Morphologic Evidence of Anti-Tumor Specificity of T Cells Activated by Denritic Cells Derived from Peripheral Blood Mononuclear Cells of Thyroid Cancer Patients

  • Recent studies suggest that immunization with autologous dendritic cells (DCs) results in protective immunity and rejection of established tumors in various human malignancies. The purpose of this study is to determine whether DCs are generated from peripheral blood mononuclear cells (PBMNs) by using cytokines such as F1t-3 ligand (FL), granulocyte macrophage-colony stimulating factor (GM-CSF), IL-4, and TNF-${\alpha}$, and whether cytotoxic T cells activated against the thyroid cancer tissues by the DCs. Peripheral blood was obtained from 2 patients with thyroid cancer. DCs were established from PBMNs by culturing in the presence of FL, GM-CSF, IL-4, and TNF-${\alpha}$ for 14 days. At day 14, the differentiated DCs was analyzed morphologically. The immunophenotypic features of DCs such as CDla, CD83, and CD86 were analyzed by immunofluorelescence microscopy. At day 18, DCs and T cells were incubated with thyroid cancer tissues or normal thyroid tissues for additional 4 days, respectively. DCs generated from the PBMNs showed the typical morphology of DCs. Activated cytotoxic T lymphocytes (CTLs) were observed also. DCs and the CTLs were attached to the cancer tissues on scanning electron microscope. The DCs activated the CTLs, which able to specifically attack the thyroid cancer. This study provides morphologic evidence that the coculture of T cells/cancer tissues activated the T cells and differentiated CTLs. The CTLs tightly adhered to cancer tissues and lysed cancer tissues vigorously. Therefore DCs could be used as potential vaccines in the immunotherapy.

A Study on Progressive Sampling with Distinct Morphologic Features (지성선을 이용한 밀도증가식 표본추출법에 관한 연구)

  • Digital Terrain Model is composed of two basic elements, namely, sampling and interpolation. They represent the procurement of data and the geometric reconstruction of terrain relief respectively. For evaluation DTM data, following two paremeters appear to be the most important factor, that is, the accuracy and efficiency and, in particular, the break point information significantly affect to the accuracy of DTM data. The main objective of this study is to improve the accuracy and efficiency of DTM by applying Progressive Sampling with distinct morphologic information. In this study, the total of 240 individual numerical tests has been implemented and the appropriate computer program is also developed for the test. The result of investigation shows that the Progressive Sampling with break point information improves the accuracy of DTM by 30 percent approximately.

Fine Needle Aspiration Cytological Findings of Angioimmunoblastic T-cell Lymphoma -A Brief Case Report - (혈관면역모세포 T세포 림프종의 세침흡인 세포소견 -짧은 증례 보고-)

  • Recently, we experienced a case of Angioimmunoblastic T-cell lymphom (AITL) in a 60-year-old man presented with lymphadenopathy, which is first cytological report in Korea. The cytological features showed a heterogeneous population of small to medium-sized lymphocytes, immunoblasts, and plasma cells. Characteristically, there were also a distinct population of follicular dendritic cells admixed with lymphoid cells, forming the so-called dendritic cell-lymphocyte complexes. Histological features showed the classic morphologic features of AITL. Recognition of the characteristic cytological features can suggest the possibility of AITL.

Cytologic Feature of Papillary Carcinoma of the Thyroid (세침흡인 검사로 진단된 갑상선 유두상 암종의 세포학적 소견)

