회전근 개 파열에 대한 관절경적 교량형 봉합술의 결과: 지방 변성이 중등도 이하인 전층 파열에 대한 단기 추시 임상적 결과 (Outcome of Arthroscopic Suture Bridge Technique for Rotator Cuff Tear: Short Term Clinical Outcome In Full-thickness Tear With Fatty Degeneration Less Than Moderate Degree)

  • 목적: 회전근 개 건 전층 파열 시 관절경적 교량형 봉합술 시행 후 단기 추시 관찰을 통해 이 술식의 임상적 결과에 대해 알아보고자 하였다. 대상 및 방법: 회전근 개 건 전층 파열로 관절경적 교량형 봉합술을 시행 받고 조기 재활을 시행한 29명 (남자:17명, 여자:12명)의 환자를 대상으로 시행하였다. 평균 나이는 56.4세 (34~73세)였으며 추시 관찰 기간은 평균 13개월 (12~15개월)이었다. 임상적 및 기능적 평가로 The University of California at Los Angeles (UCLA) score, The Korean Shoulder Scoring system (KSS) 및 Visual Analogue Scale (VAS) score를 이용하였다. 15예에서 최종 추시시 자기 공명 영상을 촬영하여 Sugaya 분류를 통해 회전근 개의 구조적 상태를 분석하였다. 결과: 수술 후의 UCLA score는 수술 전과 수술 후 각각 16.4에서 31.6으로 측정되어 유의할만한 호전을 보였고 (p< 0.05), KSS는 수술 후 6개월에 88이었고, 최종 추시 시 92로 측정되었다. VAS score는 수술 전 8.6에서 수술 후 3개월에 2.1로, 수술 후 6개월에는 1.4로 나타났다. 28예 (96.5%)에서 수술 후 관절 운동 범위가 증가되었다. 추시 시 자기 공명 영상을 촬영한 15예에서 Sugaya 분류에 의해 I형이 6예, II형이 7예, III형이 1예, V형이 1예이었다. 결론: 회전근 개 건 전층 파열에 대한 관절경적 교량형 봉합술을 이용한 회전근 개 봉합술은 임상적으로 96.5%에서 양호 이상의 만족할 만한 결과를 보였으므로 믿을 만한 수술적 방법 중 하나라고 생각한다.

  • 본 연구의 목적은 니켈 티타늄 전동파일의 피로 파절에 파일의 단면 형태가 미치는 영향을 평가하고자 하였다. 6 종의 ProFile (Dentsply), Mtwo (VDW), K3 (SybronEndo), Heroshaper (MicroMega), NRT (Mani), Alpha system (KOMET) 니켈-티타늄 전동 파일(ISO 30 size/.04 taper)을 각 10개씩 실험군으로 배정하고 실험을 진행하였다. 니켈 티타늄 전동파일에 반복적인 장력과 압축력을 재현시킬 수 있는 피로파절 실험기(Denbotix)와 연결된 토크 조절 전동모터(Aseptico)에 각 군의 파일을 연결하여 300rpm 속도로 만곡도가 60도이고 5mm의 반경을 가진 인공 금속 근관 내를 6 mm pecking depth로 작동시켰다. 각 파일의 파절시간을 측정하고 통계 분석하여 유의성을 분석하였으며, 각 군의 파절된 모든 파일의 파절면을 주사전자현미경을 이용한 fractographic analysis를 통해 파절 역학을 규명하였다. 또한 각 군에서 3개의 사용하지 않은 새 파일을 clear resin에 매몰하고tip에서부터 3mm 지점을 횡절단하여 Image-Pro Plus (Imagej 1.34n, NIH) 소프트웨어로 절단면의 단면적을 측정하고 단면적의 크기와 피로 파절과의 상관성도 평가하였다. 실험 결과 NRT와 ProFile이 다른 실험군에 비해서 유의성 있게 높은 피로 파절 저항성을 보였다(P < .05). 또한 파일의 단면적은 피로파절 저항성과 통계학적인 유의성은 보이지 않았다. Fractographic analysis 결과 모든 시편에서 파절면은 ductile fracture와 brittle fracture가 혼재된 양상으로 나타났다. 결론적으로 니켈 티타늄 전동 파일의 단면적의 크기는 피로파절 저항과는 상관성이 적었다.

