자한(自汗)과 수족한(手足汗)에 대한 한의학 및 의학적 고찰 (Different Pathology between General and palms-and-soles hyperhidrosis in Korean Medicine and Medicine)

  • Objectives: We noticed that hyperhidrosis can be differentiated by whether it is topical or systemic in both Korean medicine(KM) and Modern medicine(MM). Comparing between topical and systemic sweating, we will figure out similarity between KM and MM about stimuli on sweat. Methods: All research is done by finding information on text-book, article, books. Results: Hyperhidrosis is differentiated by whether it is topical or systemic in both Korean medicine(KM) and Modern medicine(MM). First, systemic sweating(SS) is affected by body temperature. In KM, Heat and Cold(plus yang deficiency) can make human sweat systemically. In MM, heat is also mentioned as stimulus. Second, topical sweating(TS) can occur on emotionally-stressed situation especially on palms-and-soles. In KM, this phenomenon is explained by heart spirit(心神) and disease transmitted by pericardium meridian(手厥陰心包經 是動病). In MM, anatomically hyperhidrosis on palms-and-soles is generated by adrenergic sympathetic nerve which is involved with stress. Third, sweating on palms-and-soles also can be generated by internal organ. In KM, hyperhidrosis on palms-and-soles is explained as illness on stomach meridian(足陽明胃經). The 70% of parasympathetic nerve is vagus nerve which is located at internal organs-usually gastrointestinal tract. In that point, stomach and parasympathetic nerve seem to be involved in hyperhidrosis on palms-and-soles. Conclusion: Hyperhidrosis is differentiated similarly by whether it is topical or systemic in both Korean medicine and Modern medicine. Conserving each perspective of KM and MM, one perspective can be useful to other by supplementing other's weak point.

초기 근대 의약품 광고 담론분석: 근대적 아픔의 주체와 경험에 대한 소고 (The Modern subject and experience of pain described in medicine advertisements in the early modern times.)

  • 이 연구는 근대 의약품 광고에 드러난 신체 담론에 관한 연구이다. 최근 근대 형성기에 대한 미시적 접근은 일상사의 영역연구에서 두드러지는데, 일상적인 영역이야말로 사람들의 구체적인 경험이기 때문이다. 특히 근대 광고에는 당시 생활세계를 구성했던 대중문화, 소비문화, 성, 가족, 의 식 주, 질병 등의 구체적인 영역이 고스란히 담겨있다. 근대 이후 일상생활의 재조직화에서 신체는 이전까지 상속되고 신분적 질서에 의해 규정되었던 전근대적인 신체와 달리 어떤 규칙과 가치 속에서 자신의 신체를 자기 스스로 만들고 조립하여야 하는 근대적인 신체로 변화했다. 이 연구는 의약품 광고를 통해서 그 당시 사람들이 자신의 신체를 어떻게 인식하는지를 분석하고자 했고 분석방법으로 푸코의 담론형성이론을 활용하였다. 분석결과, 근대 초기의 의약품 광고 각각의 텍스트를 가로지르는 몇 가지 공통의 규칙, 즉 담론-장치를 찾을 수 있었고, 그 규칙성 속에서 의약품 광고가 근대적 아픔의 주체를 구성하는 과정을 살필 수 있었다. 근대 의약품 광고에 투영된 '임상의학적 시선의 내면화'와 '자기감시'라는 규율 권력적 요소는 '근대적 아픔의 주체' 형성에 기여했고, 사회 권력과 접합된 방식으로 표상되기도 했다.

양생도인법(養生導引法)과 현대운동요법(現代運動療法)의 비교(比較) 고찰(考察) (A Study on Comparison of Yangsaeng-Doyin Therapy and Modern Exercise Therapy)

  • Today the interest about maintenance and improvement of health has been increasing more and more. Realizing the necessity of study about exercise remedy, by consideration of the exercise remedy in the Occident and the Orient, I come to conculsions as follows : First, both Modern Exercise therapy and Yangsaeng-Doyin therapy(養生導引法) are preserve methods founded on breathing principle and are used efficiently in disease treatment and health maintenance. Second, compared with Yangsaeng-Doyin therapy(養生導引法), Modern Exercise therapy concentrates on physical training which emphasize momentum, Yangsaeng-Doyin therapy(養生導引法), however, focuses on discipline at once in mind and body based on the care of moral culture. Third, Modern Exercise therapy has been develope in view of Cure medicine, but Yangsaeng-Doyin therapy(養生導引法) to Psychosomatic preserving therapy including preserving health, treating a diseade and longevity in the light of comprehensive Priventive medicine. Fourth, while Modern Exercise therapy in the field of Clinical medicine is organized systematically and practically, Yangsaeng-Doyin therapy(養生導引法) is not sufficient to practical study in real clinic despite the abundance of content.

