Background/Aims There has been major progress in our understanding of the irritable bowel syndrome (IBS), and novel treatment classes have emerged. The Rome IV guidelines were published in 2016 and together with the growing body of Asian data on IBS, we felt it is timely to update the Asian IBS Consensus. Methods Key opinion leaders from Asian countries were organized into 4 teams to review 4 themes: symptoms and epidemiology, pathophysiology, diagnosis and investigations, and lifestyle modifications and treatments. The consensus development process was carried out by using a modified Delphi method. Results Thirty-seven statements were developed. Asian data substantiate the current global viewpoint that IBS is a disorder of gut-brain interaction. Socio-cultural and environmental factors in Asia appear to influence the greater overlap between IBS and upper gastrointestinal symptoms. New classes of treatments comprising low fermentable oligo-, di-, monosacharides, and polyols diet, probiotics, non-absorbable antibiotics, and secretagogues have good evidence base for their efficacy. Conclusions Our consensus is that all patients with functional gastrointestinal disorders should be evaluated comprehensively with a view to holistic management. Physicians should be encouraged to take a positive attitude to the treatment outcomes for IBS patients.
Internuclear Ophthalmoplegia (INO) is characterized by lesion side eye's adduction limitation in lateral horizontal gaze and nystagmus in the abducting eye due to lesions in the Medial Longitudinal Fasciculus (MLF). Brainstem infarction is one of the causes of INO, but related issues have seldom been reported in the Korean medical literature. The present case was a pontine infarction patient with INO who complained of diplopia and eye movement disorder. The patient was treated with Korean medicine treatment, including herbal medicine, acupuncture, cupping, and moxibustion for 30 days. Her changes in clinical symptoms were measured with a Numeric Rating Scale (NRS) and a length of External Ocular Movement (EOM), in which the patient's right eyeball moved to the left from the midline in the left gaze. After treatment, her clinical symptoms were improved. Diplopia decreased from NRS 10 to NRS 2, and eye movement disorder was improved such that EOM increased from 1mm to 5 mm. Therefore, integrative Korean medicine treatment may be effective in the treatment of INO patients caused by pontine infarction.
Primary cilia, single hair-like appendage on the surface of the most mammalian cells, were once considered to be vestigial cellular organelles for a past century because of their tiny structure and unknown function. Although they lack ancestral motility function of cilia or flagella, they share common ground with multiciliated motile cilia and flagella on internal structure such as microtubule based nine outer doublets nucleated from the base of mother centrioles called basal body. Making cilia, ciliogenesis, in cells depends on the cell cycle stage due to reuse of centrioles for cell division forming mitotic spindle pole (M phase) and assembling cilia from basal body (starting G1 phase and maintaining most of interphase). Ciliary assembly required two conflicting processes such as assembly and disassembly and balance between these two processes determines the length of cilia. Both process required highly conserved transport system to supply needed substance to grow tip of cilia and bring ciliary turnover product back to the base of cilia using motor protein, kinesin and dynein, and transport protein complex, IFT particles. Disruption of ciliary structure or function causes multiple human disorder called ciliopathies affecting disease of diverse ciliated tissues ranging from eye, kidney, respiratory tract and brain. Recent explosion of research on the primary cilia and their involvement on animal development and disease attracts scientific interest on how extensively the function of cilia related to specific cell physiology and signaling pathway. In this review, I introduce general features of primary cilia and recent progress in understanding of the ciliary length control and signaling pathways transduced through primary cilia in vertebrates.
Objectives : The aim of this study was to investigate the relationship between headache and gastric dysmotility in patients with functional dyspepsia by using electrogastrogram and bowel sounds analysis. Methods : 127 patients(male 40, female 87) with functional dyspepsia were enrolled. By using inquiry and the questionnaire, we examined whether each patient had headache and abdominal pain. All patients were divided into 4 groups according to the above symptoms. Gastric motility was measured during the fasting-postprandial state by using electrogastrogram and bowel sounds analysis. Results : 33 patients complained of headache. There was a significant difference between headache patients and non-headache patients in gastric arrhythmia of the fasting state. Moreover, in headache patients without abdominal pain, postprandial improvement of gastric arrhythmia was poorer than the other groups. So, headache patients without abnormal pain had more severe gastric myoelectrical rhythm disorder than headache patients with abdominal pain. Conclusion : Headache could be one of symptoms induced by gastric dysmotility in patients with functional dyspepsia. Especially, headache in functional dyspepsia was related to gastric arrhythmia.
IBS(Irritable bowel syndrome) means disorder of intestinal motility. To increase the cure rate of IBS, we have observed and analysed 26 persons who have administered Ansinbobitang(安神補脾湯), Ansinbobitang(安神補脾湯) consists of herb medicines which have tranquilizational(安神), reinforcing the spleen(補脾), and tonifying the kidney's Yang(補腎陽) effects. From this study we obtained as follows. 1. 16 cases of IBS patients were male and 10 cases were female. Male of female ratio was 1 : 1.6, and the peak age in incidence of IBS was in End and 3rd decade. 2. Duration of IBS attack, the most frequence is $0{\sim}1$ year and $3{\sim}5$ years. 3. Seasonal distribution of IBS attack reveals the peak occurrence in autumn. 4. Duration of treatments, the most frequence is $2{\sim}3$ weeks. 5. Ansinbobitang(安神補脾湯) shows 88.5% of effective rate.
