Alternative Prevention on Human Error of Fatal Injuries by the Mobile Cranes (이동식 크레인 사망재해의 인지공학적 대안)

  • According to the statistics, occupational fatal injuries by mobile cranes were about 12 per year in whole industrial. Mobile cranes are widely used in various parts of industries to improve the efficiency of the work. However considerable number of fatal injuries happen each year during the operation of the machines. In this study, the current regulations to be adequate in industrial site have to be renew in order to prevent the fatal injuries by mobile cranes. Fatal injury analyses were conducted with several accident cases by the mobile cranes. For each accident, the causes of the injuries were examined and proper safety measures were proposed. In this study, the mobile crane showed a high fatality rate in industrial accidents and no detailed cause analysis of fatal accidents was conducted in terms of unsafe acts or conditions. This study proposed a revision of the standard guideline as an accident prevention measures through in-depth analysis of fatal accidents. First, among the mainly five machines caused the accidents, mobile crane was higher for the second showed 0.6% for number of fatalities compared to number of mobil cranes and for the third showed 11% for number of fatalities compared to number of injuries. Second, main cause of cognitive engineering agenda was visibility, responsibility, affordance. As the measures to prevent accidents before starting operation, alternative revision for the fool proof including visibility, responsibility, affordance etc. for the fool proof measures was proposed. Third, alternative revision as cognitive accident prevention for the fail safe measures was proposed.

  • As buildings become larger, higher and more complex in most construction sites, construction projects have to deal with transportation of more materials, labor and equipment, necessitating more use of construction equipment. Notably, in the high-rise building and plant construction projects mobile cranes are adopted more frequently among different types of construction machinery, which also results in serious industrial accidents relating to the use of crane. To reduce serious industrial accidents involving cranes, researches on its stability is in need. The research herein aims to study how to select sling and lug of mobile crane and review its stability. The research outcomes herein will make considerable contributions to selecting mobile cranes efficiently and ensuring their stability.

A study on Cause Analyses of Fatal injuries by the Mobile Cranes (이동식 크레인 사고사망재해 심층분석 연구)

Fatal injury analyses were conducted with several accident cases by the mobile cranes. For each accident, the causes of the injuries were examined and proper safety measures were proposed. This study proposed a revision of the standard guideline as an accident prevention measures through in-depth analysis of fatal accidents. First, fatality rate per 10,000 persons by the mobiles cranes was 0.57 in 2011. Second, the in-depth analysis of the mobile crane accidents showed that the main factors of the accidents were the overturn of mobile crane, the fracture of connection part between mobile crane boom and rope, the breaking of rope and the misunderstanding over the signal between a mobile crane driver and workers around a mobile crane. Third, the order of high defection of connecting parts was bucket bolt, bracket and extension parts. Fourth, since there were several cases of overturn of mobile crane by itself in the in-depth analysis of the mobile crane, the over slop-proof device was proposed as the main alternative in this study. Fifth, the revision of standards of safety factor that shall be more than 10, of connecting parts of mobile crane was proposed.

A Quantitative Analysis of Fatal Accidents Related to Cranes Using the FMEA Method (FMEA 기법을 활용한 크레인 관련 중대 재해의 정량적 분석에 관한 연구)

  • As buildings become higher, larger, and more complex, safety issues for construction workers working at such environments become more important. We analyzed 83 critical accident cases reported to the KOSHA(Korea Occupational Safety & Health Agency) for construction cranes by types of cranes and by patterns of accidents and causes. There are more number of accidents related to mobile cranes than that related to tower cranes, but the numbers of dead were similar in both cases. The most dominant cause of crane accidents was "fall of materials". We also analyzed the cases of crane accidents using the FMEA(Failure Mode and Effect Analysis) in order to set up a priority for safety management and also to prioritize research and development items relating tower cranes. In the process, we tried to eliminate subjective indexes such as an expert group survey and use objective and quantitative indexes. As a result, it was found that critical crane accidents occurs most during the "lifting and translating" activity.