디지털 트윈을 적용한 고감도 충돌 시뮬레이션 개발을 위한 연구 (A Study on the Development of High Sensitivity Collision Simulation with Digital Twin)

  • 연구목적: 원자로 내부 시설물 해체 등의 고위험 시설이나 고비용 작업에 앞서 시뮬레이션을 통한 작업의 안정성과 생산성을 최대화하기 위해 실제 제어 장비의 제원을 시뮬레이션 상에 모사하고 이를 통해 정밀 제어될 수 있는 디지털 트윈 기술을 이용하고자 한다. 디지털 트윈 기술을 적용함에 정밀 제어 장비와 시뮬레이션의 시간적 격차로 인해 발생할 수 있는 동작 제어 오차는 위험 시설물과 제어 장비 간의 충돌 등과 같은 위험 요소들을 발생시킬 수 있다. 이러한 상황을 제거하고 통제하기 위해서는 사전 연구가 필요하다. 연구방법: 현재 시뮬레이션을 개발함에 가장 대중적으로 사용되는 엔진으로는 Unity 3D가 있다. 하지만 Unity 3D 엔진 내부의 시간 보정으로 인해 발생할 수 있는 제어 오차가 존재한다. 그 오차는 여러 환경에 예상되고 그 오차는 시스템 사양 등의 개발 환경에 따라 다를 수 있다. 이를 입증하기 위해 Unity 3D 엔진을 이용하여 충돌 시뮬레이션 개발하고 이를 통해 다양한 조건의 충돌실험을 진행하고 그에 따른 결과를 정리하고 분석하여 이를 토대로 정밀 제어 장비의 허용 오차를 도출한다. 연구결과: 충돌실험 시뮬레이션을 통한 실험에서 엔진 내부 함수호출에 1/1000초 단위의 시간 보정으로 인해 충돌체의 이동제어에 단위 시간당 거리오차가 발생하고 거리오차는 충돌체의 이동속도와 비례한다. 결론: 디지털 트윈을 이용한 원격해체 시뮬레이터는 하드웨어와 소프트웨어 환경과 수동 제어 시 정밀 제어 장치의 요구 정밀도에 따른 이동속도의 제한이 필요할 것으로 판단된다. 그리고 운용 제어 장비의 가용 및 허용 오차와 작업의 요구 속도를 시스템 개발 환경, 하드웨어 사양과 시뮬레이션에 모사된 제어 장비 및 시설물 등의 모델링 데이터의 크기도 반드시 고려한다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. Our department has developed an information system for routine hematology using down-sized computer system. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer. Because the point of maximun temperature of laser-irradiated gelatin exists below the surface due to surface cooling, evaporation at the boiling temperature is made explosively from below the surface. The important parameters of this process are the conduction loss to laser power absorption (defined as the conduction-to-laser power parameter, Nk), the convection heat transfer at the surface to conduction loss (defined as Bi), dimensionless extinction coefficient (defined as Br.), and dimensionless irradiation time (defined as Fo). Dependence of Fo on Nk and Bi has been observed by experiment, and the results have been compared with the numerical results obtained by solving a 2-dimensional conduction equation. Fo and explosion depth (from the surface to the point of maximun temperature) are increased when Nk and Bi are increased.To find out the minimum laser power for explosive evaporative removal process, steady state analysis has been also made. We obtained a two-dimensional current density distribution within a phantom by processing the real and imaginary MR images from a 4.77 NMR machine. We implemented a resistivity mage reconstruction algorithm using the finite element method and sensitivity matrix. Optimal stimulus parameters, which can minimize muscle fatigue and can achieve effective muscle contraction were determined in slow and fast muscles in Sprague-Dawley rats. Stimulus patterns of our stimulator were designed to simulate electromyographic activity monitored during locomotion of normal subjects. Muscle types of the lower extremity were classified according to their mechanical property of contraction, which are slow muscle (msoleus m.) and fast muscle (medial gastrocneminus m., rectus femoris m., vastus lateralis m.). Optimal parameters of electrical stimulation for slow muscles were 20 Hz, 0.2 ms square pulse. For fast muscle, 40 Hz, 0.3 ms square pulse was optimal to produce repeated contraction. Higher stimulus intensity was required when synergistic muscles were stimulated simultaneously than when they were stimulated individually. Electrical stimulation for each muscle was designed to generate bipedal locomotion, so that individual muscles alternate contraction and relaxation to simulate stance and swing phases. Portable electrical stimulator with 16 channels built in microprocessor was constructed and applied to paraplegic patients due to lumbar cord injury. The procedure has the following three distinct stages. First, the initial center is estimated by the initial center estimation algorithm which is applied to decimated image. Second, the center estimation algorithm is applied to original image and then best-fit elliptic model estimation is processed. Third, best-fit boundary is detected by the cost function which is based on the best-fit elliptic model.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer. Because the point of maximun temperature of laser-irradiated gelatin exists below the surface due to surface cooling, evaporation at the boiling temperature is made explosively from below the surface. The important parameters of this process are the conduction loss to laser power absorption (defined as the conduction-to-laser power parameter, Nk), the convection heat transfer at the surface to conduction loss (defined as Bi), dimensionless extinction coefficient (defined as Br.), and dimensionless irradiation time (defined as Fo). Dependence of Fo on Nk and Bi has been observed by experiment, and the results have been compared with the numerical results obtained by solving a 2-dimensional conduction equation. Optimal stimulus parameters, which can minimize muscle fatigue and can achieve effective muscle contraction were determined in slow and fast muscles in Sprague-Dawley rats. Stimulus patterns of our stimulator were designed to simulate electromyographic activity monitored during locomotion of normal subjects. Muscle types of the lower extremity were classified according to their mechanical property of contraction, which are slow muscle (msoleus m.) and fast muscle (medial gastrocneminus m., rectus femoris m., vastus lateralis m.). Optimal parameters of electrical stimulation for slow muscles were 20 Hz, 0.2 ms square pulse. For fast muscle, 40 Hz, 0.3 ms square pulse was optimal to produce repeated contraction. Higher stimulus intensity was required when synergistic muscles were stimulated simultaneously than when they were stimulated individually. Electrical stimulation for each muscle was designed to generate bipedal locomotion, so that individual muscles alternate contraction and relaxation to simulate stance and swing phases. Portable electrical stimulator with 16 channels built in microprocessor was constructed and applied to paraplegic patients due to lumbar cord injury. kV X선 영상, 전자조사문 영상, 디지털화재구성 영상 간 기하학적 일치성 평가 (Evaluation of Geometric Correspondence of kV X-ray Images, Electric Portal Images and Digitally Reconstructed Radiographic Images)