  • Fine needle aspiration biopsy cytology (FNA) is an important diagnostic tool in the management of thyroid nodule. Especially, papillary carcinoma of the thyroid has distinct morphologic features that allow a definite cytologic diagnosis with high degree of accuracy. We examined the characteristic cytologic features of 57 cases of papillary carcinoma of the thyroid, and their frequency and diagnostic significance were evaluated. The results obtained are summerized as follows; 1. In pattern of cellular arrangement, papillary structure with or without stroma is predominant feature (96%). 2. In individual cell morphology, grooved nuclei (95%), intranuclear cytoplasmic invagination (89%) and nuclear lobulation (74%) are most frequent and important cytologic findings. 3. Chromatin pattern is usually fine. Coarse chromatin is infrequent finding (37%). Nucleoli are inconspicuous. Cytoplasm us plump and distinctly eosinophilic. 4. Psammoma bodies are identified only in 4 cases (7%), but they are considered as helpful diagnostic features. 5. There are other associated findings including multinucleated giant cells (51%), macrophages (37%) and cystic degeneration (16%).

Efficiacy of CT Colonography in the Detection of Colorectal Polypoid Lesions (대장의 폴립양 병변의 발견에 있어 CT 대장조영술의 유용성)

Purpose: We wished to compare CT colonography with conventional colonoscopy for the detection of colorectal poiypoid lesions, and we wanted to evaluate the role of IV contrast-enhanced CT colonography for the differentiation between benign polypoid lesions and malignant polypoid lesions. Materials and Methods: Thirty-four consecutive patients underwent CT colonography prior to conventional colonoscopy, Precontrast prone-position CT images and post contrast supine position CT images were obtained and the virtual colonoscopic images were reconstructed, Axial, sagittal and presence, size and morphologic features of colorectal polypoid lesions, and thor these findings were compared with the colonoscopic findings. The degree of enhancement of colorecralpolypoid lesions was measured by subtracting the attenuation valves obtained with precontrastand postcontrast CT images for the differentiation of benignity and malignancy of the colorectal polypoid lesions. Results. Among 75 colorectal polypoid lesions identified on conventional colonoscopy, 49neoplasms were found on CT coloaographv, and the overall detection rate was 65,3%.Detection rate of lesions smaller than l0mm was 52.1%(24/46), and the detection rate for lesions equal to or larger than 10mm was 86.2%(25/29), Morphologic features of the sessile type lesions on CT colonography were well correlated with those noted on colonoscopy, but the stalks were not identified in 6 of 13 polyps on CT colonography. There was no statistical correlation between benignity and malignancy and the degree of contrast enhancement on CT colonography, Conclusion CT colonography is a useful modality for the detection of colorectal polypoid lesionsequal to or polyps. However, CT colonography cannot differentiate benignity from malignancy.

Diagnostic Imaging Features of Asymptomatic Extrahepatic Portosystemic Shunt Detected by CT in Dogs (개에서 컴퓨터단층촬영술을 이용하여 진단한 임상증상이 없는 간외성 전신문맥단락의 영상학적 평가)

  • This study was performed to compare clinical and diagnostic imaging features between asymptomatic and symptomatic extrahepatic portosystemic shunts in dogs. The data of thirty patients diagnosed with extrahepatic PSS by multi-detector CT were reviewed, and the dogs were divided into asymptomatic (9/30) and symptomatic (21/30) groups. Signalments, hematologic results, liver size, morphologic classifications and main portal vein to abdominal aortic ratio (PV/AO) at the porta hepatis level from CT images were evaluated in two groups. Shih-tzu (5/9) was the most frequent breed in asymptomatic group, and various breeds were presented in symptomatic group. Mean age of asymptomatic group ($9.2{\pm}3.2$ years) was significantly higher than that of symptomatic group ($4.5{\pm}3.2$ years). The most morphologic form of shunt vessel was the splenophrenic shunt (16/30). PV/AO of asymptomatic group ($1.1{\pm}0.19$) was significantly higher than the values of symptomatic group ($0.55{\pm}0.19$). Clinical signs, hematologic results and diagnostic imaging findings of asymptomatic PSS are too nonspecific to suspect PSS. Therefore, considering of patient's age and CT examination with application of PV/AO ratio could be useful for the diagnosis of asymptomatic PSS.