  • The specifications needed for the mobile cranes are summarized as the following : 1) there may be not occured the oscillation of the cargo at unloading point. 2)the required time from departure point to destination point may be as short as possible. 3) there may be not a collapse of cargo caused by the oscillation in the course that the crago is mobilling. In this paper, the linear fractional transformation method is adopted as a method in order to improve the above mentioned problems. A design method of servo system is developed by modifying Davison's method for the case that the homogeneous differential equations of reference input and disturbance are different types. The real time control of a mobile crane system is implemented by 16bits microcomputer with A/D and D/A converters to illustrate the application of the adopted method. The experimental results for the three types of the design methods; linear fractional transformation method, servo system design method and optimal control method are shown for the comparison.

Development of Standard Risk Indicators for the Prevention of Serious Accidents in Mobile Crane Operations (Focused on Construction Industry) (이동식크레인 작업의 중대재해예방을 위한 표준 리스크 평가 지수 개발 (건설업을 중심으로))

  • Purpose: Mobile cranes are machines that contribute to high mortality, and the High Risk Factor (SIF) information, which analyzed 2,574 accidental deaths in the construction industry in the past 6 years (2016~2021), resulted in a total of 61 mobile crane accidents. Despite safety measures in the field, it is not used properly. In this study, we present standard risk assessment indicators that contribute to accident prevention. Method: Through expert interviews, fatal accident case analysis, field analysis, and literature research, we present the standard risk assessment index method of the 4M risk assessment method. Result: As a result of analyzing the risk assessment of eight sites, it was concluded that it cannot make a significant contribution to disaster prevention and should be applied as an improvement measure of the Standard Risk Assessment Index Law. Conclusion: Switching to the standard risk assessment index method at construction sites has been proposed to make it easier for health and safety personnel and workers to use, contributing to the reduction of accidents.

  • A new concept of ocean transport system, called mobile harbor, was introduced as a feasibility study in Korea in 2009. Target of the mobile harbor is a smart distance transport of containers with or without cargo handling cranes. Although the mobile harbor project has a lot of topics to deal with, this paper is to focus on only ship-to-ship stabilized mooring, which plays a key role in cargo handling. The ship-to-ship stabilized mooring system was developed and installed on beard a barge of LOA 32m and breadth 12m. The dockside tests as sea test were carried out so as to ascertain ascertained whether the systems can work well to control the barge's motion. The results of dockside test showed that the heave motion of the barge's motion can be reduced by more than 45%.

  • Although various policies have been conducted on safety work of construction equipment such as lifts, tower cranes, and mobile cranes, institutional and managerial problems, and improvement measures, accidents are still not decreasing. This study tried to identify differences in the viewpoints of supervisors, safety managers, and construction machinery operators and to obtain survey data on the awareness and necessity of related safety education. In conclusion, accidents would be reduced when workers received effective training on accurate work methods, safety rules, and recognition and removal of risk factors.

  • The concept of the Mobile Harbor had been made recently as a kind of feeder vehicle to transfer a certain amount of container boxes (i.e. 250 TEU at a time) from main ocean container vessels over 5,000 TEU capacity to the container terminal on land. In a harbor a short distance apart from the land, the container loading/unloading operation has to be performed on the main deck of the Mobile Harbor using the container cranes in the state of side-by-side mooring with protection of fenders and robot arms in the gap. Even under the ocean condition of the sea state class 2 or 3, the operation has to be confirmed to be safely performed. In this situation, the floating bodies considering the multiple-body interaction effect also has to be examined whether they might behave safely or not. Especially, this study focuses on the dynamic behavior of the Mobile harbor when a container box is hanged on the crane and the impact load due to the slewing motion is imposed in a certain sea state. The motion response should be controlled within the motion level to assure the safe operation.
