Drug selection for sedation and general anesthesia in children undergoing ambulatory magnetic resonance imaging

  • The demand for drug-induced sedation for magnetic resonance imaging (MRI) scans have substantially increased in response to increases in MRI utilization and growing interest in anxiety in children. Understanding the pharmacologic options for deep sedation and general anesthesia in an MRI environment is essential to achieve immobility for the successful completion of the procedure and ensure rapid and safe discharge of children undergoing ambulatory MRI. For painless diagnostic MRI, a single sedative/anesthetic agent without analgesia is safer than a combination of multiple sedatives. The traditional drugs, such as chloral hydrate, pentobarbital, midazolam, and ketamine, are still used due to the ease of administration despite low sedation success rate, prolonged recovery, and significant adverse events. Currently, dexmedetomidine, with respiratory drive preservation, and propofol, with high effectiveness and rapid recovery, are preferred for children undergoing ambulatory MRI. General anesthesia using propofol or sevoflurane can also provide predictable rapid time to readiness and scan times in infants or children with comorbidities. The selection of appropriate drugs as well as sufficient monitoring equipment are vital for effective and safe sedation and anesthesia for ambulatory pediatric MRI.

  • 달에 떨어지는 유성체(lunar meteoroid)는 그대로 표면에 충돌하여 섬광(flash)을 일으킨다. 이 현상은 매우 희미하고 순간적이지만 고감도 비디오카메라를 이용하면 지상 관측이 가능하다고 알려져 있다. 2013년 10월에 발사된 NASA의 Lunar Atmosphere and Dust Environment Explorer(LADEE)가 달 주위의 대기 및 먼지 환경을 측정하고 있는 동안 전 세계 지상관측 네트워크도 달 표면 충돌 감시 관측을 수행 중에 있다. 충남대학교에서도 LADEE 미션 시작인 10월부터 16인치 망원경에 고감도 비디오카메라를 장착한 시스템을 구성하여 매달 초승부터 상현까지 관측을 진행해왔다. 관측은 달 표면의 어두운 영역을 초당 30프레임으로 녹화하였으며, NASA에서 제공한 LunarScan 소프트웨어를 사용하여 섬광을 찾는 분석 작업을 수행하였다. 현재까지 약 70시간 동안 관측하고 분석하여 충돌 섬광 후보를 발견하였다. 본 발표에서 달 충돌 섬광 관측시스템에 대해 소개하고, 관측된 충돌 섬광 후보의 분석 결과를 제시할 것이다.

Healthy Assessment of Generator Stator Cores using EL-CID (ELectromagnetic Core Imperfection Detector) (EL-CID를 이용한 발전기 고정자 철심의 건전성 평가)

  • The ELectromagnetic Core Imperfection Detector (EL-CID) test was performed on a small generator in the laboratory and a gas turbine generator in the field to assess the fault condition of generator stator core. Artificial defects with six different sizes were introduced in the small generator. The scan results on six defects show a very large increase in the magnitude of fault current compared to that obtained with a healthy core. After the stator core heats up, a thermal imaging camera was used to detect hot spot on the inner surface of the core for comparison. Several faults were found during inspection of the gas turbine generator with the EL-CID. It has been shown that the existence of a fault can be determined by monitoring the magnitude of fault current.

  • A switch is a component with two or more ports that selectively transmits, redirects, or blocks optical power in a fiber transmission line. Our switch uses rotation mechanism using stepping motor, hence the common optical fiber can scan and allign to one of the arrayed N optical fibers to provide optical path by electronic precise control. The developed switch is consisted of switching module and its control module. The performance parameters of a switching loss and a repeatability are considered very important. We performed the study to reduce the switching loss and improve the repeatability of switch. The switch can be widely used as a test instrument of optical device and of optical cable in factory, also of optical cable monitoring systems.

  • 원자력교육원 2회기 기준발전소인 영광 1호기 발전소의 디지털 경보설비 (IAS : Intelligent Annunciator System) 는 MPU, SSU, ACU 및 ADU로 구성되어 있으며, 특히 MPU에서는 SSU 로부터 수신된 Scan Data를 해석하고 경보를 저장하고, 경보의 내용을 프린터 및 CRT를 비롯하여 다양한 형태로 병렬출력이 가능하도록 하고 있다. 본 논문에서는 전력연구원에서 수행중인 '원자력 교육원 시뮬레이터 성능개선' 과제의 일부로 IAS의 구조와 이를 시뮬레이터 주컴퓨터 (Host Computer)상의 다이나믹 모델과 연동시켜 구현하는 방법론을 다루고 있다. 특히, 시뮬레이터에 대한 규제기관의 인허가가 중요한 현안으로 대두될 전망임에 따라 ANSI/ANS-3.5, Regulatory Guide 1.149 등 규제요건에서 제시하는 성능기준 및 검사기준을 만족하도록 설계함으로 향후 규제기관에 대한 인허가 획득에서 대비할 예정이다.