한의학 출산생리(出産生理)의 기경팔맥론(奇經八脈論) 적용에 대한 고찰 (A Study on the Application of Eight Extra Meridians Theory to Birth Physiology of Korean Medicine)

  • Subject : The application of eight extra Meridians theory to birth physiology of Korean Medicine. Objective : This study suggests physiological theories in child birth in Korean Medicine. Method : First, the application of eight extra Meridians and extraordinary organs theories to physiology, pathology, pregnancy, postnatal period of women were examined based on major publications and theories of some historical characters. This work provided clues for application of eight extra Meridians theory to birth physiology. Second, the progressing birth physiology of modern medicine based on the importance of the brain function was examined, with which the application of eight extra Meridians and extraordinary organs theories to the birth process showed close relationship. Conclusion : The following conclusions could be drawn. In traditional publications of Korean Medicine, the contents explaining birth physiology with eight extra Meridians theory were scarce, mainly due to the doctor's perception of birth as part of the natural female physiology along with menstruation and pregnancy. However, the contents explaining menstruation, pregnancy, postnatal physiology and pathology through eight extra Meridians theory could be found in classical texts such as Neijing, Jebyeong-wonhuron, Imjeunginameuian. Therefore, it is reasonable to apply the eight extra Meridians theory to the birth process as well. Modern medicine's understanding that pituitary and hypothalamus hormones are in play on womb in the birth process, such as Oxytocin, is in continuous progress. Moreover, the environmental condition that has been traditionally known ideal to the birth process in both the East and West-dark, warm, quiet and secluded-is being confirmed in modern research. The application of eight extra Meridians theory to birth physiology can help to explain these modern information, and thus make them more acceptable.

Real-World Impact of Modern Reperfusion Therapy for Acute Ischemic Stroke : A Nationwide Population-Based Data Study in Korea

  • Objective : The treatment paradigm for acute ischemic stroke has undergone several major changes in the past decade, contributing to improved patient prognosis in clinical practice. However, the extent to which these changes have affected patient prognosis in the real-world is yet to be clarified. This study aimed to evaluate the real-world impact of modern reperfusion therapy for acute ischemic stroke using data from the National Health Insurance Service in Korea. Methods : This study included patients aged 18-80 years who were admitted via the emergency room with an I63 code between 2011 and 2020. The rates of intravenous thrombolysis use and endovascular treatment according to the year of admission were investigated. Furthermore, the rates of decompressive craniectomy and 3-month mortality were also analyzed. The 10-year observational period was divided into three periods based on the 2015 guideline change as follows : prior, 2011-2014; transitional, 2015-2016; and modern, 2017-2020. Results : A total of 307117 patients (mean age, 65.7±10.9 years) were included, and most patients were male (59.7%). The rate of endovascular treatment gradually increased during the study period from 0.71% in the prior period to 1.32% in the transitional period and finally to 1.85% in the modern period. Meanwhile, the 3-month mortality rate gradually decreased from 4.78% in the prior period to 4.03% in the transitional period and to 3.71% in the modern period. Conclusion : In Korea, the mortality rate decreased as the rate of modern reperfusion therapy increased in patients with acute ischemic stroke. Overall, technical and scientific advances in reperfusion therapy have improved the outcome of patients with acute ischemic stroke in Korea.

이마의 온도 분포 진단법, 소위 액맥진단(額脈診斷)에 관한 고찰 (A Study on the Forehead Temperature Distribution Palpation, so called 'Emaizhenduan')

  • Objectives As a method of body temperature distribution diagnosis, palpation of infants' forehead had been widely introduced in ancient East Asian medical classics. However, few attention have been paid to this diagnostic method - so called 'emaizhenduan (額脈診斷)'. So we studied the history of emaizhenduan and it's implication in modern clinical field. Materials and methods: To search the medical classic that contains any contents of emaizhenduan, we mainly used the electronic texts of Zhonghuayidian (中華醫典). To search modern study article on emaizhenduan, we used China National Knowledge Infrastructure (www.cnki.net), National Digital Science Library (ndsl.kr) etc. Results 20 kinds of East Asian medical classics were found to contain contents on emaizhenduan. The oldest one was Huoyoukouyi (活幼口議) which was written in Yuan dynasty of China. Only one modern TCM book was found to have comments on emaizhenduan, while some textbooks of pediatrics in Korean Medicine was found to have relatively sound introduction on emaizhenduan. Conclusion It is shown in ancient East Asian medical classics that infants' forehead temperature palpation was introduced as early as late 13th century in China, and have continuously been referred in many East Asian medical classics before modern era.

