A Case Report of "Spinal Cord Apoplexy" Elicited by Metastatic Intramedullary Thyroid Carcinoma

  • A 31-year-old man presented with acute onset of paraplegia. The patient's history was significant for thyroid carcinoma that had been treated 2 years earlier by thyroidectomy. A magnetic resonance imaging scan showed an enhancing intramedullary lesion at T7-8. Patient underwent surgical treatment and a tumor with hematoma was resected via posterior midline myelotomy. Postoperatively, the patient's motor weakness was improved to grade 3. The lesion showed typical histologic features consistent with papillary thyroid carcinoma. Early diagnosis and microsurgical resection can result in improvement in neurological deficits and quality of life of patients with an ISCM.

Acute Cervical Spinal Subdural Hematoma Not Related to Head Injury

  • We report an extremely rare case of traumatic cervical spinal subdural hematoma not related to intracranial injury. There has been no report on traumatic cervical spinal subdrual hematoma not related to intracranial injury. A 27-year-old female patient was admitted to our emergency room due to severe neck pain and right arm motor weakness after car collision. On admission, she presented with complete monoplegia and hypoesthesia of right arm. Magnetic resonance imaging (MRI) revealed subdural hematoma compressing spinal cord. Lumbar cerebrospinal fluid (CSF) analysis revealed 210,000 red blood cells/$mm^3$. She was managed conservatively by administrations of steroid pulse therapy and CSF drainage. Her muscle power of right arm improved to a Grade III 16 days after admission. Follow-up MRI taken 16th days after admission revealed almost complete resolution of the hematoma. Here, the authors report a traumatic cervical spinal SDH not associated with intracranial injury.

어혈비로 변증한 Brown-Sequard Syndrome 치험 1예 (A Case of Brown-Sequard Syndrome Treated as Eo-Hyeol Bee-Jeung)

  • Brown-Sequard syndrome is an incomplete spinal cord lesion characterized by ipsilateral weakness, loss of proprioceptive and vibratory sensation on the same side. and loss of pain and temperature sensation on the contralateral side. This clinical case was analysed on a patient with Brown-Sequard syndrome. The patient was admitted on January 26th, 2005. and departed on March 28th, 2005. He was treated as Eo-Hyeol Bee-Jeung. After treatment, His motor grade and weakness were improved and sensory ability about pain and temperature were regaine. We suggest that oriental medicine therapy is effective in treatment on Brown-Sequard syndrome.

  • Recently, the concern for the reliability of railway is increasing, also the study about reliability is being processed at a good pace. However, in case of an existing railway operating it is true that the railway system is being operated without a clear RAM standard. There is not a clear RAM standard about product menual of a city railroad, and there are only basic provisions concerning traction motor which can be called a main equipment. In this paper, we calculated the availability and reliability of subway having MKBSF, MKBF, MTBF, MTTR analyzing accumulated maintenance data about a main equipment of a VVVF direct subway being operated in line #4. By watching the reliability grade of a VVVF subway being operated and manufactured within the country and being applied with the development of a city railway reflected by RAM menual when the new subway is manufactured, and with the basic data of RCM, we'd like to grope the methods which can be improved by operation stability of a city railway.

Neurologic Complication Following Spinal Epidural Anesthesia in a Patient with Spinal Intradural Extramedullary Tumor

  • Paraplegia following spinal epidural anesthesia is extremely rare. Various lesions for neurologic complications have been documented in the literature. We report a 66-year-old female who developed paraplegia after left knee surgery for osteoarthritis under spinal epidural anesthesia. In the recovery room, paraplegia and numbness below T4 vertebra was checked. A magnetic resonance image (MRI) scan showed a spinal thoracic intradural extramedullary (IDEM) tumor. After extirpation of the tumor, the motor weakness improved to the grade of 3/5. If a neurologic deficit following spinal epidural anesthesia does not resolve, a MRI should be performed without delay to accurately diagnose the cause of the deficit and optimal treatment should be rendered for the causative lesion.

화농성 척추염환자 치험 1례에 대한 임상적 고찰 (Clinical Study on a Case of Pyogenic Spondylitis)

  • Objectives : This report intended to estimate effect taken by using acupuncture, herbal medication on the patient with Pyogenic Spondylitis Methods : From 17 December, 2003 to 30 August, 2004. The patient received TDP, Infra-Red and Acupuncture therapy. Results : After being hospitalized for 8 months, the patient's Motor Grade, VAS, ODI, Nurick's grading system were improved each from III to IV+, from 5 to 4, from 21.25 to 17.5, from Gr. IV to Gr. II. Conclusion : We considered that oriental treatment has an useful effect on Pyogenic spondylitis patient's treatment and recovery.

