule for mouth guard use in sports activities. Therefore, oral and maxillofacial trauma prevention education program is needed.

The Effects of Risk Perception on e-WOM in Internet Shopping of Chinese Consumers in Their 20s (중국 20대 소비자들의 인터넷 쇼핑 위험지각이 구전(e-WOM)에 미치는 영향)

  • This study examined the risk perception in internet shopping by Chinese and analyzed the effects of risk perception on internet word-of-mouth acceptance and transmission. This analysis was conducted with data collected from 373 Chinese individuals in their 20s. Data were analyzed with factorial analysis, cluster analysis, ANOVA, multiple regression analysis, Chi-square test, and Cronbach's ${\alpha}$ using SPSS 18.0. The results were: 1) The factor analysis of risk perception extracted four factors: fashion/social psychological risk, product risk, transaction risk, and economic risk. The cluster analysis classified them into: Group with low-risk, Group with high-risk, Group with economic risk, and Group with medium-risk. 2) Group with high-risk and Group with economic risk had a higher word-of-mouth acceptance than other groups. The Group with a high-risk had more word-of-mouth transmission than other groups. 3) It was found that when word-of-mouth was accepted, the factual information about fashion products (size and material) was most referred to, and that the overall evaluation of satisfaction and dissatisfaction was most conveyed when word-of-mouth was conveyed. 4) Internet word-of-mouth acceptance was affected by product risk, economic risk, and transaction risk. Internet word-of-mouth transmission was affected by economic risk, and fashion/social psychological risk, product risk, and transaction risk.

The Effect of Corporate Green Marketing on Consumer Word of Mouth Intention (기업의 그린마케팅이 소비자 구전의도에 미치는 영향)

  • In the midst of increasing interest in the environmental responsibility of companies, this study investigates how eco-friendly activities affect the creation of management performance. To this end, the effect of companies' green marketing on consumers' word-of-mouth intention, the mediating effect of brand reliability, and the moderating effect of consumers' pro-environmental behavior were verified. The results of the collected data analysis are as follows. First, companies' green marketing has a positive (+) effect on consumers' word-of-mouth intention. Second, brand reliability takes a positive (+) mediating role in the relationship between companies' green marketing and consumers' word-of-mouth intention. Third, consumers' pro-environment behavior has a moderating role between Brand Reliability and their word-of-mouth intention. Therefore, companies' eco-friendly strategies and their implementation will help enhance both brand reliability and consumers' word-of-mouth intention. By revealing the effect of companies' eco-friendly strategies and their implementation on brand reliability and consumers' word-of-mouth intention it is meaningful to analyze the relationship between green marketing and the possibility of consumers' spontaneous word-of-mouth intention. The results suggest to companies that it is necessary to establish an eco-friendly marketing strategy for sustainable growth.

Research Trend of Oriental Medical Treatments for Burning Mouth Syndrome (구강작열감증후군의 한의학적 치료에 대한 최근 국내외 임상 연구 동향)

  • Objectives : The purpose of this study is to analyze the oriental medical treatments of burning mouth syndrome, understand the tendency of treatment, and apply it to clinical settings. Methods : We collected case reports, retrospective cohort studies and RCT studies related to oriental medical treatments of burning mouth syndrome using domestic and Chinese databases(CNKI, KISS, RISS, OASIS, KCI). Search terms include 'Burning Mouth Syndrome', 'BMS', 'Burning Tongue', '灼口综合征', and 'oriental medicine', 'oriental medical treatment', 'Korean medicine' and '中医'. A search was conducted by appropriately combining keywords. Results : A total of 27 papers were included in the analysis. Among them, 9 are case studies, 1 is a retrospective cohort study, 1 is a before and after study and 16 are RCT studies. Treatments for burning mouth syndrome included herbal medicine, acupuncture, electro-acupuncture, acupoint injection, auricular acupuncture treatment, external use herbal medicine and gargling. In all studies, symptoms of burning mouth syndrome were alleviated after oriental medical treatments. In 14 RCT studies comparing western medical treatments, the results of the treatment group that included oriental medical treatments were found to be more significant, except for one. Conclusions : As a result of the study, oriental medical treatments are effective in treating burning mouth syndrome. In the future, we hope that clinical research related to oriental medical treatments of burning mouth syndrome will be actively conducted so that evidence-based treatment can be implemented.

THE INFLUENCE OF METABOLIC ACIDOSIS, AIRWAY RESISTANCE AND VAGOTOMY ON THE DEVELOPMENT OF MOUTH BREATHING (대사성 산증, 기도저항 변화 및 미주신경 절단이 구호흡 발생에 미치는 영향)

  • Respiration is one of the most important functions which are carried out in stomatognathic system. When nasal orifice is obstructed or the resistance of upper airway is increased mouth breathing is initiated. Mouth breathing is regarded as an important etiologic factor of dentofacial anomalies. This experiment was performed to observe the influences of metabolic acidosis, tracheal resistance and vagotomy on mouth breathing. After rabbits were anesthetized with sodium pentobarbital, a pair of wire electrode was inserted into mylohyoid muscle, anterior belly of digastric muscle and dilator naris muscle to record EMG activity. Femoral vein and artery were cannulated for infusion of 0.3N HCl and collection of blood sample to determine the blood pH, and tracheal intubation was done to control airway resistance. Mouth breathing was induced by metabolic acidosis. Increase of the airway resistance through tracheal cannula intensified the activity of dilator naris, mylohyoid and digastric muscle. The higher the resistance, the larger the EMG amplitude. After bilateral vagotomy, respiratory volume and inspiatory time were increased and the activities of dilator naris, mylohyoid and digastric muscle were strengthened. It was concluded that the muscle activity related to mouth breathing was induced by metabolic acidosis and increase of tracheal tube resistance.