  • 본 연구에서는 온보드 영상장치(On-Board Imager; OBI, Varian Medical Systems, USA)의 kV X선 영상 및 전자조사문 영상(Electric portal image), 디지털화재구성 영상(Digitally reconstructed radiographic image) 내 관심지점에서의 기하학적 일치성 여부를 평가하고자 하였다. 이를 위해 상용화된 IMRT 팬텀(Wellhofer, Germany)에 포함된 부품의 윗면 및 왼쪽면에 각 3개씩 총 6개의 볼베어링을 부착하여 위치 확인용 팬텀을 제작하였다. 각 영상의 획득을 위해 팬텀을 전자조사문 영상을 이용하여 각 방향에서 정확하게 중심이 일치하도록 셋업한 후 직교하는 kV X선 영상 및 전자조사문 영상을 획득하여 각 볼베어링의 절대 위치를 비교하였다. 또한 2차원-2차원 정합 후 보정 결과의 정확성 평가를 위하여 팬텀의 중심을 정확히 회전중심점에 일치시킨 후 갠트리 각도 $315^{\circ}$에서 획득된 전자조사문 영상을 기준 영상으로 한 뒤 카우치를 각 방향으로 $-5{\sim}5\;mm$ 임의로 이동한 후 OBI에서 지원하는 2차원-2차원 정합 기능을 이용하여 셋업 오차를 보정하도록 하였다. 이후 다시 갠트리 각도 $315^{\circ}$에서 전자조사문 영상을 획득한 후 기준 전자조사문 영상과 비교하였으며 이 과정을 10회 반복하였다. 연구 결과 kV X선 영상과 전자조사문 영상, 디지털화재구성 영상 간 볼베어링 중심의 위치는 1 mm 이내에서 일치하였으며 실제 위치와도 1 mm 이내에서 일치하였다. 또한 기준 전자조사문 영상과 2차원-2차원 정합 후 획득된 전자조사문 영상 내 각 볼베어링의 위치들도 1 mm 이내에서 일치하였다. 본 연구를 통해 OBI의 kV X선 영상이 기하학적으로도 전자조사문 영상과 동일함을 확인할 수 있었다