위전도는 피부전극을 이용하여 위에서 발생하는 전기신호를 측정하여 위운동상태를 측정하는 비관혈적인 검사방법이다. 본 연구에서는 임상적으로 유용한 위전도신호 측정 및 분석시스템을 개발하였으며, 개발시스템은 크게 하드웨어(생체증폭기, 필터)와 소프트웨어(유저인터페이스, 분석알고리듬, 환자데이터베이스)로 구성된다. 개발시스템의 생체신호증폭기는 신호분석에 용이하도록 3채널로 구성되어 있으며, 신호의 저장 및 분석은 PC에서 수행하도록 구성되어 있다. 위수축 정보의 획득을 위해서 분석신호는 16Hz로 샘플링하였으며, 위전도신호분석은 임상이용에 유용한 여러 가지 분석 파라메터를 추출할 수 있도록 하였다. 개발된 시스템은 동물실험을 통하여 성능을 평가하였으며, 현재 환자를 대상으로 임상시험이 진행중이다.
Diabetic gastroparesis is a disorder in a diabetic of delayed gastric emptying in the absence of mechanical obstruction. It is estimated that about 20%-50% of diabetics suffer from gastroparesis. Clinical Sympoms include early satiety, bloating, nausea, anorexia, vomiting, abdominal pain, and weight loss. Severe gastroparesis might result in recurrent hospitalization, malnutrition, and even death, but, no exact treatment has yet been established. Electrogastrography has been known to be a simple, non-invasive, and effective method in assessing gastric motility and EGG has been used to diagnosis diabetic gastroparesis and to estimate its prognosis. Recently, two cases of diabetic gastroparesis were observed. For this cases, manual acupucture, electroacupucture, lumbar skin warming and herb medicine were applied. After application of these therapies, gastrointestinal symptoms improved and these therapeutic effects were confirmed in EGG. Moreover, blood glucose control improved, so patients were able to discontinue insulin injection and change to p.o.medication. In light of this encouraging application of oriental medicine, this is reported along with investigation of the literature.
Objectives: This case study reports the effectiveness of Korean medical treatment in a patient with cyclic vomiting syndrome. Methods: A 29-year-old female Korean patient with cyclic vomiting syndrome received acupuncture, electroacupuncture, herbal medicine, and moxibustion for 2 weeks in hospital. Changes in symptoms were evaluated using the Gastrointestinal Symptom Rating Scale (GSRS), Visual Analog Scale (VAS), Index for Nausea, Vomiting, and Retching (INVR), Nausea Severity Scale (NSS), Nepean Dyspepsia Index-Korean version (NDI-K), quality of life using the Functional Dyspepsia Related Quality of Life questionnaire (FD-QoL), and gastric motility using electrogastrography (EGG). Results: Post-treatment, the patient showed high satisfaction and improvement in symptoms of nausea and vomiting. The following changes were observed in scores: GSRS: 23 to 19; VAS of nausea: 88 to 95; VAS of dyspepsia: 95 to 12; INVR: 13 to 1; NSS: 17 to 5; NDI-K: 107 to 78; and FD-QoL 84 to 27. We also found positive results in Channel 3 of EGG parameters, implying the improvement of gastric motility disorder. Conclusion: Korean medical treatment can be a therapeutic option for cyclic vomiting syndrome.
뇌혈관 손상으로 인해 발생하는 신경학적 질환인 뇌졸중과 관련이 있는 편측무시(Unilateral neglect)는 대뇌반구의 반대측 자극에 대하여 인지 또는 반응하지 못하는 증상으로, 주로 우측 대뇌반구 손상에서 나타난다. 본 논문에서는 편측무시에 대한 행동적 평가 척도로 사용되고 있는 한국형 캐서린 버지고 척도(Korean Catherine Bergego Scale)의 10가지 문항과 내용을 기반으로 가상현실(Virtual Reality)을 이용한 콘텐츠를 구현하였다. 구현된 콘텐츠는 편측무시 환자의 증상으로 나타나는 신체 무시, 운동성 무시, 시각 및 공간 무시, 청각 주의 등을 평가할 수 있으며, 시각적으로 인지하고 반응하는 동안의 시간과 머리 회전각을 측정함으로써 질환의 진행 정도를 정량적으로 확인하였다. 이 방법을 이용하여 편리한 편측무시 진단과 이를 응용한 콘텐츠를 활용하여 재활 훈련에 유용하게 사용될 수 있을 것으로 기대된다.
Congenital central hypoventilation syndrome은 혈중 이산화탄소 증가와 저산소에 대한 자율 신경계와 호흡 조절 기능의 선천성 결함으로 호흡의 저환기가 주로 수면시에 발생하는 질환이다. 이는 신경 이주장애 질환(neurocristopathy)에 속한다고 알려져 있으며 선천성 거대결장 등의 질환과 잘 동반된다. 아직까지 확실한 완치법은 없는 상태이고 환아들은 평생을 환기 보조에 의존하여 생존해야 하며 적절한 환기 보조를 통해서 생존 기간을 연장할 수 있다. 저자들은 출생시부터 반복되는 수면시의 무호흡과 청색증이 있는 환아에서 congenital central hypoventilation 및 선천성 거대 결장이 동반된 1례를 경험하였기에 보고하는 바이다.