Case Report of Prostate Cancer Patient with Only Lymph Node Involvement on F-18 FDG PET/CT

  • We report a case of a patient with locally advanced prostate cancer who had only lymph node involvement without bone metastasis on F-18 FDG PET/CT. A 62-year-old Korean male was admitted to our hospital due to dysuria. His PSA level on admission was 79.35 ng/mL. A transrectal ultrasound-guided prostate biopsy confirmed prostate cancer and his Gleason score was 10 (5+5). F-18 FDG PET/CT demonstrated a hypermetabolic mass lesion with SUVmax 7.0 in the prostate and hypermetabolism with SUVmax 4.7 of the abdominal and pelvic lymph nodes. Tc-99m HDP bone scan showed no significant bone metastasis. The patient underwent hormonal therapy for 9 months. Follow-up F-18 FDG PET/CT showed significantly reduced size and FDG uptake in the prostate and abdominal and pelvic lymph nodes. In this case, treatment monitoring with F-18 FDG PET/CT showed decreased mass size and FDG uptake in the prostate and abdominal and pelvic lymph nodes.

Power Doppler ultrasound-guided sialography using the phenomenon of increased blood flow: A technical report

  • Purpose: This report presents a procedure for performing power Doppler ultrasound-guided sialography using the phenomenon of increased blood flow and illustrates its application to practical patient cases. Materials and Methods: The salivary gland was scanned using ultrasound equipment (GE LOGIQ5 Expert® device; GE Medical Systems, Milwaukee, WI, USA) to identify pathological findings related to the patient's chief complaint. To identify the orifice of the main duct, it should be cannulated using a lacrimal dilator. After inserting the catheter into the cannulated main duct, the position of the catheter within the duct was confirmed by ultrasound. A contrast agent was injected until the patient felt fullness, and ultrasound (B-mode) was used to confirm whether the contrast agent filled the main canal and secondary and tertiary ducts. Then, power Doppler ultrasound was performed to determine whether the salivary gland had increased blood flow. Results: In 2 cases in this report, a power Doppler ultrasound scan showed a significant increase in blood flow after contrast medium injection, which was not observed on a preoperative scan. Conclusion: Power Doppler ultrasound was found to be a simple, safe, and effective tool for real-time sialography monitoring.

Detection of High-Velocity Impact Damage in Composite Laminates Using PVDF Sensor Signals (고분자 압전 필름 센서를 이용한 복합재 적층판의 고속 충격 손상 탐지)

  • The mechanical properties of composite materials may severely degrade in the presence of damage. Especially, the high-velocity impact such as bird strike, a hailstorm, and a small piece of tire or stone during high taxing, can cause considerable damage to the structures and sub-system in spite of a very small mass. However, it is not easy to detect the damage in composite plates using a single technique or any conventional methods. In this paper, the PVDF(polyvinylidene fluoride) film sensors were used for monitoring high-velocity impact damage initiation and propagation in composite laminates. The WT(wavelet transform) and STFT(short time Fourier transform) are used to decompose the sensor signals. A ultrasonic C-scan and a digital microscope are also used to examine the extent of the damage in each case. This research shows how various sensing techniques, PVDF sensor in particular, can be used to characterize high-velocity impact damage in advanced composite.

An active intrusion-confronting method using fake session and Honeypot (거짓 세션과 허니팟을 이용한 능동적 침입 대응 기법)

  • In the coming age of information warfare, information security patterns need to be changed such as to the active approach using offensive security mechanisms rather than traditional passive approach just protecting the intrusions. In an active security environment, it is essential that, when detecting an intrusion, the immediate confrontation such as analysing the intrusion situation in realtime, protecting information from the attacks, and even tracing the intruder. This paper presents an active intrusion-confronting system using a fake session and a honeypot. Through the fake session, the attacks like Dos(Denial of Service) and port scan can be intercepted. By monitoring honeypot system, in which the intruders are migrated from the protected system and an intrusion rule manager is being activated, new intrusion rules are created and activated for confronting the next intrusions.

Identification of Impact Damage in Smart Composite Laminates Using PVDF Sensor Signals (고분자 압전센서 신호를 이용한 스마트 복합적층판의 충격 손상 규명)

  • An experimental procedure to identify failure modes of impact damage using sensor signals and to analyze their general features is examined. A series of low-velocity impact tests from low energy to damage-induced high energy were performed on the instrumented drop weight impact tester to monitor the stress wave signals due to failure modes such as matrix cracking, delamination, and fiber breakage. The wavelet transform(WT) and Short Time Fourier Transform(STFT) are used to decompose the piezoelectric sensor signals in this study. The extent of the damage in each case was examined by means of a conventional ultrasonic C-scan. The PVDF sensor signals are shown to carry important information regarding the nature of the impact process that can be extracted from the careful signal processing and analysis.