길랑바레증후군으로 진단받은 환자의 한방치료 증례보고 1례 (Case Report of Patients Diagnosed with Guillain-Barre Syndrome Improved by Traditional Korean Medical Treatment)

  • Objective: The present study reports on one patient diagnosed with Guillain-Barre syndrome.Method: One patient was treated by Sukjiyanggeun-tang, electroacupuncture, and physical therapy. Any improvement in symptoms was assessed by measuring changes in motor grade; scores in the modified Barthel index, the disability scale, and the numerical rating scale (NRS) of lower back pain; and ambulatory condition during hospitalization.Results: After 48 days of treatment, the patient's symptoms of Guillain-Barre syndrome showed improvement.Conclusions: Traditional Korean medical treatment appeared to be effective in reducing Guillain-Barre syndrome symptoms, but more research is required to confirm these results.

Dietary modulation of gut microbiota for the relief of irritable bowel syndrome

  • Irritable bowel syndrome (IBS) is a frequently diagnosed gastrointestinal (GI) disorder characterized by recurrent abdominal pain, bloating, and changes in the stool form or frequency without any structural changes and overt inflammation. It is not a life-threatening condition but causes a considerable level of discomfort and distress. Among the many pathophysiologic factors, such as altered GI motility, visceral hypersensitivity, and low-grade mucosal inflammation, as well as other immunologic, psychologic, and genetic factors, gut microbiota imbalance (dysbiosis), which is frequently found in IBS, has been highlighted as an etiology of IBS. Dysbiosis may affect gut mucosal homeostasis, immune function, metabolic regulation, and even visceral motor function. As diet is shown to play a fundamental role in the gut microbiota profile, this review discusses the influence of diet on IBS occurring through the modulation of gut microbiota. Based on previous studies, it appears that dietary modulation of the gut microbiota may be effective for the alleviation of IBS symptoms and, also an effective IBS management strategy based on the underlying mechanism; especially because, IBS currently has no specific treatment owing to its uncertain etiology.

Coinfection of Sphingomonas paucimobilis meningitis and Listeria monocytogenes bacteremia in an immunocompetent patient: a case report

  • This report describes a case of coinfection of Sphingomonas paucimobilis meningitis and Listeria monocytogenes bacteremia in a 66-year-old immunocompetent female patient. The patient had undergone traditional procedures, including acupuncture, which possibly caused the coinfection. During treatment with susceptible antibiotics for bacterial meningitis, she developed hydrocephalus on the third day. Consequently, the patient recovered with a mild neurological deficit of grade 4 motor assessment in both upper and lower extremities at discharge. S. paucimobilis and L. monocytogenes are rare pathogens in developed countries, occurring only during environmental outbreaks. S. paucimobilis meningitis is rarely reported. Hence, the various presentations of S. paucimobilis meningitis and the antibiotic regimen for its treatment are hereby reported, in addition to a review of other similar reported cases. This case is a possible traditional procedure-related infection. Appropriate oversight and training should be emphasized regarding preventive measures of this kind of infection. A team approach with neurologists and neurosurgeons is imperative in treating patients with hydrocephalus-complicated meningitis.

상흉추강내에 발생한 신경장 낭종 - 증 례 보 고 - (Neurenteric Cyst in Upper Thoracic Spinal Canal - Case Report -)

  • Spinal neurenteric cyst results from the persistence of an abnormal communication between endodermal and ne-uroectodermal layer. Embryologically, neurenteric cyst is derived from endoderm that is fused with the developing notochord during the third week of gestation. It is a rare malformation that lead to spinal cord compression. The patient is 19-year-old male presented with chest pain, paresthesia and progressive weakness in his low extremities(grade II/II). Preoperative MR imaging revealed intradural extramedullary cyst with intracystic hemorrhage in T1 and T2 level that is ventrally located and compressed the spinal cord. Involved vertebral bodies were scalloped and fused. The cystic tumor were totally removed through costotransversectomy approach. Postoperatively, motor weakness of the low extremities were improved to the level of grade IV/V. And chest pain and paresthesia were gradually disappeared. Postoperative MR imaging showed the decompression of the thoracic spinal cord. Histologic examination revealed a ciliated columnar epithelial neurenteric cyst. The pre- and postoperative clinical, radiological features of a case of upper thoracic neurenteric cyst is described with review of literature.