  • 목적: 이 연구의 목적은 3D FDG-PET 뇌영상의 정량적 정확도와 임상적 유용성을 연구하는 것이다. 대상 및 방법: 24명의 환자에게 약 370 MBq의 FDG를 주사하고 섭취되기까지 30분을 대기한 후 GE $Advance^{TM}$ PET을 사용하여 30분간의 2D 영상과 10분간의 3D 영상을 얻었다. 백질, 회백질, 병변, 정상조직의 관심영역 분석을 통하여 영상대비와 FDG 섭취비를 구하여 비교하였다. 3D PET의 효율을 최대화하기 위하여 투과스캔을 사용한 경우와 사용하지 않은 경우의 감쇠보정 효과를 비교하였다. 결과 3D 영상의 영상대비는 2D 영상이 1 일 때 산란보정을 실행한 경우 회백질 대 백질에서 $0.95{\pm}0.12$, 정상 대 병변에서 $0.96{\pm}0.05$이었다. 2D 영상의 FDG 섭취비가 1일 때 3D 영상의 FDG 섭취비는 산란보정을 실행한 경우 $1.02{\pm}0.08$이었다. 3D 영상은 영상대비, 분해능, 영상 잡음에 대한 순위 척도평가에서 각각 81%, 83%, 81%에 해당되는 등급을 받아 선호도가 우수하였다. 3D 영상에서 계산감쇠보정 방법은 측정감쇠보정 방법에 필적하는 결과를 얻었다. 결론: 산란보정과 계산감쇠보정을 실시한 3D 영상은 일반적인 2D 영상과 비교하여 정량적으로 정확한 결과를 나타냈으며 정성적으로 유용하였다. 3D 영상은 일상 임상환경에 충분히 적용할 수 있으며 환자 스캔 시간을 단축하고 방사능 피폭량을 감소시키는 장점이 있었다.