  • Through the translation and comparative study of the enriching blood and nourishing vital essence(補陰血方劑) in "The Elimination & Supplement about the Famous Prescription Comments(刪補名醫方論)" of "The Golden Mirror of Medicine(醫宗金鑑)" with "The Famous Prescription Comments on Ancient and Modern Times(古今名醫方論)", we confirmed that about 50% of the sentences from "The Elimination & Supplement about the Famous Prescription Comments(刪補名醫方論)" were quoted in "The Famous Prescription Comments on Ancient and Modern Times(古今名醫方論)", and that many of the text were not quoted unchanged, but were revised and supplemented. In organization, the prescription with the fewer number of component drugs is given first, followed by that with more component drugs, and that with similar component drugs is explained subsequently to facilitate understanding. In the prescription notes, it is emphasized that when enriching blood, the invigorative method(補氣法) is very important and that cold or pungent herb should be very carefully used.

의료전직 전문화적 필요조건화중의학 (The Requisites of the Professionalization and Chinese Medicine)

  • This paper analyzes the requisites and the characteristics of the professionalization of modern Chinese Medicine. The requisites of the professionalization are the standardization of knowledge, the settlement of license system, the development of occupational ideology, and the strengthening of the organization. The research findings are as follows. First, Chinese Medicine choose the people-oriented development strategy. Second, The professionalization of Chinese Medicine was propelled by the government. Third, the sequence of the professionalization of Chinese Medicine was 1) the development of occupational ideology, 2) the standardization of knowledge, 3) the strengthening of the organization, and 4) the settlement of license system. The order of the professionalization of Chinese Medicine is different from that of Western Medicine. Fourth, in the process of the professionalization, modern Chinese Medicine partly employs the traditional face-to-face education.

호스피스 자원봉사자 지망생들의 암에 대한 인식도

  • Purpose: This study was designed to evaluate the perception of cancer in aspirants for hospice volunteer. Methods: Aspirants for hospice volunteer were surveyed by questionnaire before education of hospice program in September 2005 at Pyongan Hospice Center of Sam Anyang Hospital. The questionnaire was composed of total 28 items. Ten items were general characteristics and the remaining 18 items knowledge, attitude and practice of cancer and cancer pain. The data were analyzed by SPSS WIN 11.0 program using frequency, mean, percentage. Results: Eighty nine questionnaires were returned. There were 6 males and 83 females, and median age of aspirants was 46 years. Forty five aspirants(51%) had a cancer patient in th family or relatives and the others 44(49%) not. Forty eight(54%) had favorable attitude about modern clinical medicine for cancer treatment and the others 39(44%) unfavorable. Fifty eight(65%) had favorable attitude about complementary and alternative medicine for cancer treatment and the others 20(23%). In response to the question about 'How illness is a cancer', 25 aspirants(25%) responded to chronic disease, 42(47%) suffering incurable disease, the others 17(19%) death sentence, respectively. A number of aspirants had wrong knowledge and understanding of cancer, cancer pain and opioid analgesics. Seventy one percent aspirants feared cancer pain. Conclusion: The credibility to modern clinical medicine for cancer treatment was relatively low in aspirants for hospice volunteer compared with complementary and alternative medicine. They recognized a cancer as suffering incurable disease and death sentence than chronic disease. A number of aspirants had wrong knowledge of cancer, cancer pain and opioid analgesics. Therefore, further studies and trial will be warranted to evaluated the causes of these results and improve the credibility of modern clinical medicine for cancer treatment in general population.

음허증(陰虛證)의 임상적 질환 범위에 대한 고찰 (Study on Clinical Diseases of Yin Deficiency Pattern)

  • The purposes of this article are understanding the meaning of yin deficiency interpreted with a perspective of Traditional Korean Medicine and a modern perspective a study and assigning modern diseases to yin deficiency pattern types. Clinical papers were searched in China National Knowledge Infrastructure(CNKI) from 1995 to 2013. Results are as follows. First, yin deficiency written in the "Neijing" has been understood in many ways. It is translated such as deficiency of yin qi, inner qi, essence, cubit pulse, yin meridians qi, viscera yin and kidney. Second, yin deficiency pattern are related with disorders of the endocrine system, immunity, energy metabolism, blood circulation, cytokine, microelements, lipid metabolism and capability of getting rid of oxygen free radicals. Third, from pattern types, diverse diseases classified in types involving the heat from yin deficiency, which reflects pathologic conditions of deficiency heat which is distinct characteristics of yin deficiency pattern. Various diseases classified in types related with liver or kidney are reported, which reflects two viscera are more related with yin deficiency than other viscera. Fourth, levels of pattern types surveyed are more specific than Korean Standard Classification of Diseases(KCD) and specific enough to be applied clinically. This article surveyed the categories of modern diseases yin deficiency pattern types is assigned to but the detailed relation between them will be necessary to be studied in the future.