Morphologic and Genetic Identification of Diphyllobothrium nihonkaiense in Korea

  • Diphyllobothrium nihonkaiense was first described by Yamane in 1986 but the taxonomical features have been obscure due to lack of critical morphologic criteria in its larval and adult stages. In Korea, this tapeworm had long been known as Diphyllobothrium latum. In this study, we observed 62 specimens collected from Korean residents and analyzed them by morphological features and nucleotide sequences of mitochondrial cox1 gene as well as the ITS1 region. Adult tapeworms were examined after carmine or trichrome stain. Longitudinal sections of the gravid proglottids showed an obtuse angle of about 150 degree between the cirrus sac and seminal vesicle. This angle is known as a major differential point compared with that of D. latum. Nucleotide sequence differences between D. latum and the specimens from Koreans represented 17.3% in mitochondrial DNA cox1 gene. Sequence divergence of ITS1 among 4 Korean isolates was 0.3% and similarity was 99.7% with D. nihonkaiense and D. klebanovskii. All of the Korean specimens analyzed in this study were identified as being D. nihonkaiense (n = 62). We propose its Korean name as "Dong-hae-gin-chon-chung" which means 'long tapeworm of the East Sea' for this newly analyzed diphyllobothriid tapeworm in Korea.

CT Image Analysis of Hepatic Lesions Using CAD ; Fractal Texture Analysis

  • We investigated whether the CT images of hepatic lesions could be analyzed by computer-aided diagnosis (CAD) tool. We retrospectively reanalyzed 14 liver CT images (10 hepatocellular cancers and 4 benign liver lesions; patients who presented with hepatic masses). The hepatic lesions on CT were segmented by rectangular ROI technique and the morphologic features were extracted and quantitated using fractal texture analysis. The contrast enhancement of hepatic lesions was also quantified and added to the differential diagnosis. The best discriminating function combining the textural features and the values of contrast enhancement of the lesions was created using linear discriminant analysis. Textural feature analysis showed moderate accuracy in the differential diagnosis of hepatic lesions, but statistically insignificant. Combining textural analysis and contrast enhancement value resulted in improved diagnostic accuracy, but further studies are needed.

Metastatic Adenoid Cystic Carcinoma of the Lung Diagnosed by Fine Needle Aspiration Biopsy (세침 흡인 생검으로 진단된 폐의 전이성 선양 낭포암종 1예 보고)

  • A case of metastatic adenoid cystic carcinoma of the lung, originated from the trachea, was diagnosed by fine needle aspiration. Although the cytologic features of adenoid cystic carcinoma have been well described, it is easy to confuse adenoid cystic carcinoma with more common primary small cell neoplasms of the lung, i.e., small cell carcinoma, well differentiated adenocarcinoma, and carcinoid tumor of the lung. The features distinguishing adenoid cystic carcinoma from these neoplasms include 1) tight, globular, honeycomb pattern of cells, 2) acellualr basement membrane material in the lumen, and 3) cells lacking true nuclear melding and having bland chromatin pattern. The morphologic feature of metastatic adenoid cystic carcinoma in this case was so distinctive as to permit a definite diagnosis by aspiration cytology.

Neurilemmoma in the floor of the mouth: a case report

  • Neurilemmomas are well-encapsulated, benign, slow-growing tumors originating from Schwann cells of the nerve sheath surrounding cranial, peripheral, or autonomic nerves. Intraoral neurilemmomas are relatively rare and have a wide variety of morphologic and radiologic features. This makes differential diagnosis difficult, and only histopathological features can lead to a definitive neurilemmoma diagnosis. In this report, we present the case of a 30-year-old woman whose chief complaint was a solitary, nodular mass on the right floor of the mouth. After computed tomography and magnetic resonance imaging, we performed an incisional biopsy that showed the typical characteristics of a neurilemmoma. The mass was removed completely through an intraoral surgical approach. Despite losing a portion of the lingual nerve, the patient did not complain of any specific discomfort. Wound healing was uneventful and there were no signs or symptoms of recurrence.