횡격막의 움직임을 이용한 최적화된 호흡 위상의 선택: 폐암의 호흡 동기 방사선치료 중심 (The Optimum of Respiratory Phase Using the Motion Range of the Diaphragm: Focus on Respiratory Gated Radiotherapy of Lung Cancer)

  • 본 연구는 각 호흡 위상에 따른 계획용표적체적(planning target volume. PTV)의 움직임 및 체적(PTV volume)의 변화를 횡격막(diaphragm)의 움직임을 이용하여 정량적으로 분석함으로써 폐암의 호흡 동기 방사선치료를 위한 최적화된 호흡 위상을 알아보고자 하였다. 비특이적 호흡이나 불규칙적인 호흡에 의한 체계적 오류(system error)를 화하기 위하여 모의 호흡 훈련을 시행하였다. 정규화된 호흡 동기 방사선치료 절차에 따라 각 호흡 위상 i에 따른 4차원 전산화치료계획(4-dimensional computed tomography. 4DCTi)을 시행하였으며 0~90%, 30~70%, 40~60% 호흡 위상으로 재구성된 4DCTi 영상에서 PTV를 정의하고 PTVi의 움직임 및 체적의 변화를 정략적으로 분석하였다. 모의호흡 훈련에 의한 평균 호흡 주기는 $3.4{\pm}0.5$초로 나타났으며 임상적으로 유도되는 예상 값과 실제 측정값의 일치 정도를 나타내는 R-제곱 값은 1에 근접하여 유의하였다. 또한 각 호흡 위상 i에 따른 PTVi의 움직임은 0~90% 호흡 위상의 경우 $13.4{\pm}6.4mm$, 30~70% 호흡 위상의 경우 $6.1{\pm}2.9mm$, 40~60% 호흡 위상의 경우 $4.0{\pm}2.1mm$ 이었으며 PTVi의 체적 변화는 30~70% 호흡 위상의 경우 $32.6{\pm}8.7%$, 40~60% 호흡 위상의 경우 $41.6{\pm}6.2%$ 감소되었다. 결론적으로 짧은 호흡 위상(40~60%: 30% duty cycle) 폭을 적용하였을 때 PTV의 움직임 및 체적의 변화가 감소되어 호흡을 고려한 PTV 마진이 4mm 이내이면서 PTV 내 선량의 균일성을 얻을 수 있었다.

선속 폭(Field Width) 변화에 따른 종축선량 분석 (Analysis on Longitudinal Dose according to Change of Field Width)

  • 목 적: 호흡에 의하여 영향을 받는 장기의 토모 치료 시 선속 폭(Field Width) 변화에 따른 종양 용적선량의 선량 전달 정확성을 분석하고 자체 제작한 Moving car를 사용하여 선량 변화의 차이를 확인하고 실질적인 종양 선량의 전달 선량을 평가해 보고자 한다. 대상 및 방법: 자체 제작한 Moving car를 사용하여 종축 움직임 차이(0.0 cm, 1.0 cm, 1.5 cm, 2.0 cm)를 적용하고 선속 폭(Field Width) 변화(1.05 cm, 2.50 cm, 5.02 cm)에 따른 측정된 선량의 값을 계산된 선량 값과 비교하여 오차범위를 파악하였다. Gafchromic EBT 필름을 이용하여 측정한 DQA (Delivery Quality Assurance) 필름의 감광정도를 선량윤곽(Dose Profile)과 Gamma Histogram을 이용하여 비교 분석하였다. 결 과: 선속 폭(Field Width) 1.05 cm, 2.50 cm, 5.02 cm일 때 오차범위가 각각 -2.00%, -0.39%, -2.55%의 선량 전달율을 나타내었다. Gafchromic EBT 필름 분석의 선량 윤곽(Dose profile)에서는 Moving car의 움직임이 있는 상태에서 종축방향으로의 움직임이 클수록, 선속 폭(Field Width)이 좁아질수록 고선량 부분에서 계산된 선량과의 오차가 크게 나타났다. Gamma histogram에서는 gamma index가 선속 폭(Field Width)이 좁을수록 움직임의 영향이 상당히 증가하는 것을 알 수 있었다. 결 론: 자체 제작한 Moving car를 이용하여 움직이는 장기의 종축선량 차이를 확인할 수 있었으며 토모 치료 시 선속 폭(Field Width)을 작게 하고 호흡에 의한 장기의 움직임을 최소화하기 위하여 호흡조절 장치와 고정기구의 개발이 필요하리라 사료된다.

초음파를 이용한 견관절 관절강내 주사의 정확성과 임상적 결과: 유착성 관절낭염 환자의 견봉 쇄골 접근법 (Accuracy and Clinical Outcomes of Ultrasound-guided Glenohumeral joint Injection: Acromioclavicular Approach in Patients with Adhesive Capsulitis)

  • 목적: 유착성 관절낭염 환자에서 초음파를 이용한 관절와 상완 관절강 내 스테로이드 주사를 시행한 후 주사요법의 정확성이 임상적 결과에 미치는 영향을 평가하고자 하였다. 대상 및 방법: 임상적으로 유착성 관절낭염으로 진단받은 환자들 중 외상이 없고, 약물 치료 및 물리 치료를 받았으나 만족할 만한 호전을 보이지 않은 환자를 대상으로 하였다. 환자들은 총 33명으로 남자 15명, 여자 18명으로 평균 연령은 55.1세(42~72세)였다. 이를 대상으로 견봉 쇄골 접근법을 통해 스테로이드 칵테일 10 ml(triamcinolone 1 ml(40 mg/ml), lidocaine 4 ml(2%), normal saline 5 ml)와 조영제 5 ml ($Telebrix^{(R)}$, Guerbet, France)를 주입하였으며, 이후 바로 방사선 촬영을 시행하여 주사의 견관절 관절강 내 주입 여부를 확인하였다. 또한 조영제가 관절강내로 정확히 들어간 경우, 부분적으로 들어간 경우 그리고 조영제가 관절강 바깥으로 들어간 경우로 3군으로 분류하여 각각의 군에서 주사요법 전, 후 수동적 운동범위 및 VAS score를 이용한 통증 완화 정도를 분석하였다. 결과: 총 33례 중 25례(76%)에서 조영제가 정확히 관절강 내로 주입되었음을 확인하였다. 6례(18%)에서는 관절강에 부분적으로 조영제가 관찰되었고, 2례(6%)에서는 조영제가 관절강 바깥에서 관찰되었다. 조영제가 관절강내로 정확히 들어간 군은 시행 전 굴곡은 평균 111도(80~140도), 외회전은 48도(0~90도)였고, 시술 후 굴곡은 평균 134도(90~150도), 외회전은 70도(30~90도)로 호전되었다(p<0.01). 그리고 조영제가 관절강내로 부분적으로 들어간 군은 시행 전 굴곡은 평균 120도(90~150도), 외회전은 70도(10~90도)였고, 시술 후 굴곡은 평균 139(135~140도)도, 외회전은 78도(50~90도)로 호전된 결과가 관찰되었다(p<0.01). 조영제가 관절강 바깥으로 들어간 군은 환자수의 부족으로 통계적으로 의미를 갖지 않았다. 시술 전, 후 VAS 점수의 변화는 조영제가 관절강 내로 정확히 들어간 군은 7.1점(3~9점)에서 2.6점(0~5점)으로 호전되었고(p<0.01), 부분적으로 들어간 군은 7.5점(7~9점)에서 3.3점(2~4점)으로 호전되었고(p<0.01), 관절강 바깥으로 들어간 군은 7.5점에서 2점으로 호전되었다. 또한 당뇨 환자군과 비 당뇨 환자군을 비교한 결과 임상적 결과에 통계적으로 유의한 차이가 없이 모두 호전되었다. 결론: 유착성 관절낭염 환자에서 초음파 하에 견봉 쇄골 접근법을 통한 견관절 관절강 내 스테로이드 주사는 기존의 방법들보다 좋은 정확도(94%)를 보고하였으며, 이 중 관절강 바깥으로 새어나간 군과 새어나가지 않은 군 간에는 임상적으로 유의한 차이가 발견되지 않았다.
